α-Imino metal carbenes are versatile intermediates in organic synthesis, and have broad applications in the assembly of divergent N-heterocycles. However, the catalytic enantioselective desymmetrization based on α-imino metal carbenes has not been developed to date. Herein, we disclose an enantioselective desymmetrizing C（sp2）–H functionalization of azide-ynamides via α-imino copper carbenes, leading to the efficient assembly of divergent chiral indoloazepines in generally moderate to excellent yields with high enantioselectivities. Notably, this reaction represents the first enantioselective desymmetrization based on α-imino metal carbenes. Further synthetic transformations and biological tests show the potential utility of this method. Moreover,computational studies are employed to elucidate the reaction mechanism and the origin of enantioselectivity.

BACKGROUND Acute-on-chronic liver disease(AoCLD)accounts for the majority of patients hospitalized in the Department of Hepatology or Infectious *** To explore the characterization of AoCLD to provide theoretical guidance for the accurate diagnosis and prognosis of *** Patients with AoCLD from the Chinese Acute-on-Chronic Liver Failure(ACLF)study cohort were included in this *** clinical characteristics and outcomes,and the 90-d survival rate associated with each clinical type of AoCLD were analyzed,using the Kaplan-Meier method and the log-rank *** A total of 3375 patients with AoCLD were enrolled,including 1679(49.7%)patients with liver cirrhosis acute decompensation(LC-AD),850(25.2%)patients with ACLF,577(17.1%)patients with chronic hepatitis acute exacer-bation(CHAE),and 269(8.0%)patients with liver cirrhosis active phase(LC-A).The most common cause of chronic liver disease(CLD)was HBV infection(71.4%).The most common precipitants of AoCLD was bacterial infection(22.8%).The 90-d mortality rates of each clinical subtype of AoCLD were 43.4%(232/535)for type-C ACLF,36.0%(36/100)for type-B ACLF,27.0%(58/215)for type-A ACLF,9.0%(151/1679)for LC-AD,3.0%(8/269)for LC-A,and 1.2%(7/577)for *** HBV infection is the main cause of CLD,and bacterial infection is the main precipitant of *** most common clinical type of AoCLD is *** diagnosis and timely intervention are needed to reduce the mortality of patients with LC-AD or ACLF.

Objective:This cross-sectional study aimed to determine the epidemiology of olfactory and gustatory dysfunctions related to COVID-19 in ***:This study was conducted by 45 tertiary Grade-A hospitals in *** and offline questionnaire data were obtained from patients infected with COVID-19 between December 28,2022,and February 21,*** collected information included basic demographics,medical history,smoking and drinking history,vaccination history,changes in olfactory and gustatory functions before and after infection,and other postinfection symptoms,as well as the duration and improvement status of olfactory and gustatory ***:Complete questionnaires were obtained from 35,566 *** overall incidence of olfactory and taste dysfunction was 67.75%.Being female or being a cigarette smoker increased the likelihood of developing olfactory and taste *** received four doses of the vaccine or having good oral health or being a alcohol drinker decreased the risk of such *** infection,the average olfactory and taste VAS scores were 8.41 and 8.51,respectively;after infection,they decreased to 3.69 and 4.29 and recovered to 5.83 and 6.55 by the time of the *** median duration of dysosmia and dysgeusia was 15 and 12 days,respectively,with 0.5%of patients having symptoms lasting for more than 28 *** overall self-reported improvement rate was 59.16%.Recovery was higher in males,never smokers,those who received two or three vaccine doses,and those that had never experienced dental health issues,or chronic accompanying ***:The incidence of dysosmia and dysgeusia following infection with the SARS-CoV-2 virus is high in *** and prognosis are influenced by several factors,including sex,SARS-CoV-2 vaccination,history of head-facial trauma,nasal and oral health status,smoking and drinking history,and the persistence of accompanying symptoms.

Background:Acupuncture has been proven to improve the pregnancy rate in infertile *** is a common cause of *** far,there are no studies of the efficacy of acupuncture on endometriosis-associated infertility(EAI).Therefore,we will conduct a randomized controlled trial(RCT)to investigate whether acupuncture could have higher pregnancy rates in patients with EAI compared with sham *** study protocol is reported in this ***/design:The current trial is a multicenter randomized controlled study undergoing in Shanghai,*** 224 eligible patients with EAI will be randomly assigned into the acupuncture group and the sham acupuncture group in a 1:1 *** the acupuncture group,patients will be treated with acupuncture on Guanyuan(CV4),Zhongji(CV3),Zigong(EX-CA1),Xuehai(SP10),Zusanli(ST36),Sanyinjiao(SP6),and Taichong(LR3),twice a week,for 3 menstrual cycles,and 12 menstrual cycles will be followed *** Park sham device will be used to perform non-transdermal acupuncture in the sham acupuncture *** operation points will be located at 1.5 cm away beside these acupoints in the acupuncture *** intervention period is the same as that in the acupuncture *** primary outcome will be pregnancy *** outcomes will include ultrasound results,cancer antigen 125(CA125),antiMullerian hormone(AMH),sex hormones,and the Endometriosis Health Profile-5(EHP-5).Outcomes will be collected at baseline,the end of treatment,and follow-up visits at 3,6,and 12 menstrual *** data,including major adverse events,will be recorded in electronic case report forms and analyzed using SPSS ***:This study will help to evaluate whether acupuncture is effective in increasing the pregnancy rate of infertile women with endometriosis.

