Correlations between serum kidney injury molecule-1,cystatin C and immunosuppressants:A cross-sectional study of renal transplant patients in Bahrain

Renal transplant patients receive several immunosuppressive drug regimens that are potentially nephrotoxic for *** creatinine is the standard for monitoring kidney function;however,cystatin C(Cys C)and kidney injury molecule-1(KIM-1)have been found to indicate kidney injury earlier than serum creatinine and provide a better reflection of kidney ***,we assessed Cys C and KIM-1 serum levels in renal transplant patients receiving mycophenolate mofetil,tacrolimus,sirolimus,everolimus,or cyclosporine to evaluate kidney *** used both the Chronic Kidney Disease Epidemiology Collaboration(CKD-EPI)2021 equation,which is based on creatinine and combined creatinine with Cys C,and the CKD-EPI 2012 equation,which is based on Cys C alone,to estimate glomerular filtration rate(GFR).Then,we assessed the association between serum KIM-1 and GFR<90 mL per minute per 1.73 *** observed significantly higher serum Cys C levels in patients with the elevated serum creatinine,compared with those with normal serum *** estimated GFRs based on creatinine were significantly higher than those based on the other equations,while a significant positive correlation was observed among all *** KIM-1 levels were negatively correlated with the estimated GFRs by the CKD-EPI Cys C and the combined creatinine with Cys C equations.A serum KIM-1 level above 0.71 ng/mL is likely to indicate GFR<90 mL per minute per 1.73 *** observed a significant correlation between serum creatinine and Cys C in our renal transplant ***,serum KIM-1 may be used to monitor renal function when using potentially nephrotoxic drugs in renal transplants.

Postpartum depression (PPD) is a term applied to describe depressive symptoms occurring during the first year of the postpartum period and is characterized by low mood, loss of enjoyment, reduced energy, and activity, marked functional impairment, reduced self-esteem, ideas or acts of self-harm or suicide. The women’s change into motherhood is a difficult period that involves significant changes in the psychological, social and physiological aspects, and is considered to increase vulnerability for the development of mental illness. In Somalia, 1 in 20 women aged 15 - 49 die due to pregnancy- or birth-related complications every year. This makes being pregnant in Somalia a risk that has its own stress and anxiety. Objective: To evaluate postpartum depression and associated factors among mothers who gave birth in the last 12 months in Mogadishu, Somalia. Methodology: The study is analytic cross sectional study design in which postpartum depression and its associated factors are measured. We used Cochran’s formula (Z2p(1 – p)/d2). Results: One fifth of the study population was found to have postpartum depression and the factors relating to it included marital status, substance abuse, mode and place of delivery. Conclusion: This shows that neglected mental health of the mothers in Somalia has its burden on the society. Recommendation: Mental health care component should be integrated with maternal care programs available in the country to work on prevention and control of motherhood related stress and depression among mothers who are giving birth under the very difficult circumstances in the country.

This work;It was made to determine the positive and negative usage information, attitudes and behaviors of social networking networks for users. Today, it is well known how the wide social effects of social networking sites are widespread and effective. As far as it can be followed from written and visual media, it causes significant damages for attitudes and behaviors for individuals. Looking at the results of the study, it seems impossible to limit the time and necessity of these networks for people, but it is understood that people force their limits and capacity. It was determined that the general acceptance and rejection areas are mixed and the dominant distinctive effects were weakened. Unable to prevent social dissolution and deterioration was realized by these social networks of networks. However, we cannot only talk about negative effects because their positive effects should be mentioned. Data were obtained in this direction. For example, some segments encourage the users of these sites to have more freedom, to develop democracy and human rights awareness. However, the real problem is when and where to stand. This study;Social networking networks use information, attitudes and behaviors;It was carried out for the purpose of evaluating the use of nicknames and networks for the use of networks. The data of the study were obtained by the application of a questionnaire and the data obtained were analyzed with appropriate statistical analysis methods. Non Alcoholic Fatty Liver Disease: Assessment of Lipid Profile Estimation in Different Grades of Fatty Liver on Ultrasound

