Comparative Evaluation of Implant Placement with Conventional and Digital Surgical Guide Techniques: Two Case Reports

Background: Implant placement using a conventional surgical guide and digital surgical guide techniques is well documented in the literature. The most frequently reported disadvantages of conventional surgical guide placement are lack of accuracy in implant placement when compared to three-dimensional assessment in digital technique. Other factors listed are longer time duration and the need for impression techniques. In this case report, the authors present a comparison between the two techniques and the time taken between both cases one done conventionally and another case by digital technique. Case Presentation: For the digital surgical guide, a 44-year-old, male reported with the chief complaint of missing teeth needing replacement was considered. For the conventional technique, a female patient aged fifty-seven who had gone through various dentists with an existing bridge was considered. This patient wanted a good outcome at a reasonable cost. In both cases, molars were missing and needed replacement. The steps for digital flow for a surgical guide and step-by-step conventional methods are both highlighted in this article. Conclusion: Hence the digital technique saved time and was accurate when compared to the conventional in our experience.

Effect of Water Saving Irrigation Management Practices on Rice Productivity and Methane Emission from Paddy Field

Irrigation water supply is one of the vital components for sustainable rice farming, which is becoming a limiting resource in the changing climatic condition. An experiment was conducted at the research field of Bangladesh Agricultural University, Mymensingh during dry season from January-June of 2017 to investigate the suitability of Alternate Wet and Dry Irrigation (AWDI) for sustainable rice production and reducing methane emission. The modern rice variety BINA Dhan 10 was used as test crop. There were five irrigation treatments viz. T1 (saturated condition), T2 (continuous flooded, 5 cm standing water), T3 (AWDI-10 cm;irrigated when water level fell 10 cm from surface), T4 (AWDI-15 cm;irrigated when water level fell 15 cm from surface) and T5 (AWDI-20 cm;irrigated when water level fell 20 cm from surface). Results of the field trial showed satisfactory grain yield and low seasonal methane emission along with significantly high irrigation water savings (%) in AWDI treated field plots. Among the treatments, T3 (AWDI-10 cm) and T4 (AWDI-15 cm) showed higher yield performance (6250kg.ha-1 and 5810 kg.ha-1, respectively) with lower CH4 emission (reduced up to 36% and 40%, respectively) compared to continuous flooded treatment (T2, CF 5 cm water). Introduction: The pneumocystosis is an infection caused by a saprophytic germ Pneumocystis jiroveci. It is exceptional in immunocompetent patients. Case report: This is a 30-year-old patient, admitted to the service for an acute respiratory failure. She was treated for pulmonary tuberculosis 6 months ago. The review has objectified clinique cyanosis of the lips and extremities with a blood pressure 120/70 mmHg, a heart rate of 70 bats/min, a temperature of 38°C and SaO2 to 82% in the open air. The chest radiograph objectified reticulonodular opacities on the right. The patient was put under bi-broad-spectrum antibiotics. Due to the non improvement of the patient’s state, the search for Pneumocystis jiroveci in induced sputum was made and it was positive. The search for a field of immunosuppression was negative.

Lung Cancer in Women

At the same smoking level, women have a higher risk of lung cancer. The aim of the study was to evaluate epidemiological, clinical and histological profile of women＇s lung cancer. A retrospective study including women patients with a confirmed lung cancer followed at the Pneumology Department of University Hospital Center Mohammed VI of Marrakesh in Morocco between January 2010 and December 2014. 14 cases were included in the study. The average age of patients was 54.2 years. Active smoking was noted in 2 patients （14.4%）. The chest X-ray showed an atelectasis in 6 cases （42.8%）, hilar opacity in 6 cases （42.8%） and alveolar opacity in 2 cases （14.4%）, pleural effusion was present in 3 cases （21.4%）. Histological confirmation was obtained in 10 cases （71.4%） by bronchial biopsy, 2 cases with pleural biopsy （14.4%）, one case （7.1%） by lymph node biopsy and one case trans-parietal biopsy （7.1%）. An adenocarcinoma was found in 9 cases （64.3%）, squamous cell carcinoma in 4 cases （28.6%） and small cell carcinoma in one case （7.14%）. The study shows that the lung cancer in women in our context occurs more often in non-smoker patients and it＇s usually an adenocarcinoma type.

