Feasibility and clinical benefits of the double-ProGlide technique for hemostasis after cryoballoon atrial fibrillation ablation with uninterrupted oral anticoagulants

OBJECTIVE To access the efficacy and safety of the double-ProGlide technique for the femoral vein access-site closure in cryoballoon ablation with uninterrupted oral anticoagulants(OAC),and its impact on the electrophysiology laboratory time as well as hospital stay after the procedure in this observational study.METHODS Patients with atrial fibrillation undergoing cryoballoon ablation with uninterrupted OAC at Department of Cardiology,Beijing Anzhen Hospital,Capital Medical University,Beijing,China from May 2019 to May 2021 were enrolled in this study.From October 2020,double-ProGlide technique was consistently used for hemostasis(ProGlide group),and before that conventional manual compression was utilized(manual compression group).The occurrence of vascular and groin complications was accessed during the hospital stay and until the three-month follow-up.RESULTS A total of 140 participants(69.30%of male,mean age:59.21±10.29 years)were evaluated,70 participants being in each group.Immediate hemostasis was achieved in all the patients with ProGlide closure.No major vascular complications were found in the ProGlide group while two major vascular complications were occurred in the manual compression group.The incidence of any groin complication was obviously higher in subjects with manual compression than patients with ProGlide devices(15.71%vs.2.86%,P=0.009).In addition,compared with the manual compression group,the ProGlide group was associated with significantly shorter total time in the electrophysiology laboratory[112.0(93.3–128.8)min vs.123.5(107.3–158.3)min,P=0.006],time from sheath removal until venous site hemostasis[3.8(3.4–4.2)min vs.8.0(7.6–8.5)min,P<0.001],bed rest time[8.0(7.6–8.0)h vs.14.1(12.0–17.6)h,P<0.001]and hospital stay after the procedure[13.8(12.5–17.8)h vs.38.0(21.5–41.0)h,P<0.001].CONCLUSIONS Utilization of the double-ProGlide technique for hemostasis after cryoballoon ablation with uninterrupted OAC is feasible and safe,which has the clinical benefit in reducing the total electrophysiology

Pediatric and adult spinal cord injuries (SCI) are distinct entities.Children and adolescents with SCI must suffer from lifelong disabilities,which is a heavy burden on patients,their families and the society.There are differences in Chinese and foreign literature reports on the incidence,injury mechanism and prognosis of SCI in children and adolescents.In addition to traumatic injuries such as car accidents and falls,the proportion of sports injuries is increasing.The most common sports injury is the backbend during dance practice.Compared with adults,children and adolescents are considered to have a greater potential for neurological improvement.The pathogenesis and treatment of pediatric SCI remains unclear.The mainstream view is that the mechanism of nerve damage in pediatric SCI include flexion,hyperextension,longitudinal distraction and ischemia.We also discuss the advantages and disadvantages of drugs such as methylprednisolone in the treatment of pediatric SCI and the indications and timing of surgery.In addition,the complications of pediatric SCI are also worthy of attention.New imaging techniques such as diffusion tensor imaging and diffusion tensor tractography may be used for diagnosis and assessment of prognosis.This article reviews the epidemiology,pathogenesis,imaging,clinical characteristics,treatment and complications of SCI in children and adolescents.Although current treatment cannot completely restore neurological function,patient quality of life can be enhanced.Continued developments and advances in the research of SCI may eventually provide a cure for children and adolescents with this kind of injury.

