A Comparative Analysis on Surgical Outcomes and Complications of Endoscopic and Open Vein Harvesting Techniques

Objective: The great saphenous vein (GSV) is commonly used as a conduit for grafting during CABG surgery, and open GSV harvesting (OVH), commonly used with long incision to expose the vein. However, endoscopic vein harvesting (EVH) is an alternative approach, utilizing specialized instruments and small incisions to harvest the vein. Methods: A retrospective analysis was conducted on a cohort of patients who underwent Coronary artery bypass graft (CABG) requiring great saphenous vein (GSV) harvesting which was done by EVH or OVH procedures. Demographic variables, including age and gender, were assessed for both groups. Intraoperative variables such as the number of grafts, cardiopulmonary bypass time, X clamp time, and type of procedure were analyzed. Postoperative variables, including infection and bleeding rates, were also evaluated. Results: The study included 30 patients each undergoing Coronary artery bypass graft (CABG) with need of great saphenous vein harvesting which was done by EVH and OVH. Demographic variables were well-matched between the two groups in terms of age, while a significant difference in gender distribution was observed. Obesity and smoking were more prevalent in the OVH group, and EVH was associated with a higher mean number of grafts compared to OVH. Conversion to an open technique occurred in a portion of the EVH cases, and infection rates did not significantly differ between the EVH and OVH groups. However, the incidence of postoperative bleeding was significantly higher in the EVH group. Conclusion: This study provides valuable insights into the demographic, intraoperative, and postoperative variables associated with EVH and OVH techniques. EVH demonstrated advantages in terms of reduced infection rates compared to OVH. However, the higher incidence of postoperative bleeding associated with EVH raises concerns about potential risks.

Assessment of fibroblast growth factor 19 as a non-invasive serum marker for hepatocellular carcinoma

BACKGROUND Fibroblast growth factor 19(FGF-19)is one of the founding members of the endocrine FGF ***,it has been the subject of much interest owing to its role in various physiological processes affecting glucose and lipid metabolism and the regulation of bile acid secretion as well as cell proliferation,differentiation,and ***,FGF-19 secretion in an autocrine style has reportedly contributed to cancer progression in various types of malignancies including hepatocellular carcinoma(HCC).AIM To estimate the serum FGF-19 concentrations in HCC cases and assess its diagnostic performance for the detection of *** We recruited 90 adult participants and divided them into three equal groups:Healthy controls,cirrhosis patients,and HCC *** FGF-19 concentrations were measured using the Human FGF-19 ELISA *** We detected a high statistically significant difference in serum FGF-19 levels among the three *** highest level was observed in the HCC group,followed by the cirrhosis and control groups(236.44±40.94 vs 125.63±31.54 vs 69.60±20.90 pg/mL,respectively,P≤0.001).FGF-19 was positively correlated with alpha fetoprotein(AFP;r=0.383,P=0.003)and international normalised ratio(r=0.357,P=0.005),while it was negatively correlated with albumin(r=-0.500,P≤0.001).For the detection of HCC,receiver operating characteristic curve analysis showed that the best cut-off point of AFP was>8.2 ng/mL with an area under the curve(AUC)of 0.78,sensitivity of 63.33%,specificity of 83.33%,positive predictive value(PPV)of 79.2%,negative predictive value(NPV)of 69.4%,and total accuracy of 78%.However,FGF-19 at a cut-off point>180 pg/mL had an AUC of 0.98,sensitivity of 100%,specificity of 90.0%,PPV of 90.0%,NPV of 100%,and total accuracy of 98%.CONCLUSION FGF-19 represents a possible novel non-invasive marker for *** may improve the prognosis of HCC patients due to its utility in several aspects of HCC detection and management.

Evaluation of chronic idiopathic tinnitus and its psychosocial triggers

埃及和中国都是历史悠久的文明古国,各自拥有其独特的民族传承与文化内涵。随着两国之间的经济交流日益密切,汉语学习在埃及学生群体中逐渐普及,前来授学的汉语教师也随之增多。但因为地理位置、历史进程和和宗教信仰等原因,两国的风土民俗存在着诸多差异,导致在教学中,师生之间时常会出现许多跨文化沟通上的壁垒,甚至发生一些本不必要的误解与误会。本论文主要立足于研究和分析在埃及汉语教学过程中中国教师和埃及学生之间最容易发生的交流障碍。全文主要以“案例分析”的研究方式,客观生动地展示几则常见案例,并逐一说明其产生的背景、情境、原因分析及解决方法。本文主要从语言、非语言和社会文化等方面追因溯源,最后结合实际情况来对埃及学生、中国教师和埃及学校提出一些建议,以致力于使两国师生在能够在今后更加顺畅的开展语言课程及文化交流。

Exploitation of Gene Expression and Cancer Biomarkers in Paving the Path to Era of Personalized Medicine

