Isolated mass-forming IgG4-related sclerosing cholangitis masquerading as extrahepatic cholangiocarcinoma:A case report

BACKGROUND IgG4-related sclerosing cholangitis(IgG4-RSC)is an uncommon benign disease,and its rarer,isolated and mass-forming subtype poses a significant challenge to differential diagnosis from cholangiocarcinoma of the extrahepatic bile *** herein report a case of isolated IgG4-RSC with an obstructing bile duct mass,for which extrahepatic bile duct resection was performed under the impression of proximal common bile duct(CBD)*** SUMMARY A 79-year-old male was admitted for jaundice that had developed 1 mo *** was no family history for autoimmune diseases or biliary *** tomography(CT)and magnetic resonance cholangiopancreaticography revealed a short segmental concentric wall thickening of the proximal CBD with diffuse dilatation of the bile duct to the *** endoscopic biopsy specimen showed no malignant *** emission tomography-CT showed a focal hypermetabolic lesion(SUVmax 4.2)in and around the proximal CBD *** the impression of proximal CBD cancer,we performed segmental resection of the extrahepatic bile *** demonstrated marked sclerosis with diffuse lymphoplasmacytic infiltration and some *** staining for IgG4 showed increased positivity in some areas(up to 30/high-power field)and IgG4+/IgG+cell ratio as 30%-50%.Pathologists’impression was IgG4-related sclerosing ***-up serum IgG4 levels were continuously elevated;however,no evidence of relapse or other organ involvement related to IgG4-RSC *** Isolated and mass-forming IgG4-RSC displays striking similarity with *** avoid unnecessary major surgery,high index of suspicion is needed.

This work aims to understand the relationship between the member angle limit and the energy ratio for 24 test samples of composite beams and CFT square steel tube columns. This work also compares the formula applicability for member angle limit with the previous test result to provide the basic data for the design of composite beam-CFT column. The evaluation of the member angle limit was performed with concrete compressive strength (fC =22.16 MPa, 30.49 MPa), breadth-to-thickness ratio (B/t=25.0, 33.3, 43.5), and axial capacity ratio (N/N0=0, 0.3, 0.4, 0.5) as the main variables of the test samples. For the relationship between the member angle limit (Ru) and the energy ratio (ES/EC ), the test result shows that the energy ratio becomes lower as the axial capacity ratio and the breadth-to-thickness ratio increase. The energy ratio is lower for the Type B test sample compared with that of Types A and C. For the formula suggested by SATO, the test samples are distributed evenly for comparison between test values and the member angle limit; however, other formulas indicate a deviation. Specifically, for the comparison between R u,cal and R u (Test), Maeda's formula shows severe deviation.

In conventional source-filter models, voiced and unvoiced components were considered independently. However, in practice it was difficult to separate the source into two parts. An actual source consists of a mixture of two sources and the ratio varies according to the content or the intention of speaker. It had been investigated to separate the voiced and unvoiced components for different source models. Source signals were modeled based on the residual signal measured from inverse filtering. Three different source models were assumed. The parameters of each model were optimized for the original speech signal using a genetic algorithm. The resulting parameters were compared in terms of the mel-cepstral distance to the original signal, the spectrogram and the spectral envelope from the synthesized signal. The optimization method achieves an improvement of 15% for the Klatt model, but there is little improvement in the modified residual case.

This paper presents a new approach to estimate damage seventy tor slaear-wall DUlltllngs using alagona~ terms of a modal flexibility matrix estimated from dynamic properties. This study aims to provide a fundamental concept for quantifying the damage of realistic buildings by investigating an idealized shear-wall building. Numerical studies were performed on a 5-story shear-wall building model to validate the applicability of the presented approach, using two damage patterns. With the numerical simulations, the proposed approach accurately determined the damage ratio of the specimens. Experiments were also conducted on a 5-story shear-wall building model for which the system parameters were almost the same as those in numerical simulations. The estimated damage-quantification results from the experimental validations demonstrated that the performance of the presented method for shear-wall buildings was both suitable and accurate.

The formability of austenitic high-Mn steels is a critical issue in automotive applications under nonuniformly-deformed environments caused by dynamic strain *** austenite stabilizing alloying elements in those steels,Cu has been known as an effective element to enhance tensile properties via controlling the stacking fault energy and stability of *** effects of Cu addition on formability,however,have not been sufficiently reported *** this study,the Cu addition effects on formability and surface characteristics in the austenitic high-Mn TRIP steels were analyzed in consideration of inhomogeneous microstructures containing the segregation of Mn and *** reveal determining factors,various mechanical parameters such as total elongation,post elongation,strain hardening rate,normal anisotropy,and planar anisotropy were correlated to the hole-expansion and cup-drawing test *** respect to microstructural parameters,roles of(Mn,Cu)-segregation bands and resultant Cu-rich FCC precipitates on the formability and surface delamination were also discussed.

The physicochemical processes of dielectric barrier discharge （DBD） such as insitu formation of chemically active species and emission of ultraviolet （UV）/visible light were utilized for the treatment of a simulated wastewater formed with Acid Red 4 as the model organic contaminant. The chemically active species （mostly ozone） produced in the DBD reactor were well distributed in the wastewater using a porous gas diffuser, thereby increasing the gas-liquid contact area. For the purpose of making the best use of the light emission, a titanium oxide-based photocatalyst was incorporated in the wastewater treating system. The experimental parameters chosen were the voltage applied to the DBD reactor, the initial pH of the wastewater, and the concentration of hydrogen peroxide added to the wastewater. The results have clearly shown that the present system capable of degrading organic contaminants in two ways （photocatalysis and ozonation） may be a promising wastewater treatment technology.