铊短期暴露对SBR活性污泥性能的影响

以序批式活性污泥(SBR)工艺为例,从脱氮除磷性能、微生物及酶活性等方面研究了Tl暴露浓度对活性污泥性能的短期扰动及其恢复行为.结果表明:活性污泥的脱氮除磷性能和COD去除均易受Tl的影响.当Tl为500µg/L时,脱氮除磷被抑制,但Tl暴露消除后,性能可恢复.而当Tl为1000µg/L时,尽管除磷效率可恢复至65%,但脱氮过程完全被抑制且不可逆.低浓度Tl提高了COD去除率且高浓度Tl对COD去除的影响是可逆的.此外,200µg/L的Tl提高了微生物活性,但1000µg/L的Tl会导致过氧化氢酶和超氧化物歧化酶相对产量分别降低至71.9%和30.8%,极大抑制了微生物活性;比耗氧速率对Tl的存在与消失均很敏感,可作为污水处理厂Tl暴露的指示指标.傅里叶红外光谱和X射线光电子能谱表征结果表明,Tl与游离态胞外聚合物的羟基结合或由Tl^(+)氧化成Tl^(3+),可能是活性污泥降低Tl对其细胞毒性的作用机制.因此Tl短期暴露时,需增强活性污泥的脱氮除磷性能并强化其除Tl能力,以确保污水处理厂稳定运行.本研究揭示了Tl短期暴露对活性污泥系统的潜在影响并提出了应对措施.

Patients with age-related hearing loss face hearing difficulties in daily *** causes of age-related hearing loss are complex and include changes in peripheral hearing,central processing,and cognitive-related ***,the factors by which aging relates to hearing loss via changes in audito ry processing ability are still *** this cross-sectional study,we evaluated 27 older adults(over 60 years old) with age-related hearing loss,21 older adults(over 60years old) with normal hearing,and 30 younger subjects(18-30 years old) with normal *** used the outcome of the uppe r-threshold test,including the time-compressed thres h old and the speech recognition threshold in noisy conditions,as a behavioral indicator of auditory processing *** also used electroencephalogra p hy to identify presbycusis-related abnormalities in the brain while the participants were in a spontaneous resting *** timecompressed threshold and speech recognition threshold data indicated significant diffe rences among the *** patients with age-related hearing loss,information masking(babble noise) had a greater effect than energy masking(speech-shaped noise) on processing *** terms of resting-state electroencephalography signals,we observed enhanced fro ntal lobe(Brodmann’s area,BA11) activation in the older adults with normal hearing compared with the younger participants with normal hearing,and greater activation in the parietal(BA7) and occipital(BA19) lobes in the individuals with age-related hearing loss compared with the younger *** functional connection analysis suggested that compared with younger people,the older adults with normal hearing exhibited enhanced connections among networks,including the default mode network,sensorimotor network,cingulo-opercular network,occipital network,and frontoparietal *** results suggest that both normal aging and the development of age-related hearing loss have a negative effect on

【目的】探讨健胃消胀片对大鼠胃癌前病变的治疗作用及机制。【方法】将40只雄性SD大鼠随机分为正常组、模型组、叶酸组和健胃消胀片组,每组10只。除正常组,其他3组大鼠采用雷尼替丁水溶液灌胃联合N-甲基-N’-硝基-N-亚硝基胍(MNNG)溶液饮用法制备胃癌前病变模型。成功造模后,相应给药治疗7周。记录造模及给药期间大鼠体质量变化,观察胃部大体观并进行病理评分,测定脾脏、肝脏系数,采用苏木素-伊红(HE)染色法观察胃组织病理形态变化,酶联免疫吸附分析(ELISA)检测血清胃泌素(GAS)、胃动素(MTL)、胰高血糖素(GC)含量,阿利新蓝-过碘酸雪夫氏(AB-PAS)染色法观察胃组织黏膜层厚度,蛋白免疫印迹(Western Blot)法检测胃组织磷脂酰肌醇3-激酶(PI3K)、磷酸化PI3K(p-PI3K)、蛋白激酶B(Akt)、磷酸化Akt(p-Akt)、内皮型一氧化氮合酶(eNOS)蛋白的表达,免疫荧光染色法检测胃组织血管内皮细胞生长因子A(VEGFA)蛋白的表达。【结果】与正常组比较,模型组大鼠实验期间体质量增长慢,胃部大体观病理评分显著升高(P<0.01),脾脏系数和肝脏系数显著降低(P<0.01),胃组织出现杯状细胞增生、肠化生现象,胃组织炎症评分显著升高(P<0.01),血清GAS含量显著升高(P<0.01),MTL、GC含量显著降低(P<0.05),胃组织黏膜层厚度显著减小(P<0.05),PI3K、p-PI3K、Akt、p-Akt、eNOS蛋白表达水平降低(P<0.01),VEGFA蛋白表达水平降低(P<0.01);与模型组比较,健胃消胀片组和叶酸组上述指标均得到明显改善(P<0.05或P<0.01),其中,在胃组织杯状细胞增生、肠化生现象,血清GAS含量,胃组织黏膜层厚度方面,健胃消胀片组改善效果更优。【结论】健胃消胀片可改善大鼠胃癌前病变,其机制可能与激活PI3K-Akt-eNOS通路进而促进胃损伤组织的血管生成和修复能力有关。
