We described a 59-year-old male patient who underwent liver transplantation in 1989 for hepatocellular carcinoma （HCC） complicating hepatitis B virus （HBV） cirrhosis. In 2001 （12 years after liver transplantation）, he developed a lung metastasis of HCC without intrahepatic recurrence and the resection was done. In July 2003, he was symptom free without any recurrence. HCC metastasis can develop even after a very long time of liver transplantation. Many HCCs grow slowly, and the growth rate of recurrent tumors in patients receiving immunosuppressive therapy is significantly greater than that of those who do not receive immunosuppressive therapy.

The aim of this paper is to develop a neuro-fuzzy-sliding mode controller （NFSMC） with a nonlinear sliding surface for a coupled tank system. The main purpose is to eliminate the chattering phenomenon and to overcome the problem of the equivalent control computation. A first-order nonlinear sliding surface is presented, on which the developed sliding mode controller （SMC） is based. Mathematical proof for the stability and convergence of the system is presented. In order to reduce the chattering in SMC, a fixed boundary layer around the switch surface is used. Within the boundary layer, where the fuzzy logic control is applied, the chattering phenomenon, which is inherent in a sliding mode control, is avoided by smoothing the switch signal. Outside the boundary, the sliding mode control is applied to drive the system states into the boundary layer. Moreover, to compute the equivalent controller, a feed-forward neural network （NN） is used. The weights of the net are updated such that the corrective control term of the NFSMC goes to zero. Then, this NN also alleviates the chattering phenomenon because a big gain in the corrective control term produces a more serious chattering than a small gain. Experimental studies carried out on a coupled tank system indicate that the proposed approach is good for control applications.

BACKGROUND: Chronic pancreatitis (CP) is a risk factor of pancreatic adenocarcinoma (PA). The discovery of a pancreatic head lesion in CP frequently leads to a pancreaticoduo denectomy (PD) which preceded by a multidisciplinary meeting(MM). The aim of this study was to evaluate the relevance between this indication of PD and the definitive pathological ***: Between 2000 and 2010, all patients with CP who underwent PD for suspicion of PA without any histological proof were retrospectively analyzed. The operative decision has always been made at an MM. The definitive pathological finding was retrospectively confronted with the decision made at an MM, and patients were classified in two groups according to this concordance (group 1) or not (group 2). Clinical and biological parameters were analyzed, preoperative imaging were reread and confronted to pathological findings in order to identify predictive factors of malignant ***: During the study period, five of 18 (group 1) patients with CP had PD were histologically confirmed to have PA, and the other 13 (group 2) did not have PA. The median age was52.5 ±8.2 years (gender ratio 3.5). The main symptoms were pain (94.4%) and weight loss (72.2%). There was no patient’s death. Six (33.3%) patients had a major complication (ClavienDindo classification ≥3). There was no statistical difference in clinical and biological parameters between the two groups. The rereading of imaging data could not detect efficiently all patients with ***: Our results confirmed the difficulty in detecting malignant transformation in patients with CP before surgery and therefore an elevated rate of unnecessary PD was found. A uniform imaging protocol is necessary to avoid PD as a less invasive treatment could be proposed.

The study is to synthesize composite materials with news formulations of thermosetting matrices by the incorporation of 40% of mineral fillers to reduce the cost of composite materials and to improve implementation by increasing the viscosity and reducing the withdrawal to its minimum value. The mineral filler increases significantly in rheological parameters, whatever it nature of the mineral filler. Compared to the control without charges, an increase of 60% of the shear stress and that of plastic viscosity were recorded for the epoxy resin. The software Rheowine viscometer enabled us to model and identified the rheological behavior of these mixtures with resin. The results obtained in this modeling, confirmed that the resin mixtures have shear-thinning rheological behavior, this last allows the model of the Ostwald de Waele.

Long Term Survival after Resection of a Small Bowel Metastasis of Lung Adenocarcinoma: A Case Report

Background: Small bowel metastases of lung adenocarcinoma represent a rare clinical entity, often diagnosed after complications due to obstruction, bleeding or bowel perforation. Case Report: We report a case of ileal metastasis discovered during an intestinal perforation. The patient was a 47-year-old man hospitalized in emergency for acute abdominal pain, with right lower quadrant abdominal defense and a clinical history of left lower pulmonary lobectomy for lung adenocarcinoma 6 months before. Exploratory laparotomy for peritonitis revealed a single ileal tumor which was resected with lymph nodes excision. Pathology finding revealed that it was compatible with an ileal metastasis of his lung adenocarcinoma. Conclusions: The suspicion of a small bowel metastasis of lung carcinoma, although rare, must be taken in consideration and to carry out an extensive surgery, because only this will have an improvement on the survival of these patients. The purpose of this investigation is to add a new case report to the few that exist in order to provide a therapeutic approach to these exceptional cases.

FTIR Analysis and Rheological Behavior of Bisphenol： A Diglycidyl Ether Resin Filled Fume-Silica

Silica-fume as fillers into polymers materials affect significantly the physical, mechanical and the electrical properties of the polymers. The polymer that we studied here is an epoxy based resin (STR) and silica fume (FS). This paper presents the dependency of the rheological behavior of the STR epoxy resin on the FS content within a temperature at which the reaction thermoset is neglected. We used a cone-plate viscometer for the study of the rheological behavior, and a scanning electron microscope is used for the analysis of the dispersion state of the FS in STR epoxy resin. Compared to the control without loads, an increase of 50% of the shear stress and the plastic viscosity were recorded for the composite. The software Rheowine viscometer enabled us to model and identified the rheological behavior of these mixtures with resin. The results obtained in this modeling, confirmed that the resin mixtures have shear-thinning rheological behavior, this last give us the model of Ostwald de Waele.

AIM:To analyze the impact of previous cyst-enterostomy of patients underwent congenital bile duct cysts(BDC)***:A multicenter European retrospective study between 1974 and 2011 were conducted by the French Surgical *** Todani subtypes I and IVb were *** imaging studies and operative and pathology reports underwent central *** with and without a previous history of cystenterostomy(CE)were ***:Among 243 patients with Todani types I and IVb BDC,16 had undergone previous CE(6.5%).Patients with a prior history of CE experienced a greater incidence of preoperative cholangitis(75%vs 22.9%,P<0.0001),had more complicated presentations(75%vs 40.5%,P=0.007),and were more likely to have synchronous biliary cancer(31.3%vs 6.2%,P=0.004)than patients without a prior *** morbidity(75%vs 33.5%;P<0.0008),severe complications(43.8%vs 11.9%;P=0.0026)and reoperation rates(37.5%vs 8.8%;P=0.0032)were also significantly greater in patients with previous CE,and their Mayo Risk Score,during a median follow-up of 37.5 mo(range:4-372 mo)indicated significantly more patients with fair and poor results(46.1%vs 15.6%;P=0.0136).CONCLUSION:This is the large series to show that previous CE is associated with poorer short-and longterm results after Todani types I and IVb BDC resection.
