We aimed to develop and validate a clinical nomogram predicting bladder outlet obstruction(BOO)solely using routine clinical parameters in men with refractory nonneurogenic lower urinary tract symptoms(LUTS).A total of 750 eligible patient ≥50 years of age who had previously not responded(International Prostate Symptom Score[IPSS]improvement2 cmH20 ml^-1 s^-1.A stepwise multivariable logistic regression analysis was conducted to determine the predictors of BOO,and^-coefficients of the final model were selected to create a clinical nomogram.The final multivariable logistic regression model showed that age,IPSS,maximum urinary flow rate,postvoid residual volume,total prostate volume,and transitional zone index were significant for predicting BOO;these candidates were used to develop the final nomogram.The discrimination performance of the nomogram was 88.3%(95%Cl:82.7%-93.0%,Pmining further treatment,primarily focused on prostatic surgery for BOO,without impeding the detection of possible BOO in men with LUTS that is refractory to empirical medications.

We evaluated whether the prostate-specific antigen (PSA)mass or free PSA (fPSA)mass (i.e.,absolute amount of total circulating PSA or fPSA protein,respectively),versus serum PSA or fPSA concentration,improves the accuracy of predicting the total prostate volume (TPV)in relation to obesity.Among men whose multicore (≥12)transrectal prostate biopsy was negative,586 who had a PSA of mining TPVs of ≥30 ml and ≥50 ml in obese and overweight men,they did not improve the accuracy in most other combinations of the degrees of obesity with TPV cut-off points.Thus,compared with serum PSA or fPSA,the absolute amount of PSA or fPSA protein mass improved the accuracy of predicting TPV in obese men very minimally and only for certain TPV cut-off points.Hence,these indicators may not provide clinically meaningful improvement in predicting TPV in obese men.

Endogenous endophthalmitis is an intraocular infection that results from hematogenous spread of organisms from a distant source of infection. Facial cellulitis is rarely reported as a focus of infection. We report a case of 51-year-old unconscious woman presenting with fever, facial swelling, and decreased visual acuity secondary to facial cellulitis, endogenous endophthalmitis and end-stage renal disease(ESRD). Generally systemic antibiotics in endophthalmitis have low efficacy because of the blood-ocular barrier. The management of endophthalmitis begins with intravitreal antibiotic injections and if the response is not favorable to do a vitrectomy. Twelve hours after the intravitreal antibiotic injection, vitrectomy was considered because of worsening of the vitreal cloudiness. However, the patient's general medical condition precluded vitrectomy. We experienced successful treatment with intravitreal antibiotic injection and continuous intravenous antibiotic administration because of the breakdown of the blood-ocular barrier due to ocular inflammation, especially in the setting of systemic vascular and fluid homeostatic changes.

A comparison of transanal excision and endoscopic resection for early rectal cancer

AIM:To compare the outcomes of endoscopic resection with transanal excision in patients with early rectal cancer.METHODS:Thirty-two patients with early rectal cancer were treated by transanal excision or endoscopic resection between May 1999 and December 2007.The patients were regularly re-examined by means of colonoscopy and abdominal computed tomography after resection of the early rectal cancer.Complications,length of hospital-stay,disease recurrence and follow up outcomes were assessed.RESULTS:Sixteen patients were treated by endoscopic resection and 16 patients were treated by transanal excision.No significant differences were present in the baseline characteristics.The rate of complete resection in the endoscopic resection group was 93.8%,compared to 87.5% in the transanal excision group(P=0.544).The mean length of hospital-stay in the endoscopic resection group was 2.7 ± 1.1 d,compared to 8.9 ± 2.7 d in the transanal excision group(P=0.001).The median follow up was 15.0 mo(range 6-99).During the follow up period,there was no case of recurrent disease in either group.CONCLUSION:Endoscopic resection was a safe and effective method for the treatment of early rectal cancers and its outcomes were comparable to those of transanal excision procedures.

In recent years,metal halide perovskites have received significant attention as materials for next-generation optoelectronic devices owing to their excellent optoelectronic properties.The unprecedented rapid evolution in the device performance has been achieved by gaining an advanced understanding of the composition,crystal growth,and defect engineering of perovskites.As device performances approach their theoretical limits,effective optical management becomes essential for achieving higher efficiency.In this review,we discuss the status and perspectives of nano to micron-scale patterning methods for the optical management of perovskite optoelectronic devices.We initially discuss the importance of effective light harvesting and light outcoupling via optical management.Subsequently,the recent progress in various patterning/texturing techniques applied to perovskite optoelectronic devices is summarized by categorizing them into top-down and bottom-up methods.Finally,we discuss the perspectives of advanced patterning/texturing technologies for the development and commercialization of perovskite optoelectronic devices.

