Histopathology and TB-PCR kit analysis in differentiating the diagnosis of intestinal tuberculosis and Crohn’s disease

AIM: To compare the histopathologic features of intestinal tuberculosis (ITB) and Crohn’s disease (CD) and to identify whether polymerase chain reaction for Mycobacterium tuberculosis (TB-PCR) would be helpful for differential diagnosis between ITB and ***: We selected 97 patients with established diagnoses (55 cases of ITB and 42 cases of CD) who underwent colonoscopic *** features of ITB and CD were reviewed,and eight pathologic parameters were *** cases of acid fast bacilli culture-positive specimens and 10 normal colonic tissue specimens were evaluated as the positive and negative control of the TB-PCR test,*** assays were done using two commercial kits: kit detected IS6110 and MPB64,and kit detected IS6110 only;a manual in-house PCR method was also performed on formalin-fi xed,paraffi n-embedded colonoscopic biopsy ***: Statistically significant differences were noted between ITB and CD with regard histopathologic criteria: size of granulomas (P = 0.000),giant cells (P = 0.015),caseation necrosis (P = 0.003),confluent granulomas (P = 0.001),discrete granulomas (P = 0.000),and granulomas with lymphoid cuffs (P = 0.037).However,29 cases (52.7%) of ITB showed less than fi ve kinds of pathologic parameters,resulting in confusion with *** sensitivities and specificities of the TB-PCR test by kit ,kit ,and the in-house PCR method were 88.9% and 100%,88.9% and 100%,and 66.7% and 100% in positive and negative controls,*** PCR test done on endoscopic biopsy specimens of ITB and CD were signifi cantly different with kit (P = 0.000) and kit (P = 0.000).The sensitivities and specifi cities of TB-PCR were 45.5% and 88.1%,36.4% and 100%,and 5.8% and 100%,for kit and kit and inhouse PCR method on endoscopic biopsy *** the 29 cases of histopathologically confusing CD,10 cases assayed using kit and 6 cases assayed using kit were TB-PCR positive.A combi

Gastric cancer patients at high-risk of having synchronous cancer

AIM： To identify patients with a high-risk of having a synchronous cancer among gastric cancer patients. METHODS： We retrospectively analyzed the prospective gastric cancer database at the National Cancer Center, Korea from December 2000 to December 2004. The clinicopathological characteristics of patients with synchronous cancers and those of patients without synchronous cancers were compared. Multivariate analysis was performed to identify the risk factors for the presence of a synchronous cancer in gastric cancer patients. RESULTS： 111 of 3291 gastric cancer patients （3.4%） registered in the database had a synchronous cancer. Among these 111 patients, 109 had a single synchronous cancer and 2 patients had two synchronous cancers. The most common form of synchronous cancer was colorectal cancer （42 patients, 37.2%） followed by lung cancer （21 patients, 18.6%）. Multivariate analyses revealed that elderly patients with differentiated early gastric cancer have a higher probability of a synchronous cancer. CONCLUSION： Synchronous cancers in gastric cancer patients are not infrequent. The physicians should try to find synchronous cancers in gastric cancer patients, especially in the elderly with a differentiated early gastric

Microscopic characteristics of the inferior tarsal muscle and its surroundings in Korean

AIM: To investigate the detailed microscopic anatomic structures of the lower eyelid in Korean cadavers. METHODS: Eight lower eyelids of 4 formalin-fixed Asian cadavers (4 males; age range, 48-69 years; mean age, 60.2 years) were examined. Three perpendicular dissected sections with a 2mm thickness were obtained from each eyelid to investigate anatomic shapes, size and relationship with surrounding structures. One section was obtained from the midline and 2 the other sections were obtained from a 3mm apart from the lateral and medial tarsus margins. RESULTS: The inferior tarsal muscle fibers were not directly attached to the tarsus but were only linked to the tarsus with enclosed fibrous fascia. The inferior tarsal muscles connected loosely with the capsulopalpebral fascia anteriorly and the conjunctiva posteriorly. The inferior tarsal muscle runs horizontally to the tarsus according to the shape of muscle fibers. The capsulopalpebral fascia consisted of an anterior and posterior layer. The anterior layer reached the orbital septum and subcutaneous fat but the posterior layer forwarded into the tarsus. Lockwood’s ligament was separated from the inferior tarsal muscle and capsulopalpebral fascia or fused into the capsulopalpebral ***: This study suggests that the inferior tarsal muscle which runs horizontally and doesn’t insert directly into the tarsus, plays an important role in the movement and localization of the lower eyelid.

Red ginseng is a well-known alternative medicine with anti-inflammatory *** exerts pharmacological effects through the transformation of saponin into metabolites by intestinal *** that intestinal microflora vary among individuals,the pharmacological effects of red ginseng likely vary among *** order to produce homogeneously effective red ginseng,we prepared probiotic-fermented red ginseng and evaluated its activity using a dextran sulfate sodium(DSS)-induced colitis model in *** analysis of intestinal damage indicated that the administration of probiotic-fermented red ginseng significantly decreased the severity of colitis,compared with the control and the activity was higher than that induced by oral administration of ginseng powder or probiotics *** analysis of the levels of serum IL-6 and TNF-α,inflammatory biomarkers that are increased at the initiation stage of colitis,were significantly decreased in probiotic-fermented red ginseng-treated groups in comparison to the control *** levels of inflammatory cytokines and mR NAs for inflammatory factors in colorectal tissues were also significantly decreased in probiotic-fermented red ginseng-treated ***,oral administration of probiotic-fermented red ginseng reduced the severity of colitis in a mouse model,suggesting that it can be used as a uniformly effective red ginseng product.

