EFFECTS OF SUBSTITUTION OF MINERAL NITROGEN WITH ORGANIC AMENDMENTS ON NITROGEN LOSS FROM SLOPING CROPLAND OF PURPLE SOIL

Nitrogen loss from purple soil can lead to large negative impacts to the environment considering the wide distribution of this soil type in the upper reaches of the Yangtze River.Therefore,nitrogen loss patterns from sloping cropland of purple soil in the Sichuan Basin with the following fertilization regimes were studied in a wheat-maize rotation system:100%organic fertilizer(OM),using pig manure to replace 30%of mineral N(OMNPK)and crop residue to replace 15%of the mineral N(CRNPK)plus standard mineral fertilization(NPK)and no fertilizer control.The cumulative hydrological N loss could be as high as 45 kg·ha^(−1) N.The interflow accounted for up to 90%of the total N loss followed by sediment and overland flow losses.The high N loss via interflow found in this study highlighting that sloping cropland of purple soil may be one of the hot spots of N leaching.Compared to the NPK regime,organic substitution regimes(i.e.,OM,OMNPK and CRNPK)decreased total hydrological N loss loadings by 30%−68%.In addition,they can maintain annual crop yields and decrease yield-scaled total hydrological N losses by 18%−71%.In conclusion,long-term substitution of mineral N with organic amendments can maintain high crop productivity and reduce environmental N loss loadings,and thereby recommended as good N management practices to minimize the risk of agricultural non-point source pollution in the purple soil region of China.

The excessive exposure to high concentrations of fluoride in drinking water can lead to a serious disease called fluorosis.The upstream region of the Yongding River is an ecological protection area for Beijing.Some studies have reported that there is a high concentration of fluoride in the groundwater in this area.However,there are few data on the distribution of fluoride in surface water and health risk assessments in this area.In this study,the fluoride concentrations were determined by using the spectrophotometric method using data from 2013 to 2017 from 9 surface water quality monitoring stations in the upper reaches of the Yongding River.The health risks of fluoride were assessed using the approach developed by the United States Environmental Protection Agency(US EPA).The results indicated that the fluoride content in the drinking water ranged from 0.30 to 1.50 mg L 1,with an average of 0.86 mg L"1.In total,22.7%of the analyzed samples exceeded the Chinese limit of 1.0 mg L"1 for fluoride,and 5.5%of samples had less than the permissible limit of 0.5 mg L"1.Higher fluoride concentrations and fluorosis hotspots were found to be predominately located downstream of the Yanghe River close to the Guanting Reservoir,where 71.4%of samples exceeded the limit of 1.0 mg/L'1.The spatial distribution of high fluoride concentrations was found to be primarily determined by industry.The hazard quotient(HQ)index for children,teenagers and adults indicated that 19.6%,15.6%,and 5.1%of the samples in the upper reaches of the Yongding River,respectively,posed health hazards to the associated groups.Furthermore,the HQ index more than 1 for children,teenagers and adults had values of 64.3%,56.1%,and 19.4%,respectively,in samples from the downstream region of the Yanghe River.Therefore,there are potential risks of dental and skeletal fluorosis in the upper river reaches of the Yongding River.It is imperative to take measures to reduce the fluoride pollution in surface water and control fluorosis.Action should be taken to improve th

Introduction: Cardiovascular disease is the leading cause of death in diabetics. The objective of our study was to investigate the echocardiographic aspects of type 2 diabetics. Patients and Method: Descriptive and cross-sectional study of 12 months from June 2020 to June 2021. We included hospitalized type 2 diabetics who underwent transthoracic cardiac ultrasound in the Department of Medicine and Endocrinology at the Mali Hospital. Results: We collected 128 type 2 diabetics. The predominance was male with a sex ratio of 1.2. The mean age of patients was 60.06 ± 11.54 years with extremes of 28 and 84 years. Echocardiographic abnormalities were dominated by abnormal relaxation of left ventricle in 62.5%, increased of left ventricle mass in 54.7% and left atrium dilation in 28.1%. Patients with type 2 diabetes mellitus and hypertension had more left atrium dilation with a p of 0.02. Disorders of global kinetics and systolic dysfunction were more prevalent in smoking patients with statistically significant associations, respectively, p = 0.02;p = 0.03. Dyslipidemia had a statistically significant association with segmental kinetic disorders with a p of 0.008. Duration of diabetes greater than 5 years was associated with left atrium dilation and p-value was 0.04. Conclusion: Diabetes is responsible for cardiovascular manifestations that can be identified with transthoracic echocardiography. Its performance in diabetic patients makes it possible to refine the patient’s management.

Introduction: Fibromyomas are benign tumours of frequent uterine location, most often found in black women, whose diagnosis of certainty is histological. The extra uterine location is rare and the physio-pathogenesis is poorly understood. This location poses diagnostic problems on imaging. Management is based on tumour resection. Observation: The authors report a clinical case of a 31-year-old woman who was seen for non-febrile hypogastric pain with a large abdominopelvic mass on physical examination. There was no particular history. The biology did not show any abnormality. Ultrasound with Doppler found a mass that was difficult to characterize, and was completed by a CT scan and a pelvic MRI. The management was surgical with an evolution marked by complications. Conclusion: Ectopic leiomyoma is a rare condition. This location poses diagnostic problems on imaging. Management depends on the type of extra uterine presentation of the myoma.

