Early diagnosis and treatment of acute or subacute spinal epidural hematoma

Background Despite low morbidity, acute or subacute spinal epidural hematoma may develop quickly with a high tendency to paralysis. The delay of diagnosis and therapy often leads to serious consequences. In this study we evaluated the effects of a series of methods for the diagnosis and treatment of the hematoma in 11 patients seen in our hospital. Methods Of the 11 patients （8 males and 3 females）, 2 had the hematoma involving cervical segments, 2 cervico-thoracic, 4 thoracic, 1 thoraco-lumbar, and 2 lumbar. Three patients had quadriplegia, including one with central cord syndrome; another had Brown-Sequard＇s syndrome; and the other seven had paraplegia. Five patients were diagnosed at our hospitals within 3-48 hours after appearance of symptoms, and 6 patients were transferred from community hospitals within 21-106 hours after development of symptoms. Key dermal points, key muscles and the rectal sphincter were determined according to the American Spinal Injury Society Impairment Scales as scale A in two patients, B in 5 and C in 4. Emergency MRI in each patient confirmed that the dura mater was compressed in the spinal canal, with equal intensity or hyperintensity on T1 weighted image and mixed hyperintensity on T2 weighted image. Preventive and curative measures were taken preoperatively and emergency operation was performed in all patients. Open laminoplasty was done at the cervical and cervico-thoracic segments, laminectomy at the thoracic segments, laminectomy with pedicle screw fixation at the thoraco-lumbar and lumbar segments involving multiple levels, and double-sided laminectomy with the integrity of articular processes at the lumbar segments involving only a single level. During the operation, special attention was given to hematoma evacuation, hemostasis and drainage tube placement. Results Neither uncontrollable hemorrhage nor postoperative complications occurred. Different plasma levels of interleukins and chemokines： comparison between children and adults with AIDS in China

Background The immunological differences between children and adults with AIDS in China are not well documented. Th1/Th2 cytokines and chemokines are two types of immune factors intimately involved in disease progression of HIV-1 infection. This study aimed to identify changes in plasma levels of Th1/Th2 cytokines inerleukin （IL）-18, IL-16, IL-10 and chemokines regulated on activation, normal T cell expressed and secreted （RANTES）, stromal cell-derived factor-1 （SDF-1） and monocyte chemoattractant protein-1 （MCP-1） in HIV-l-infected children and adults in China. Methods Seventy-five children with AIDS and 35 adult AIDS patients were recruited and clinical data were collected. CD4^＋ T lymphocyte counts were measured by flow cytometery and plasma HIV RNA levels were measured by quantitative RT-PCR. Plasma levels of IL-18, IL-10, IL-16, RANTES, MCP-1, SDF-1a and SDF-1β were quantified by enzyme-linked immunosorbent assay. The levels of β2-microglobulin （β2-MG） and soluble Fas （sFas） were measured to validate the level of humoral and cellular immune activation. Results The mean levels of all cytokines in pediatric and adult AIDS patients were significantly higher than in their healthy controls （P 〈0.01）. The mean levels of these cytokines were higher in pediatric patients than in adult patients （P 〈0.05, except for SDF-la and β2-MG）. Some of the cytokine levels in patients younger than 6 years old was higher than in older children and adults with AIDS （IL-10, IL-18, SDF-la, MCP, RANTES and sFas, P 〈0.05）. Levels of IL-18, IL-10, RANTES and β2-MG of pediatric patients increased as the levels of viral load increased （P 〈0.05）. Conclusions Abnormal immune activation can be measured in Chinese pediatric and adult patients with AIDS, and is higher in children than in adult patients. The cytokines levels coincide with disease progression of AIDS, but have no direct relationship with total CD4^＋ T cell count.

Plastic limit load analysis for pressure pipe with incomplete welding defects based on the extended Net Section Collapse Criteria

With von Mises yield criterion,the loading range of Net Section Collapse(NSC) Criteria is extended from combined tension and bending loadings to combined bending,torsion and internal pressure loadings.A new theoretical analyzing method of plastic limit load for pressure pipe with incomplete welding defects based on the extended NSC Criteria is presented and the correlative formulas are deduced,the influences of pipe curvature,circumferential length and depth of incomplete welding defects on the plastic limit load of pressure pipe are considered as well in this method.Meanwhile,according to the orthogonal experimental design method,the plastic limit loads are calculated by the finite element method and compared with the theoretical values.The results show that the expressions of plastic limit load of pressure pipe with incomplete welding defects under bending,torsion and internal pressure based on extended NSC criteria are reliable.The study provides an important theoretical basis for the establishment of safety assessment measure towards pressure pipe with incomplete welding defects.

