Inhibitory effects of regorafenib, a multiple tyrosine kinase inhibitor, on corneal neovascularization

AIM:To evaluate the inhibitory effects of regorafenib(BAY 73-4506),a multikinase inhibitor,on corneal neovascularization(NV).METHODS:Thirty adult male Sprague-Dawley rats weighing 250-300 g,were *** NV was induced by NaOH in the left eyes of each *** the establishment of alkali burn,the animals were randomized into five groups according to topical *** 1(n=6)received 0.9%NaCl,Group 2(n=6)received dimethyl sulfoxide,Group 3(n=6)received regorafenib 1 mg/mL,Group 4(n=6)received bevacizumab 5 mg/mL and Group 5(n=6)received 0.1%dexamethasone *** the 7d,the corneal surface covered with neovascular vessels was measured on photographs as the percentage of the cornea’s total area using computer-imaging *** corneas obtained from rats were semiquantitatively evaluated for caspase-3 and vascular endothelial growth factor by ***:A statistically significant difference in the percent area of corneal NV was found among the groups(P0.005).There was a statistically significant difference among the groups in apoptotic cell density(P=0.002).The staining intensity of vascular endothelial growth factor in the epithelial and endothelial layers of cornea was significantly different among the groups(P<0.05).The staining intensity of epithelial and endothelial vascular endothelial growth factor was significantly weaker in Groups 3,4 and 5 thanin Groups 1 and ***:Topical administration of regorafenib 1mg/mL is partly effective for preventing alkali-induced corneal NV in rats.

Time of arrival and in-hospital evaluation processes among patients with acute ischemic stroke at Yozgat City Hospital in Turkey: A retrospective study

Objective: To reveal the factors leading to delay in the evaluation processes of patients with suspected acute ischemic stroke at Yozgat City Hospital in Turkey and suggest potential solutions. Methods: Patients who visited the emergency service of Yozgat City Hospital between 1 April 2017 and 1 July 2017 and those hospitalized with a diagnosis of ischemic stroke, were included in this retrospective study. The clinical information of the patients was collected via hospital files and telephone interviews. In addition, the potential association between arrival time and the clinical parameters was investigated. Results: A total of 87 patients were included. The median arrival time to emergency service was 5 (IQR=9) h. Forty-four percent of patients arrived within the first 4.5 h from symptom onset. However, intravenous thrombolytic treatment was performed in only 7% of the patients. The median time from arrival to neuroimaging performing was 20.0 (IQR=34) min. Fifty-one percent of patients were screened within the first 20 min from arrival to emergency service. There was no association between arrival time and transfer method of the patients. Conclusions: The main problem regarding acute stroke care in our region may be inefficient use of emergency ambulance. This study provides basis for measures to shorten the arrival time.

Stroke with persisting false-negative diffusion-weighted imagings: A case report

Rationale:Diffusion-weighted imaging(DWI)is a cardinal tool for detecting acute brain ischemia;however,recent studies have shown that DWI may not show the ischemic lesion during the hyperacute *** concerns:A 76-year old male patient was admitted due to an acute onset of dysarthria and right lower paresis which progressed on the second day of ***:The preliminary diagnosis was an ischemic stroke and the final diagnosis was left medullary ***:Antiaggregant and anticoagulant therapies were initiated after excluding hemorrhagic stroke by cranial computed ***,DWIs,which were recorded on the 6th hour and 19th hour after the symptom onset,were in normal ***,the patient was referred to a senior hospital for further etiological ***:The patient was discharged after treatments with aspirin and atorvastatin,and the neurological examination showed mild-moderate dysarthria and moderate right-sided paralysis(3/5).Lessons:Clinicians should pay more attention to stroke patients with negative DWI and perform DWI several times when the diagnostic is unclear.

Tardive phenomenon presenting as isolated dysarthria: A rare entity mimicking stroke

Distinguishing stroke mimics constitutes a considerable challenge for clinicians in emergency department. Here, we illustrate an extremely rare patient presenting with acute onset isolated dysarthria, who finally received diagnosis of tardive phenomenon associated with betahistine. Through the presentation of this case, we point out tardive phenomenon as an alternative differential diagnosis of stroke. Furthermore, this case adds substantial data presenting an interesting manifestation of isolated dysarthria as a tardive phenomenon, occurring due to betahistine usage which is extremely rare in literature.

目的:分析Duane眼球后退综合征(DRS)患者的临床特点及手术疗效。方法:评估94例DRS患者临床数据。记录完整的眼科数据,包括年龄、性别、侧位、DRS类型、偏角、异常头部姿势(AHP)、眼球后退、过冲及手术入路类型。结果:患者年龄为15.4±9.18岁,其中78例男性,16例女性。单侧和双侧受累分别为87%和13%。在单侧DRS患者中,左眼占65%。94例患者中,59.5%的患者为I型,29.7%的患者为II型,8.5%的患者为III型,2.1%的患者为IV型。大多数患者(36,38%)为嗜热性,43例(96%)患者术前AHP出现10°以上。有40例(43%)患者出现过度调校,通常为II型。对45例(48%)患者进行手术治疗。共32例(78%)行水平直肌手术后获得了较好疗效,24例(53%)患者AHP改善。结论:侧直肌的Y分裂和后榫固定在有明显过校的情况下都有令人满意的结果。垂直直肌转位增强术在限制外展方面取得了最佳效果。

Objective:To determine post-discharge mortality and associated factors of the first-wave multicenter Turkish Thoracic Society(TTD)-TURCOVID ***:In this retrospective cohort study,we analyzed the data of 18 of 26 centers included in the first TTD-TURCOVID study,and 1112 cases diagnosed with COVID-19 between 11 March and 31 July 2020 participated in the *** causes of death after COVID-19 discharge were ***:The mean age of the patients was(51.07±16.93)years,with 57.6%male *** the cohort group,89.1%of COVID-19 treatment locations were hospital wards,3.6%were intensive care units(ICUs),and 7.2%were community *** the longterm follow-up,the in-hospital mortality rate was 3.6%(95%CI 2.6-4.8),the post-discharge mortality rate was 2.8%(95%CI 1.9-3.9),and the total mortality was 6.3%(95%CI 5.0-7.8).After discharge,63.3%of mortality overall occurred during the first six *** rates in post-discharge follow-ups were 12.7%(95%CI 8.0-30.6)in cancer patients,10.8%(95%CI 6.3-22.9)in chronic obstructive pulmonary disease patients,11.1%(95%CI 4.4-22.7)in heart failure patients,7.8(95%CI 3.8-14.3)in atherosclerotic heart disease patients,and 2.3%(95%CI 0.8-5.6)in diabetes mellitus *** smokers/ex-smokers,the all-mortality rates were higher than in ***:This multicenter study showed that patients over 65 years of age,males,former/active smoker,ICU stay,lung,heart disease,and malignancy should be followed up for at least the first six months after discharge due to COVID-19.
