Thoracic giant cell tumor after two total en bloc spondylectomies including one emergency surgery:A case report

BACKGROUND For patients with acute paraplegia caused by spinal giant cell tumor(GCT)who require emergency decompressive surgery,there is still a lack of relevant reports on surgical *** study is the first to present the case of an acute paraplegic patient with a thoracic spinal GCT who underwent an emergency total en bloc spondylectomy(TES).Despite tumor recurrence,three-level TES was repeated after denosumab *** SUMMARY A 27-year-old female patient who underwent single-level TES in an emergency presented with sudden severe back pain and acute paraplegia due to a thoracic spinal *** emergency TES,the patient's spinal cord function recovered,and permanent paralysis was *** postoperative histopathological examination revealed that the excised neoplasm was a rare ***,the tumor recurred 9 months after the first *** 12 months of denosumab therapy,the tumor size was reduced,and tumor *** prevent recurrent tumor progression and provide a possible cure,a three-level TES was performed *** patient returned to an active lifestyle 1 month after the second surgery,and no recurrence of GCT was found at the last *** This patient with acute paraplegia underwent TES twice,including once in an emergency,and achieved good therapeutic *** in emergency surgery is feasible and safe when conditions permit;however,it may increase the risk of tumor recurrence.

结直肠癌术后腹腔感染的危险因素及风险模型建立

目的分析结直肠癌术后患者腹腔感染的临床特征及危险因素,并构建风险预测模型。方法选取2020年1月-2022年12月东营市东营区人民医院收治的179例结直肠癌行手术治疗患者,根据术后是否发生腹腔感染分为感染组43例和未感染组136例,回顾性... 详细信息

目的分析结直肠癌术后患者腹腔感染的临床特征及危险因素,并构建风险预测模型。方法选取2020年1月-2022年12月东营市东营区人民医院收治的179例结直肠癌行手术治疗患者,根据术后是否发生腹腔感染分为感染组43例和未感染组136例,回顾性收集患者临床资料,多因素Logistic回归分析结直肠癌患者术后腹腔感染的危险因素,构建风险预测模型并评估其预测价值。结果Logistic回归分析结果显示,合并糖尿病、有低蛋白血症、术后造口、留置引流管时间≥10 d与结直肠癌术后腹腔感染有关(P<0.05);构建的预测模型:Logit(P)=-2.410+合并糖尿病×1.031+低蛋白血症×0.919+术后造口×0.987+留置引流管时间×1.000,Hosmer-Lemeshow拟合优度检验显示模型拟合效果较好(Chi-Square=3.412,P=0.578),校准曲线结果显示,预测概率与实际概率接近,提示该回归模型具有良好的区分、校准和预测能力,受试者工作特征(ROC)曲线分析结果显示,模型预测结直肠癌术后腹腔感染的曲线下面积(AUC)值为0.828。结论结直肠癌术后腹腔感染与合并糖尿病、有低蛋白血症、术后造口、留置引流管时间有关,据此构建的风险预测模型具有较好的预测价值。

为了实现凹非球面的快速、高精度与通用化检测,文中提出了一种将非球面当做球面,直接采用干涉仪检测的非零位干涉检测方法,并结合相应的数据处理方法,获得非球面的面形误差检测结果。首先,介绍了该方法的检测原理,建立了回程误差、调整误差的计算与去除模型,研究了面形误差的数据处理方法。然后,以两个不同非球面度的凹非球面为例,对其回程误差和调整误差进行了仿真计算,验证了该方法的有效性。最后,搭建了凹非球面的非零位检测实验装置,成功测量得到其面形误差。通过与自准直零位检测法或LUPHOScan轮廓测量法检测结果对比发现,两种方法测量得到的面形分布和评价指标具有高度一致性,验证了该检测方法的正确性。该检测方法在保证高精度测量的同时兼备一定的通用性与便捷性,为凹非球面的通用化检测提供了一种有效手段。

Microstructure evolution and phase transformation of the mushy zone in a quenchedβ-solidifying TiAl alloy

