The development of Grave’s ophthalmopathy following radioiodine treatment for Grave’s disease is well recognized. However, development of Grave’s ophthalmopathy following radioiodine therapy given for toxic adenoma or toxic nodular goiter has been reported but very rarely. Hereby, we reported a case of a 69-year-old male patient who developed ophthalmopathy after receiving 30 mCi (1110 mBq) 131 I radioiodine treatment for a toxic nodule. In addition, a review of the current literature with regards to this topic is presented.

Management of Thymic Neoplasms: Egyptian NCI Experience

Background: Thymic tumors are rare entity with little information regarding outcomes after therapy with curative intent. They are heterogeneous group of neoplastic lesions with wide spectrum of morphologic appearances. They show different presentations with a variable & unpredictable evolution ranging from indolent attitude to highly infiltrative and metastasizing one. The optimal treatment includes surgical resection, chemotherapy, and radiotherapy. Objectives: To analyze, to report & to better understand the clinico-pathologic features, results of treatment, and prognostic factors of these tumors. Materials and Methods: From 2008 to 2014, 13 patients (8 men, 5 women) underwent surgical resection of thymic tumor at a mean age of 47 years. Patient demographics, extent of surgical resection, and outcomes were compiled. Demographic variables, use of chemotherapy or radiotherapy, peri-operative variables, recurrence rates, and long term survival were analyzed retrospectively. The Masaoka stage and tumor diameter were recorded along with other variables that potentially influenced survival such tumor grade, site & number of metastatic disease. Results: The mean age of our patients was 47 years (range: 21 to 58 years) & 8 cases (61.5%) were men. The distribution of patients according to the Masaoka staging at presentation was, stage I (6 cases, 47%), stage II (3 cases, 23%), stage III in (1 patient, 7%), and stage IV (3 cases, 23%). Neoadjuvant chemotherapy was administered to 3 patients (23%) whose tumors were deemed to be more locally advanced precluding R0 resection from the start & 2 patients (15%) received neoadjuvant concomitant chemo-radiotherapy. No patient demonstrated an associated immunologic disorder such as myasthenia gravis. In all patients (13 cases, 100%) pathologic confirmation of thymic tumor was by CT guided fine needle aspiration/biopsy. Integrating Radar Altimeters and Optical Imagery Data for Estimating Water Volume Variations in Lakes and Reservoirs (Case Study: Lake Nasser)

Monitoring of variations in water for lakes and reservoirs is a requirement for meeting human needs and assessing ongoing climatic changes. However, regular gauging networks fail to provide the information needed for water volume data. The aim of this study is to evaluate an approach to estimate water volume variation for the southern part of Lake Nasser in Egypt without in-situ gauge measurements and bathymetry maps. Combination of both Hydroweb satellite altimetry and Landsat 8 satellite imagery data was used. As compared to in-situ water levels, satellite altimetry provided accurate water levels variations for Lake Nasser;the RMSE was 0.28 m, with excellent agreement (R2 is 0.98). The lowest water level of altimetry database i.e. 174.57 m was used as a reference level for estimating water volumes variations for the study duration 8/2014-6/2015. All water altimetry levels were converted to differences of recorded water level above the lowest altimetry Level (ΔWL). Series of Landsat 8 imagery data were selected to extract surface areas corresponding to radar altimetry water levels dates. Areas-ΔWL relationship model was established as a polynomial function: A = f(ΔWL), and therefore, the relationship of the water volume above the lowest water level for the study time (ΔV) and ΔWL was obtained through the analytical integration of (Area-ΔWL) model. Another approach (Heron method) was also applied for estimating water volume variations. Validation of these two approaches showed that estimated water volume variations above reference water level using both methods i.e. integration and Heron agreed well with in-situ measurements of volume variation deduced from recent bathymetry map and in-situ water levels (R2 for both methods = 0.98). The RMSE for integration method is 323.89 MCM and for Heron method was 318.09 MCM, being approximately 13.2% of the mean volume variations above the lowest reference water level for mean surface area ≈

Novel CYP1B1 mutations and a possible prognostic use for surgical management of congenital glaucoma

