align="justify"> Introduction: The coronavirus disease 2019 epidemic emerged in December 2019 and spread worldwide. Since workplaces are high-risk location for occupational exposure, understanding of the dynamics of COVID-19 clusters in occupational settings helps reduce the transmission of infectious disease in workplaces. This study presents an overview of the epidemiological characteristics of COVID-19 workplace clusters, preventive strategies adopted and outbreak responses undertaken in Muscat governorate. Materials and Methods: This is a descriptive study on the epidemiological characteristics of cases and distribution of workplace-related clusters of COVID-19 in Muscat Governorate in Oman. Results: A total of 36,798 COVID-19 cases were confirmed in Muscat from 24th February to 31st July 2020 in which 40.5% was belonging to clusters. Out of them 61% were Workplace-Dormitory clusters, predominantly expatriates. 78.6% of employees were symptomatic at time of examination. Fever and cough were the two most common symptoms reported in workplace related clusters. The number of affected employees ranged from 2 to 358 per cluster. Construction, retail, food, beverages, services, industrial manufacturing, oil & gas and transportation were identified as most at risk settings. Within the cases in workplace-related clusters, there was significant higher prevalence of diabetes, hypertension and obesity among females compared to males as also Omani nationals compared to expatriates and smoking among expatriate males compared to Omani males. In conclusion, understanding the epidemiological characteristics of the affected cases in organization setting will assist the policymakers to understand patterns of epidemiological spread and plan for robust interventions. <

Introduction: The coronavirus disease 2019 (COVID-19) was declared as pandemic by WHO by March 11th. First case reported in Oman was on 24th February 2020 and later the country went through stages of epidemic progression. This study describes the sociodemographic and epidemiological characteristics of confirmed COVID-19 cases in Muscat governorate and related outcomes. Materials and Methods: This is a descriptive, exploratory analysis of all lab confirmed COVID 19 cases that were reported from 1st February to 31st May 2020. Data for the study was primarily extracted from notifications system established for surveillance (Tarassud). Secondary data sources were, contact listings and hospital medical records. Results: 11,648 initial cases of confirmed COVID-19 infections were included. The mean age was 35 years, 84.7% (N = 9862) were males, 25.9% (N = 3017) were Omanis, and 74.1% (N = 8631) were expatriates of which Indian origin were the majority (37%). Fever and cough were the most common presentations (46.3% and 29.5% respectively). Diabetes and hypertension were the most common comorbidities (4.9% and 4.6% respectively). Hospital admission was required for 7% (N = 811) of the total reported cases, out of them 171 cases (21%) were admitted to ICU, where 107 (13.2%) were ventilated. The case fatality rate (CFR) was 0.9%. 158 clusters containing 2949 contacts were identified from case records and categorised into 3 groups based on their exposure settings. The incubation period measured was 8 days (IQR 4.0 - 15.0) for workplace, 8 days (IQR 4 - 17) for dormitory and 4 days (IQR 2.0 - 7.0) for family groups. The secondary attack rate (SAR) estimated was 41.6% (95% CI: 0.34 - 0.48), 52% (95% CI: 0.40 - 0.63) and 33% (95% CI: 0.27 - 0.38) for workplace, dormitory and family groups, respectively. AIM:To investigate the associations between CDH1 gene polymorphisms and gastric cancer(GC) risk ***:We analyzed four CDH1 polymorphisms(+54 T>C,-160 C>A,-616 G>C,-3159 T>C) in an Omani population,by extraction of genomic DNA from the peripheral blood of 192 patients with GC and 170 control participants and performed CDH1 genotyping using DNA ***:CDH1-160-AA genotype was associated with an increased risk of GC(OR = 3.6,95% CI:1.1-11.8)(P = 0.03).There was no significant association between the other polymorphisms and GC *** haplotype analysis of +54 T>C,-160 C>A,-616 G>C,-3159 T>C genotypes revealed that the OR of CCGC and CAGC haplotypes was 1.5(95% CI:0.7-3.5) and 1.5(95% CI:0.2-3.0),but did not reach statistical ***:The current study suggests that the-160 AA genotype was associated with an increased risk of GC in Oman.

Microplastic content was analyzed in road dust, soils alongside roads and stormwater run-offs in al Ain City, Abu Dhabi. Apart from tire wear material, fibers and degradation products of meso- and macroplastics such as plastic bags and plastic bottle tops were found to be the most dominant plastic microparticles. Speed bumps and artificial turf/lawns were also evaluated as potential sources of microplastics on and alongside roads. It was found that in arid regions Aeolian transport of microplastics may be more important than transport by water.

