Effect of Pre-Existing Chronic Medical Conditions on Clinical Features and Disease Outcomes of COVID-19 Infection in Jordanian Children

Background: The COVID-19 pandemic emerged unexpectedly, impacting millions worldwide and, though waning in many regions, remains a significant concern. This study focused on understanding COVID-19’s clinical presentation, disease progression, and outcomes among hospitalized pediatric patients. Additionally, it sought to investigate the impact of concurrent chronic medical conditions on disease severity. Methods: Conducted retrospectively between September 2020 and December 2021, this study observed pediatric patients (below 18 years) hospitalized with confirmed COVID-19 at an Irbid, Jordan tertiary healthcare center. Patients meeting inclusion criteria, including a positive nasal swab PCR test and hospitalization need, were analyzed. Epidemiological and clinical data were collected and segregated into two groups (A and B) based on the presence of chronic conditions. Comparative analysis encompassed presentation, disease trajectory, and outcomes between these groups. Results: Among 451 tested positive patients, 112 were included in the study. Significant differences emerged between the two pediatric groups hospitalized for COVID-19. Those with concurrent chronic illnesses exhibited heightened symptom severity—fever, dyspnea, fatigue—and a higher incidence of abnormal chest radiographs, often requiring intensive care and experiencing elevated mortality rates. Conclusion: This study underscores the criticality of prioritizing COVID-19 management strategies for pediatric patients with pre-existing chronic medical conditions. The findings highlight the increased vulnerability and poorer outcomes experienced by this subgroup, emphasizing the necessity for tailored interventions and focused care approaches.

The Whale Optimization Algorithm(WOA)is a swarm intelligence metaheuristic inspired by the bubble-net hunting tactic of humpback *** spite of its popularity due to simplicity,ease of implementation,and a limited number of parameters,WOA’s search strategy can adversely affect the convergence and equilibrium between exploration and exploitation in complex *** address this limitation,we propose a new algorithm called Multi-trial Vector-based Whale Optimization Algorithm(MTV-WOA)that incorporates a Balancing Strategy-based Trial-vector Producer(BS_TVP),a Local Strategy-based Trial-vector Producer(LS_TVP),and a Global Strategy-based Trial-vector Producer(GS_TVP)to address real-world optimization problems of varied degrees of ***-WOA has the potential to enhance exploitation and exploration,reduce the probability of being stranded in local optima,and preserve the equilibrium between exploration and *** the purpose of evaluating the proposed algorithm's performance,it is compared to eight metaheuristic algorithms utilizing CEC 2018 test ***,MTV-WOA is compared with well-stablished,recent,and WOA variant *** experimental results demonstrate that MTV-WOA surpasses comparative algorithms in terms of the accuracy of the solutions and convergence ***,we conducted the Friedman test to assess the gained results statistically and observed that MTV-WOA significantly outperforms comparative ***,we solved five engineering design problems to demonstrate the practicality of *** results indicate that the proposed MTV-WOA can efficiently address the complexities of engineering challenges and provide superior solutions that are superior to those of other algorithms.

In this work,we use the(m+1/G')-expansion method and the Adomian decomposition method to study the 3D potential Yu-Toda-Sasa-Fukuyama(3D-pYTSF)equation which has a good application in the twolayer liquid *** the first time,the(m+1/G')-expansion and the Adomian decomposition methods are used to establish novel exact wave solutions and to study some numerical solutions for the 3D-pYTSF equation,*** using the analytical method,kink-type wave,singular solution and some complex solutions to the suggested equation are successfully *** obtained wave solutions are represented with some figures in 3D and contour plots.

Objective:To determine the temporal trend and epidemiology of animal bite cases in Gerash City,***:This retrospective cross-sectional study analyzed 630 cases of people with animal bites between 2011 and 2021 in Gerash *** collected data were analyzed using Chi-square ***:The mean age of victims was(30.9±17.5)years.80.54%Of victims were male,39.70%were self-employed,and 64.60%were adults(≥18 years).Most cases of bites occurred in 2019(91 cases),2020(74 cases)and 2021(87 cases),and most of the bites were related to the upper limbs(62.70%).Most of the wounds were superficial(78%),most of the biting animals were domestic animals(91.74%),and most of the victims had Iranian nationality(97.62%).Conclusions:Given the increasing trend of animal bites in Gerash City,health authorities should carry out basic measures such as education and awareness among the public,especially at-risk groups such as adult ***,since most cases of bites are due to dogs,it seems necessary to plan for vaccination of dogs,especially those with owners.

目的模拟训练(simulation-based training,SBT)是一种有效的教育方法,广泛应用于包括肿瘤学在内的多种临床环境中。该文对肿瘤学中与SBT相关的研究进行了范围综述,全面了解SBT在提高医护人员技能方面的作用,从而提高护理质量和患者安全。方法根据Arksey和O’Malley提出的范围综述方法框架,在Medline、Scopus和Web of Science数据库中检索于2012年至2022年发表的,与SBT在提高肿瘤科医护人员技能方面的相关研究文献,仅纳入经过同行评审的英文或法语文献。由2名研究员独立进行文献的筛选、提取和分析。结果检索出1013篇文章,共纳入29篇文献。其中25篇文献侧重于非技术技能,如决策技能、沟通和团队合作技能以及认知能力;13篇文献侧重于技术技能。纳入文献研究结果表明,通过SBT项目,肿瘤科医护人员的技能得到了显著提高。其中14篇文献对SBT的应用进行了主观评估,9篇文献对其进行了客观评估,6篇文献采用了主观和客观相结合的评估方法。结论SBT是提高肿瘤科医护人员技能的有效方法,支持和推广SBT对提供高质量的医疗服务和确保患者安全至关重要。

