Differentiation of Exophytic Renal Masses with Determination of the Angular Interface with Renal Parenchyma in US and CT

Purpose: To determine whether benign exophytic renal masses can be distinguished from malignant lesions by using the angular interface sign in ultrasonography (US) and computerized tomography (CT). Materials and Methods: A total of 71 cases with exophytic renal mass (2 cm or greater) were examined on the basis of angular interface in US (n = 23), CT (n = 21) and US + CT (n = 16) between January 2008 and June 2010 were included in this study. The renal interface relationships were examined by 2 radiologists and classified as having angular or wide interface. Results: No statistically significant difference was found between the findings of two readers. There was almost perfect interobserver agreement for the interface sign. For cystic lesions, the angular interface sign was determined in all but two Bosniak category 1 cases. Also, the angular interface sign was positive in all but one Bosniak category 2 - 3. For cystic lesions with solid component and pure solid lesions, in the benign group, the angular interface sign was positive in all except three cases (vascular malformation, oncocytoma and Xanthogranulomatous pyelonephritis). In the malignant group, the angular interface sign was determined in only two RCC cases;in other primary or metastatic malignant lesions there was a wide interface sign. Conclusion: Exophytic renal masses can be differentiated as malignant or benign with 87% accuracy using only the angular interface sign in US or CT and also in opposition to dynamic-contrast examinations. This method entails a lack of additional radiation or contrast media exposure, time-saving, and costeffectivity.

Background/Aim: We aimed to investigate the effects of colchicine on clinical recovery, as well as oxidative stress markers and total antioxidant capacity (TAC) in whole blood of patients with knee osteoarthritis (OA). Materials and methods: Sixty patients with grade 2 - 3 knee OA according to ACR knee OA criteria whom examination of the knee joint synovial fluid by polarized light microscopy demonstrated CPDD crystals existence were included in the study. Fifty healthy subjects were included as a control group. Patients were divided randomly into two groups. The first group (paracetamol group) was given only paracetamol 3 gr daily p.o and the second group (colchicine and paracetamol group) was given colchicine 1,5 gr and paracetamol 3 gr daily p.o for six months. For outcome measures WOMAC and VAS were used. Superoxide dismutase (SOD), Catalase (CAT) ezyme activities and Glutathione (GSH) and Malondialdehyde (MDA) levels and TAC all were measured. Results: WOMAC scores were improved in both patient groups compared with pre-treatment evaluation (p 0.05). TAC was signifcantly increased only in colchicine/paracetamol group. Oxidant parameter MDA levels were significantly decreased in both paracetamol group and colchicine/paracetamol group. CAT, SOD enzyme activities and GSH levels did not change before and after treatment protocols in both patient groups. Conclusion: Both paracetamol 3000 mg/day and 3000 mg paracetamol plus 1,5 gr/day colchicine is effective in the treatment of patients with knee osteoarthritis. But the addition of colchicine to paracetamol produced significantly greater symptomatic benefit than paracetamol alone. Our study also showed that colchicine lowers whole blood MDA which is a lipid peroxidation compound and elevates TAC levels in patients with knee OA. This may show probable disease modifying effect of colchicine in knee OA which require further long period laboratory and radiologic investigations.

Crossed renal ectopia is the second most common fusion anomaly of the kidney after horseshoe kidney. The incidence of both fused and unfused cases is 1 in 7000 in autopsies. The unfused crossed renal ectopia is much rare with an incidence of 1/75000 in autopsies. Ninety percent ofectopic kidneys are fused to their ipsilateral mate and the ureter of the ectopic kidney inserts into its normal position in the bladder at the contralateral side. But demonstration of the anomaly may provide to search for accompanying congenital anomalies and to plan a follow-up program for possible future complications in most of the cases. However it may remain asymptomatic for years as in the patients. Here the authors are presenting a 64-year-old male patient, with a right sided unfused crossed renal ectopia anomaly without any complaints until this age.

Backround: Chronic constipation is a common, chronic and frequent problem of the general population. The aim of this study is to assess the efficacy of defecography in diagnosing the etiology of constipation and the relation between constipation and rectocele. Material-method: We have investigated 250 patients who have been admitted to our general surgery out-patient clinic with complaint of constipation using Rome III criteria and diagnostic defecography. Results: Out of 250 patients who were evaluated with defecography only 24 had normal findings. 136 patients were found to have rectocele. Conclusion: We propose that rectocele is an important etiology of constipation, and defecography should be considered early in the diagnosis of rectocele.