Catalytic asymmetric dearomative azo-Diels–Alder reaction of 2-vinlyindoles

Due to the high electrophilic nature of azo-dienophiles, azo-Diels–Alder proceeds rapidly even without the need of a catalyst and is therefore regarded as the “click reaction”. This spontaneity causes strong background reaction and poses a daunting challenge to chemists for developing the catalytic asymmetric version. Reported herein is the first catalytic asymmetric dearomative azo-Diels–Alder reaction between2-vinylindoles and triazoledione. This protocol makes use of the high energy barrier of dearomatization to avert the strong background reaction of azo-Diels–Alder reaction, allowing the implementation of the projected reaction at ambient temperature. Density functional theory calculations have been performed to gain insights into the reaction mechanism and the origins of the enantioselectivity. By using this method,a variety of tetracyclic indole derivatives have been readily prepared in good to excellent yields and with excellent diastereo-and enantio–selectivities(33 examples, up to 97% yield and >99% ee, >20:1 dr).

BACKGROUND Hepatocellular carcinoma(HCC)is one of the leading causes of death due to its complexity,heterogeneity,rapid metastasis and easy recurrence after surgical resection.We demonstrated that combination therapy with transcatheter arterial chemoembolization(TACE),hepatic arterial infusion chemotherapy(HAIC),Epclusa,Lenvatinib and Sintilimab is useful for patients with advanced HCC.CASE SUMMARY A 69-year-old man who was infected with hepatitis C virus(HCV)30 years previously was admitted to the hospital with abdominal pain.Enhanced computed tomography(CT)revealed a low-density mass in the right lobe of the liver,with a volume of 12.9 cm×9.4 cm×15 cm,and the mass exhibited a“fast-in/fast-out”pattern,with extensive filling defect areas in the right branch of the portal vein and an alpha-fetoprotein level as high as 657 ng/mL.Therefore,he was judged to have advanced HCC.During treatment,the patient received three months of Epclusa,three TACE treatments,two HAIC treatments,three courses of sintilimab,and twenty-one months of lenvatinib.In the third month of treatment,the patient developed severe side effects and had to stop immunotherapy,and the Lenvatinib dose had to be halved.Postoperative pathological diagnosis indicated a complete response.The patient recovered well after the operation,and no tumor recurrence was found.CONCLUSION Multidisciplinary conversion therapy for advanced enormous HCC caused by HCV infection has a significant effect.Individualized drug adjustments should be made during any treatment according to the patient's tolerance to treatment.

执行力附着于执行名义,是权利人请求执行及执行机关实施执行的法律效力。执行力主观范围包括“为谁,对谁”为强制执行,原则上以执行名义记载为准,但在例外情况下向第三人扩张。执行主观范围的消极扩张,表现为经申请执行人的申请,执行法院裁定变更、追加第三人为被执行人。此类第三人可分为“承继型”和“责任型”两种。执行力主观范围的消极扩张须满足必要性、正当性和妥适性三个要件。“承继型”第三人兼具必要性和正当性。对“责任型”第三人而言,必要性即被执行人的财产不足以清偿债务,正当性依赖于实体法的规定或第三人的意定。妥适性指的是变更、追加的争议适合通过内嵌于强制执行的略式程序进行审查判断。

Trimethylamine N-oxide aggravates vascular permeability and endothelial cell dysfunction under diabetic condition:in vitro and in vivo study

