Background： The radiochemotherapy regimen concomitantly employing temozolomide （TMZ） chemotherapy and radiotherapy （RT） 4 weeks after surgery, followed by 6 cycles of TMZ is a common treatment for glioblastoma （GBM）. However, its median overall survival （OS） is only 14.6 months. This study was to explore the effectiveness and safety of early TMZ chemotherapy between surgery and chemoradiotherapy plus the standard concomitant radiochemotherapy regimen. Methods： A randomized, parallel group, open-label study of 99 newly diagnosed GBM patients was conducted at 10 independent Chinese neurosurgical departments from June 2008 to June 2012. Patients were treated with concomitant radiochemotherapy regimen plus early postsurgical temozolomide （early TMZ group） or standard concomitant radiochemotherapy regimen （control group）. Overall response was assessed based on objective tumor assessments, administration ofcorticosteroid and neurological status test. Hematological, biochemical, laboratory, adverse event （AE）, and neurological condition were measured for 24 months of tbllow-up. The primary efficacy endpoint of this study was overall survival （OS）. The secondary endpoint was progression free survival （PFS）. Results： The median OS time in the early TMZ group was 17.6 months, compared with 13.2 months in the control group （log-rank test P 0.021 ）. In addition, the OS rate in the early TMZ group was higher at 6, 12, and 18 months than in the control group, respectively （P 〈 0.05）. The median PFS time was 8.7 months in the early TMZ group and 10.4 months in the control group （log-rank test P = 0.695）. AEs occurred in 29 （55.8%） and 31（73.8%） patients respectively in early and control groups, including nausea （15.4% vs. 33.3%）, vomiting （7.7% vs. 28.6%）, fever （7.7% vs. 11.9%）, and headache （3.8% vs. 23.8%）. Only 30.8% and 33.3% were drug-related, respectively. BACKGROUND With the continuous development and progress of medical technology,the position of surgical nursing in the field of clinical medicine is becoming in-creasingly prominent.As an important branch of the surgical field,the nursing requirements and difficulty of gastrointestinal surgery are also increasing.In order to improve the teaching quality of nursing care in gastrointestinal surgery,many educators and researchers are actively exploring new teaching methods.Among them,the teaching method case-based learning(CBL),scene-simulated learning(SSL),task-based learning(TBL),combining self-evaluation and training mode is considered as an effective method.This method aims to help students to better master knowledge and skills and improve their comprehensive quality by cultivating their self-evaluation ability.AIM To explore the practical effect of CBL-SSL-TBL combined with training mode and student self-assessment in nursing teaching of gastrointestinal surgery.METHODS Seventy-one nursing interns in our hospital from December 2020 to December 2021 were selected.According to different teaching modes,they were divided into observation group CBL-SSL-TBL combined with training mode combined with student self-assessment and control group(conventional teaching mode),of which 36 were in observation group and 35 were in control group.The results of operational skills,theoretical knowledge,nursing students'satisfaction,learning effectiveness questionnaire and teaching effect were compared between the two groups.RESULTS Compared between the two groups,the operational skills and theoretical knowledge scores of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).Compared between the two groups,the total satisfaction ratio of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).Compared between the two groups,the observation group was lower than the control group in the questionnaire results

The goal of this paper is to deal with a new dynamic system called a differential evolution hemivariational inequality(DEHVI)which couples an abstract parabolic evolution hemivariational inequality and a nonlinear differential equation in a Banach space.First,by apply ing surjectivity result for pseudomonotone multivalued mappins and the properties of Clarke's subgradient,we show the nonempty of the solution set for the parabolic hemivariational inequality.Then,some topological properties of the solution set are established such as boundedness,closedness and convexity.Furthermore,we explore the upper semicontinuity of the solution mapping.Finally,we prove the solution set of the system(DEHVI)is nonempty and the set of all trajectories of(DEHVI)is weakly compact in C(I,X).

Lateral predictive coding is a recurrent neural network that creates energy-efficient internal representations by exploiting statistical regularity in sensory inputs.Here,we analytically investigate the trade-off between information robustness and energy in a linear model of lateral predictive coding and numerically minimize a free energy quantity.We observed several phase transitions in the synaptic weight matrix,particularly a continuous transition that breaks reciprocity and permutation symmetry and builds cyclic dominance and a discontinuous transition with the associated sudden emergence of tight balance between excitatory and inhibitory interactions.The optimal network follows an ideal gas law over an extended temperature range and saturates the efficiency upper bound of energy use.These results provide theoretical insights into the emergence and evolution of complex internal models in predictive processing systems.