Coronary Artery Bypass with Myxedema

＜正＞ The present study aimed to elucidate the therapeutic effects of melatonin against E. coli - induced renal injury. Wistar albino rats, injected intrarenally with E. Coli. were administered with either saline or melatonin （10 mg/kg/day;intraperitoneally）.Twenty - four hours or one week after pyelonephritis induction, rats were decapitated. In kidney samples, malondialdehyde （MDA）,glutathione （GSH） levels, myeloperoxidase （MPO） activity and collagen content were measured and histological analyses were made. In the saline - treated pyelonephritis group, a decrease in renal GSH along with increases in MDA level. MPO activity, and collagen content were observed （p＜0.05-0.00l）. while serum TNF -α. lactate dehydrogenase. BUN and creatinine levels were elevated as compared to control. However, melatonin treatment reversed all these biochemical indices （p ＜ 0.05）. as well as renal injury observed histologically. The protective effects of melatonin may be due to its ability to inhibit neutrophil infiltration and to balance oxidant- antioxidant status, suggesting a future role for melatonin in acute pyelonephritis treatment.

Introduction Hypothyroidism increases the risk of cardiovascular complications in patients scheduled for coronary artery bypass grafting. Atrial fibrillation is one of its cardiac complications. Case Report Our 45-year-old male patient was admitted to the Emergency Medicine Clinic of our hospital with chest pain that started in the left arm and extended to the chin 2 days ago. It was noted that the patient had hypothyroidism, but did not have any medication for the disease. On physical examination, dry and pale skin, sparse coarse hair, non-pitting edema were diagnosed with acute coronary artery disease. No signs of ischemia were observed on the ECG at the time of admission, but bradycardic sinus rhythm was recorded. The patient was started to be followed up with the diagnosis of NON-ST myocardial infarction. Nitroglycerin 0.25 - 2 mcgr/h and morphine 2 mg were administered intravenously to the patient whose anginal complaints continued after admission. Anginal complaint continued was operated in 1:1 mode by attaching an intraaortic balloon pump (Maquet Sensation 7Fr 40 cc, Datascope CS300 console) via the left femoral artery. Surgery procedure: The patient was performed with median sternotomi (aortic, two-stage cannulation). While the patient was cooled to 32 degrees and given blood cardioplegia (St. Thomas II) and applied topical cold. After distal anastomoses were performed with saphenous vein graft. Total cross-clamp time was 60 min. Epinephrine was given for bradycardia sinus rhythm. No cardiovascular complications were encountered while being followed in the intensive care unit. The patient was discharged on the 6th postoperative day. Conlucion: Our case, who was taken to emergency CABG operation with myxedema, was discharged without any cardiovascular, respiratory or metabolic complications both in the perioperative and postoperative periods. AIM: To assess lung parenchymal changes in ankylosing spondylitis (AS) using high resolution computed tomography (HRCT). METHODS: We included 78 AS patients whose average age was 33.87 (18-56) years with a ratio of 53 males to 25 females who were followed up for 3.88 (1-22) years on average. neumonia and tuberculosis were excluded. In a detailed examination of lung HRCT findings, we investigated the presence of parenchymal micronodules,parenchymal bands, subpleural bands, interlobular and intralobular septal thickening, irregularity of interfaces,ground glass opacity, consolidation, mosaic pattern,bronchial wall thickening, bronchial dilatation, tracheal dilatation, pleural thickening, emphysema, thoracic cage asymmetry, honeycomb appearance, structural distortion, apical fibrosis and other additional ***: In detailed HRCT evaluations, lung parenchymal changes were found in 46 (59%) of all patients. We found parenchymal bands in 21 (27%) cases, interlobular septal thickening in 9 (12%), emphysema in 9 (12%), apical fibrosis in 8 (10%), ground-glass opacities in 7 (9%), parenchymal micronodules in 5 (6%), irregularity in interfaces in 3 (4%), bronchial dilatation in 3 (4%), mosaic pattern in 2 (3%), pleural thickening in 2 (3%), consolidation in 1 (1%), bronchial wall thick ening in 1 (1%) and a subpleural band in 1 (1%) case. Furthermore, we detected subsegmental atelectasis in 2 patients and a cavitary lesion in 1 patient. CONCLUSION: Our study had the highest number of AS cases of all previous studies in evaluating lung paren chymal changes. The rate of lung parenchymal changes was slightly lower than that reported in recent literature.

目的:分析Duane眼球后退综合征(DRS)患者的临床特点及手术疗效。方法:评估94例DRS患者临床数据。记录完整的眼科数据,包括年龄、性别、侧位、DRS类型、偏角、异常头部姿势(AHP)、眼球后退、过冲及手术入路类型。结果:患者年龄为15.4±9.18岁,其中78例男性,16例女性。单侧和双侧受累分别为87%和13%。在单侧DRS患者中,左眼占65%。94例患者中,59.5%的患者为I型,29.7%的患者为II型,8.5%的患者为III型,2.1%的患者为IV型。大多数患者(36,38%)为嗜热性,43例(96%)患者术前AHP出现10°以上。有40例(43%)患者出现过度调校,通常为II型。对45例(48%)患者进行手术治疗。共32例(78%)行水平直肌手术后获得了较好疗效,24例(53%)患者AHP改善。结论:侧直肌的Y分裂和后榫固定在有明显过校的情况下都有令人满意的结果。垂直直肌转位增强术在限制外展方面取得了最佳效果。
