BACKGROUND The advent of cutting-edge systemic therapies has driven advances in the treatment of hepatocellular carcinoma(HCC),and therapeutic strategies with multiple modes of delivery have been shown to be more efficacious than mono-therapy.However,the mechanisms underlying this innovative treatment modality have not been elucidated.AIM To evaluate the clinical efficacy of targeted therapy plus immunotherapy combined with hepatic arterial infusion chemotherapy(haiC)of FOLFOX in patients with unresectable HCC.METHODS We enrolled 53 patients with unresectable HCC who received a combination of targeted therapy,immunotherapy,and haiC of FOLFOX between December 2020 and June 2021 and assessed the efficacy and safety of the treatment regimen.RESULTS The objective response rate was 60.4%(32/53),complete response was 24.5%(13/53),partial response was 35.9%(19/53),and stable disease was 39.6%(21/53).The median duration of response and median progression-free survival were 9.1 and 13.9 months,respectively.The surgical conversion rate was 34.0%(18/53),and 1-year overall survival was 83.0%without critical complicating diseases or adverse events(AEs).CONCLUSION The regimen of haiC of FOLFOX,targeted therapy,and immunotherapy was curative for patients with unresectable HCC,with no serious AEs and a high rate of surgical conversion.

The integrity of retinal ganglion cells is tightly associated with diabetic macular degeneration that leads to damage and death of retinal ganglion cells,affecting vision.The major clinical treatments for diabetic macular edema are anti-vascular endothelial growth factor drugs and laser photocoagulation.However,although the macular thickness can be normalized with each of these two therapies used alone,the vision does not improve in many patients.This might result from the incomplete recovery of retinal ganglion cell injury.Therefore,a prospective,non-randomized,controlled clinical trial was designed to investigate the effect of anti-vascular endothelial growth factor drugs combined with laser photocoagulation on the integrity of retinal ganglion cells in patients with diabetic macular edema and its relationship with vision recovery.In this trial,150 patients with diabetic macular edema will be equally divided into three groups according to therapeutic methods,followed by treatment with anti-vascular endothelial growth factor drugs,laser photocoagulation therapy,and their combination.All patients will be followed up for 12 months.The primary outcome measure is retinal ganglion cell-inner plexiform layer thickness at 12 months after treatment.The secondary outcome measures include retinal ganglion cell-inner plexiform layer thickness before and 1,3,6,and 9 months after treatment,retinal nerve fiber layer thickness,best-corrected visual acuity,macular area thickness,and choroidal thickness before and 1,3,6,9,and 12 months after treatment.Safety measure is the incidence of adverse events at 1,3,6,9,and 12 months after treatment.The study protocol hopes to validate the better efficacy and safety of the combined treatment in patients with diabetic macula compared with the other two monotherapies alone during the 12-month follow-up period.The trial is designed to focus on clarifying the time-effect relationship between imaging measures related to the integrity of retinal ganglion cells and best-corrected visual acuity.

The 2017 Nobel Prize in Economics was awarded to Thaler, whose concept of nudges has been widely used. Although existing research has proposed numerous nudging strategies, there is no research comprehensively examining the role of nudging,resulting limited attention across various fields.To fill this gap, this study aims to present how to analyze the role of nudges in the system using a systems thinking approach. The analysis indicates that nudges have a multiplier effect in the system and can achieve significant results at a small cost. We use three cases to analyze the effective impact of nudges on decision making: changing the default setting in organ donation, providing the check list to avoid mistakes in surgery, and the approach to prevent the coronavirus epidemic applied in China.