Copper element was added in pure titanium to produce a new biomedical titanium-copper alloy by laser powder bed fusion(LPBF).Addition of copper can eliminate the mismatch of high strength but poor duc-tility problem caused by lathα’martensite,which is the usual microstructure of nearαtitanium alloy fabricated by LPBF.Instead of by the usual trade-offrelationship between strength and ductility,which is a long-standing challenge for martensitic titanium alloys,in this study,we proposed a boundary engineer-ing strategy and aim to synergistically enhance the strength and ductility of martensitic titanium alloy fabricated by LPBF.It is hypothesized that whilst both low-angle and high-angle grain boundaries are beneficial to the strength,high-angle grain boundary can simultaneously improve the ductility of materi-als.To test this strategy,a Ti-5Cu(wt.%)alloy is selected to compare against pure titanium and Ti-6Al-4V at the same laser processing conditions.EBSD,TEM and XRD analysis show that the as-fabricated LPBF Ti-5Cu alloy is comprised of partially tempered martensite with extraordinarily high number density of both high-angle and low-angle grain boundaries as well as low dislocation density.Such microstructure enables a high tensile strength of 940-1020 MPa,which is at a similar level as LPBF Ti-6Al-4V,and an excellent elongation of 13%-16%,twice as much as that of LPBF Ti-6Al-4V.The mechanism of microstruc-ture refinement in LPBF Ti-5Cu at different levels from prior-βgrains,martensitic packets,blocks to laths is also discussed.

Diffusion tensor imaging of the hippocampus reflects the severity of hippocampal injury induced by global cerebral ischemia/reperfusion injury

At present,predicting the severity of brain injury caused by global cerebral ischemia/reperfusion injury(GCI/RI)is a clinical problem.After such an injury,clinical indicators that can directly reflect neurological dysfunction are lacking.The change in hippocampal microstructure is the key to memory formation and consolidation.Diffusion tensor imaging is a highly sensitive tool for visualizing injury to hippocampal microstructure.Although hippocampal microstructure,brain-derived neurotrophic factor(BDNF),and tropomyosin-related kinase B(Trk B)levels are closely related to nerve injury and the repair process after GCI/RI,whether these indicators can reflect the severity of such hippocampal injury remains unknown.To address this issue,we established rat models of GCI/RI using the four-vessel occlusion method.Diffusion tensor imaging parameters,BDNF,and Trk B levels were correlated with modified neurological severity scores.The results revealed that after GCI/RI,while neurological function was not related to BDNF and Trk B levels,it was related to hippocampal fractional anisotropy.These findings suggest that hippocampal fractional anisotropy can reflect the severity of hippocampal injury after global GCI/RI.The study was approved by the Institutional Animal Care and Use Committee of Capital Medical University,China(approval No.AEEI-2015-139)on November 9,2015.

Damage behavior of the KKV direct hit against fluid-filled submunition payload

The damage effects of fluid-filled submunition payload impacted by the kinetic kill vehicle(KKV)are investigated by simulations and ground-based experiments.Numerical simulations showed that the damage level and number of submunitions were significantly influenced by the diameter of the KKV compared with its length.Based on that,a high velocity penetrator formed by shaped charge explosion was used to simulate the direct hit experiment of a KKV impacting submunition payload.Experimental results demonstrated that the damage modes of submunitions mainly included the slight damage,perforation and total smash,showing a good agreement with the simulations.To understand the multiple damage modes of submunitions,the damage behavior of the submunitions in direct hit process were analyzed based on the AUTODYN-3D code.Numerical results presented that increased KKV diameter can increase the crater diameter and expand the damage volume,which will achieve a higher direct hit lethality.Further analysis indicated that there were other mechanical behaviors can enhance the damage to submunitions not lying in the KKV flight path,such as secondary debris kill,neighboring submunitions collision with each other,and high-speed fluid injection effect.

Dear Editor,Alzheimer’s disease(AD)is the most common cause of dementia among the older population,and is characterized by amyloid-beta(Aβ)accumulation,hyperphosphorylation of tau,and finally neurodegeneration in the brain[1].Aβ is the major pathological agent in AD,thus currently most efforts in developing therapies for AD target this peptide.The most promising therapy for this disease is immunotherapy.

