A retrospective study on use of palliative care for patients with alcohol related end stage liver disease in United States

BACKGROUND Palliative care(PC)has been shown to be beneficial in end stage liver disease(ESLD),yet the hospitalization data for PC utilization is *** To identify the trend of PC utilization for the special population of alcoholassociated ESLD patients,factors affecting its use and ascertain its impact on healthcare *** We analyzed around 78 million discharges from the 2007-2014 national inpatient sample and 2010-2014 national readmission database including adult patients admitted for decompensated alcohol-associated *** identified patients with PC consultation as a secondary *** ratios(OR)and means were adjusted for confounders using multivariate regression analysis *** Out of the total 1421849 hospitalizations for decompensated liver cirrhosis,62782(4.4%)hospitalizations had a PC consult,which increased from 0.8%(1258)of all alcohol-associated ESLD hospitalizations in 2007 to 6.6%in 2014(P3 d,prolonged ventilation,and administration of total parenteral nutrition(all P<0.01).Palliative encounters in alcohol-associated ESLD and acute-onchronic liver failure(ACLF)score were associated with increased odds of discharge to a rehabilitation facility,but significantly lower odds of 30-d readmissions(aOR:0.35,95%CI:0.31-0.41),lower total hospitalization charges and lower mean hospitalization days(all P<0.01).CONCLUSION Inpatient PC is sparingly used for patients with decompensated alcohol related liver disease,however it has increased over the past *** consultation is associated with lower 30-d readmission rates on multivariate analysis,and lower hospitalization cost and length of stay in patients with ACLF score≥2.

Prediction of promiscuous T-cell epitopes in the Zika virus polyprotein:An in silico approach

Objective:To predict immunogenic promiscuous T-cell epitopes from the polyprotein of the Zika virus using a range of bioinformatics *** date,no epitope data are available for the Zika virus in the IEDB ***:We retrieved nearly 54 full length polyprotein sequences of the Zika virus from the NCBI database belonging to different outbreaks.A consensus sequence was then used to predict the promiscuous T cell epitopes that bind MHC 1 and MHC II alleles using Propred1 and Propred immunoinformatic algorithms *** antigencity predicted score was also calculated for each predicted epitope using the Vaxi Jen 2.0 ***:By using Pro Pred1,23 antigenic epitopes for HLA class I and 48 antigenic epitopes for HLA class II were predicted from the consensus polyprotein sequence of Zika *** greatest number of MHC class I binding epitopes were projected within the NS5(21%),followed by Envelope(17%).For MHC class II,greatest number of predicted epitopes were in NS5(19%) followed by the Envelope,NS1 and NS2(17% each).A variety of epitopes with good binding affinity,promiscuity and antigenicity were predicted for both the HLA ***:The predicted conserved promiscuous T-cell epitopes examined in this study were reported for the first time and will contribute to the imminent design of Zika virus vaccine candidates,which will be able to induce a broad range of immune responses in a heterogeneous HLA ***,our results can be verified and employed in future efficacious vaccine formulations only after successful experimental studies.

Partial pre-image attack on Proof-of-Work based blockchains

Blockchain is a type of distributed ledger technology that consists of a growing list of records,called blocks,that are securely linked together using *** blockchain-based solution deploys a specific consensus algorithm that guarantees the consistency of the ledger over *** most famous,and yet claimed to be the most secure,is the Proof-of-Work(PoW)consensus *** this paper,we revisit the fundamental calculations and assumptions of this algorithm,originally presented in the Bitcoin white *** break down its claimed calculations in order to better understand the underlying assumptions of the *** also propose a novel formalization model of the PoW mining problem using the Birthday *** utilize this model to formalize and analyze partial pre-image attacks on PoW-based blockchains,with formal analysis that confirms the experimental results and the previously proposed *** build on those analyses and propose new concepts for benchmarking the security of PoW-based systems,including Critical Difficulty and Critical Difficulty per given *** calculations result in several important findings,including the profitability of launching partial pre-image attacks on PoW-based blockchains,once the mining puzzle difficulty reaches a given ***,for any compromised portion of the network(q<0.5;honest majority assumption still holds),the attack is formally proven profitable once the PoW mining puzzle difficulty reaches 56 leading zeros.

