Discussions on real-world acupuncture treatments for chroni clow-back pain in older adults

Chronic low-back pain(CLBP) is one of the most common pain conditions. Current clinical guidelines for low-back pain recommend acupuncture for CLBP. However, there are very few high-quality acupuncture studies on CLBP in older adults. Clinical acupuncture experts in the American Traditional Chinese Medicine Association(ATCMA) were interested in the recent grant on CLBP research announced by the National Center for Complementary and Integrative Health. The ATCMA experts held an online discussion on the subject of real-world acupuncture treatments for CLBP in older adults. Seven participants, each with more than 20 years of acupuncture practice, discussed their own unique clinical experience while another participant talked about the potential mechanism of acupuncture in pain management. As a result of the discussion, a picture of a similar treatment strategy emerged across the participants for CLBP in older adults. This discussion shows that acupuncture may have complicated mechanisms in pain management, yet it is effective for the treatment of chronic pain involving maladaptive neuroplasticity;therefore, it should be effective for CLBP in older adults.

Spin effect on redox acceleration and regioselectivity in Fe-catalyzed alkyne hydrosilylation

Iron catalysts are ideal transition metal catalysts because of the Earths abundant, cheap, biocompatible features of iron salts. Iron catalysts often have unique open-shell structures that easily undergo spin crossover in chemical transformations, a feature rarely found in noble metal catalysts. Unfortunately, little is known currently about how the open-shell structure and spin crossover affect the reactivity and selectivity of iron catalysts, which makes the development of iron catalysts a low efficient trial-and-error program. In this paper, a combination of experiments and theoretical calculations revealed that the iron-catalyzed hydrosilylation of alkynes is typical spin-crossover catalysis. Deep insight into the electronic structures of a set of well-defined open-shell active formal Fe(0) catalysts revealed that the spin-delocalization between the iron center and the 1,10-phenanthroline ligand effectively regulates the iron center's spin and oxidation state to meet the opposite electrostatic requirements of oxidative addition and reductive elimination, respectively,and the spin crossover is essential for this electron transfer process. The triplet transition state was essential for achieving high regioselectivity through tuning the nonbonding interactions. These findings provide an important reference for understanding the effect of catalyst spin state on reaction. It is inspiring for the development of iron catalysts and other Earth-abundant metal catalysts, especially from the point of view of ligand development.

Acupuncture price in forty-one metropolitan regions in the United States: An out-of-pocket cost analysis based on Ok ***

Few studies have focused on the cost of acupuncture treatments although acupuncture has become popular in the United States(U.S.). The purpose of the current study was to examine the out-of-pocket costs incurred from acupuncture services based on an online website, Ok ***. We examined descriptive statistics(range, median and 20% intervals) for the cost of acupuncture "first-time visits"and "follow-up visits" in 41 metropolitan regions in the U.S. The acupuncture prices of 723 clinics throughout 39 metropolitan regions were included, except for Birmingham, Alabama and Detroit, Michigan as there was no online data available at the time of the study for these two regions. The cost range for a first-time acupuncture visit was $15–400;the highest median was $150 in Charleston, South Carolina,while the lowest was $45 in St. Louis, Missouri. The top 10 cities for the highest median were:Baltimore, Washington, D.C., New York, San Francisco, San Jose, Boston, Atlanta, Seattle, Portland and Indianapolis, with the median $120, while the median for all 723 clinics was $112. For the follow-up visits,the cost range was $15–300;the highest median was $108 in Charleston, South Carolina, and the lowest$40 in Miami, Florida. The 10 cities with highest median follow-up acupuncture visit costs were: New York, Baltimore, New Orleans, Washington, D.C., Philadelphia, San Francisco, San Jose, Seattle, Boston and Atlanta, with the median $85, while for all 723 clinics the median price was $80. The estimation of the average gross annual income of each acupuncturist from the regions studied was $95,760, while the total annual cost of patients seeking acupuncture services in the U.S. was about $3.5 billion in 2018.

Effectiveness of two different acupuncture strategies in patients with vulvodynia Study protocol for a pilot pragmatic controlled trial