This study investigates the phase constitutions and transformations that occur in the mushy zone and in the adjacent phase fields of a directionally solidified Ti-44Al-8Nb-1Cr alloy via quenching *** results indicate that the mushy zone consists of unmeltedβdendrites and interdendritic liquid,whose formation can be attributed to the difference in melting point aroused by local heterogeneity in ***βdendrite is composed of numerous subgrains with various *** quenching,theβdendrite transforms into Widmanstättenαvia a precipitation reaction,owing to the decreasing cooling rate caused by heat transfer from the surrounding ***,after quenching,the interdendritic liquid is transformed intoγ*** the singleβphase field and the lower part of the mushy zone,a massive transformation ofβtoγ***,in theβ+αphase field,bothβandαphases are retained to ambient *** the heating process,the transformation ofα→βgives rise to the formation ofβvariants,which affects the orientation ofβdendrites in the mushy *** growth kinematics of theα→βtransformation was elucidated,revealing the preferential growth directions of111and112forβ***,this study presents an illustration of the formation process of the mushy zone and the microstructural evolution during the heating and quenching process.

A scheme for generating nonisospectral integrable hierarchies is *** on the method,we deduce a nonisospectral hierarchy of soliton equations by considering a linear spectral *** follows that the corresponding expanded isospectral and nonisospectral integrable hierarchies are deduced based on a 6 dimensional complex linear space ■.By reducing these integrable hierarchies,we obtain the expanded isospectral and nonisospectral derivative nonlinear Schr?dinger *** using the trace identity,the biHamiltonian structure of these two hierarchies are also ***,some symmetries and conserved quantities of the resulting hierarchy are discussed.

The electrical microenvironment plays an important role in bone ***,the underlying mechanism by which electrical stimulation(ES)promotes bone regeneration remains unclear,limiting the design of bone microenvironment-specific electroactive ***,by simple co-incubation in aqueous suspensions at physiological temperatures,biocompatible regenerated silk fibroin(RSF)is found to assemble into nanofibrils with aβ-sheet structure on MXene nanosheets,which has been reported to inhibit the restacking and oxidation of *** electroactive hydrogel based on RSF and bioencapsulated MXene is thus prepared to promote efficient bone *** MXene/RSF hydrogel also acts as a piezoresistive pressure transducer,which can potentially be utilized to monitor the electrophysiological *** sequencing is performed to explore the underlying mechanisms,which can activate Ca^(2+)/CALM signaling in favor of the direct osteogenesis *** is found to facilitate indirect osteogenesis by promoting the polarization of M2 macrophages,as well as stimulating the neogenesis and migration of *** improvements in bone regeneration and angiogenesis are observed with MXene/RSF hydrogels under ES in ***,the MXene/RSF hydrogel provides a distinctive and promising strategy for promoting direct osteogenesis,regulating immune microenvironment and neovascularization under ES,leading to re-establish electrical microenvironment for bone regeneration.

Sparse Modal Decomposition Method Addressing Underdetermined Vortex-Induced Vibration Reconstruction Problem for Marine Risers

When investigating the vortex-induced vibration(VIV)of marine risers,extrapolating the dynamic response on the entire length based on limited sensor measurements is a crucial step in both laboratory experiments and fatigue monitoring of real *** problem is conventionally solved using the modal decomposition method,based on the principle that the response can be approximated by a weighted sum of limited vibration ***,the method is not valid when the problem is underdetermined,i.e.,the number of unknown mode weights is more than the number of known *** study proposed a sparse modal decomposition method based on the compressed sensing theory and the Compressive Sampling Matching Pursuit(Co Sa MP)algorithm,exploiting the sparsity of VIV in the modal *** the validation study based on high-order VIV experiment data,the proposed method successfully reconstructed the response using only seven acceleration measurements when the conventional methods failed.A primary advantage of the proposed method is that it offers a completely data-driven approach for the underdetermined VIV reconstruction problem,which is more favorable than existing model-dependent solutions for many practical applications such as riser structural health monitoring.

In this article,we study strong limit theorems for weighted sums of extended negatively dependent random variables under the sub-linear *** establish general strong law and complete convergence theorems for weighted sums of extended negatively dependent random variables under the sub-linear *** results of strong limit theorems are more general than some related results previously obtained by Thrum(1987),Li et al.(1995)and Wu(2010)in classical probability space.