In order to clarify the oxidation mechanisms and make better use of the low-grade vanadiferous titanomagnetite concentrate with high titanium(LVTC),the oxidation behavior of LVTC was investigated.The results showed that oxidation degree was achieved within 90 min when temperature was not lower than 700°C,and the main phases of the oxidized LVTC consisted of Fe9TiO15,Fe2O3,CaSiTiO5 and a small amount of Fe2.75Ti0.25O4.Increasing temperature is favorable to the formation of Fe2TiO5.The surface of LVTC gradually becomes rough,with fine particles of needle-like and granular shape appearing on the surface,which finally turn from laminar to creamy,spread out,and are interspersed with many tiny holes.The phase oxidation paths in LVTC were as follows:(1)Fe2.75Ti0.25O4→Fe9TiO15+Fe2O3;(2)Fe2.75Ti0.25O4→Fe2O3+FeTiO3→Fe2TiO5;(3)FeTiO3→Fe2O3+Fe2Ti3O9→Fe2TiO5.LVTC is predominantly mesoporous whether oxidized or not,with the pores mainly distributed in the range of 2–40 nm,and the specific surface area of LVTC decreases significantly with increasing temperature.

The oxidation behavior of a novel Ni-based single-crystal 4774DD1 superalloy for industrial gas turbine applications was investigated by the isothermal oxidation at 980℃ and discontinuous oxidation weight gain methods.The phase constitution and morphology of surface oxides and the characteristics of the crosssection oxide film were analyzed by XRD,SEM and EDS.Results show that the oxidation kinetics of the 4774DD1 superalloy follows the cubic law,indicating its weak oxidation resistance at this temperature.As the oxidation time increases,the composition of the oxide film evolves as following:One layer consisting of a bottom Al_(2)O_(3)sublayer and an upper(Al_(2)O_(3)+NiO)mixture sublayer after oxidized for 25 h.Then,two layers composed of an outermost small NiO discontinuous grain layer and an internal layer for 75 h.This internal layer is consisted of the bottom Al_(2)O_(3)sublayer,an intermediate narrow CrTaO_(4)sublayer,and an upper(Al_(2)O_(3)+NiO)mixture sublayer.Also two layers comprising an outermost relative continuous NiO layer with large grain size and an internal layer as the oxidation time increases to 125 h.This internal layer is composed of the upper(Al_(2)O_(3)+NiO)mixture sublayer,an intermediate continuous(CrTaO_(4)+NiWO_(4))mixture sublayer,and a bottom Al_(2)O_(3)sublayer.Finally,three layers consisting of an outermost(NiAl2O_(4)+NiCr2O_(4))mixture layer,an intermediate(CrTaO_(4)+NiWO_(4))mixture layer,and a bottom Al_(2)O_(3)layer for 200 h.

The spent lithium-ion batteries recovery has been brought into focus widely for its environmental imperatives and potential profits from the metal components,such as lithium,cobalt,nickel and manganese.However,the weaker pollution and fewer profits of LiMn2O4cathode dispel the enthusiasm and responsibility of industry companies.Thus,a simplified and efncient method to regenerate the sodium-ion cathode materials and separate Li from spent LiMn2O4materials for the profit improvement is proposed.In detail,adjusting the parameters of carbothermal reduction process appropriately,the LiMn2O4spinel structure is destroyed within a short period time and transformed into simple metal oxide.As anticipated,nearly 95 wt.%lithium can be obtained and recovered during the water leaching,while 99 wt.% of manganese can be extracted in acid solution.Noted that the leaching residue can return to the carbothermic reduction,leading to a closed-loop economic recycling process.The regenerated Na0.67Ni0.3Mn0.7O2cathode displays excellent electrochemical performance with superior cycling stability(the initial capacity reaches 95.9 mAh·g-1,and the retention rate reached 98.3% after 100 cycles at 1 C).The delicate strategy of sodium-ion cathode material regenerated from spent LiMn2O4aims to realize lithium separation and material utilization of manganese simultaneously,providing the instructive suggestion to rise up the recycling profits of spent batteries.