Background： Vulvodynia, or vulvar pain, is a common condition in women; however, there are few evidence-based clinical trials evaluating nonpharmacological therapies for this condition. Acupuncture is one complementary and integrative medicine therapy used by some patients with vulvodynia. This study evaluates two different acupuncture strategies for the treatment of vulvodynia and aims to evalu- ate whether either of the acupuncture protocols reduces vulvar pain, pain duration or pain with inter- course. The study also examines how long the effect of acupuncture lasts in women with vulvodynia. Methods/design： The study is designed as a randomized controlled trial, focused on two acupuncture protocols. Fifty-one patients who have had vulvodynia for more than 3 months will be recruited. Among them, 34 patients will be randomized into Groups la and 1b; those who are unwilling to receive acupuncture will be recruited into the standard care group （Group 2）. Patients in Group la will have acupuncture focused on the points in the pudendal nerve distribution area, while patients in Group lb will receive acupuncture focused on traditional （distal） meridian points. Patients in Group 2 will receive routine conventional treatments, such as using pain medications, local injections and physical therapies or other nonsurgical procedures. Acupuncture will last 45 min per session, once or twice a week for 6 weeks. The primary outcome measurement will be objective pain intensity, using the cotton swab test. The secondary outcome measurement will be subjective patient self-reported pain intensity, which will be conducted before cotton swab test. Pain intensities will be measured by an 11-point Numeric Pain Rating Scale. Pain duration and pain score during intercourse are recorded. Local muscle tension, tender- ness and trigger points （Ashi points） are also recorded. A solubility limited pyrene-4,5,9,10-tetraone-based covalent organic framework for high-performance aqueous zinc-organic batteries

Pyrene-4,5,9,10-tetraone(PTO),with a high theoretical capacity of 408 mAh·g^(−1),is a promising candidate for rechargeable aqueous zinc-ion batteries(RAZIBs),but its zincated products during discharge process suffer from high solubility in electrolytes.Herein,aβ-ketoenamine-linked two-dimensional(2D)covalent organic framework(COF)based on a 2,7-diaminopyrene-4,5,9,10-tetraone(4KT-BD)monomer and a 2,4,6-trihydroxy-benzene-1,3,5-tricarbaldehyde(Tp)node(4KT-Tp-COF)is synthesized to address the above issue.The well-designed 4KT-Tp-COF displays low solubility in 3 M Zn(CF3SO3)2 owing to the favorableπ-πstacking as well as extended structure.Besides,the ingenious structural design of the active molecule and the long-range ordered nano-channels alter the intramolecular electron distribution,which facilitates the ionic diffusion.Consequently,the 4KT-Tp-COF cathode exhibits a stable capacity of 181 mAh·g^(−1)at 0.2 A·g^(−1),superior rate capability of 139 mAh·g^(−1)at 20 A·g^(−1),and a long lifetime of 1000 cycles without capacity loss at 30 A·g^(−1).Even at a low temperature of−20°C,the electrode also performs an ultralong cycling life of 9000 cycles with a capacity of 106 mAh·g^(−1)at 5 A·g^(−1).The comprehensive characterizations of ex-situ analyses together with the theoretical calculations validate that the groups of C=O can contribute highly accessible redox-active sites for Zn^(2+)storage.The ultra-stable 4KT-Tp-COF cathode provides important insights for designing robust organic electrodes for sustainable and large-scale electrochemical energy storage.

Nitrogen(N)-doped carbon materials as metal catalyst supports have attracted signifi cant attention,but the eff ect of N dopants on catalytic performance remains unclear,especially for complex reaction processes such as Fischer-Tropsch synthesis(FTS).Herein,we engineered ruthenium(Ru)FTS catalysts supported on N-doped carbon overlayers on TiO_(2)nanoparticles.By regulating the carbonization temperatures,we successfully controlled the types and contents of N dopants to identify their impacts on metal-support interactions(MSI).Our fi ndings revealed that N dopants establish a favorable surface environment for electron transfer from the support to the Ru species.Moreover,pyridinic N demonstrates the highest electron-donating ability,followed by pyrrolic N and graphitic N.In addition to realizing excellent catalytic stability,strengthening the interaction between Ru sites and N dopants increases the Ru^(0)/Ru^(δ+)ratios to enlarge the active site numbers and surface electron density of Ru species to enhance the strength of adsorbed CO.Consequently,it improves the catalyst’s overall performance,encompassing intrinsic and apparent activities,as well as its ability for carbon chain growth.Accordingly,the as-synthesized Ru/TiO_(2)@CN-700 catalyst with abundant pyridine N dopants exhibits a superhigh C_(5+)time yield of 219.4 mol CO/(mol Ru·h)and C_(5+)selectivity of 85.5%.

Comparative pharmacokinetics of six components in normal and rheumatoid arthritis rats after intragastrical administration of Qianghuo Shengshi Decoction granules by LC-MS/MS

