Bilateral Spontaneous Achilles Tendon Rupture without Any Predisposing Factors

Besides spontaneous rupture of achilles tendon is a rare condition which is usually associated with corticosteroid or fluoroquinolon usage, spontaneous rupture of Achilles tendon without any risk factors is extremely rare. In this report we aimed to present clinical features of a case of bilateral spontaneous achilles tendon rupture and tried to investigate potential etiologic factors. A 54 year old man was admitted to our emergency department as complaining bilateral ankle pain at the retrocalcaneal region. He had no trauma, any comorbidities or any drug consumption history. Painful achilles tendon gaps proximal to his ankles had been palpated and bilateral Thompson’s test positivity had been noted (Figures 1 and 2). He was unable to rise upon his toes however he was able to walk. American Orthopedic Foot and Ankle Society Hindfoot Scoring was calculated as 78 points. The rupture had been verified with a magnetic resonance imaging and his laboratory findings were all within normal limits (Figures 3 and 4).The patient did not approve an operation so conservative treatment with dorsiflexion limiting brace was applied and strengthening and stretching exercises of gastrosoleus was suggested. Atraumatic and bilateral cases with achilles rupture should be well investigated whether any systemic factors are coexistent. Besides well known corticosteroids or fluoroquinolone exposure, endocrinologic and rheumatological diseases, hypercholesterolaemia should be evaluated in terms of predisposing causes. Patients who have to use corticosteroids or fluoroquinolone, should be informed about achilles tendon rupture and be suggested do regular physical exercises in order to reduce the rupture risk. Despite predisposing factors, sometimes nothing can be associated with achilles tendon rupture as in our case. Genetic predisposition or a degenerative process of a tendon due to age may be underlying cause like in rotator cuff tears.

Let G be a locally compact abelian group. The main purpose of this article is to find the space of multipliers from the Lorentz space. L（p1, q1）（G） to L（p＇2, q＇2）（G）. For this reason, the authors define the space A p1,q1^ p2,p2（G）, discuss its properties and prove that the space of multipliers from L（p1, q1）（G） to L（p＇2, q＇2）（G） is isometrically isomorphic to the dual of A p1,q1^p2,q2 （G）.

Power System Analysis of an Aero-Engine

The aim of this study is analyzed in detail for better understanding of energy and power of an aero-engine. In this regard, this study presents energy equations were applied to the turbofan engine components. The engine has a thrust range of 82 to 109 kN. It consists of fan, axial low pressure compressor (LPC), axial high pressure compressor (HPC), an annular combustion chamber, high-pressure turbine (HPT) and low pressure turbine (LPT). The results show that power of the engine flow approaches a maximum value to be 82.85 MW in the combustor outlet, while minimum power is observed at LPC inlet with the value of 1.37 MW. Furthermore, important parameters of the engine are also analyzed from reverse-engineering method. It is expected that results of this study will be beneficial of power, cogeneration and aero-propulsive generation systems in similar environment.

Benign Giant Cell Tumor is a relatively common benign lesion which usually appears as an enlarging painless mass and has a synovial origin. Although benign giant cell tumors generally involve tendon sheaths, they are infrequently documented in the foot. A 45 years old female presented with a complaint of a lump on the top of her left foot. Under general anesthesia with a pneumatic tourtniquet the mass excised with great care not to leave any residual tumor tissue that can cause recurrence. Benign giant cell tumor of the foot can be associated with talonavicular joint capsule which can be detected with MRI imaging and total excision of the lesion is mandatory to prevent recurrence.