AIM: To identify CYP1B1 gene mutations and evaluate their possible role as a prognostic factor for success rates in the surgical management of Egyptian congenital glaucoma ***: Totally 42 eyes of 29 primary congenital glaucoma patients were operated on with combined trabeculotomy/trabeculectomy with mitomycin-C and followed up at 1 d, 1 wk, 1, 6 and 12 mo postoperatively. Genomic DNA was extracted from peripheral blood leukocytes. Coding regions of CYP1B1 gene were amplified using 13 pairs of primers, screened for mutations using single-strand conformation polymorphism followed by sequencing of both strands. Efficacy of the operation was graded as either a success [maintaining intraocular pressure(IOP) less than 21 mm Hg with or without antiglaucoma medication], or a failure(IOP more than 21 mm Hg with topical antiglaucoma medications). RESULTS: Seven novel mutations out of a total of 15 different mutations were found in the CYP1B1 genes of 14 patients(48.2%). The presence of CYP1B1 gene mutations did not correlate with the failure of the surgery(P=0.156, odds ratio=3.611, 95%CI, 0.56 to 22.89); while the positive consanguinity strongly correlated with failure of the initial procedure(P=0.016, odds ratio=11.25, 95%CI, 1.57 to 80.30). However, the Kaplan-Meier survival analysis revealed a significantly lower time of IOP control in the subgroup with mutations in CYP1B1 versus the congenital primary glaucoma group without mutations(log rank test, P=0.015).CONCLUSION: Seven new CYP1B1 mutations are identified in Egyptian patients. Patients harboring confirmed mutations suffered from early failure of the initial surgery. CYP1B1 mutations could be considered as a prognostic factor for surgery in primary congenital glaucoma.

Nanostructured Microemulsion Phase Behavior Using AOT or Extended Surfactant Combined with a Cationic Hydrotrope

The phase behavior for systems containing sodium bis (2-ethylhexyl sulfosuccinate) (AOT), with the cationic hydro-trope tetraethyl ammonium chloride (TEAC), in the presence of water and heptane were studied. Formulations of mi-croemulsion for such systems and for systems containing the so called “extended surfactant” were carried out at different salt concentrations. Anisotropy was detected using cross polarizers and polarized microscopy. Ultralow interfacial tension for microemulsion was measured, first theoretically using the Chun-Huh equation, and then experimentally using spinning drop experiments. The presence of short chain hydrotropes (TEAC) was found to destabilize the liquid crystalline region, observed in the AOT/water/heptane phase diagram. Ultralow interfacial tensions were observed for microemulsions formulated with surfactant concentrations as low as 0.5%, for both AOT and “extended surfactant” systems. The effect of temperature on the phase behavior of a microemulsion formed with AOT system was studied and found to behave in an opposite manner compared to the nonionic surfactant.

The Association between Mycoplasma Species Infection and Tubal Obstruction

Objectives: Numerous factors can cause infertility. Tubal factor accounts for approximately 30% of infertility in females. This study was conducted to evaluate the rate of asymptomatic Mycoplasma hominis and Ureaplasma urealyticum infection in females diagnosed with tubal obstruction. Methods: This is a retrospective case-control study conducted at New Life Fertility Centre. We have identified and reviewed the health records of 167 subfertile women that had HSG and an endocervical swab for Mycoplasma hominis and Ureaplasma urealyticum done. Results: We compared the endocervical swab results of ureaplasma and mycoplasma in the patients with tubal obstruction (group 1) to the patients with normal patent tubes (group 2). Diagnosis of tubal patency was based on the HSG results. Our results show that there is a significantly higher rate of Ureaplasma urealyticum/Mycoplasma hominis infection in group 1 compared to group 2. Conclusion: Our data shows that there is a high rate of U. urealyticum and M. hominis infection in patients diagnosed with tubal factor of infertility and it can be a marker in the prediction of the tubal pathology.

The Effectiveness of Video Distraction on Children Preoperative Anxiety: An Integrative Literature Review

Background: Children who undergo a day case surgery experience high levels of preoperative anxiety. Preoperative anxiety interferes with anesthesia induction compliance and is associated with many short and long term postoperative complications. Recently, video distraction intervention has been evaluated for its anxiolytic effects in preoperative children. Aim: The aim of this integrative review was to demonstrate the varying methodological approaches utilized to evaluate the effectiveness of a video distraction intervention in reducing preoperative anxiety in children undergoing day case surgery. Methods: A total of 8 articles, meeting the inclusion criteria, were evaluated and included. Findings: The eight studies investigating the effect of video distraction on children preoperative anxiety concluded that video distraction was significant in controlling children preoperative anxiety. Four of the eight studies (50%), investigated video distraction effectiveness against pharmacological comparisons and demonstrated superior or equal anxiolytic effect of video distraction on different points along the surgical continuum. Three of the eight studies compared video distraction against parental presences and video distraction showed superior anxiolytic effect. Three of the eight studies evaluated the effect of video distraction on anesthesia induction compliance and emergence delirium. A significant effect on anesthesia induction was demonstrated while non-significant effect on emergence delirium was documented. Conclusion: Video distraction is a safe, time and cost effective non pharmacological anxiolytic intervention. It can be provided by nurses to control children high level of anxiety before surgery and during anesthesia induction.
