Spatial and Temporal Variation of the Hydrological Parameters in the Wouri-Nkam Section of the Cameroon Estuary, Central African Atlantic Coast

This study aimed to provide relevant knowledge about the dynamics of the hydrological parameters in the river-estuary continuum of the Wouri-Nkam river estuary for a sustainable management program. The hydrological parameters were recorded in eleven stations spanned out on the basis of population density and human activities. Water quality parameters (Temperature, salinity, dissolved oxygen, pH, Total dissolved solutes, Redox potential and conductivity) were collected in subsurface water using a multiple parameter. Surface currents and morphometric (depth and width) parameters were recorded using a drifter, sonar depth and GPS. The field measurements took placed between 18/05/2019 to 08/09/2020 and were divided into six (06) cruises. The data were later subjected to an analysis of variance (ANOVA) and Principle Component Analysis using XLSTAT 2017 (2.7 version) software. Results obtained revealed that, the water quality parameters were spatially more stable notsignficant at (df = 9, p 0.05) with a relatively low temperature (25.5°C - 27°C) during the wet period. Estimation of the Direct Cost of Management of Venous Thromboembolism in Three Reference Hospitals in the City of Yaoundé: A Retrospective Study over a Three-Year Period

Background: Venous thromboembolism (VTE) is a major public health problem due to its increasing frequency, mortality and management cost. This cost may require major financial efforts from patients, especially in developing countries like ours where less than 7% of the population has health insurance. This study aimed to estimate the direct cost of managing VTE in three reference hospitals in Yaoundé. Methods: This was a cross-sectional retrospective study over a three-year period (from January 1st 2018 to December 31 2020) carried out in the Cardiology departments of the Central and General Hospitals, and the Emergency Centre of the city of Yaoundé. All patients managed during the study period for deep vein thrombosis and pulmonary embolism confirmed by venous ultrasound coupled with Doppler and computed tomography pulmonary angiography respectively were included. For each patient, we collected sociodemographic and clinical data as well as data on the cost of consultation, hospital stay, workups and medications. These data were analysed using SPSS version 23.0. Results: A total of 92 patient’s records were analysed. The median age was 60 years [48 - 68] with a sex ratio of 0.53. The median direct cost of management of venous thromboembolism was 766,375 CFAF [536,455 - 1,029,745] or $1415 USD. Management of pulmonary embolism associated with deep vein thrombosis was more costly than isolated pulmonary embolism or deep vein thrombosis. Factors influencing the direct cost of management of venous thromboembolism were: hospital structure (p = 0.015), health insurance (p 0.001), type of pulmonary embolism (p = 0.021), and length of hospital stay (p = 0.001). Conclusion: Management of VTE is a major financial burden for our patients and this burden is influenced by the hospital structure, health insurance, type of pulmonary embolism and length of hospital stay.

Functional Capacity of Patients with Venous Thromboembolic Disease Six to Twelve Months after Treatment

Background: Venous thromboembolism (VTE) is among the leading causes of hospital-related disability-adjusted life years lost. We aimed to determine the prevalence and determinants of functional capacity impairment six to twelve months after an acute VTE event. Methods: This was a cross-sectional study conducted between January and April 2021 in two referral hospitals of Yaoundé, including consenting adult patients admitted to these hospitals six to twelve months ago for VTE. We excluded dead patients and those with any comorbidity or symptoms limiting physical activity. The functional outcome was assessed with the six-minute walk test. Functional capacity impairment was defined as walking distance lower than the expected value. Results: We included 27 cases in this study with a mean age of 53.2 ± 14.4 years. The prevalence of functional capacity impairment was 29.6% (95% CI: 14.8 - 48.1). Factors associated with poor functional outcome were obesity (OR: 59.5;95% CI: 4.6 - 767.2;p - 207.4;p = 0.017), massive PE (OR: 30;95% CI: 2.5 - 354;p = 0.004), and poor adherence to treatment (OR: 30.3;95% CI: 2.5 - 333.3;p = 0.004). Conclusion: Functional capacity impairment is common in the medium-term after VTE and factors associated with this poor outcome are obesity, the severity of the VTE, and poor adherence to treatment.