Contemporary characteristics and outcomes of adults with familial dilated cardiomyopathy listed for heart transplantation

BACKGROUND Familial dilated cardiomyopathy(FDCM) account for 20%-30% of non-ischemic cardiomyopathies(NICM). Previous published data showed that some patients with FDCM tend to have rapidly progressive disease; however, five-year mortality was not significantly different in the familial and non-familial forms of NICM with optimal medical *** To better define the characteristics and clinical outcomes of FDCM patients listed for heart transplantation(HT).METHODS We queried the United Network for Organ Sharing Registry to identify FDCM patients listed for HT between January 2008 and September 2015 and compared them to NICM and ischemic cardiomyopathy(ICM) patients. We included all patients ≥ 18 years old and we separated patients to three groups: FDCM, NICM and ICM. Chi-square test was used to compare between categorical variables, the t-test was used to compare between continues variables, and Cox-proportional hazards model was used to perform time-dependent survival *** Of the 24809 adults listed for HT, we identified 677 patients(2.7%) with the diagnosis of FDCM. Compared to patients with NICM and ICM, FDCM patients were younger(FDCM 43.9 ± 13.5 vs NICM 50.9 ± 12.3, P al artificial heart(FDCM 1.3% vs NICM 0.6%, P = 0.039; vs ICM 0.4%, P =0.002). Additionally, patients with FDCM were less frequently delisted for clinical deterioration or death and more likely to be transplanted compared to those with NICM [hazard ratio(HR): 0.617, 95% confidence interval(CI): 0.47-0.81; HR: 1.25,95%CI: 1.14-1.37, respectively], and ICM(HR: 0.5, 95%CI: 0.38-0.66; HR: 1.18,95%CI: 1.08-1.3, respectively). The Incidence of Splenic Injury Following Blunt Abdominal Trauma (BAT), Sultan Qaboos University Hospital Experience

The aim of the study was to review traumatic splenic injury following blunt abdominal trauma (BAT), during the period from January 2009 to January 2015 at SQUH. The data for this study was retrospectively collected. It included 768 patients admitted to SQUH general surgery department following BAT. 43 patients with splenic injury were identified (34 males, 9 females). The mean age of patients with splenic injury was 36.0 years (34.4 years for males, 42.1 years for females). The most common mechanism of injury was motor vehicle collision (90.7%). Grade I, II are the most common grades of splenic injury. Non-Omani patients accounted for (51.2%) and most of them were pedestrians at the time of trauma. Ribs fracture is the most common injury associated with splenic injury. 36 (83.7%) patients were managed conservatively and 7 (16.3%) patients were splenectomized. Angioembolization was done for 11 (30.6%) patients. Despite the small population of Oman, high incidence of motor vehicle collision (MVC) increases the incidence of splenic injury among young age group.

NAT2 polymorphism in Omani gastric cancer patients-risk predisposition and clinicopathological associations

AIM: To study whether N-acetyltransferase 2 (NAT2) genotypes and phenotypes are associated with increased risk factor for gastric cancer in Omani patients and to study the clinico-pathological correlations and the prognostic signifi cance of ***: Genomic DNA was extracted from peripheral blood of 100 gastric cancer patients and 100 control subjects. NAT2 genotyping was performed using DNA sequencing. The prognostic significance of NAT2 and other clinicopathological features was assessed by univariate and multivariate ***: We observed no significant association between NAT2 genotypes and phenotypes and gastric cancer risk. The NAT2 phenotype polymorphisms and gastric cancer risk predisposition were not modified by concomitant H pylori infection and smoking. The emergence of novel severe acute respiratory syndrome coronavirus-2(SARS-CoV-2)has caused the global outbreak and major public health *** the outbreak human-to-human transmission was confirmed with or without symptoms of upper and lower respiratory tract *** to date,there has been evidence that COVID-19 is beyond that of a typical pulmonary disease and revealing pathomechanics of COVID-19-associated acute respiratory distress syndrome(CARDS),which include severe inflammation and pulmonary edema leading to impaired alveolar homeostasis,and resulting in an alteration of lung physiology,lung fibrosis,inflammation of endothelium,vascular thrombosis,as well as exaggerated immune re*** this pathophysiology,the use of quercetin as phytotherapeutic may merit in the management of COVID-19 *** this review,the authors wish to elaborate on the molecular effect of quercetin on SARS-CoV-2 by giving a detailed mechanism of quercetin against the binding of the S-protein of the virus to angiotensin-converting enzyme 2(ACE2)receptors,the main protease(M^(pro))or 3C-like protease(3CL^(pro)),papain-like protease(PL^(pro)),and RNA-dependent RNA polymerase(RdRP).Recent clinical evidence supporting the use of quercetin in COVID-19 management is also discussed in this paper.
