Comparison of one and three initial monthly intravitreal ranibizumab injection in patients with macular edema secondary to branch retinal vein occlusion

AIM： To compare three initial monthly intravitreal ranibizumab（IVR） injections followed by pro re nata（PRN） dosing with one initial monthly IVR injections followed by PRN dosing for macular edema（ME） secondary to branch retinal vein occlusion（BRVO）.METHODS： Forty-two eyes of 42 patients who had IVR injections for BRVO were retrospectively studied. Eighteen eyes received 1 initial IVR injection（1＋PRN group） and 24 eyes received 3 monthly IVR injections（3＋PRN）. At 1, 3, 6 and 12mo; spectral-domain optical coherence tomography（SD-OCT） was performed. Central macular thickness（CMT）, the integrity of the external limiting membrane（ELM）, the presence of subretinal fluid, cyst size, the presence of inner segment/outer segment（IS/OS） defect were ***： At baseline the mean CMT was 521.3±153.2 μm in the 3＋PRN group while it was 438.1±162.4 μm in 1＋PRN group. At the final visit, mean CMT was 278.3±87.8 μm in the 3＋PRN group and 285.2±74.2 μm in the 1＋PRN group（P=0.079）. The changes in CMT over the entire study period were also comparable in both groups（243±160 μm in the 3＋PRN group, and 152.9±175.3 μm in the 1＋PRN group; P=0.090）. At baseline, best-corrected visual acuity（BCVA） was 0.92±0.60 logarithm of the minimal angle of resolution（logMAR） in the 3＋PRN group, while it was 0.72±0.46 logMAR in the 1＋PRN group. Final BCVA was 0.42±0.55 logMAR in the 3＋PRN group and 0.38±0.50 logMAR in the 1＋PRN group（P=0.979）. Additionally, the BCVA changes from baseline to final visit were not significantly different（-0.50±0.45 logMAR in the 3＋PRN group, and-0.33±0.39 logMAR in the 1＋PRN group; P=0.255）.CONCLUSION： No significant differences in the anatomical or functional results are found between 3＋PRN and 1＋PRN regimens in the patients receiving ranibizumab for ME secondary to BRVO. Intact IS/OS and baseline BCVA are good predictor of the visual gain, while baseline CMT is a good predictor of the anatomical gain.

Objective:To investigate the effect of the use of color codes for patient triage on physicians’clinical ***:This prospective study was conducted among female patients aged 18-65 years who visited the emergency department(ED)with complaints of acute abdominal pain.A 3-level of triage system[red(very urgent),yellow(urgent)and green(less urgent)]was used in our *** patients were green *** of these patients remained at the green level(the green group),and the remaining patients were re-labeled as false yellow(the false yellow group)in the order of ED *** tests,consultation requests,intravenous treatment,length of hospital stay,and cost were compared between the two groups of ***:In total 393 patients were included with 198 patients in the green group and 195 in the false yellow *** was no statistically significant difference between the two groups in age,temperature,systolic blood pressure,diastolic blood pressure,pulse and oxygen saturation(P>0.05).It was observed that more tests(P=0.001),consultations(P<0.001),and intravenous treatment were requested(P<0.001),and the duration of stay in the ED was longer(P<0.001)and cost(P<0.001)was higher in the false yellow ***:Triage do affect the decisions of physicians on female patients with acute abdominal pain.

Tubular flow reactors are mainly used in chemical industry and waste water discharged units. Control of output variables is very difficult because of the existence of high dead-time in these types of reactors. In the present work, sodium hydroxide and acetic acid solutions were sent to the tubular flow reactor. The aim was to control p H at 7 in the nonlinear region. The p H control of a tubular flow reactor with high time delay and a highly nonlinear behavior in p H neutralization reaction was investigated experimentally in the face of the various load and set point changes. Firstly, efficiency of conventional Proportional-Integral-Derivative(PID) algorithm in the experiments was tested. Then self-tuning PID(STPID) control system was applied by using the ARMAX model. The model parameters were calculated from input–output data by using PRBS signal as disturbance and Bierman algorithm. Lastly, the experimental fuzzy control of p H based on fuzzy model was achieved to compare the success of fuzzy approach with the performance of other control cases studied.

Rationale: Necrotizing fasciitis is an aggressive infection of subcutaneous tissues, which tends to spread rapidly through the fascial planes. Colonic diverticulosis is a common disease in advanced age, although it rarely causes a lethal necrotizing soft-tissue infection. Patient concerns: A 58-year-old woman complained of left leg pain for 15 d without abdominal pain. Diagnosis: Diverticulitis perforation presented as necrotizing fasciitis of the left thigh. Interventions: Extensive debridement. Outcomes: The patient died due to sepsis-induced multiple organ failure and severe metabolic acidosis. Lessons: Clinicians should be aware of presentations of diverticulitis in patients who have a soft-tissue infection in lower extremity even though patients may not have had a history of diverticulosis or abdominal pain.

In this study, several N-(p-substituted-arylsulfonyl)phthalimides (1a-e) were synthesized. The synthesized compounds were then examined with respect to their substitution reactions with t-butylamine, diethylamine, cyclohexylamine, and trans-1,2-diaminocyclohexane in acetonitrile. In order to determine the mechanism, substituent effect, activation entropy, and nucleophile effect were used as criteria.