Prognostic Factors in Cardiorenal Syndrome Type 1: Retrospective Observational and Analytical Study

Introduction: Type 1 cardiorenal syndrome (CRS 1) is characterized by acute impairment of cardiac function leading to acute renal dysfunction. CRS1 is present in 25% of patients admitted for heart failure. The objective of our study is to analyze the epidemiological, clinical, therapeutic profile and the risk and prognostic factors of these patients. Materials and Methods: We identified 120 patients with cardiorenal syndrome (CRS) over a one-year period to determine the prevalence and risk factors for developing CRS 1. We analyzed the clinical, biological, and evolutionary profiles of patients with CRS 1 and determined the risk factors for the occurrence of acute kidney injury (AKI) as well as the mortality factors in these patients. Résultats: The average age of our patients with CRS1 is 58 ± 9 years, with a sex ratio of 1.4. The average eGFR of our patients is 35 ± 6.5 ml/min/1.73m2. Diabetes was found in 17% of our patients and hypertension in 14%. The etiology of cardiac impairment is predominantly acute coronary syndrome (ACS), followed by rhythm disorders. Renally, all our patients have acute kidney injury (AKI), with 86% having functional acute renal failure and 14% having acute tubular necrosis. Therapeutically, 50% of our patients are on diuretics, 42% receive beta-blocker treatment, and RAAS blockers are used in 29% of cases. Renal replacement therapy (RRT) sessions were required in 13.8% of cases. In univariate analysis, male gender, tachyarrhythmia, and hypertension are associated with the early onset of acute kidney injury (AKI). The use of diuretics, anemia, and low left ventricular ejection fraction (LVEF) are linked to a higher risk of developing CRS 1 (p = 0.021, p = 0.037, p = 0.010 respectively). In multivariate analysis, advanced age is significantly associated with increased mortality risk in CRS 1 patients (p = 0.030), while beta-blocker use is considered a protective factor (p = 0.014). Extra Renal Rhabdoid Tumor: A Rare Cause of Congenital Soft Tissue Tumor

Rhabdoid tumors (RTs) are a well-defined entity in the kidney or central nervous system of infants or children. However, soft-tissue involvement is uncommon. It’s an exceptional neonatal tumor of soft tissue. The imaging characteristics of this tumor are not specific. Biopsy allows diagnosis;the histomorphological characteristics of rhabdoid tumors, their immunoreactivity to epithelial markers and vimentin, and the INI-1 loss are important tools for diagnosis. RT tumors are aggressive and have a rapidly fatal clinical course in most cases. Despite multidisciplinary therapy, the survival rate is very low. We report a rare case occurring in a male neonate who presents at birth with a voluminous right axillary mass. A CT scan showed a well-limited tumor mass with lobulated contours. An ultrasound-guided biopsy was performed on day 8, showing the morphology and immunoprofile of RT. The mass showed rapid growth. The child was admitted for respiratory distress at 3 weeks. A thoraco-abdominal CT showed an increase in the size of the mass with the appearance of multiple lymph nodes and pleural, hepatic, and renal metastases. The child died two days later.

Unraveling the Impact of Direct-Acting Antivirals on Hepatitis-Linked Cirrhosis: A Comprehensive Analysis of Fibrosis, Child Score, and Disease Progression

The treatment of hepatitis C has undergone a significant boom since the advent of direct acting antivirals (DAA). Indeed, the interferon-ribavirin combination that has been used to treat hepatitis C has a virological response in only 45% of cases with significant side effects. The advent of direct-acting antivirals has changed the prognosis of cirrhotic patients with hepatitis C. DAAs have ensured a sustained viral response in the majority of patients. Our work aims to see the evolution of hepatitis C patients at the cirrhosis stage under DAA. We conducted a retrospective study over 15 years (January 2009, January 2024) including all patients with post-viral cirrhosis C, whom we divided into two groups: group A, cirrhotic patients who received ribavirin and interferon, and group B, patients on DAA. From January 2009 to January 2024, we conducted a study of 182 patients with viral hepatitis C, including 102 cirrhotic patients. The mean age was 55 years. 66% of patients were initially treated with the ribavirin interferon combination, while 34% received direct-acting antivirals (DAAs). Since the introduction of DAAs, the most commonly used regimens have been sofosbuvir/daclatasvir with or without ribavirin and sofosbuvir/ledipasvir with or without ribavirin. Group A achieved sustained virological response (SVR) in 60% of cases, with notable side effects. In Group B, SVR was 98.18%, with improved tolerability and fewer side effects than previous treatments. Fifteen patients developed hepatocellular carcinoma (HCC), with a significantly lower mortality rate in those treated with DAAs compared with pegylated dual therapy (p: 0.001).

Dunbar Syndrome and Hypoxic Hepatitis: An Unusual Presentation

Hypoxic hepatitis, also known as ischemic hepatitis, is characterized by acute hepatocellular injury due to inadequate oxygen delivery to the liver. Celiac trunk stenosis can lead to hepatic ischemia and subsequent liver damage. We present the case of an 81-year-old patient with a history of hypertension, ischemic heart disease, hypothyroidism, and biliary lithiasis, who developed hypoxic hepatitis secondary to Dunbar syndrome and a stenosis of the superior mesenteric artery. The patient improved symptoms and liver function tests with conservative management, including intravenous fluids and supportive care. Long-term management involved continued antiplatelet therapy and statins, with consideration of further interventions for celiac trunk stenosis. This case underscores the importance of recognizing Dunbar syndrome as well as superior mesentery trunk stenosis as a potential cause of hypoxic hepatitis. It highlights the need for multidisciplinary management in such cases.
