目的探讨对终末期肾病患者采用维持性血液透析+西那卡塞治疗的有效性。方法随机选取2020年1月—2023年5月沭阳中兴医院收治的64例终末期肾病患者为研究对象,根据投掷硬币法进行分组。参照组(32例)采用维持性血液透析;研究组(32例)在参照组基础上给予西那卡塞药物治疗;比较两组治疗效果。结果治疗后,与参照组碱性磷酸酶(Alkaline Phosphatase, AKP)水平(101.29±14.95)u/l、全段甲状旁腺激素(Intact Parathyroid Hormone, iPTH)水平(329.65±47.35)ng/l比较,研究组AKP水平(81.32±6.41)u/l、iPTH水平(212.39±23.16)ng/l均较低,差异有统计学意义(t=6.944、12.584,P均<0.05)。治疗后,研究组炎性因子水平、肾功能指标、免疫功能指标均优于对照组,差异有统计学意义(P均<0.05)。结论临床对终末期肾病患者在展开疾病治疗期间,将维持性血液透析方法以及西那卡塞治疗充分结合,可将患者的炎性反应、肾功能以及免疫功能等有效改善。

为探究凸型加筋锥柱壳在静水压与深水爆炸载荷联合作用下的动态响应,在塑性弦线模型基础上考虑静水压载荷、锥角因素,将问题简化为求解拥有初边界值的波动方程,利用特征值展开将肋间板壳径向位移表示为无穷级数的形式,并对每个特征值计算相应的卸载时间,以此显示冲击波载荷的衰减特性。使用有限元程序Abaqus对半锥角为20°的凸型加筋锥柱壳开展最大深度500 m、最大冲击因子0.79 kg 0.5/m的水下爆炸数值模拟研究,对邻近结合处的柱段、锥段肋间板壳的理论模型计算结果进行验证对比和讨论。研究结果表明:与不计静水压相比,静水压使得肋间板壳刚度减小——最大位移出现时刻延滞,最终径向位移随水深而增大;在不同冲击因子下,理论模型与数值模拟最终径向位移误差最大为21.7%(锥段),最小为2.0%(柱段);由于锥角的存在,肋间板壳位移不再关于中心点对称分布,中心点最终位移较柱段减小40%以上。

背景：体外淋巴细胞增殖实验常用于检测医疗器械潜在的免疫原性，但在相关标准中均未给出详尽的浸提条件及作用剂量。目的：考察供试品不同浸提条件及作用剂量对体外人淋巴细胞增殖的影响，思考在选择体外淋巴细胞增殖实验条件时需考虑的因素。方法：实验检测同种骨修复材料与肝素修饰人工晶状体两种供试品，均分为以下12组：(1)实验组1：供试品24 h完全培养基(含体积分数10%胎牛血清的RPMI改良培养基)浸提液200μl+淋巴细胞悬液50μl；(2)阴性对照组1:24 h完全培养基200μl+淋巴细胞悬液50μl；(3)实验组2：供试品24 h完全培养基浸提液100μl+淋巴细胞悬液100μl；(4)阴性对照组2:24 h完全培养基100μl+淋巴细胞悬液100μl；(5)实验组3：供试品72 h RPMI改良培养基浸提液(实验前加体积分数10%胎牛血清)200μl+淋巴细胞悬液50μl；(6)阴性对照组3:72 h RPMI改良培养基(实验前加体积分数10%胎牛血清)200μl+淋巴细胞悬液50μl；(7)实验组4：供试品72 h RPMI改良培养基浸提液(实验前加体积分数10%胎牛血清)100μl+淋巴细胞悬液100μl；(8)阴性对照组4:72 h RPMI改良培养基(实验前加体积分数10%胎牛血清)100μl+淋巴细胞悬液100μl；(9)阳性对照组1：含10μg/ml植物血凝素M的完全培养基200μl+淋巴细胞悬液50μl；(10)阳性对照组2：含10μg/ml植物血凝素M的完全培养基100μl+淋巴细胞悬液100μl；(11)空白对照组1:250μl完全培养基；(12)空白对照组2:200μl完全培养基。培养3 d后，采用CCK-8法检测淋巴细胞增殖。结果与结论：(1)不同实验条件下，同种骨修复材料浸提液均可增强人淋巴细胞的活性，以RPMI改良培养基浸提72 h、浸提液与淋巴细胞悬液的体积比为4∶1的实验条件最为显著；肝素修饰人工晶状体在该条件下对淋巴细胞活性有明显的抑制作用，可能与浸提液中的肝素有关，但在完全培养基浸提24 h、浸提液与淋巴细胞悬液的体积比为4∶1的实验条件下对淋巴细胞活性有轻微的增强作用；(2)供试品不同浸提条件及作用剂量下，人体外淋巴细胞增殖实验结果可能会有较大差异，实验条件的选择需结合产品临床应用情况，也需考虑产品的固有特性。

Background Agricul.ure has greatly influenced water quality, habitats, and fish assemblages in streams of the Mississippi Al.uvial Plain(MAP) ecoregion. However, MAP streams have historically been understudied compared to streams in other agricul.ural regions of the uSA. In this study, water quality, habitat, and fish assemblage composition were assessed seasonally(spring, summer, and fall) in eight representative MAP streams located across three u.S. states. The study design included four streams containing highly agricul.ural watersheds(herein termed “agricul.ure” streams) and four streams containing mostly forested watersheds(herein termed “forest” streams), which were intended to represent reference conditions for MAP *** In general, forest streams contained significantly better instream and riparian habitats than agricul.ure streams(P = 0.010–0.040) whereas agricul.ure streams contained significantly greater levels of primary nutrients(P ul.ure and forest streams with respect to other physical and chemical variables were intermittent and season dependent. Fish assemblages in agricul.ure and forest streams were structured primarily along an environmental gradient reflecting instream habitat conditions, water nutrient concentrations, and benthic chlorophyll-a production. Structurally, fish assemblages in both stream types contained many regionally common species, though some species appeared to exhibit affinities for a particular stream type. Functionally, fish assemblages in agricul.ure streams contained more tolerant species, more omnivores, and fewer insectivores compared to forest stream assemblages, which were nearly all insectivores. 为探究洛阳传统特色食品浆面条核心元素绿豆酸浆的细菌群落组成,利用Il.umina MiSeq高通量测序技术对洛阳6家浆坊的绿豆酸浆进行细菌16S r DNA测序。结果表明:从6个绿豆酸浆样本中共鉴定出8个细菌门,涉及75个属和106个种;其中厚壁菌门为优势菌门(占比78.57~97.17%),乳杆菌属(占比20.95%~90.36%)和乳球菌属(占比3.26%~62.68%)为优势菌属,寡发酵乳杆菌、未分类乳球菌属是对应的优势菌种,而植物乳杆菌、弯曲乳杆菌、食木薯乳杆菌等是常见的次优势菌种;这些优势细菌均为乳酸菌,具有广泛益生作用。对绿豆酸浆中细菌群落进行功能预测,发现细菌群落的碳水化合物代谢和氨基酸代谢丰度高,特别是呈味氨基酸代谢通路和l-乳酸脱氢酶丰度均较高,推断绿豆酸浆独特的酸香味与细菌的氨基酸代谢以及乳酸合成有关。本研究结果为进一步研究绿豆酸浆发酵机理及开发相应发酵剂奠定了理论基础。