Clinical and Bacteriological Profile of Infections in Sickle Cell Children in Two Referral Hospitals in Niamey, Niger

Introduction: Infections are significant causes of mortality in sickle cell children in resource-limited countries. This study aimed to determine the clinical profile and bacterial ecology of infections in children with sickle-cell disease in two referral hospitals in Niamey. Patients and methods: A retrospective descriptive study was conducted from January 2018 to July 2020 in two referral hospitals in Niamey. All children aged one (1) to 15 years with sickle cell disease admitted for suspected infection, including at least one bacterial culture, were studied. Bacteriological analysis was performed using the appropriate culture media, using BactAlert (Reference 4700003 BTA3D60 BioMérieux). Results: Over 36-months, 350 children with a mean age of 10.9 months were admitted. The sex ratio was 1.2. The SS electrophoretic profile was the most common (93.4%). Immunization status was up to date in 66% of patients. Fever was the common reason for consultation (55.1%). Infection was confirmed in 62 patients (17.7%). The primary diagnoses were bacterial gastroenteritis (24.2%) and urinary tract infection (19.4%). Blood cultures were isolated from Salmonella typhi (13.0%) and Escherichia coli (8.7%). Klebsiella spp (7.1%) and Escherichia coli (5.0%) were detected in cytobacteriological examination of urine. Salmonella typhi (23.5%) and Escherichia coli (5.9%) were isolated on coproculture. Conclusion: Bacterial ecology appears not different from that usually observed in sickle-cell children. Salmonella and Escherichia coli were predominant.

The feasibility of a sustainable non-autoclaved cellular concrete,based on flax vegetable co-products,for the production of usable specimen in the lightweight construction field,has been investigated *** produced specimen,containing various volume ratios of flax particles with respect to preformulatd Tradical PF70 lime binder of 0,1,and 2,were lightened by creating a porous structure in the matrix through the addition of 0.3%*** powder(able to react with calcium hydroxide from the binder and result in microscopic air-bubbles).Fresh and hardened specimen properties,including hydration,fresh density,porosity,hardened density,compressive and flexural strengths,toughness energy,and dry thermal conductivity at different temperatures,were assessed for varying flax-to-binder *** have shown that the addition of Aluminum powder leads to restrain the setting time delay of binder-based ***,the hardened material displays a significant decrease in specimen density,thereby resulting in a compressive strength level compatible with that required in the cellular construction materials *** also highlighted the ability of added flax to induce a change in the specimen from brittle to ductile ***,a high degree of thermal insulation can be achieved,which makes the cellular specimen based on flax particle suitable as insulated-bearing walls material.

A New Method for Sentiment Analysis Using Contextual Auto-Encoders

Sentiment analysis, a hot research topic, presents new challenges for understanding users＇ opinions and judg-ments expressed online. They aim to classify the subjective texts by assigning them a polarity label. In this paper, weintroduce a novel machine learning framework using auto-encoders network to predict the sentiment polarity label at theword level and the sentence level. Inspired by the dimensionality reduction and the feature extraction capabilities of theauto-encoders, we propose a new model for distributed word vector representation ＂PMI-SA＂ using as input pointwise-mutual-information ＂PMI＂ word vectors. The resulted continuous word vectors are combined to represent a sentence. Anunsupervised sentence embedding method, called Contextual Recursive Auto-Encoders ＂CoRAE＂, is also developed forlearning sentence representation. Indeed, CoRAE follows the basic idea of the recursive auto-encoders to deeply composethe vectors of words constituting the sentence, but without relying on any syntactic parse tree. The CoRAE model consistsin combining recursively each word with its context words （neighbors＇ words： previous and next） by considering the wordorder. A support vector machine classifier with fine-tuning technique is also used to show that our deep compositionalrepresentation model CoRAE improves significantly the accuracy of sentiment analysis task. Experimental results demon-strate that CoRAE remarkably outperforms several competitive baseline methods on two databases, namely, Sanders twittercorpus and Facebook comments corpus. The CoRAE model achieves an efficiency of 83.28% with the Facebook dataset and97.57% with the Sanders dataset.

