Long-term follow-up of intravesical abobotulinumtoxinA (Dysport®) injections in women with idiopathic detrusor overactivity

Objective:Only a few numbers of studies have been published on the use of abobotulinumtoxinA(Dysport®)in idiopathic detrusor overactivity(IDO).This study reported the long-term follow-up of women with IDO who were treated with intravesical Dysport®injections.Methods:Two hundred and thirty-six patients with IDO who had failed first-line conservative and antimuscarinic therapy received 500-900 units of Dysport®between April 2014 and July 2015.All patients were followed up for 5 years after their initial injection and interviewed on the phone.Results:A total of 236 women with IDO aged from 18 years to 84 years(mean±standard deviation:49.6±15.9 years)were included in our study.The median follow-up time for patients was 36.5(range:10-70)months,and the median recovery time after injection was 18.5(range:0-70)months.A total of 83(35.2%)patients stated that they had subjective improvement of their symptoms whereas 84(35.6%)patients did not report any improvement in symptoms.The initial International Consultation on Incontinence Questionnaire Overactive Bladder mean score was 6.9(standard deviation 3.4).There was a positive association between the median recovery time and the components of the International Consultation on Incontinence Questionnaire Overactive Bladder questionnaire.Conclusion:In a sub-population of overactive bladder patients with IDO who have failed first-line therapy,a single intravesical Dysport®injection can resolve patient symptoms completely or reduce the symptoms to an acceptable level that can be controlled with antimuscarinics or re-injection on demands.

Effects of anestrus dog serum on superovulation in rats and mice

Objective:To study the effects of anestrus dog serum in rodents (contains high amount of FSH) compared with two common superovulatory programs.Methods: Rats (n=30) at diestrus phase were evenly divided into pregnant mare serum gonadotrophins (PMSG) group (administrated with 30 IU PMSG, and 48 h later following by 25 IU hCG), recombinant follicle stimulating hormone (rFSH) group (reducing dose every 12 h from 5 to 1 IU, then following by 25 IU hCG) and anestrus dog serum group (reducing dose every 12 h from 0.6 to 0.1 mL, then following by 25 IU of hCG). Mice (n=30) were also evenly divided into PMSG group (5 IU PMSG used at 13 pm, and 48 h later following by 5 IU of hCG), rFSH group (from 13 pm, reducing dose every 12 h from 2.5 to 0.5 IU, then following by 5 IU hCG) and anestrous dog serum group (from 13 pm, reducing dose every 12 h from 0.100 to 0.025 mL, then following by 5 IU hCG). Mice and rats placed with males for 24 h after last injection. Histology samples of ovaries were prepared and the numbers of corpus lutea were counted on day 14 after mating. Results: In mice, the differences among mean number of corpus lutea in all groups (P=0.01) and between FSH and dog serum group were significant (P=0.0007). But no significant differences were found between the mean number of corpus lutea in dog serum and PMSG, or between FSH and PMSG groups. In rats, mean number of corpus lutea were significant differences among three groups (P=0.01), and between PMSG and dog serum groups (P=0.02). Conclusions: Superovulatory response in anestrus dog serum group is similar to PMSG group in mice, which is relatively similar to FSH in rats.

Pediatric palliative care for children with cancer:a concept analysis using Rodgers’evolutionary approach

Background Providing access to pediatric palliative care(PPC)for children living with a serious illness,such as cancer,is of critical importance,although this specialized intervention,as a novel concept,still seems vague and complicated.The present study analyzed the concept of PPC for children with cancer.Methods Rodgers'evolutionary method was employed for the concept analysis.Articles on PPC,particularly those for children suffering from cancer,published between 2010 and 2021 were searched in valid academic research databases.The inclusion criteria for the full-text articles were based on the characteristics,antecedents,and consequences of PPC for children with cancer.Results In total,19 relevant articles were selected and then reviewed and analyzed after applying the inclusion criteria and the final sampling.The analysis of the concept of PPC for children affected with cancer revealed four characteristics,including"PPC as holistic and integrated care","PPC as patient-and family-centered care","PPC as early-start continuous care",and"PPC as interdisciplinary and team-based care".Some effective factors could also act as antecedents for this concept,i.e.,health care providers'training and expertise as well as human resources and financing.Moreover,improved quality of life,symptom reduction,and coordination between patient care and family support were among the PPC consequences for such children.Conclusions The study results demonstrated that delivering PPC to children with cancer demanded a comprehensive view of its various dimensions.Furthermore,numerous factors need to be delineated for its accurate and complete implementation.