AIM:To provide the direct evidence for the crucial role of trimethylamine N-oxide(TMAO)in vascular permeability and endothelial cell dysfunction under diabetic condition.METHODS:The role of TMAO on the in vitro biological effect of human retinal microvascular endothelial cells(HRMEC)under high glucose conditions was tested by a cell counting kit,wound healing,a transwell and a tube formation assay.The inflammation-related gene expression affected by TMAO was tested by real-time polymerase chain reaction(RT-PCR).The expression of the cell junction was measured by Western blotting(WB)and immunofluorescence staining.In addition,two groups of rat models,diabetic and non-diabetic,were fed with normal or 0.1%TMAO for 16wk,and their plasma levels of TMAO,vascular endothelial growth factor(VEGF),interleukin(IL)-6 and tumor necrosis factor(TNF)-αwere tested.The vascular permeability of rat retinas was measured using FITC-Dextran,and the expression of zonula occludens(ZO)-1 and claudin-5 in rat retinas was detected by WB or immunofluorescence staining.RESULTS:TMAO administration significantly increased the cell proliferation,migration,and tube formation of primary HRMEC either in normal or high-glucose conditions.RT-PCR showed elevated inflammation-related gene expression of HRMEC under TMAO stimulation,while WB or immunofluorescence staining indicated decreased cell junction ZO-1 and occludin expression after high-glucose and TMAO treatment.Diabetic rats showed higher plasma levels of TMAO as well as retinal vascular leakage,which were even higher in TMAO-feeding diabetic rats.Furthermore,TMAO administration increased the rat plasma levels of VEGF,IL-6 and TNF-αwhile decreasing the retinal expression levels of ZO-1 and claudin-5.CONCLUSION:TMAO enhances the proliferation,migration,and tube formation of HRMEC,as well as destroys their vascular integrity and tight connection.It also regulates the expression of VEGF,IL-6,and TNF-α.

Potassium-ion batteries(PIBs)by virtue of their strong cost competitiveness and similar electrochemical properties to lithiumion batteries have been deemed to be a promising electrochemical energy storage technology.To promote the application in the commercial market,developing electrode materials with high specifi c capacities,superior cycling stability,and reliable safety is of great importance.Anode materials as an important component of PIBs play a decisive role,among which twodimensional transition metal chalcogenides(2D TMCs)have attracted wide attention owing to their unique material and electrochemical properties.In the 2D TMCs’family,molybdenum chalcogenides as fl agship are the most studied materials and demonstrated the potential as anodes.With the deepening of research on 2D TMCs,another shining member that possesses similar properties to molybdenum chalcogenides,tungsten chalcogenides(WS_(2),WSe_(2),and WTe_(2)),has aroused tremendous attention.Despite many inspiring results,various challenges remain to be further addressed;meanwhile,some results are still unclear and disputed.Herein,this review fi rst introduces their material properties and electrochemical storage mechanisms.Then,we systematically overview the research progress and put forward promoting improvement strategies.Finally,challenges and opportunities that would be future research directions are discussed.

Phase Transformation of Amorphous Calcium Carbonate to Single-Crystalline Aragonite with Macroscopic Layered Structure in the Presence of Egg White Protein and Zinc Ion

Highly oriented calcium carbonate lamellas are exquisite structure produced by biomineralization. Strategies mimicking nature have been developed to synthesize inorganic materials with excellent structures and optimal properties. In our strategy, egg white protein and zinc ion were employed in the solution to induce the crystallization of calcium carbonate, resulting in the macroscopic aragonite laminate with an average length of 1.5 mm, which was comprised of single-crystalline tablets. During the crystallization at initial stage, it was found that the particles displayed the characteristics of amorphous calcium carbonate, which was then transformed into the sophisticated structured aragonite through a multistage assembly process. The rebuilt nacre structure in vitro was achieved owing to the synergistic effects of egg white protein and zinc ion.

Fast Mineralization of Densely Packed Hydroxyapatite Layers in the Presence of Over-expressed Recombinant Amelogenin

Learning from the process of biominerals formation provides tremendous ideas for developing advanced synthesis techniques. According to the structure formation of tooth enamel, a recombinant amelogenin his-Amel X was designed and constructed. The protein was over-expressed and could be conveniently purified in one-step heat treatment. The mineralization process of hydroxyapatite was initiated by enzyme AP and regulated by the recombinant amelogenin. Effects of solution pH value and mineralization duration were studied. It was demonstrated that his-Amel X could induce the nucleation of apatite and quicken the growth rate at pH 7.0-7.4, while impeded hydroxyapatite growth at pH 6.8. Moreover, a much denser layer of hydroxyapatite was achieved with the addition of his-Amel X. The present study may not only provide insight into the formation of natural biomaterials but also open a new path to prepare materials under environmentally benign conditions.