A paintable and adhesive hydrogel cardiac patch with sustained release of ANGPTL4 for infarcted heart repair

The infarcted heart undergoes irreversible pathological remodeling after reperfusion involving left ventricle dilation and excessive inflammatory reactions in the infarcted heart,frequently leading to fatal functional *** attempts have been made to attenuate pathological remodeling in infarcted hearts using cardiac patches and anti-inflammatory drug *** this study,we developed a paintable and adhesive hydrogel patch using dextran-aldehyde(dex-ald)and gelatin,incorporating the anti-inflammatory protein,ANGPTL4,into the hydrogel for sustained release directly to the infarcted heart to alleviate *** optimized the material composition,including polymer concentration and molecular weight,to achieve a paintable,adhesive hydrogel using 10%gelatin and 5%dex-ald,which displayed in-situ gel formation within 135 s,cardiac tissue-like modulus(40.5 kPa),suitable tissue adhesiveness(4.3 kPa),and excellent mechanical ***4 was continuously released from the gelatin/dex-ald hydrogel without substantial burst *** gelatin/dex-ald hydrogel could be conveniently painted onto the beating heart and degraded in ***,in vivo studies using animal models of acute myocardial infarction revealed that our hydrogel cardiac patch containing ANGPTL4 significantly improved heart tissue repair,evaluated by echocardiography and histological *** heart tissues treated with ANGPTL4-loaded hydrogel patches exhibited increased vascularization,reduced inflammatory macrophages,and structural maturation of cardiac *** novel hydrogel system,which allows for facile paintability,appropriate tissue adhesiveness,and sustained release of anti-inflammatory drugs,will serve as an effective platform for the repair of various tissues,including heart,muscle,and cartilage.

Mpulse Breakdown Strength of Nano-ZnO/XLPE Nanocomposite Material on Temperature Rise

Development of new materials using composite materials has been much interest. XLPE is a kind of power cable in high voltage insulation. Recently research for cable insulating material has shown that nano-size filler added to XLPE is electrically and physically stable. In this paper, Impulse strength was measured in XLPE that composite by adding na-no-ZnO with different mass proportions. There is positive and negative impulse voltage. However, there is no differ-ence between them on the film specimen. Therefore we tested only positive voltage. In order to understand temperature properties of XLPE nanocomposite sample, experiment of impulse breakdown strength were measured at room temper-ature and maximum allowable temperature (90℃). Dynamic Response of Mold Oscillator Interacting with Steel Slabs

A mathematical model to show the dynamic response of the mold oscillator was suggested. The model con- sidered a frictional interaction between the mold oscillator and slab as several connected nodes. The governing equation considered the slab as a multi-degree-of-freedom （DoF） system, and included a hysteresis model to describe elastic-plastic behavior of the slab; the mold oscillator was given two DoF by utilizing pressure and displacement experiment data. Simulations indicate that the mold and slab execute various vibrations, and that mold oscillation marks are caused by a stick-and-slip phenomenon during intervals, in which the slab contacting the mold moves downward compared to the other slab （negative strip time）. The slab shows the formation of mold oscillation marks to previous formation criterion equally when the mold velocity is faster than the casting speed about downward. The oscillation mark will grow up over 2 Hz exciting frequency with constant 4 mm stroke in simulations. Finally, the negative strip time was compared to the frictional force, hysteresis variable, and plastic force to investigate formation mechanism of the oscillation marks.

Dissemination of gastric cancer may usually occur by direct spread through the perigastric tissues to adjacent organ, lymphatic spread, and hematogenous spread. We report a rare case of gastric cancer with mucosal metastastic lesion on the upper esophagus that was diagnosed by endoscopy and endosonography. A biopsy of the esophageal mass was performed and the pathologic findings with immunohistochemical stain for Mucin-5AC are proved to be identical to that of gastric adenocarcinoma, suggesting metastasis from main lesion of the gastric cancer. The lesion could not be explained by lymphatic or hematogenous spread,and its metastasis mechanism is considered to be different from previous studies. We suggest that the gastroesophageal reflux of cancer cells could be one of the possible metastatic pathways for metastasis of esophagus from an adenocarcinoma of the stomach.

Effect of Sn on the Microstructure and Mechanical Properties of Mg-5AI-2Si Alloys

The effect of Sn addition on the microstructures and mechanical properties of Mg-5Al-2Si alloys was investigated with variations of Sn contents （3 and 6 wt pct）. The microstructure of the alloy was characterized by the presence of Mg2Sn particles within matrix and at grain boundaries. As the Sn contents increased, yield and ultimate tensile strength were increased at room temperatures and 150℃. Creep properties were improved with the increasing amount of Sn due to the fine precipitation of Mg2Sn phases within grain during creep.
