Mathematical analysis of corneal remodelling after intracorneal ring surgery in keratoconus

AIM：To represent mathematically the intersection between the ectatic corneal geometry and the plane of intracorneal ring implants（ICRS）in order to determine the corneal response to ICRS surgery in keratoconus（KC）.Thereafter,to present the concept and early results of a newly derived topography-guided nomogram for ICRS surgery for the treatment of keratectasia. METHODS：The corneal rings plane intersection was modelled to a conic section. Ring effect was the result of：the ring size,position（steep vs flat）,location（distance from the geometric centre of the cornea）,and the discrepancy between the ring＇s curvature and the tunnel＇s curvature.Femtosecond laser was used to create the tunnels and the incision sites were chosen according to the nomogram in order to place the thickest ring in the steepest portion of the cornea regardless of the astigmatism axis of refraction.RESULTS：The conic section had a more prolate shape in the steep area of the cornea than in the flat area,depending on the corneal sagittal curvature. Equal ring size had more flattening effect in the steep area than in the flat area. Thick segment should be implanted under the steep portion of the cornea regardless of the cylinder axis of refraction. Single segment in the steep area was sufficient in early and moderate cases of KC. The new nomogram provided more topographic regularity with significant reduction of astigmatism and better improvement in uncorrected visual acuity（UCVA）and best-corrected visual acuity（BCVA）than the conventional nomogram. CONCLUSION：The newly derived nomogram can produce better results than the conventional nomogram. Moreover,based on this concept,a new nomogram can be integrated into the femtosecond laser software to create topography-guided,customized,elliptical tunnels with modified focal asphericity that allows for customized focal flattening of the irregularly steepened ectatic cornea.

Management of cataract in keratoconus: early visual outcomes of different treatment modalities

A review of 31 eyes with keratoconus who developed cataract and underwent phacoemulsification. Visual acuities were measured 1 mo postoperatively. Six eyes with a history of good corrected distance visual acuity(CDVA) and a similar refractive and topographic astigmatic axis were implanted with toric intraocular lenses(IOLs). The mean postoperative uncorrected distance visual acuity(UDVA) was 0.2 logMAR with a spherical equivalent(SE): 0.75 D. eleven eyes with a history of good CDVA and different refractive and topographic axis were implanted with monofocal IOL+/-Toric implantable collamer lenses to treat anisometropia and ametropia;mean UDVA was 0.25 logMAR with a mean SE:-0.51 D postoperatively. Six eyes with poor CDVA were first treated with intra-corneal ring segments, followed by phacoemulsification, the mean postoperative UDVA was 0.82 logMAR with an SE: 0.22 D. Eight eyes had advanced ectesia and received combined phacoemulsification and penetrating keratoplasty. Our approach is efficient in addressing ametropia after cataract surgery in keratoconic eyes.

In this study,our aim is to address the problem of gene selection by proposing a hybrid bio-inspired evolutionary algorithm that combines Grey Wolf Optimization(GWO)with Harris Hawks Optimization(HHO)for feature selection.Themotivation for utilizingGWOandHHOstems fromtheir bio-inspired nature and their demonstrated success in optimization problems.We aimto leverage the strengths of these algorithms to enhance the effectiveness of feature selection in microarray-based cancer classification.We selected leave-one-out cross-validation(LOOCV)to evaluate the performance of both two widely used classifiers,k-nearest neighbors(KNN)and support vector machine(SVM),on high-dimensional cancer microarray data.The proposed method is extensively tested on six publicly available cancer microarray datasets,and a comprehensive comparison with recently published methods is conducted.Our hybrid algorithm demonstrates its effectiveness in improving classification performance,Surpassing alternative approaches in terms of precision.The outcomes confirm the capability of our method to substantially improve both the precision and efficiency of cancer classification,thereby advancing the development ofmore efficient treatment strategies.The proposed hybridmethod offers a promising solution to the gene selection problem in microarray-based cancer classification.It improves the accuracy and efficiency of cancer diagnosis and treatment,and its superior performance compared to other methods highlights its potential applicability in realworld cancer classification tasks.By harnessing the complementary search mechanisms of GWO and HHO,we leverage their bio-inspired behavior to identify informative genes relevant to cancer diagnosis and treatment.

Blockchain technology has garnered significant attention from global organizations and researchers due to its potential as a solution for centralized system challenges.Concurrently,the Internet of Things(IoT)has revolutionized the Fourth Industrial Revolution by enabling interconnected devices to offer innovative services,ultimately enhancing human lives.This paper presents a new approach utilizing lightweight blockchain technology,effectively reducing the computational burden typically associated with conventional blockchain systems.By integrating this lightweight blockchain with IoT systems,substantial reductions in implementation time and computational complexity can be achieved.Moreover,the paper proposes the utilization of the Okamoto Uchiyama encryption algorithm,renowned for its homomorphic characteristics,to reinforce the privacy and security of IoT-generated data.The integration of homomorphic encryption and blockchain technology establishes a secure and decentralized platformfor storing and analyzing sensitive data of the supply chain data.This platformfacilitates the development of some business models and empowers decentralized applications to perform computations on encrypted data while maintaining data privacy.The results validate the robust security of the proposed system,comparable to standard blockchain implementations,leveraging the distinctive homomorphic attributes of the Okamoto Uchiyama algorithm and the lightweight blockchain paradigm.