Background: Non Alcoholic Fatty Liver Disease (NAFLD) is common hepatic disorder which is recognized as a great health problem causing different diseases worldwide. To determine non-alcoholic fatty liver and assess the relation of fasting total lipids with different grades of fatty liver (NAFLD) subjects diagnosed through ultrasound. By identifying the risk factors of Lipid Profile and NAFLD, the health care provider can properly manage it, even awareness specific for patients and community as general being launched to diminish the morbidity and mortality by this study. Methods: This cross-sectional research carried out at Medicine Department of PMCH, Shaheed Benazirabad. This study comprises 300 subjects of NAFLD. Patients who attended the medicine department with abdominal complains after examination consultant advised ultrasound. The ultrasound performed in Radiology department, patients with findings of fatty liver selected, and history taken from all the patients with special regard to alcoholism. Fasting lipid profile done in all patients included in present study. The blood samples collected from a vein and immediately sent to the laboratory. Results: This present study enlisted total 300 patients out of them 203 (67.7%) belonged to male gender and 97 (32.3%) were females. A ratio of 2.1:1 observed in male and female subjects. There were 176 (58.7%) patients in grade I, while 82 (27.3%) patients in grade II and 42 (14%) patients in grade III. The cholesterol value was abnormal in 186 (62%), while normal in remaining 114 (38%) patients. Triglycerides were abnormal in 152 (50.7%) while in 148 (49.3%) patients were normal. HDL in 155 (51.7%) patients was abnormal while 145 (48.3%) patients had normal values. Low density lipoprotein value in 117 (39%) patient was abnormal and 183 (61%) patient normal. Spontaneous Subcutaneous Emphysema with Pneumo-Mediastinum: A Case Report

Aim: To report a rare case of Spontaneous subcutaneous emphysema and pneumo-mediastinum Introduction: Spontaneous subcutaneous emphysema and pneumo-mediastinum may be defined as the presence of free gas or air in the subcutaneous tissue or mediastinal structures without an apparent precipitating cause. It most commonly occurs in adolescent in good physical health group devoid of severe existing lung pathology. Case Report: In our case, a middle aged married housewife was referred to Department of Medicine, Peoples Medical University Hospital Nawabshah, Sindh, Pakistan, by her local GP for the worsening of her condition due to development of acute neck pain, difficulty in breathing, eating and swallowing (mainly for solid foods) with swelling of neck. She was not complaining of any respiratory symptoms. The chest and neck radiographs showed subcutaneous emphysema and pneumo-mediastinum, there was no any evidence of air leakage from esophagus. She was subsequently put on free fluids, light diet, antibiotics, analgesia and other supportive measures along with close observation. After three days of admission, her clinical symptoms were alleviated to a great extent. She was discharged well from hospital after four days. Conclusion: Spontaneous subcutaneous emphysema with pneumo-mediastinum is medical and surgical emergency. Diagnosis may be made by routine chest X-rays and CT scan of the chest. Prompt diagnosis and immediate management may affect the morbidity and mortality outcomes in this condition.

Effects of primary suture and fibrin sealant on hemostasis and liver regeneration in an experimental liver injury

AIM: To investigate the effects of fibrin sealant on hemostasis and liver regeneration and intra-abdominal adhesions in an experimental liver injury. METHODS: Thirty-six Wistar rats were randomly divided into primary suture group (n = 15), fibrin sealant group (n = 15) and control group (n = 6). A wedge resection was performed on the left lobe of the liver. In primary suture group, liver was sutured using polypropylene material, while fibrin glue was administrated on the liver surface in fibrin sealant group. RESULTS: More intra-abdominal adhesions were observed in the primary suture group compared to the fibrin sealant group on 3rd (2.50 ± 0.5 vs 0.25 ± 0.5, P = 0.015), 10th (2.75 ± 0.5 vs 0.50 ± 0.6, P = 0.06) and 20th (1.75 ± 0.5 vs 0.70 ± 0.5, P = 0.015) postoperative days. Histopathological scores were better in the fibrin sealant group in comparison with the primary suture group on 3rd (8.75 ± 0.5 vs 6.75 ± 0.5, P = 0.006), 10th (7.50 ± 1.0 vs 5.5 ± 0.6, P = 0.021) and 20th (6.40 ± 1.7 vs 3.20 ± 1.6, P = 0.025) postoperative days. CONCLUSION: Out data suggest that fibrin sealant is preferred over primary suture in appropriate cases including liver trauma since it causes less intra-abdominal adhesions while allowing shorter hemostasis time as assessed in experimental liver trauma.