Objective:To investigate the plasma pharmacokinetics of six representative components(nodakenin,osthole,5-0-methylvisammioside,ferulic acid,liquiritigenin,and liquiritin),which were the ingredients of Qianghuo Shengshi Decoction(QSD) granules,in normal and rheumatoid arthritis(RA) rats administrated QSD granules intragastrically.Methods:A rapid and accurate ultra-high performance liquid chromatography-tandem mass spectrometry(LC-MS/MS) method was developed for the simultaneous determination of six components in plasma,and it showed a good specificity,linearity,intra-day and inter-day precision,intra-day and inter-day accuracy,extraction recovery,stability,and the less matrix effect.Results:The validated LC-MS/MS method was successfully used to compare the plasma pharmacokinetics of six ingredients between normal and RA rats after intragastrical administration of QSD granules and differences in the pharmacokinetics were found in two types of rats.The absorption rate in the RA rats was lower for nodakenin,osthole,5-0-methylvisammioside,liquiritigenin and liquiritin than in the normal group,while the absorption rate of ferulic acid remained constant in two groups.In comparison with the normal rats,the exposure concentration of nodakenin was higher and that of other five components except for nodakenin was lower under pathological conditions.Additionally,the absorptive amount of nodakenin,osthole,5-0-methylvisammioside and liquiritin was increased and that of ferulic acid and liquiritigenin was reduced in the RA rats than in the normal rats.Compared with the normal rats,the retention time of nodakenin,ferulic acid and liquiritin was reduced in vivo,whereas the retention time of osthole,5-0-methylvisammioside and liquiritigenin was raised in the body for the RA rats.In contrast to the normal rats,the data demonstrated an increase in the elimination velocity of nodakenin and a decrease in the elimination velocity of the other five components except for nodakenin in the pathological state.Conclusion:This study showed th

The compatibility of the gate dielectrics with semiconductors is vital for constructing efficient conducting channel for high charge transport.However,it is still a highly challenging mission to clearly clarify the relationship between the dielectric layers and the chemical structure of semiconductors,especially vacuum-deposited small molecules.Here,interfacial molecular screening of polyimide(Kapton)dielectric in organic field-effect transistors(OFETs)is comprehensively studied.It is found that the semiconducting small molecules with alkyl side chains prefer to form a high-quality charge transport layer on polyimide(PI)dielectrics compared with the molecules without alkyl side chains.On this basis,the fabricated transistors could reach the mobility of 1.2 cm^(2) V^(−1)s^(−1) the molecule with alkyl side chains on bare PI dielectric.What is more,the compatible semiconductor and dielectric would further produce a low activation energy(E_(A))of 3.01 meV towards efficient charge transport even at low temperature(e.g.,100 K,0.9 cm^(2) V^(−1)s^(−1)).Our research provides a guiding scheme for the construction of high-performance thin-film field-effect transistors based on PI dielectric layer at room and low temperatures.

雄安新区地下水动态特征及影响控制因素,对保障雄安新区地下水资源可持续利用和供水安全具有重要意义。基于2000—2021年逐年分层地下水水位监测数据,对雄安新区浅深层地下水水位埋深空间分布特征及演变规律进行了分析,采用随机森林、数值模拟等方法确定了地下水动态变化的影响控制因素。结果表明:(1)在地下水超采治理措施的综合影响下,2014年以来雄安新区地下水水位呈企稳回升态势,浅层地下水水位年均回升0.11 m,2017年以来深层承压水水位年均回升0.64 m;(2)地下水开采、降水和河湖生态补水是影响雄安新区地下水水位变化的主控因素。近20年来开采量、降水量对地下水水位变化的贡献率平均为55%、42%,分阶段贡献率分析表明,大规模生态补水对雄安新区地下水资源形成了有效补给。(3)2019—2021年降水偏多对雄安新区水位回升的贡献率高达63%,扣除降水影响,需要持续深入推进河湖生态环境复苏补水、水源置换等行之有效的治理措施,以巩固保持已取得的治理成效。

探明土地利用方式与水质的关系对改善地表水环境具有重要意义.基于2021年天津市16个国家地表水水质监测站的月数据及土地利用数据,利用GIS空间分析和数理统计方法研究不同尺度缓冲区的土地利用方式对地表水水质的影响.结果表明:(1)研究区土地利用类型以建设用地、耕地和水域为主,对河流水质影响显著.除水温(WT)和pH外,耕地、建设用地和水域与各水质指标均呈负相关;林地和草地与溶解氧(DO)和总氮(TN)呈正相关,与其他水质指标均呈负相关.(2)水质指标在不同季节表现出明显的空间差异.pH、DO与TN浓度在旱季较高,而高锰酸盐指数、氨氮(NH_(4)^(+)-N)与总磷(TP)浓度在雨季较高.(3)冗余分析(RDA)结果表明,800 m缓冲区土地利用对旱季水质变化具有最大的解释能力(50.4%),而3 000 m缓冲区土地利用可以最大程度解释雨季水质变化情况(49.6%);从旱雨季的平均解释率来看,3 000 m缓冲区是天津市土地利用对水质指标的最佳影响尺度(50.0%);(4)偏最小二乘回归(PLSR)分析可知,3 000 m缓冲区内建设用地、耕地和水域是影响地表水水质变化最显著的地类.旱季大多数水质指标PLSR模型的预测能力比雨季强.在旱季,除WT和pH外,其余水质指标均受耕地的影响最大.在雨季,建设用地对WT和NH_(4)^(+)-N浓度的影响最大,其余水质指标的最重要影响因子仍是耕地.研究表明合理规划河流或湖库3 000 m内的土地利用方式有利于改善地表水水环境质量.