Cardiac CT Image Segmentation for Deep Learning-Based Coronary Calcium Detection Using K-Means Clustering and Grabcut Algorithm

＜正＞Polyporus umbellatus（Syn.Grifola umbellata） is an edible and medicinal mushroom,belongs to the Polyporales of Basidiomycota.This study was initiated to evaluate the anti-oxidant,anti-inflammatory and anti-xanthine oxidase activities of P.umbellatus sclerotia extracted with acetone,methanol and hot water.The antioxidant activities were performed on reducing power,1,l-diphenyl-2-picrylhydrazyl free radical scavenging, and ferrous chelating activities.In addition to this,polyphenol and flavonoid contents were also analyzed.At 8 mg/ml,hot water extract showed the highest reducing power of 0.574.The scavenging activities of P.umbellatus on 1,1-diphenyl-2-picrylhydrazyl radicals,acetone extract was more effective than methanol and hot water extracts.The strongest chelating effect（67.50%） was obtained from the methanol extract at 1.0 mg/ml concentration.The antioxidant activities of the extracts from sclerotia of P.umbellatus were increased with the increasing concentration.The xanthine oxidase inhibitory activities of 3 different extracts of the sclerotia were not good as allopurinol,a positive control,and the inhibitory activities were increased with the increasing concentration of the extracts.The anti-inflammatory activity of hot water extract of the sclerotia was measured on carrageenan-induced hind paw edema of albino rat and the mean increase in paw volume and%inhibition in paw volume was measured by plethysmometer at different time intervals after different concentrations of carrageenan were injected.The injection of hot water extract with 20 mg/kg showed significant reduction in the carrageenan-induced paw edema compared with the control.The highest concentration of polyphenol was found in hot water,whereas acetone extract showed the highest concentration of flavonoid.The experimental results showed that acetone and methanol extract showed good,while hot water extract showed moderate xanthine oxidase inhibitory activities at the concentration tested.This investigation suggests that sclerotia of P.umbellatus

Specific medical data has limitations in that there are not many numbers and it is not standardized.to solve these limitations,it is necessary to study how to efficiently process these limited amounts of data.In this paper,deep learning methods for automatically determining cardiovascular diseases are described,and an effective preprocessing method for CT images that can be applied to improve the performance of deep learning was conducted.The cardiac CT images include several parts of the body such as the heart,lungs,spine,and ribs.The preprocessing step proposed in this paper divided CT image data into regions of interest and other regions using K-means clustering and the Grabcut algorithm.We compared the deep learning performance results of original data,data using only K-means clustering,and data using both K-means clustering and the Grabcut algorithm.All data used in this paper were collected at Soonchunhyang University Cheonan Hospital in Korea and the experimental test proceeded with IRB approval.The training was conducted using Resnet 50,VGG,and Inception resnet V2 models,and Resnet 50 had the best accuracy in validation and testing.Through the preprocessing process proposed in this paper,the accuracy of deep learning models was significantly improved by at least 10%and up to 40%.

Previous studies inv estigating prostate can cer(PCa)features in youn ger men have reported con flicting fin dings.This study aimed to investigate pathologic outcomes and biochemical recurrenee(BCR)status in younger men who underwent radical prostatectomy(RP)for PCa.Records of 2057 patients who underwent RP at Seoul National University Bundang Hospital(Seongnam,Korea)between 2006 and 2015 were reviewed;patients were divided according to age into the younger and older groups(men aged50 years,respectively).Postoperative BCR status and functional outcomes and clinicopathologic features were compared betwee n both groups.All an alyses were repeated after propensity score matchi ng.Younger men were more likely to have lowrisk disease(P<0.001),lower pathologic Gleason score(P<0.001)and pathologic stages(P<0.001)than older men.The pathologic Gleason score(P=0.002)and rates of extracapsular extension(P=0.004)were lower in younger men after propensity score matching.In multivariate analysis,age at RP was not an independent predictor of BCR-free survival after RP(P=0.669).Moreover,at 1 year after RP,younger men with preoperative 5?item International Index of Erectile Function score≥22(n=228)showed more favorable results for urinary con tine nee(defi ned as nonuse of pads daily)(99.4%vs 95%,P=0.009)and erections sufficie nt for vaginal in tercourse(81.8%vs 55.5%,P=0.001).Youn ger men had more favorable clinicopathologic features at RP than their older counterparts.Although age was not an independent predictor of BCR status outcome,younger men had better functional outcomes following RP.

Deep Learning Model Ensemble for the Accuracy of Classification Degenerative Arthritis

Artificial intelligence technologies are being studied to provide scientific evidence in the medical field and developed for use as diagnostic tools.This study focused on deep learning models to classify degenerative arthritis into Kellgren–Lawrence grades.Specifically,degenerative arthritis was assessed by X-ray radiographic images and classified into five classes.Subsequently,the use of various deep learning models was investigated for automating the degenerative arthritis classification process.Although research on the classification of osteoarthritis using deep learning has been conducted in previous studies,only local models have been used,and an ensemble of deep learning models has never been applied to obtain more accurate results.To address this issue,this study compared the classification performance of deep learning models,includingVGGNet,DenseNet,ResNet,TinyNet,EfficientNet,MobileNet,Xception,and ViT,on a dataset commonly used for osteoarthritis classification tasks.Our experimental results verified that even without applying a separate methodology,the performance of the ensemble was comparable to that of existing studies that only used the latest deep learning model and changed the learning method.From the trained models,two ensembles were created and evaluated:weight and specialist.The weight ensemble showed an improvement in accuracy of 1%,and the proposed specialist ensemble improved accuracy,precision,recall,and F1 score by 5%,6%,6%,and 6%,respectively,compared with the results of prior studies.
