Genotypic characterization of Helicobacter pylori isolates among Egyptian patients with upper gastrointestinal diseases

Objective： Over 50% of the world populations are infected with Helicobacter pylori （H. pylori）. Most subjects are asymptomatic; however, in 1994, H. pylori has been categorized as group I carcinogen. The aim of the study was to investigate the relationship between H. pylori infection and gastric cancer. Methods： Thirty gastric cancer patients （GCs） and 30 gastritis patients were enrolled in the study. H. pylori was cultured on non-selective and selective medias, infection density was assessed by quantitative culture. Antibiotic sensitivity testing was performed. PCR was done for the H. pylori 16S rRNA gene in addition to cagA, vacA and iceA genes. Results： H. pylori could be cultured from 100% of specimens obtained from all patients. The density of H. pylori was higher in cancer cases than in gastritis patients. The 16S rRNA was detected in all GC patients （100%） while it was only detected in 70% of gastritis patients. The cagA gene was found in 53.3% vs 13.3% of GC and gastritis patients, respectively. The vacA gene was present in all GC patients （by at least one of its alleles） while it was only found in 33.3% of gastritis patients. The vacA slml combination was the most predominant genotype in GC patients, while m2 was the commonest allele in gastritis patients （10%）. The iceA gene was found in 86.7% vs 40% of GC and gastritis patients, respectively. Simultaneous presence of multiple H. pyiori strains was proved, both phenotypically and genotypically. Conclusion： The development of GC is linked to infection with H. pylori harboring certain virulence genes. Higher infection density of H. pylori was found in GC patients. Co-existence of more than one strain of H. pylori in the same patient occurs in both malignant and benign lesions.

Surgical management of primary hyperparathyroidism guided by double-phase Tc-99m-MIBI scintigraphy

Objective:The aim of this work was to study the clinicopathological features of cases with primary hyperparathyroidism (PHPT) referred to National Cancer Institute (NCI), Cairo University in the last six years and to study the role of nuclear medicine techniques in the initial diagnosis and preoperative localization directing surgical management of these cases. Methods: This study included 27 patients with PHPT properly diagnosed and treated in the NCI, Cairo University from January 2005 to December 2010. Preoperative neck U/S and Tc-99m-MIBI scintigraphy were done in all cases. If preoperative localization detected a single lesion, unilateral exploration was done. Bilateral exploration was done if multiple foci of active parathyroid glands or no lesions were detected. Results: This study included 27 patients (8 males and 19 females). The median age was 43 years (range from 19 to 68 years). All cases presented with bone disease in the form of bony pain in 23 patients, bony swellings in 15 patients and pathological fractures in 6 cases. Twenty one patients (77.8%) had single adenoma, 5 cases (18.5%) had parathyroid hyperplasia, and only one case had double adenomas. The sensitivity of neck U/S in detecting single adenoma was 61.9% (13/21), with 81% positive predictive value (PPV) while the sensitivity of Tc-99m-MIBI scintigraphy was 90.5% (19/21) with 100% PPV. Based on preoperative localization tests, unilateral exploration was done in 19 patients with solitary adenomas. All cases in this group were cured with no reported case of persistent or recurrent hypercalcemia. Conclusion: Presentation of PHPT may mimic malignant bone tumors but fortunately these patients were correctly diagnosed with bone scan which confirm the presence of metabolic bone disease rather than bone metastases. Prevalence of occult hepatitis C in egyptian patients with non alcoholic fatty liver disease

This study aim is to assess the prevalence of occult HCV infection among Egyptian patients with non alcoholic fatty liver disease (NAFLD) with elevated AST and ALT, and to correlate presence of occult HCV with severity of liver disease. Patients and Methods: After informed consent 27 patients with elevateed liver enzymes diagnosed as NAFLD were examined for demographic, clinical, laboratory data and Ultrasonography. Liver biopsy was done and tested for HCV RNA in tissue. Genotyping using RFLP analysis of PCR products in the 5’NCR was done for positive cases. Results: HCV RNA in tissue was positive in 11/27 patients (40.7%);genotype was 4a in all positive cases. AST and ALT values showed significantly lower values in occult HCV than the non HCV NAFLD group. Liver biopsy of studied patients showed no significant difference as regard inflammation and fibrosis according to METAVIR score. Conclusions: Occult HCV is highly prevalent among Egyptian NAFLD patients. It seems to induce a mild liver disease. Patients with elevated ALT and negative HCV RNA in sera might be investigated for tissue HCV RNA. Follow up is recommended for the occult HCV patients to monitor progression to overt disease.

Does Argan Oil Supplementation Affect Metabolic Parameters and Behavior in Wistar Rats?