Characterization of hot deformation behavior of 30Si2MnCrMoVE low-alloying ultra-high-strength steel by constitutive equations and processing maps

Isothermal compression tests of as-forged 30Si2MnCrMoVE low-alloying ultra-high-strength steel were carried out on a Gleeble 3500 thermal simulator at the deformation temperatures of 950-1150℃and strain rates of 0.01-10 s^−1.Based on the classical stress-dislocation density relationship and the kinematics of the dynamic recrystallization,the constitutive equations of the work hardening dynamical recovery period and dynamical recrystallization period were developed by using the work hardening curve and Avrami equation,which shows good agreement with the experimental value.Processing maps at the strain of 0.90 were constructed based on dynamic material model and were analyzed combined with microstructure observation under different conditions.The optimum parameter based on the processing maps was obtained and verified by a supplementary experiment.The power dissipation maps and instability maps at strains of 0.05-0.90 were also constructed,and the evolution law was analyzed in detail.The established constitutive equation and hot processing maps can provide some guidance for hot working process.

ASCFL:Accurate and Speedy Semi-Supervised Clustering Federated Learning

The influence of non-Independent Identically Distribution(non-IID)data on Federated Learning(FL)has been a serious concern.Clustered Federated Learning(CFL)is an emerging approach for reducing the impact of non-IID data,which employs the client similarity calculated by relevant metrics for clustering.Unfortunately,the existing CFL methods only pursue a single accuracy improvement,but ignore the convergence rate.Additionlly,the designed client selection strategy will affect the clustering results.Finally,traditional semi-supervised learning changes the distribution of data on clients,resulting in higher local costs and undesirable performance.In this paper,we propose a novel CFL method named ASCFL,which selects clients to participate in training and can dynamically adjust the balance between accuracy and convergence speed with datasets consisting of labeled and unlabeled data.To deal with unlabeled data,the prediction labels strategy predicts labels by encoders.The client selection strategy is to improve accuracy and reduce overhead by selecting clients with higher losses participating in the current round.What is more,the similarity-based clustering strategy uses a new indicator to measure the similarity between clients.Experimental results show that ASCFL has certain advantages in model accuracy and convergence speed over the three state-of-the-art methods with two popular datasets.

Screening hepatoprotective effective components of Lonicerae japonica Flos based on the spectrum-effect relationship and its mechanism exploring

Lonicerae japonicae Flos(LF)is a kind of healthcare food with hepatoprotective function.This study was designed to explore the spectrum-effect relationships between UPLC fingerprints and the hepatoprotective effects of LF.Fingerprints of ten batches of LF were established by UPLC-PDA.The inhibitory levels of AST and ALT were used as pharmacological indexes,and secoxyloganin,isochlorogenic acid A and isochlorogenic acid C were screened as hepatoprotective active compounds by grey relational analysis(GRA)and partial least squares regression analysis(PLSR).Caspase-3 was obtained by network pharmacology as a key target of hepatoprotective active compounds.Molecular docking is used to explore the interaction between small molecules and proteins.This work provided a general model of the combination of UPLC-PDA and hepatoprotective effect to study the spectrum-effect relationship of LF,which can be used to considerable methods and insight for the fundamental research of the material basis of similar healthcare food.

Cancer-associated fibroblasts(CAFs)play an important role in facilitating the progression of triple-negative breast cancer(TNBC)by deteriorating the tumor mechanical microenvironment(TMME).Herein,we designed a CAFs-targeting nanomedicine by conjugating doxorubicin(DOX)-loaded hydroxyethyl starch-IR780 nanoparticles(NPs)with Cys-Arg-Glu-Lys-Ala(CREKA)peptide,which had a special affinity for fibronectin overexpressed on CAFs.After systemic administration,the NPs efficiently targeted CAFs and generated hyperthermia upon light irradiation,decreasing CAFs through the combination of chemo-and photothermal-therapies.Thus,a series of changes in TMME were achieved by reducing CAFs,which further disrupted the niche of cancer stem cells(CSCs)to affect their survival.As a result,the tumor growth was significantly inhibited in 4T1 tumors.The strategy of TMME modulation and CSCs elimination through targeting and depleting CAFs provides a novel therapeutic treatment for desmoplastic solid tumors.