Atypical Coarctation of the Aorta Revealed by Arterial Hypertension in a 22-Year-Old Young Man

Non-isthmic coarctation of the aorta is a rare congenital malformation in adults. Arterial hypertension is a frequent circumstance of discovery. We reported the case of a 22-year-old Guinean man who had been followed for 5 years for hypertension. Clinically, he presented with hypertension of the upper limbs with a systolic pressure gradient of 100 mmHg. The diagnosis was confirmed by thoracic angioscan, which showed a 65.8% coarctation of the abdominal aorta. He was on triple antihypertensive therapy combining Atenolol 100 mg, Amlodipine 10 mg and Perindopril 10 mg. He is awaiting interventional treatment. His blood pressure is stable at around 140/90 mmHg.

Vaccination against COVID-19 is the most recognised means of containing the pandemic. Vaccines are not without side effects, particularly vascular thrombosis. But before blaming the vaccines, a thorough assessment of thrombotic risk factors is necessary. We report a case of arterial and venous thrombosis after vaccination with AstraZeneca revealing an exaggeration of factor VIII in a 37-year-old female patient. The angioscanner showed a venous thrombosis of the right subclavian, a pulmonary embolism and the presence of a thrombus in the aorta. The biology was in favour of a high level of factor VIII. The patient was treated with an antivitamin K, and the clinical evolution was favourable.

Takayasu disease is a chronic vasculitis of the large vessels. It is rare in sub- Saharan Africa and probably under-diagnosed. Arterial hypertension (AH) is a frequent finding. We report the case of a 46-year-old man with Takayasu disease revealed by hypertension. The American College of Rheumatology (ACR) criteria were used for diagnosis, with 4 out of 6 criteria. Echodoppler and angioscanner showed signs of left subclavian stenosis and circumferential, regular arterial thickening. He had a biological inflammatory syndrome. He was treated with prednisone and amlodipine. After 6 months of follow-up, he was diagnosed with tuberculosis and put on anti-tuberculosis treatment.

Background: Multi-scar uterus is a uterus with two or more scars due to surgery or trauma. Objective: The aim was to compare the maternal and fetal prognosis of emergency C-sections of bi-scar uteruses to those of uteruses with at least 3 scars at the Nianankoro Fomba Hospital in Segou. Materials and Methods: It was a descriptive and analytical cross-sectional study with prospective data collection over a 24-month period from March 20, 2018, to March 20, 2020. Results: In 2 years, we collected 103 emergency C-sections for multi-scar uterus out of 1198 C-sections with a frequency of 8.6%. The age group of 20 to 35 years was the most represented with 86.4%. The bi-scar uteruses were the most frequent with 77.7%. In 71.8% of cases, the C-section was performed during the latent phase of labor. The C-section was performed under loco-regional anesthesia in 89.3% of cases. Difficulties in hemostasis and bladder injury were the most frequent intraoperative accidents. Surgical site infection was the main postoperative complication. No maternal deaths were recorded. The perinatal prognosis was marked by 4.3% fresh stillbirths in bi-scar uterus against 3.7% in three or more scar uteruses and including 2 fresh stillbirths and 1 macerated. We did not record any early neonatal deaths after C-section. Conclusion: The frequency of emergency C-sections in patients with a multi-scar uterus remains very high in our hospital. A better awareness of the patients, their spouses and all the actors involved in the management of pregnancies and childbirth, can contribute to curb this trend.

Lipid Profile among the Diabetic and Non-Diabetic Obese Patients

Objective: To study the lipid profile among the diabetic and non-diabetic obese patients. Methods: It was a descriptive and prospective study carried out over a period of 9 months from March to December 2010. It concerned all the patients who came for consultation in endocrinology at the internal medicine of Point G University Hospital Center and at the National Center for Diabetes Research. All the patients were included without any gender or age difference, voluntary and with a BMI ≥ 30 kg/m2. Results: Out of the 1543 patients received in consultation, we had identified 178 obese people that is to say 11, 53% of the cases. Among these cases 60 abided by our criteria with 30 diabetic patients and 30 non-diabetic ones. The average age was 45.83 ± 14.68 years old;73.3% were more than 40 years old. The sex ratio was 0.07. Two third (66.7%) of our patients limited their eating diet to the three main meals/day and 61.7% were sedentary. The notion of family obesity was found in 93.4% of the cases, of family diabetes in 58.3% of the cases, of past personal medical history of hypertension in 43.3% of the cases. It was about an android obesity in 91% and Gynoid in 9% of the cases. The BMI was 55% of the cases comprised between 30 - 34.9 kg/m2. The lipid profile noticed was: A hypertriglyceridemia: 33% of diabetic patients and 10% of non-diabetic patients, a high LDL cholesterol: 37% of diabetic patients and 30% of our diabetic patients, a HDL hypocholesterolemia: 40% of the diabetic patients and 20% of the non-diabetic patients, a total hypercholesterolemia: 37% of the diabetic patients and 23% of non-diabetic patients. A hyperglycemia was noticed in 13.3% of non-diabetic patients. Conclusion: The frequency of metabolic troubles was higher in obese diabetic patients compared to non-diabetic obese patients mainly the hypertriglyceridemia.
