Clinico-Pathological Profile of Malignant Abdominal Tumors in Children: Experience of the Pediatric Oncology Unit in Bamako

Introduction: the objective was to describe the epidemiological and histological aspects of malignant abdominal tumors in children. Methodology: This was a descriptive, cross-sectional study conducted from January 1, 2017 to December 31, 2018. We included all patients aged 0 to 15 years with histologically confirmed abdominal malignancies. Results: We collected 46 files of malignant tumors of the abdomen (35% of pediatric cancers). The majority of patients were under 5 years old (65%). The sex ratio was 1.7. They were generally from disadvantaged socio-economic backgrounds (67%). The average consultation time was 50 days. The main reasons for consultation were abdominal mass (48%), pain (17%) or distension (9%). The mass was large, hard (89%), immobile (71%) and painful (46%). Malnutrition was found in 80% of patients. The mass was retro-peritoneal in 63% of cases and renal in 54%. It was solid in 94% of cases and heterogeneous in 76% of cases. The assay for tumor markers was not performed. Anemia was present in all patients. Fine needle aspiration was performed in 43%, biopsy in 37% and myelogram in 15% of cases. spinal cord involvement was observed in 28% of Burkitt’s cases. The main cancers diagnosed were nephroblastoma (52%), Burkitt’s lymphoma (26%) and neuroblastoma (13%). Stage 1 and stage 2 were frequent in 20% and 22% of cases, respectively;36% of cases were not staged. The average duration of chemotherapy was 11 weeks. Total resection was performed in 30% of cases. The outcome of the patients was marked by total remission in 65%. Conclusion: Abdominal malignancies are very common in children. The diagnosis of certainty is late in our context.

Multifactorial nature of hepatocellular carcinoma drug resistance: Could plant polyphenols be helpful?

Primary hepatocellular carcinoma （HCC） is a quite frequent tumor which results in high mortality and most often exhibits a poor response to present drug therapies. Clearly, a thorough understanding of the biological bases of this malignancy might suggest new strategies for its treatment. Here we examine the evidences that both ＂pharmacological＂ mechanisms （e.g. drug transporter or detoxification enzyme over-expression） and alterations in other critical factors, including the IAPs （Inhibitory of Apoptosis Proteins）, involved in enhancement of cell survival and proliferation may determine the therapeutic resistance of HCC; we also underline the possible role in the process of the activation of transcription factors, like NF-κB, capable of contemporaneously up-regulating the mechanisms discussed. On this basis, we finally comment on the possible use of natural multi-targeted antitumoral agents like plant polyphenols to achieve sensitization to treatments in HCC.

Development of an analytical method based on solid-phase extraction and LC-MS/MS for the monitoring of current-use pesticides and their metabolites in human urine

Pyrethroids,organophosphorus pesticides and fipronil have been listed as priority chemicals in human biomonitoring studies because of their wide use and potential health effects in *** determination of 13 pesticides,including pyrethroids(deltamethrin,cypermethrin,permethrin,cyfluthrin,bifenthrin),organophosphorus(chlorpyrifos,chlorpyrifosmethyl,and malathion),fipronil,neonicotinoids(imidacloprid,acetamiprid and thiacloprid)and triazole(prothioconazole),together with 13 corresponding metabolites in human urine samples was achieved by solid-phase extraction and analysis by liquid chromatography coupled to tandem mass *** targeted compounds,except malathion dicarboxylic acid,were measured with a mean within-accuracy(n=5)of 71%–114%(RSD:1%–14%)and between-run(n=15)accuracy of 80%–118%(RSD:2%–14%).Limits of quantitation of the targeted analytes ranged from 0.1 to 16 pg/m *** detection result of urine samples from 25 volunteers indicated that the detection frequencies of 3,5,6-trichloro-2-pyridinol(median:448 pg/m L),6-chloropyridine-3-carboxylic acid(median:193 pg/m L),2-methyl-3-phenylbenzoic acid(median:181 pg/m L),3-phenoxybenzoic acid(median:99 pg/m L),2-isopropyl-6-methyl-4-pyrimidinol(median:77 pg/m L),cyfluthrin(median:59 pg/m L),cis-3-(2,2-dichlorovinyl)-2,2-dimethylcyclopropanecarboxylic acid(cis-DCCA,median:53 pg/m L),trans-DCCA(median:25 pg/m L),prothioconazole(median:21 pg/m L),imidacloprid(median:7 pg/m L),and prothioconazole-desthio(median:1 pg/m L)were>50%.The obtained results show that the validated method is suitable for the human biomonitoring of these currentuse pesticides and their metabolites.

Renal cell carcinoma （RCC） is the most common renal tumor, accounting for 2%-3% of all malignancies. Though RCC is known to spread hematogenously, isolated RCC metastasis to the stomach is a rare event. In this article, we describe the clinical course of a patient who developed a pancreatic recurrence of RCC and 1 year later a gastric recurrence of RCC treated 10 years ago with a resection and interleukin-2 （IL-2）. Accumulating evidence indicates that metastatic involvement of the pancreas and stomach should be suspected in any patient with a history of RCC who presents with gastrointestinal symptoms even 10 years after RCC resection and immunotherapy.