TheÇorakyerler fossil locality in Anatolia reveals unique faunal elements.This study introduces Hystrix kayae sp.nov.,a new Late Miocene porcupine initially classified as Hystrix sp.Yavuz et al.,2018.This finding expands our knowledge of Late Miocene Anatolian porcupines,bringing the total number of known species to three.H.kayae sp.***.is larger than Hystrix aryanensis and exhibits greater upper cheek tooth crown height.Its cheek tooth morphology differs from Hystrix depereti,and it possesses distinctive U-shaped choanae unlike Hystrix primigenia.TheÇorakyerler locality may predate sites with H.aryanensis and H.primigenia but aligns temporally with Hystrix parvae localities.This study enhances our understanding of Late Miocene porcupine diversity in Anatolia,emphasizing the importance ofÇorakyerler in unraveling the evolutionary history of these fascinating mammals.

E-ticketing,which has been promoted by the Federal Highway Administration(FHWA)“every day counts”(EDC)initiative,utilizes software applications to digitally track and store information regarding highway construction materials paid by state transportation agencies(STAs)by weight in unit bid contract structures.STAs often face implementation barriers such as institutional inertia,or the resistance by stakeholders to adopt changes from the status quo,including new technologies.The purpose of this paper is to determine the progression of STA e-ticketing policy adoption,specifically with a focus on asphalt paving operations,due to the COVID-19 pandemic.To accomplish this research effort,previous FHWA data,National Cooperative Highway Research Program(NCHRP)data,and other literatures are reviewed to determine an implementation baseline.Additional data is collected from the American Association of State Highway and Transportation Officials Committee on Con-struction to gain current feedback from STAs and their highway contractor partners after the COVID-19 pandemic.Additionally,a case study featuring the Kentucky Transportation Cabinet(KYTC),the Kentucky Association of Highway Contractors(KAHC),and the Plantmix Asphalt Industry of Kentucky(PAIKY)is performed to provide more in-depth analysis.The major finding includes a statistically significant result indicating increased imple-mentation of e-ticketing for asphalt operations within the last two years,along with noting benefits including employee safety,task loading,and project documentation along with concerns regarding cellular connectivity and procurement responsibilities.These findings indicate the importance of STAs investing in partnership with con-tractors to improve stakeholder buy-in before proceeding towards e-ticketing adoption.

Mesenchymalstemcells(MSCs)areidealcandidatesfortreatingmanycardiovasculardiseases.MSCscanmodify the internal cardiac microenvironment to facilitate their immunomodulatory and differentiation abilities,which are essential to restore heart function.MSCs can be easily isolated from different sources,including bone marrow,adipose tissues,umbilical cord,and dental pulp.MSCs from various sources differ in their regenerative and therapeutic abilities for cardiovascular disorders.In this review,we will summarize the therapeutic potential of each MSC source for heart diseases and highlight the possible molecular mechanisms of each source to restore cardiac function.

Actin, a highly conserved protein, plays a dominant role in Non-small cell lung cancer (NSCLC). Late diagnosis and the aggressive nature of NSCLC pose a significant threat. Studying the clinic pathological properties of NSCLC proteins is a potential alternative for developing treatment strategies. Towards this, 35 downregulated actin cytoskeletal proteins on NSCLC prognosis and treatment were studied by examining their protein-protein interactions, gene ontology enrichment terms, and signaling pathways. Using PubMed, various proteins in NSCLC were identified. The protein-protein interactions and functional associations of these proteins were examined using the STRING database. The focal adhesion signaling pathway was selected from all available KEGG and Wiki pathways because of its role in regulating gene expression, facilitating cell movement and reproduction, and significantly impacting NSCLC. The protein-protein interaction network of the 35 downregulated actin cytoskeleton proteins revealed that ACTG1, ACTR2, ACTR3, ANXA2, ARPC4, FLNA, TLN1, CALD1, MYL6, MYH9, MYH10, TPM1, TPM3, TPM4, PFN1, IQGAP1, MSN, and ZXY exhibited the highest number of interactions. Whereas HSPB1, CTNNA1, KRT17, KRT7, FLNB, SEPT2, and TUBA1B displayed medium interactions, while UTRN, TUBA1B, and DUSP23 had relatively fewer interactions. It was discovered that focal adhesions are critical in connecting membrane receptors with the actin cytoskeleton. In addition, protein kinases, phosphatases, and adapter proteins were identified as key signaling molecules in this process, greatly influencing cell shape, motility, and gene expression. Our analysis shows that the focal adhesion pathway plays a crucial role in NSCLC and is essential for developing effective treatment strategies and improving patient outcomes.

Assessment of Radiological Hazards of Sedimentary, Igneous and Sediments Natural Rocks

Gamma-ray spectroscopy based on a 100% efficiency hyper-pure germanium detector was used to evaluate the activity concentrations of 226Ra, 232Th, and 40K natural radionuclides in sedimentary, conglomerate, igneous and sedi-ments rock samples collected from four different locations in Eastern desert in Egypt. The obtained activity concentrations are used to evaluate the radi-ological hazards indices, absorbed dose rate, annual effective dose equivalent in air, radium equivalent, external and internal hazard index, radiation level index, annual gonadal dose equivalent, excess lifetime cancer risk and expo-sure rate. The results show that 1) the absorbed dose rate depends on the rock type, 2) the annual effective dose equivalent in air in 71% of sample below 20 mSvy-1 (permissible limit for workers), 3) the conglomerate rocks show low radioactivity level, 4) sedimentary rocks are rich in radium while igneous rocks are rich in thorium and the sediments rocks are rich in both radium and thorium.