AIM: To investigate the antif ibrotic effects of peginterferon- alpha 2b and taurine on oxidative stress markers and hepatocellular apoptosis. METHODS: Sixty rats with CCl4-induced liver fibrosis were divided into 4 groups (n = 15). Group 1 was left for spontaneous recovery (SR). Groups 2-4 received peginterferon-alpha 2b, taurine, and their combination, respectively, for four weeks. Histological f ibrosis scores, histomorphometric analysis, tissue hydroxyproline, tissue MDA, GPx and SOD activities were determined. Activated stellate cells and hepatocellular apoptosis were also evaluated. RESULTS: The degree of f ibrosis decreased in all treatment groups compared to spontaneous recovery group. Taurine alone and in combination with peginterferon-alpha 2b reduced oxidative stress markers, but peginterferon-alpha 2b alone did not. Apoptotic hepatocytes and activated stellate cells were higher in groups 2-4 than in group 1. Combined taurine and peginterferon-alpha 2b further reduced fibrosis and increased activated stellate cell apoptosis, but could not improve oxidative stress more than taurine ***: Peginterferon-alpha 2b exerts anti- f ibrotic effects on rat liver fibrosis. It seems ineffective against oxidative stress in vivo. Peginterferon-alpha 2b in combination with taurine seems to be an antif ibrotic strategy.

An Assessment of the Seismic Performance of the Historic Tigris Bridge

Turkey is a country that is vulnerable to earthquakes and has experienced many major earthquakes that completely destroyed or caused significant damage to numerous historic structures. Today, using computer software, it is important to numerically model and analyze historic structures that need significant restoration and strengthening, to evaluate them from a perspective of seismic resistance, and to reinforce them without altering their originality. In this study, a finite element model of the historic Tigris Bridge on the Tigris River was created. First, the stresses and deformation caused by its own weight were determined. Subsequently, dynamic analyses were performed in the time domain using past earthquake ground motion records. Displacement and stress values obtained for each earthquake record in these time domain analysis were compared to each other to evaluate the seismic behavior of the bridge comparatively. The seismic performance of the bridge was determined on the basis of the “Guidelines on the Management of Earthquake Risks for Historic Structures” published by the Directorate General of Religious Foundations in Turkey.

Intramedullary Nailing of Femoral Shaft Fractures with Compressive Nailing Using Only Distal Dynamic Hole and Proximal Static Hole

Objectives: In this study we aimed to present our treatment results of intramedullary nailing of femoral shaft fractures with compressive nailing using proximal static hole and only distal dynamic hole with one screw. Methods: Forty-three patients who had a fracture of the femoral shaft were managed between 2005 and 2008 with intramedullary nailing and the use of only one screw for distal interlocking. Prospectively we evaluated the union time, possible reoperation, fixation and fracture alignment, range of knee motion and complications. Results: Union occurred within a mean duration of 18.7 weeks. No failures of the fixation and fracture alignment and no more than 1 cm shortness were detected. The knee range of motion was all more than 90 degree. Only one deep venous thrombosis was detected as complication. Conclusions: Compressive nailing using proximal hole and only distal dynamic hole with one screw is a convenient technique for femur fractures.

Introduction: Although Brucellosis has a limited geographic distribution;it remains a challenge in certain parts of the world such as in Mediterranean, western Asian, Latin American and African regions. We present a unique case of periacetabular Brucella osteomyelitis and increase awareness of possible widespread distrubition of Brucella osteomyelitis and its ability to affect any region of the musculoskeletal system. Case Presentation: A 44-year-old male farmer presented with symptoms of pain radiating from his left hip to his thigh of five years duration. There was a history of night sweats and fever for the past two months. A lytic area with smooth borders in left periacetabular region was detected on pelvic roentgenography of the patient. Magnetic resonance imaging revealed a cavitatory lesion in relation to hip joint. Open biopsy was undertaken with the differential diagnosis of an infectious (Brucella or Tuberculous) or tumoral lesion. Intraoperative frozen sections showed granulomatous inflammatory tissue. Post debridement, the cavity was filled with autograft taken from the patient’s right iliac wing. Postoperative immunohistochemistry confirmed diagnosis of Brucella osteomyelitis. Oral Doxycyline, Rifampicine and Ciprofloxacin were administered for 3 months. At one-year postoperatively, the patient had a painless, unrestricted range of motion and function in relation to the affected hip. Conclusion: In endemic regions, Brucella osteomyelitis should be considered in differential diagnosis in patients with arthralgia and/or spondylodiscitis in the presence of radiologically suspected osseous lesions.