Background:Constipation is a common chronic bowel disorder with an incidence of more than 50%in the elderly population.Complementary and alternative medicine is a cost-effective and satisfactory treatment for constipation used widely by the elderly.Objective:This study evaluates the efficacy of an herbal formula made from Foeniculum vulgare Mill.and Rosa damascena for the treatment of constipation in an elderly population and consequent changes to their quality of life.Design,setting,participants and intervention:This double-blind randomized active controlled clinical trial,with parallel group allocation ratio of 1:1,was conducted in a referral clinic in Afzalipour Hospital,affiliated to Kerman University of Medical Sciences in Kerman,Southeastern Iran.Individuals over 60 years of age,diagnosed with functional constipation(based on the RomeⅣcriteria),were included in this study.Participants received a sachet of 10 g F.vulgare and R.damascena(herbal formula group)or polyethylene glycol 4000(PEG 4000 group)with a glass of warm water two times a day for 4 weeks and were followed up for 4 additional weeks.Main outcome measures:Constipation severity,stool consistency,and the quality of life were used as the primary outcomes.Drug side effects were used as a secondary outcome.The outcomes were assessed using the Constipation Assessment Scale,the Bristol Stool Form Scale,and the Patient Assessment of Constipation Quality of Life questionnaire.Results:A total of 25 participants in each group completed the four-week treatment cycle and the eightweek follow-up.At the end of the four-week treatment cycle,all clinical outcomes had significant improvements in both groups(P<0.05).The analysis of constipation severity(P<0.001),stool consistency(P<0.001),and the quality of life(P<0.001)showed significant improvements with fewer side effects(mild diarrhea)and a longer duration of symptom relief in the herbal formula group compared to the PEG 4000 group.Conclusion:Although both interventions significantly improved the treatment out

An efficient built-up land expansion model using a modified U-Net

This paper introduces an improved convolutional neural network based on the conventional U-Net for simulating built-up land expansion.The proposed method hires a pixel-wise semantic segmentation approach considering the spatial drivers affecting urbanization as data cubes.Independent variables including altitude,slope,and distance from barren,crop,greenery,roads,and urban areas for 1998,2008,and 2018 were considered as covariates for the simulation of built-up land expansion in Tehran and Karaj regions in Iran.The proposed method was compared with the random forest(RF)algorithm as the baseline model.Evaluation using the area under the total operating characteristic indicated the superiority of our modified U-Net(0.87)over the RF(0.82)algorithm.Furthermore,evaluation using the percent correct metric indicated that our proposed model is capable of learning neighborhood effects effectively leading to simulate built-up land expansion accurately,independent from applying a cellular automata(CA)model.Therefore,the modified U-Net independent from the CA which can consider the neighborhood effects is recommended for the simulation of built-up land expansion precisely.

Two novel sets of UiO-66@metal oxide/graphene oxide Z-scheme heterojunction:Insight into tetracycline and malathion photodegradation

Nowadays,widespread researches have been focused on the development of effective photocatalysts to remove pollutants of the aquatic system.In accordance with the universal studies,two new sets of UiO-66@metal oxide(including ZnO and TiO2)/graphene oxide heterojunctions were synthesized for photodegradation of aromatic(tetracycline)and nonaromatic(malathion)pollutants which are challenging cases in the environment.The dosage of the photocatalyst,pH of the solution,the type of metal oxide,and the presence of various scavengers are assayed parameters in this work.In the optimum condition,maximum photodegradation efficiency is achieved in 90 min for tetracycline(81%)and malathion(100%)by the UiO-66@ZnO/graphene oxide.The superior separation of charge carriers by Z-scheme mechanism,excellent electron mobility on layers of graphene oxide and high surface area are factors that enhanced the efficiency.Furthermore,in comparison with pure UiO-66,the band gaps belong to heterojunctions revealed a red shift in the absorption edge,which can be responsible for more expand adsorption of the solar spectrum.Total organic carbon analysis verified the decontamination of these pollutants in the solution.The produced main intermediates during the photocatalytic process were identified and the possible degradation pathway proposed.In general,the superior photocatalytic activity suggests that these designed photocatalysts can be a promising choice for having a clean future.