BACKGROUND The lack of specific symptoms of gastric cancer(GC)causes great challenges in its early diagnosis.Thus it is essential to identify the risk factors for early diagnosis and treatment of GC and to improve the survival rates.AIM To assist physicians in identifying changes in the output of publications and research hotspots related to risk factors for GC,constructing a list of key risk factors,and providing a reference for early identification of patients at high risk for GC.METHODS Research articles on risk factors for GC were searched in the Web of Science core collection,and relevant information was extracted after screening.The literature was analyzed using Microsoft Excel 2019,CiteSpace V,and VOSviewer 1.6.18.RESULTS A total of 2514 papers from 72 countries and 2507 research institutions were retrieved.China(n=1061),National Cancer Center(n=138),and Shoichiro Tsugane(n=36)were the most productive country,institution,or author,respectively.The research hotspots in the study of risk factors for GC are summarized in four areas,namely:Helicobacter pylori(H.pylori)infection,single nucleotide polymorphism,bio-diagnostic markers,and GC risk prediction models.CONCLUSION In this study,we found that H.pylori infection is the most significant risk factor for GC;single-nucleotide polymorphism(SNP)is the most dominant genetic factor for GC;bio-diagnostic markers are the most promising diagnostic modality for GC.GC risk prediction models are the latest current research hotspot.We conclude that the most important risk factors for the development of GC are H.pylori infection,SNP,smoking,diet,and alcohol.

Effect of Y_(2)O_(3) on microstructure and properties of CoCrFeNiTiNb high entropy alloy coating on Ti-6Al-4V surface by laser cladding

The effects of Y_(2)O_(3) on the microstructure, microhardness, wear resistance, high-temperature oxidation resistance, hot corrosion resistance, and electrochemical corrosion behaviour of CoCrFeNiTiNb high entropy alloy coatings formed on Ti-6Al-4V alloy surfaces were studied. The results show that the addition of Y_(2)O_(3) changes the proportion of the phase but does not change its type. The average grain size is only 1/4.7 of that of the high entropy alloy(HEA) coating, and the fine-grained strengthening leads to increases in the microhardness and wear resistance of 21.8% and 26.9%, respectively. The addition of Y_(2)O_(3) enhances the denseness and bonding properties of the oxide and corrosion product layers, reducing the oxidation and hot corrosion rates by 60.3% and 40.3%, respectively. The addition of Y_(2)O_(3) doubles the corrosion resistance which is attributed to the refinement of the grains, the increased proportion of HCP and TiN, and the weakening of galvanic coupling corrosion.

The urgent demand of extreme(ultra-high/low)thermal conductivity materials is triggered by the high-power device,where exploring the theories and mechanisms of regulating thermal transport properties plays a key role.Herein,we elaborately investigate the effect of vertical(out-of-plane)piezoelectric characteristics on thermal transport,which is historically undiscovered.The different stacking-order(AA and AB)bilayer boron nitride(Bi-BN)in two-dimensional(2D)materials are selected as study cases.By performing state-of-the-art first-principles calculations,it is found that the polarization charge along the out-of-plane orientation ascends significantly with the increasing piezoelectric response in AB stacked Bi-BN(Bi-BN-AB)followed by the enhanced interlayer B–N atomic interactions.Consequently,the amplitude of phonon anharmonicity in Bi-BN-AB increases larger than that in the AA stacked Bi-BN(Bi-BN-AA),resulting in the dramatic weakening of the thermal conductivity by 20.34%under 18%strain.Our research reveals the significant role of the vertical(out-of-plane)piezoelectric characteristic in regulating thermal transport and provides new insight into accurately exploring the thermal transport performance of 2D van der Waals materials.

BACKGROUND Venoarterial(VA)extracorporeal membrane oxygenation(ECMO),an effective short-term circulatory support method for refractory cardiogenic shock,is widely applied.However,retrospective analyses have shown that VA-ECMO-assisted cases were associated with a relatively high mortality rate of approximately 60%.Embolization in important organs caused by complications of left ventricular thrombosis(LVT)during VA-ECMO is also an important reason.Although the incidence of LVT during VA-ECMO is not high,the consequences of embolization are disastrous.CASE SUMMARY A 37-year-old female patient was admitted to hospital because of fever for 4 d and palpitations for 3 d.After excluding the diagnosis of coronary heart disease,we established a diagnosis of“clinically explosive myocarditis”.The patient still had unstable hemodynamics after drug treatment supported by VA-ECMO,with heparin for anticoagulation.On day 4 of ECMO support,a left ventricular thro-mbus attached to the papillary muscle root of the mitral valve was found by transthoracic echocardiography.Left ventricular decompression was performed and ECMO was successfully removed,but the patient eventually died of multiple cerebral embolism.CONCLUSION LVT with high mobility during VA-ECMO may cause embolism in important organs.Therefore,a"wait and see"strategy should be avoided.