Prognostic factors for recurrence of gastrointestinal bleeding due to Dieulafoy's lesion

AIM： To analyze the effectiveness of the endoscopic therapy and to identify prognostic factors for recurrent bleeding. METHODS： Retrospective study of patients with gas- trointestinal bleeding secondary to Dieulafoy＇s lesion （DL） from 2005 to 2011. We analyzed the demographic characteristics of the patients, risk factors for gastro- intestinal bleeding, endoscopic findings, characteristics of the endoscopic treatment, and the recurrence of bleeding. We included cases in which endoscopy de- scribed a lesion compatible with Dieulafoy. We exclud- ed patients who had potentially bleeding lesions such as angiodysplasia in other areas or had undergone other gastrointestinal endoscopic procedures. RESULTS： Twenty-nine patients with DL were identi- fied. Most of them were men with an average age of 71.5 years. Fifty-five percent of the patients received antiaggregatory or anticoagulant therapy. The most common location for DL was the stomach （51.7%）. The main type of bleeding was oozing in 65.5% of cases. In 27.6% of cases, there was arterial （spurting） bleeding, and 6.9% of the patients presented with an adherent clot. A single endoscopic treatment was ap- plied to nine patients （31%）, eight of them with adren- aline and one with argon, while 69% of the patients received combined treatment. Six patients （20.7%） presented with recurrent bleeding at a median of 4 d after endoscopy （interquartile range = 97.75）. Within these six patients, the new endoscopic treatment ob- tained a therapeutic success of 100%. The presence of arterial bleeding at endoscopy was associated with a higher recurrence rate for bleeding （50% vs 33.3% for other type of bleeding） rP = 0.024, odds ratio （OR） = 8.5, 95% CI = 1.13-63.87]. The use of combined en- doscopic treatment prevented the recurrence of bleed- ing （10% vs 44.4% of single treatment）（P = 0.034, OR = 0.14, 95% CI = 0.19-0.99）. Tuning the Defects in AgO Nanoparticles/Graphene Bilayers System by Laser Power Density

The silver oxide nanoparticles （AgONps） used in this study was obtained by laser ablation technique. On the other hand, graphene bilayers were obtained through micro mechanic exfoliation. Both techniques are considered clean, simple and they have economic viability. The analyses have showed a positive charge in nanoparticles and alteration in Raman spectra caused by doping in the bilayer observing the appearing of D band. The alterations in the Raman spectrums are related with the introduced defects in the crystalline chain after the nanoparticles incorporation on bilayers. However, we have been doing a systematic study about the Raman laser output in only graphene bilayer and in graphene bilayer doped with nanoparticles. These measurements have shown a clear difference to both cases. The distance between defects has been showing to be inversely proportional to the ratio ID/I6 in the analyzed cases, increasing （up power） or lowering （down power） the incident laser output, with this it was possible to handle the defect distances in the case of AgO nanoparticles/graphene bilayers. Finally, we analyzed the ratio of the integrated area of D and G band, from these parameters are determined the defect density （ND） and the defects density （LD）. Thus, we clearly find a tuning between the ND and LD as a function of laser power density Raman using to λ = 532 nm.

Introduction: Congenital limb gangrene is a rare pathology. In the majority of cases, no cause is identified and its treatment is not codified. Observation: 29-week premature newborn, female from poorly followed twin pregnancy. Clinical examination at admission found an unstable neonate, the presence of necrosis of the left hand and forearm well limited, with absent humeral pulse and absence of hand movement. Surgical treatment was scheduled for amputation of the limb after the agreement of the parents whose postoperative follow-ups were simple. Conclusion: Congenital gangrene remains a rare entity in neonatology.

The introduction of tacrolimus in clinical practice has improved patient survival after organ ***,despite the long use of tacrolimus in clinical practice,the best way to use this agent is still a matter of intense *** start of the genomic era has generated new research areas,such as pharmacogenetics,which studies the variability of drug response in relation to the genetic factors involved in the processes responsible for the pharmacokinetics and/or the action mechanism of a drug in the *** variability seems to be correlated with the presence of genetic *** is an attractive option especially for the initiation of the dosing of tacrolimus;also,unlike phenotypic tests,the genotype is a stable characteristic that needs to be determined only once for any given ***,prospective clinical studies must show that genotype determination before transplantation allows for better use of a given drug and improves the safety and clinical efficacy of that *** present,research has been able to reliably show that the CYP3A5 genotype,but not the CYP3A4 or ABCB1 ones,can modify the pharmacokinetics of ***,it has not been possible to incontrovertibly show that the corresponding changes in the pharmacokinetic profile are linked with different patient outcomes regarding tacrolimus efficacy and *** these reasons,pharmacogenetics and individualized medicine remain a fascinating area for further study and may ultimately become the face of future medical practice and drug dosing.