The effects of Ca addition on the microstructure and oxidation properties of a new Mg alloy were studied.The oxidation behavior of the alloys was analyzed by thermal analysis and material characterization of the alloys exposed in flame environment;and both electrical and induction furnaces.Moreover,the surface layers were characterized using field emission scanning electron microscopy,and X-ray diffraction technique.It was found that increasing the Ca addition reduces the grain size and increases the fraction of the secondary phases,and enhances the mechanical properties.Moreover,increasing the Ca contents resulted in the formation of a dense CaO/MgO layer on the surface prohibited the oxygen diffusion and assisted in protection of the substrate against further oxidation.Therefore,ignition temperature was increased from 680℃ to 890℃ after addition of the Ca element.The mechanical properties and ignition behavior of the current materials was compared with the literature which it showed an excellent combination of the properties in the developed alloys.

Objective:Pneumocystis pneumonia(PcP)is a life-threatening infection caused by the opportunistic fungi Pneumocystis jirovecii.The emergence of the COVID-19 pandemic forced the focus of attention of health policymakers on these two infections due to their clinical and paraclinical similarities,which cause diagnostic dilemmas.This study was undertaken to evaluate and estimate the global prevalence and main leading risk factors of coronavirus-associated pneumocystosis(CAP).Methods:We searched related databases between December 2019 and May 2022 for studies reporting CAP.Meta-analysis was performed using StatsDirect software(version 2.7.9)according to the DerSimonian and Laird method applying the random-effects model.We evaluated heterogeneity using theχ2-based Q statistic(significant for P75%indicative of“notable”heterogeneity).Moreover,an odds ratio(OR)analysis was performed for eligible data.Results:Our meta-analysis included eight studies with 923 patients hospitalized with COVID-19;among them,92 were PcP cases.The overall pooled prevalence of CAP was estimated at 11.5%.The mortality among CAP patients was lower than that of non-PcP patients(OR 1.93;95%CI 0.86-4.31).Long-term corticosteroid therapy(OR 28.22;95%CI 0.54-1480.84)was the most predisposing factor for PcP among COVID-19 patients,followed by pulmonary diseases(OR 1.46;95%CI 0.43-4.98),kidney diseases(OR 1.26;95%CI 0.21-7.49),and acute respiratory destruction syndrome(OR 1.22;95%CI 0.05-29.28).Conclusions:The prevalence of PcP among the COVID-19 population is almost similar to the pre-COVID era.However,PcP-related mortality was decreased by the emergence of the COVID-19 pandemic.Women with COVID-19 are more susceptible to PcP than men.Acute respiratory distress syndrome,kidney diseases,pulmonary diseases,and long-term corticosteroid therapy increased the risk of PcP;however,transplantation and malignancy decreased the risk for PcP among COVID-19 patients.Further retrospective,case-control,prospective,and more precisely systema

Estimation Methods for the Generalized Inverted Exponential Distribution Under Type II Progressively Hybrid Censoring with Application to Spreading of Micro-Drops Data

In this article,we consider the statistical inferences of the unknown parameters of a generalized inverted exponential distribution based on the Type II progressively hybrid censored sample.By applying the expectation–maximization(EM)algorithm,the maximum likelihood estimators are developed for estimating the unknown parameters.The observed Fisher information matrix is obtained using the missing information principle,and it can be used for constructing asymptotic con-fidence intervals.By applying the bootstrapping technique,the confidence intervals for the parameters are also derived.Bayesian estimates of the unknown parameters are obtained using the Lindley’s approximation.Monte Carlo simulations are imple-mented and observations are given.Finally,a real data set representing the spread factor of micro-drops is analyzed to illustrative purposes.

Second-Order Finite Difference/Spectral Element Formulation for Solving the Fractional Advection-Diffusion Equation

The main aim of this paper is to analyze the numerical method based upon the spectral element technique for the numerical solution of the fractional advection-diffusion equa-tion.The time variable has been discretized by a second-order finite difference procedure.The stability and the convergence of the semi-discrete formula have been proven.Then,the spatial variable of the main PDEs is approximated by the spectral element method.The convergence order of the fully discrete scheme is studied.The basis functions of the spectral element method are based upon a class of Legendre polynomials.The numerical experiments confirm the theoretical results.