Argan oil is renowned for its particular biochemical profile: high-fat oleic and linoleic acids, tocopherols, sterols, polyphenols. This composition gives it nutritional, therapeutic and preventive properties against dermatological, metabolic and proliferative diseases. The composition of argan oil assigns its benefits to mental health;it would be provided with possible effects on the prevention and/or cure of stress related disorder. This work aims to evaluate the impact of argan oil dietary on the behavioral response, biochemical and hematological constants and histological profiles of adrenal involved in emotional responses to stress. The variation of these parameters was evaluated in Wistar rats receiving dietary 10 ml/Kg/day of argan oil, starting from weaning, for 13 weeks. Our results show that supplementation has resulted in an increase in locomotor activity, reduced sensitivity to frightening environments with sex dependent variation. Moreover, lipid markers, corticosterone and lymphocytes show a rising trend. If the important role of argan oil diet in cardio-metabolic health is generally well recognized;for mental health, it is the first study that needs further investigation linking between the nervous system, inflammation parameters and metabolism.

Ex situ conservation in seed banks is a potential complementary conservation strategy for native plant *** is well established that ex situ seed banking of native wild plants prolongs seed viability and thereby preserves genetic and species diversity for future *** evaluated ex situ storage potential of eight halophytic species from deserts in the United Arab Emirates(UAE)by studying seed ***,we examined the germinability of freshly collected seeds and seeds stored for three years in a seed *** also examined the effect of light conditions on fresh and stored seed *** seeds of seven of the eight species tested had a higher germination rates under 12/12 h light/dark fluctuations than did those exposed to total *** reduced light sensitivity in Halocnemum strobilaceum,Suaeda aegyptiaca,Salsola drummondii and Salsola imbricata,but increased the requirement for light in Arthrocnemum *** Anabasis setifera,storage decreased germination percentage when there was a 12-hour light/dark fluctuation,but increased germination rate when exposed to the dark *** significantly reduced germination in both the light/dark and dark treatments in Suaeda vermiculata and *** speed also responded differently to storage;whereas Timson's index significantly increased in *** and ***,it significantly decreased for ***,*** and *** of these species at a range of temperatures requires further testing;additionally,we strongly suggest that these laboratory findings be complemented by field studies.

Middle East respiratory syndrome coronavirus(MERS-Co V) has emerged in the Arabian Gulf region, with its epicentre in Saudi Arabia, the host of the ‘Hajj' which is the world's the largest mass gathering. Transmission of MERS-Co V at such an event could lead to its rapid worldwide dissemination. Therefore, we studied the frequency of viruses causing influenza-like illnesses(ILI) among participants in a randomised controlled trial at the Hajj 2013. We recruited 1038 pilgrims from Saudi Arabia, Australia and Qatar during the first day of Hajj and followed them closely for four days. A nasal swab was collected from each pilgrim who developed ILI. Respiratory viruses were detected using multiplex RT-PCR. ILI occurred in 112/1038(11%) pilgrims. Their mean age was 35 years, 49(44%) were male and 35(31%) had received the influenza vaccine pre-Hajj. Forty two(38%) pilgrims had laboratory-confirmed viral infections; 28(25%) rhinovirus, 5(4%) influenza A, 2(2%) adenovirus, 2(2%) human coronavirus OC43/229 E, 2(2%) parainfluenza virus 3, 1(1%) parainfluenza virus 1, and 2(2%) dual infections. No MERS-Co V was detected in any sample. Rhinovirus was the commonest cause of ILI among Hajj pilgrims in 2013. Infection control and appropriate vaccination are necessary to prevent transmission of respiratory viruses at Hajj and other mass gatherings.

Establishing normal ranges for fetal and neonatal small and large intestinal lengths:results from a prospective postmortem study

Objective To establish reference intervals(RIs)for fetal and neonatal small and large intestinal *** Linear measurements on small and large intestines were made upon postmortem examination of 131 preterm and term infants with gestational ages between 13 and 41 *** cases were referred from the Eastern Ontario and Western Québec regions to a tertiary care *** and sex partitions were considered and RI limits were *** Data consisted of 72 male(54.96%)and 59 female(45.04%)fetuses and neonates with mean gestational age of 25.6 *** showed that small and large intestinal lengths increased linearly with gestational *** for small intestinal length(cm)of fetuses and neonates aged 13-20 weeks were(21.1,122.4);of those aged 21-28 weeks were(57.7,203.8);of those aged 29-36 weeks were(83.6,337.1);and of those aged 37-41 weeks were(132.8,406.4).RIs for large intestinal length(cm)of fetuses and neonates from the same four age groups were(5.1,21.4),(12.7,39.7),(32.4,62.4),and(29.1,82.2).Conclusions Establishing accurate RIs for premature and term infants has clinical relevance for pathologists performing postmortem analysis and for surgeons planning postoperative management of *** results of this study reaffirm that fetal small and large intestinal lengths increase linearly with gestational age irrespective of *** studies should aim to further investigate the role of possible confounders on growth of fetal intestinal length,including maternal factors such as age and substance use during pregnancy.
