Metastatic bone cancer as a recurrence of early gastric cancer - characteristics and possible mechanisms

The surgical outcome of most early gastric cancer （EGC） is usually satisfactory. Some cases show bone metastasis even though the depth of cancer invasion is confined to the mucosa. The most frequent site for recurrence of EGC is the liver. Cases of EGC with bone metastasis are reviewed to clarify the clinicopathological characteristics of EGC giving rise to bone metastasis. Possible mechanisms and risk factors underlying this rare condition are proposed. Forty-six cases of bone metastasis from EGC are reviewed from published reports and meeting proceedings in Japan. This investigation suggests that risk factors for bone metastasis from EGC include depressed-type signet-ring cell carcinoma, poorly differentiated carcinoma, and/or the likely involvement of lymph node metastasis, even though the cancer is confined to the gastric mucosa. The risk factors do not include recurrence of EGC in the liver. We speculate that the mechanism of bone metastasis from EGC is via lymphatic channels and systemic circulation. Postoperative follow-up of cases should consider the development of bone metastasis from EGC. We propose the use of elevated alkaline phosphatase levels for the detection of bone metastasis and recommend bone scintigraphy in positive cases. 2005 The WJG Press and Elsevier Inc. Pain in Non-Athlete Patients Undergoing Hip Arthroscopy for Femoroacetabular Impingement and Hip Labral Tears: A Longitudinal Study

Background: Pain in patients with orthopedic problems is an important aspect of surgical evaluation. However, the effect on the quality of life depends not only on the degree of pain but also on its type. Hip arthroscopy has been performed for hip joint symptoms caused by femoroacetabular impingement (FAI) and hip labrum tears. However, the degree and type of pain after hip arthroscopy remains unknown. Objective: This study aimed to clarify the degree and type of pain during 6 months after hip arthroscopy. Methods: Non-athlete patients aged 20 - 65 years who underwent hip arthroscopy between December 2018 and October 2019 participated in a questionnaire survey before hospitalization and at 1, 3, and 6 months after surgery. The questionnaire comprised the Japanese Orthopaedic Association Hip-Disease Evaluation Questionnaire (JHEQ) for quality of life, International Physical Activity Questionnaire (IPAQ) for activities, and the Short-Form McGill Pain Questionnaire (SF-MPQ-2) for pain. Results: The analysis included 10 patients (men: 4, women: 6, mean age: 38 ± 8.6 years, FAI: 6 cases, hip labrum tears: 4 cases). The average time from symptom onset to surgery was 26.9 ± 19.0 months. The analysis from before surgery to 6 months after surgery showed improvement over time in all scale scores (JHEQ, IPAQ, and SF-MPQ-2). The degree of pain was significantly improved based on the JHEQ visual analog scale evaluation (P = 0.019) 3 months after surgery. Significant improvement in intermittent pain (P = 0.011) based on SF-MPQ-2 was noted 3 months after surgery;however, no significant improvement in continuous pain was noted. Conclusions: Patients who underwent hip arthroscopy showed significant improvements in the degree of pain and type of intermittent pain from before surgery to 3 months after surgery, however, no change was observed in continuous pain.

Spontaneous transition of the plasma confinement mode was observed in thehelical-axis heliotron device 'Heliotron J' for three different plasma heating schemes, ***-only, NBI-only and the combination of ECH and NBI. The transition seems to occur above a certaincritical density. In addition to the confinement transition, a spontaneous shift of the hittingposition of the divertor plasma flux on the wall was observed. This shift could be related with thechange of the edge field topology caused by non-inductive toroidal currents.

BACkGROUND Bronchogenic cysts are congenital lesions requiring radical resection because of malignant ***,a method for the optimal resection of these cysts has not been completely *** SUMMARY Herein,we presented three patients with bronchogenic cysts that were located adjacent to the gastric wall and resected *** cysts were detected incidentally with no symptoms and the preoperative diagnosis was challenging to obtain via radiological *** on laparoscopic findings,the cyst was attached firmly to the gastric wall and the boundary between the gastric and cyst walls was difficult to ***,resection of cysts alone caused cystic wall injury in Patient ***,the cyst was resected completely along with a part of the gastric wall in Patient *** examination revealed the final diagnosis of bronchogenic cyst and revealed that the cyst wall shared the muscular layer with the gastric wall in Patients 1 and *** Patient 3,the cyst was located adjacent to the gastric wall but histopathologically originated from diaphragm rather than *** the patients were free from *** The findings of this study state that a safe and complete resection of bronchogenic cysts required the adherent gastric muscular layer or full-thickness dissection,if bronchogenic cysts are suspected via pre-and/or intraoperative findings.

Comparing mass screening techniques for gastric cancer in Japan

AIM： To discuss the efficacy of endoscopic mass screening for gastric cancer.METHODS： The data used in this study were the results of mass screening programs for gastric cancer in Niigata City from 2002 to 2004. The number of participants was 35089 in 2002, 34557 in 2003 and 36600 in 2004. The finding ratio referred to the final diagnosis of gastric cancer after a double check of endoscopic files and histological findings. The costs of identifying one case of gastric cancer were calculated based on the total expense for each screening program and additional close examinations. RESULTS： From the analysis of individual screening program with endoscopy, individual screening program with X-ray （ISX） and mass screening program with photofluorography （MSP） in reference to the finding ratio of gastric cancer, endoscopic examination was the best for detecting early gastric cancer, the finding ratio was 0.87% in 2004, approximately 2.7 and 4.6 times higher than those of the ISX and MSP groups. In addition, this novel method was the cheapest means regarding the cost of identifying one case of gastric cancer, which was estimated to be 1608000 Japanese yen in 2004. CONCLUSION： Endoscopic mass screening is a promising method and can be effectively applied if a sufficient number of skilled endoscopists become available to staff the system and if city offices support it.

Melatonin Enhances Object Recognition Memory through Melatonin MT1 and MT2 Receptor-Mediated and Non-Receptor-Mediated Mechanisms in Male Mice

Melatonin (MEL) has been reported to have acute enhancing effects on some aspects of cognition. Recently, we revealed that N1-acetyl-5-methoxyquinuramine (AMk), a brain metabolite of MEL, is much more potent than MEL in converting short-term memory (STM) to long-term memory (LTM) with a single administration immediately after the acquisition trial of the novel object recognition (NOR) task. These data suggest that the memory-enhancing effects of MEL may be mediated by mechanisms independent of the activation of MEL MT1 and MT2 receptors. In the present study, we examined the contribution of MT1 and MT2 receptor-mediated and non-receptor-mediated mechanisms to the acute memory-enhancing effects of MEL using NOR task. Mice were administered with either MEL, AMk, or a highly selective MT1/MT2 receptor agonist ramelteon (RAM) immediately after the acquisition trial and the effects of varying doses of these drugs on both STM and LTM performance were compared. We found that both AMk and RAM were more potent than MEL in both facilitating STM and promoting LTM formation. We also found that pretreatment with luzindole, a MT1/MT2 receptor antagonist, markedly suppressed only the effects of RAM. These results suggest that acutely administered MEL enhances NOR memory through both MT1 and MT2 receptor-mediated and non-receptor-mediated mechanisms.