Upcycling tea waste particles into magnetic adsorbent materials for removal of Cr(Ⅵ) from aqueous solutions

In this work,locally available tea wastes were used as precursors for the development of cost-effectivebio-sorbent materials for heavy metal removal from *** and thermal treatments toprepare magnetic/non-magnetic tea waste-based adsorbents were discussed and correlated theirproperties with corresponding Cr(Ⅵ)adsorption *** bio-sorbents were thoroughly characterized by FT-IR,PXRD,N2 adsorption-desorption surface area techniques and Cr adsorption performancewas investigated in batch reactor under different conditions such as adsorbent dosage,initial Cr concentration,solution pH and contact *** bleaching steps,incorporation of magnetic ironnanoparticles and judicious temperature treatments of tea wastes resulted in high performing biosorbents with a complete removal of Cr at a dosage of 0.25 g/mL of the *** adsorption on thesetea waste-based bio-sorbents were found to be fitting to a pseudo 2nd order kinetics.

Medical image segmentation is a powerful and evolving technology in medical *** fact,it has been identified as a very effective tool to support and accompany doctors in their fight against the spread of the coronavirus(COVID-19).Various techniques have been utilized for COVID-19 image segmentation,including Multilevel Thresholding(MLT)-based meta-heuristics,which are considered crucial in addressing this ***,despite their importance,meta-heuristics have significant ***,the imbalance between exploration and exploitation,as well as premature convergence,can cause the optimization process to become stuck in local optima,resulting in unsatisfactory segmentation *** this paper,an enhanced War Strategy Chimp Optimization Algorithm(WSChOA)is proposed to address MLT *** strategies are incorporated into the traditional Chimp Optimization *** update mechanism that provides diversity in the ***,the attack and defense strategies are incorporated to improve the search space leading to avoiding local *** experimental results were conducted by comparing WSChoA with recent and well-known algorithms using various evaluation metrics such as Feature Similarity Index(FSIM),Structural Similarity Index(SSIM),Peak signal-to-Noise Ratio(PSNR),Standard deviation(STD),Freidman Test(FT),and Wilcoxon Sign Rank Test(WSRT).The results obtained by WSChoA surpassed those of other optimization techniques in terms of robustness and accuracy,indicating that it is a powerful tool for image segmentation.

The model of equations of micropolar generalized magneto-thermoelasticity is introduced within the context of the theory of two temperatures generalized thermoelasticity and we consider a problem of an isotropic homogeneous micropolar medium taking into account the heat effects and allowing the magnetic field effects. A plane wave analysis is employed to obtain the exact formulas of the two temperatures （conductive and mechanical）, displacement components, micro-rotation components, stresses, couple stresses, induced electric current, electric field and magnetic field. Arbitrary application is chosen to enable us to get the complete solution. The considered variables are presented graphically and discussions are made for the results.

Herbicide use is rising globally to enhance food production,causing harm to environment and the *** agriculture suggests variable-rate herbicide application based on weed densities to mitigate adverse effects of *** weed density estimation using advanced computer vision techniques like deep learning requires large labelled agriculture *** large agriculture data at pixel level is a time-consuming and tedious *** this paper,a methodology is developed to accelerate manual labelling of pixels using a two-step *** the first step,the background and foreground are segmented using maximum likelihood classification,and in the second step,the weed pixels are manually *** labelled data is used to train semantic segmentation models,which classify crop and background pixels as one class,and all other vegetation as the second *** paper evaluates the proposed methodology on high-resolution colour images of canola fields and makes performance comparison of deep learning meta-architectures like SegNet and UNET and encoder blocks like VGG16 and ***-50 based SegNet model has shown the best results with mean intersection over union value of 0.8288 and frequency weighted intersection over union value of 0.9869.

With the growing interest in utilizing Mg and austenitic stainless steel(ASS)in the automotive sector,joining them together in three-sheet configuration is ***,achieving this task presents considerable challenges due to the large differences in their physical,metallurgical and mechanical *** overcome these challenges,the feasibility of using weld-bonding to join Mg alloy/ASS/ASS was *** nugget formation,interface characteristics,microstructure and mechanical properties of the joints were *** results show that the connection between the Mg alloy and upper ASS was achieved through the combined effect of the cured adhesive and weld-brazing in the weld *** the other hand,a metallurgical bond was formed at the ASS/ASS *** Mg nugget microstructure exhibited fine columar grains composed predominantly of primaryα-Mg grains along with a eutectic mixture ofα-Mg andβ-*** nugget formed at the ASS/ASS interface consisted largely of columnar grains of austenite,with some equiaxed dendritic grains formed at the centerline of the *** weld-bonded joints exhibited an average peak load and energy absorption of about 8.5 kN and 17 J,respectively(the conventional RSW joints failed with minimal or no load application).The failure mode of the joints changed with increasing welding current from interfacial failure via the Mg nugget/upper ASS interface to partial interfacial failure(part of the Mg nugget was pulled out of the Mg sheet).Both failure modes were accompanied by cohesive failure in the adhesive zone.