Influences of shale microstructure on mechanical properties and bedding fractures distribution

The difference in microstructure leads to the diversity of shale mechanical properties and bedding fractures distribution patterns. In this paper, the microstructure and mechanical properties of Longmaxi marine shale and Qingshankou continental shale were studied by X-ray diffractometer(XRD), field emission scanning electron microscope(FE-SEM) with mineral analysis system, and nanoindentation.Additionally, the typical bedding layers area was properly stratified using Focused Ion Beam(FIB), and the effects of microstructure and mechanical properties on the distribution patterns of bedding fractures were analyzed. The results show that the Longmaxi marine shale sample contains more clay mineral grains, while the Qingshankou continental shale sample contains more hard brittle mineral grains such as feldspar. For Longmaxi marine shale sample, hard brittle minerals with grain sizes larger than 20 μm is18.24% and those with grain sizes smaller than 20 μm is 16.22%. For Qingshankou continental shale sample, hard brittle minerals with grain sizes larger than 20 μm is 40.7% and those with grain sizes smaller than 20 μm is 11.82%. In comparison to the Qingshankou continental shale sample, the Longmaxi marine shale sample has a lower modulus, hardness, and heterogeneity. Laminated shales are formed by alternating coarse-grained and fine-grained layers during deposition. The average single-layer thickness of Longmaxi marine shale sample is greater than Qingshankou continental shale sample. The two types of shale have similar bedding fractures distribution patterns and fractures tend to occur in the transition zone from coarse-grained to fine-grained deposition. The orientation of the fracture is usually parallel to the bedding plane and detour occurs in the presence of hard brittle grains. The fracture distribution density of the Longmaxi marine shale sample is lower than that of the Qingshankou continental shale sample due to the strong heterogeneity of the Qingshankou continental shale. The Xianshuihe-Anninghe fault extends SE–S and constitutes the southeastern margin of the Tibetan Plateau.However,the Dadu River which is associated with the fault does not flow following the path,but makes a 90ºturn within a distance of 1 km at Shimian,heading east,and joins the Yangtze River,finally flowing into the East China Sea.Adjacent to the abrupt turn,a low and wide pass near the Daqiao reservoir at Mianning separates the N–S course of the Dadu River from the headwater of the Anning River which then flows south into the Yunnan Province along the Anninghe fault.Therefore,many previous studies assumed southward flow of the paleo-Dadu River from the Shimian to the Anning River.However,evidences for the capture of the integrated N–S paleo-Dadu-Anning River,its timing,and causes are still insufficient.This study explored the paleo-drainage pattern of the Dadu and Anning Rivers based on bulk mineral and geochemical analyses of the large quantities of fluvial/lacustrine sediments along the trunk of the Dadu and Anning Rivers.Similar with sands in the modern Dadu River,the Xigeda sediments also exhibit a granitoid affinity with the bulk major mineral compositions of quartz(>50%),anorthite(about 10%),orthoclase(about 5%),muscovite(about 5%),and clinochlore(about 4%).Correspondingly,bulk major elements show high SiO_(2),with all samples>60%,and some of them>70%,low TiO_(2)(≤0.75%),P_(2)O_(5)(≤0.55%),FeO*(≤5%),and relatively high CaO(1.02%–8.51%),Na_(2)O(1.60%–2.52%),and K_(2)O(2.17%–2.71%),with a uniform REE patterns.Therefore,synthesizing all these results indicate that these lacustrine sediments have similar material sources,which are mainly derived from its course in the Songpan-Ganzi flysch block,implying that the paleo-Dadu originally flowed southward into the Anning River and provided materials to the Xigeda ancient lake.The rearrangement of the paleo-Dadu River appears to be closely related to the locally focused uplift driven by strong activities of the XianshuiheXiaojiang fault system.

Coronary artery perforation(CAP) poses a significant challenge for interventional cardiologists. Management of CAP depends on the location and severity of the perforation. The conventional method for addressing the perforation of large vessels involves the placement of a covered stent, while the perforation of distal and collateral vessels is typically managed using coils, autologous skin, subcutaneous fat, microspheres, gelatin sponge, thrombin or other substances. However, the above techniques have certain limitations and are not applicable in all scenarios. Our team has developed a range of innovative strategies for effectively managing CAP. This article provides an insightful review of the various tips and tricks for the treatment of CAP.