Purulent Hydroaeric Pleural Effusion Due to Infection with <i>Gemella morbillorum</i>: A Case Report

Gemella morbillorum (G. morbillorum) is an opportunistic bacterium, which can sometimes cause severe infections especially in immunocompromised subjects. Respiratory infections due to this germ are rare, but are described. We reported a new observation of 34-year-old female patient, admitted for unusual location and clinical presentation of respiratory infection with G. morbillorum;she was presented a left pleuritic pain, a productive cough associated with purulent and foul sputum and dyspnea with onset one month earlier. The clinical examination was objectified a polypnea at 32 cycles/minutes, intercostal indrawing and mixed pleural effusion syndrome. She had also many dental caries and bad oral health status. Chest radiography showed an air-fluid image in the left chest and the thoracocentesis had revealed purulent fluid. The culture of this fluid had isolated a G. morbillorum. The diagnosis of mixed pleural effusion due to G. morbillorum in an immunosuppressed diabetic patient was made. The patient was put on antibiotics, thoracic drainage and Chest physiotherapy with good improvement.

Surgical Management of a Collision Tumor: The Association of Intracranial Meningioma and Macroadenoma

Background: The simultaneous presence of a pituitary adenoma and a meningioma is a rare and underreported condition. The randomly encountered cases need to be reported to further allow our understanding of this collision tumors that are still not understood nor fully described. Patients and Method: We report a case of coexisting nonfunctioning pituitary adenoma and a left temporal lobe meningioma revealed by a 1-year history of progressive vision loss and occasional headaches in a 56-year-old woman. Her clinical condition worsened in the last 5 months with ptosis, cavernous sinus syndrome, and ophthalmoplegia of the right eye without papilledema. There was an improvement in the visual symptoms after subtotal resection of both lesions through a right frontotemporal craniotomy. Histology confirmed a collision tumor. The patient was referred for adjuvant treatment with gamma knife radiosurgery. He was doing well and back to his usual duties 6 months later. Conclusion: A gross total or subtotal resection with adjuvant therapy is the gold standard for the surgical management of collision tumors for a favorable patient outcome.

Reclamation of Coastal Soil Salinity towards Sustainable Rice Production and Mitigating Global Warming Potentials in the Changing Climate

Soil salinity has become a major constraint to rice productivity in the coastal region of Bangladesh, which threatened food security. Therefore, field experiment was conducted at salt stressed Shyamnagor Upazilla of Satkhira district to improve the soil salinity status, sustainable rice production and suppression of global warming potentials. Selected soil amendments viz. trichocompost, tea waste compost, azolla compost and phospho-gypsum (PG) were applied in the field plots one week prior to rice transplanting. In addition, proline solution (25 mM) was applied on the transplanted rice plants at active vegetative stage. Gas samples from the paddy field were collected by Closed Chamber technique and analyzed in by Gas Chromatograph. The 25% replacement of chemical fertilizer (i.e., 75% NPKS) with trichocompost, tea waste compost, Azolla compost and Phospho-gypsum amendments increased grain yield by 4.7% - 7.0%, 2.3% - 7.1% 11.9% - 16.6% and 9.5% - 14.2% during dry boro rice cultivation, while grain yield increments of 5.0% - 7.6%, 2.3% - 10.2%, 12.8% - 15.3% and 10.2% - 15.3% were recorded in wet Aman season respectively, compared to chemically fertilized (100% NPKS) field plot. The least GWPs 3575 and 3650 kg CO2 eq./ha were found in PG Cyanobacterial mixture with proline (T10) and tea waste compost with proline (T8) amended rice field, while the maximum GWPs 4725 and 4500 kg CO2 eq./ha were recorded in NPKS fertilized (100%, T2) and NPKS (75%) with Azolla compost (T5) amended plots during dry boro rice cultivation. The overall soil properties improved significantly with the selected soil amendments, while soil electrical conductivity (EC), soil pH and Na+ cation in the amended soil decreased, eventually improved the soil salinity status.