Cancer therapy agents have been used extensively as cytotoxic drugs against tissue or organ of a specific type of cancer. With the better understanding of molecular mcchanislns undcrlying carcinogenesis and cellular events during cancer progression and metastasis, it is now possible to use targeted therapy for these molecular events. Targeted therapy is able to identify cancer patients with dissimilar genetic defects at cellular level for the same cancer type and consequently requires individualized approach for treatment. Cancer therapy begins to shill steadily from the tra- ditional approach of ＂one regimen for all patients＂ to a more individualized approach, through which each patient will be treated specifically according to their specific genetic defects. Personalized medicine accordingly requires identification of indicators or markers that guide in the decision mak- ing of such therapy to the chosen patients for more effective therapy. Cancer biomarkcrs are fre- quently used in clinical practice for diagnosis and prognosis, as well as identification ol responsive patients and prediction of treatment response of cancer patient. The rapid breakthrough and development of microarray and sequencing technologies is probably the main tool for paving the way toward ＂individualized biomarker-driven cancer therapy＂ or ＂personalized medicine＂. In this review, we aim to provide an updated knowledge and overview of the currcnt landscape of cancer biomarkers and their role in personalized medicine, emphasizing the impact of gcnomics Oil the implementation of new potential targeted therapies and development of novel cancer biomarkers in improving the outcome of cancer therapy.

Study to Solve the Problem of Color Reversion in Refined Soybean Oil

This study carried out on the soybean oil samples taken from the processing lines during refining process (degumming, neutralization, bleaching and deodorization process) for crude soybean oil as well as on color reversed oil after deodorization step during storage for several hours to study the physical and chemical properties (color, refractive index (RI), free fatty acid (FFA), peroxide value (PV), p-Anisidine value (p-AV), total oxidative (TOTOX) value, oxidative stability (OS), saponification value (SV), iodine value (IV), unsaponifiable matter (unsap. %), soap content, minerals, waxes, total phenols content, K232 and k270 nm), the fatty acids composition, sterol compound, total tocopherols and their components (α, β, γ and δ tocopherol) and tocored compound for these oils and to find out the reason for the color reversion after a short period from storage for the deodorized soybean oil. Citric acid has been added (0.2%) to each color reversed and neutralized soybean oils, then procedure of bleaching and deodorization process on them, and studied its effect on the physical and chemical properties for them specially the color units (red and yellow), tocopherols contents and tocored compound. The results showed that refining process for soybean oil caused to a gradual decrease in values of AV, oxidative stability, IV, wax, % Unsapo., K232 and K270 nm, total polyphenols, minerals (P, Ca and Mg), total tocopherols and their compounds (α, β, γ and δ tocopherol) and also caused decrease in all sterol components, but they showed a few differences in percentages of fatty acids as result refining process. Color values (red and yellow unites) recorded gradually decreased during refining process, but these values were increased in deodorized soybean oil after storage (color reversed soybean oil). Antioxidant, hepatoprotective and nephroprotective activities of Gazania rigens against carbon tetrachloride-induced hepatotoxicity and nephrotoxicity in rats

Objective:The current study investigated the efficacy of total ethanolic extract and various fractions of Gazania rigens to protect against carbon tetrachloride-induced liver and kidney damage in rats and evaluated their antioxidant ***:The antioxidant activity of the total extract and fractions(petroleum ether,ethyl acetate,and aqueous)of Gazania rigens was examined using 2,2’-diphenyl-1-picrylhydrazyl radical scavenging and phosphomolybdate assays,and their total phenolic and flavonoid contents were *** hepatoprotective and nephroprotective potentials were studied at a dose of 300 mg/kg for six consecutive days followed by induction of hepatorenal injury using carbon tetrachloride single dose(1.5 mL/kg,1:1 v/v in olive oil,intraperitoneal)the next ***:The results revealed the potent hepato and nephroprotective effects of ethyl acetate and aqueous fractions by significantly(P<0.01 or P<0.001)reducing the serum levels of alanine aminotransferase,aspartate aminotransferase,total bilirubin,creatinine,urea,triglycerides,cholesterol,and lactate dehydrogenase,along with elevated serum albumin *** improvement of hepatic and renal antioxidant capacities and catalase activities,together with the prominent reduction of hepatic and renal malondialdehyde contents,were confirmed by histopathological ***:The potent hepatoprotective and nephroprotective effects of the total extract and different fractions of Gazania rigens may be attributed to the presence of highly phenolic and flavonoid compounds along with their complementary antioxidant scavenging properties.

Introduction of longstanding complicated sulcus intraocular lens into the intact capsular bag

AIM:To propose a surgical technique that successfully reopened the empty and intact capsular bag after long periods of closure,with repositioning of the intraocular lens(IOL)from the ciliary sulcus into its preferred habitat inside the capsular ***:This is a case series,prospective,and interventional *** technique was first performed on an aphakic high myope with a closed posterior capsule for 18 ***,five patients with recurrently displaced sulcus IOLs for a range of 1 mo to 7 y were performed for the same *** surgery,identifying a"telltale white line"was an important landmark for detecting the site of major adhesions between the edge of the capsulorhexis and the posterior *** adhesions were freed using combined manual and viscoelastic dissection,followed by an easier freeing of adhesions along the whole capsular *** IOL was safely implanted,exchanged,or introduced from the sulcus into the fibrotic and closed capsular *** were followed up for a period ranging from 6 to 17 mo ***:All the patients experienced a remarkable improvement in their subjective *** lamp examination showed a postoperative centralized IOL in the *** follow up visits confirmed visual and IOL ***:This newly-introduced surgical technique facilitates the reopening of the empty yet intact capsular bag that has been closed by fibrotic proliferations,with secured implantation of the IOL inside the capsular *** with inadvertent implantation of IOLs into the ciliary sulcus,yet having an intact capsular bag,can benefit from this technique.