Assessment of Dry Port Efficiency in Africa Using Data Envelopment Analysis

This work involves the evaluation of dry port competitiveness through analysis of efficiencies for selected dry ports in Africa. Five dry ports were selected and analysis carried out over a period of four years. The dry ports considered were Mojo and Kality in Ethiopia, Mombasa in Kenya, Isaka in Tanzania and Casablanca in Casablanca, Morocco. Data Envelopment Analysis (DEA) was applied for this work. Container throughputs for the various ports under consideration were used as the output variable for the data analysis model, while the number of reach stackers, the number of tractors, the number of forklifts and the size of the dry port were used as the input variables. From the results, the Mombasa dry port was found to be the most efficient with an average score of approximately 1 over the period under consideration. Casablanca was the second efficient dry port with an average score of 0.762, while Isaka was the least efficient with an average score of 0.142. This research is significant since the African countries have embraced the dry port concept, as witnessed in the huge investments in this sector, and would serve to highlight areas that need improvement for the few existing dry port facilities, most of which are undergoing expansion as well as modernization.

Evaluation of the Functional Capacity of a Group of Patients Aged over 65 Years: A Cross-Sectional Study at the YaoundéCentral Hospital

Background: The functional capacity of elderly patients decreases with age due to a combination of age-related decline in physiologic functions and chronic diseases. A severe decrease leads to an inability to carry out activities of daily living leading to a loss of autonomy and increased dependence. The aim of this study was to evaluate the functional capacity of the elderly followed at the Yaoundé Central Hospital. Methods: We carried out a non-probabilistic consecutive sampling of elderly patients that consulted in Yaoundé Central Hospital during a five-month’ period. Data was taken concerning their chronic conditions, use of medication, and presence of depressive symptoms (evaluated using the Geriatric Depression Scale (GDS) questionnaire). The presence of any cognitive impairment was evaluated using the Mini-Mental State Examination (MMSE). Cardiac ultrasonography and electrocardiograms were done to evaluate the cardiac morphology and physiology. Their functional capacity was assessed with the WHO Global Physical Activity Questionnaire and the six-minute walk test. A self-paced step test was equally done to estimate the maximum oxygen consumption during aerobic exercise. We carried out aunivariate, and then multivariate analyses to identify factors associated with an altered functional status. Statistical analysis was performed using the SPSS software 23.0. The threshold of significance was set at 0.05. Results: 66 participants were included (35 women) with a median age of 70 (IQR: 67 - 75) years. Among them, 39.4% were found to have an altered functional capacity, about 87.8% had at least one chronic condition and 47% had two or more. The most prevalent chronic condition was hypertension (71.2%) followed by heart failure (24.2%) and osteoarthritis (12.1%). Mild depressive symptoms were present in 1.5% of our study population. Objective: To evaluate the anthelmintic property of Saba senegalensis(***) Pichon(Apocynaceae)(S. senegalensis) on Haemonchus contortus that is traditionally used in Burkina Faso for its gastrointestinal parasites ***: The lyophilized aqueous decoction of leaves of S. senegalensis at concentrations of 0.10, 1.00, 3.00, 10.00 and 15.00 mg/m L was used on eggs and adult worms of Haemonchus contortus collected from gastrointestinal tract of small ***: The LC_(50) on adult worms was 6.79 mg/m L and 3.25 mg/m L for the leaves of S. senegalensis and the levamisole(reference drug), respectively. Inhibition of hatching assay showed a concentration-dependent manner with an inhibition of 93.63% at the concentration of 15.00 mg/m L of S. ***: These results indicate that the aqueous extract of S. senegalensis possesses an anthelmintic property and may justify its use in traditional medicine for the treatment of gastrointestinal parasites.
