Comparison of breast reconstruction using ipsilateral and contralateral pedicle transverse rectus abdominis musculocutaneous flaps

Aim: Breast reconstruction has several beneficial effects on psychosocial well-being and quality of life. The ultimate goal has always been to create the most natural breast mound. Thus in many centers, the unilateral pedicled transverse rectus abdominis myocutaneous (TRAM) flap remains the most common technique for breast reconstruction. Our objective was to retrospectively compare the outcomes of ipsilateral and contralateral pedicle TRAM flaps. Methods: The total of 110 patients underwent unilateral breast reconstruction with pedicle TRAM flap at Cancer Institute of Tehran University of Medical Science from January 1996 to June 2011. Premorbid risk factors, postoperative outcomes and demographic data were assessed. The analysis of the recordings was done by SPSS 20. Results: Out of 110 patients who were included in the study, 87 had ipsilateral and 23 contralateral pedicle TRAM flaps. The incidence of flap complications that did not require surgical intervention was 19.7% in ipsilateral and 30.4% in contralateral pedicle TRAM flap. The incidence of flap loss requiring revision was significantly higher in contralateral group (P = 0.001). Major complications were noted in 11.5% of the ipsilateral pedicle TRAM patients and 26.1% of the contralateral group (P < 0.001). Minor complications were noted in 17.2% of the ipsilateral pedicle TRAM patients and in 34.8% of the contralateral group (P < 0.001). Total early hospital stay was longer in contralateral pedicle TRAM flaps (7.66 days vs. 10.68 days, P = 0.83). H igher c omplications were e ncountered i n c ontralateral p edicle T RAM flaps compared to ipsilateral pedicle TRAM patients (39.1% vs. 19.5%, P = 0.001). The type of pedicled TRAM flap (ipsilateral vs. contralateral), had significant effect on complications (odds ratio = 0.007, P = 0.002) while other variables had no significant effect on the incidence of complications. Neuropsychological and Neurological Deficits in Obsessive-Compulsive Disorder: The Role of Comorbid Depression

Past research has found a high prevalence of neuropsychological deficits and neurological soft signs in patients with obsessive-compulsive disorder (OCD). A close relationship between OCD and depression has also been indicated in the literature. However, many aspects of this association remain unclear. In this study we compared neuropsychological functioning and neurological soft signs of 46 OCD patients who were off psychotropic medication (21 with significant depressive symptoms and 25 without) with 25 non-patient comparison subjects. The patients were matched to the comparison group with regard to age, sex, education, marital status and handedness. There were significant differences between OCD patients and the non-patient comparison group on neuropsychological functioning but not on neurological soft signs. As far as working memory concerns there was a significant difference between the OCD depressed and non-depressed groups. But we found no evidence that the neuropsychological functions such as cognitive flexibility, problem solving and spatial perception in OCD to be attributable to comorbid depression. The pattern of neuropsychological deficits in this sample of OCD patients is suggestive of orbito-frontal as well as dorsolateral prefrontal cortical dysfunction.

Characterization and frequency of antibiotic resistance related to membrane porin and efflux pump genes among Acinetobacter baumannii strains obtained from burn patients in Tehran, Iran

Objective: To explore the characterization and frequency of antibiotic resistance related to membrane porin and efflux pump genes among Acinetobacter baumannii (A. baumannii) strains obtained from burn patients in Tehran, Iran. Methods: In this cross-sectional descriptive study, 100 strains of A. baumannii isolated from burn patients visiting teaching hospitals of Tehran were collected from January 2016 to November 2017. After A. baumannii strains were confirmed, antimicrobial susceptibility testing was done via Kirby-Bauer disc diffusion method according to the Clinical and Laboratory Standards Institute guidelines. PCR amplification was performed for detection of β-lactamase adeR, OprD, adeS genes among A. baumannii strains. Results: All isolates (100%) were resistant to ceftazidime, cefotaxime, cefepime, ciprofloxacin, and piperacillin, and most isolates indicated high resistance (95%-97%) to meropenem, imipenem, gentamicin, ceftriaxone, trimethoprim-sulfamethoxazole, piperacillin-tazobactam, amikacin, and tetracycline. The most effective antibiotic against A. baumannii isolates was colistin (97% sensitivity), followed by tigecycline. The frequency of OprD, adeS, and adeR genes were 98%, 91%, and 77%, respectively. Conclusions: This study shows that the majority of A. baumannii isolates are highly resistant to the antibiotics most commonly used in burn patients. Also, high distribution of OprD and adeRS genes may be responsible for the observed resistances among A. baumannii isolates that demonstrate the possible role of both efflux pumps in simultaneous of carbapenemase production during antibiotic resistance.

The Relationships between Cognitive Neuropsychological State, Oxidative Stress and Genotoxicity in Radiology Staff

Imbalance between the production and removal of reactive oxygen species (ROS) has been used for characterizing Oxidative stress. Extra amount of ROS are hazardous and can lead damages to the cells resulting in harsh diseases. For cancer, diabetes, psychological disorders and age-related disorders, latent of ROS in induction of alteration and DNA harm is a critical factor. Regarding these finding, the current research was planned to test the relationship between cognitive neuropsychological state and oxidative markers and genotoxicity in radiology staff. Total Number of 47 radiology staff, working in the radiology center of a referral University Hospital in Shiraz, were invited in the study. The participants were occupationally exposed to low-dose ionizing radiation (x-ray) for more than 2 years. Psychological disorder was analyzed by SCL-90-R. In the plasma, lipid peroxidation, DNA harm, catalase, superoxide dismutase, myeloperoxidase, and glutathione peroxidase activity were calculated. Associations between parameters were measured by using Pearson correlation coefficient and multivariate linear regression analyses. As a result, there was a positive considerable connection between catalase and paranoid ideation, Obsessive-Compulsive, Anxiety and total. Furthermore, the correlation between psychoticism, Interpersonal Sensitivity, Paranoid Ideation and DNA damage was positive. There were positive relationships between superoxide dismutase and psychomotor speed, initiative/energy, selective attention and verbal memory. So, there were significant relationships among interpersonal sensitivity, somatization, depression and gender. But, there were no major relationships among psychological distress, history workers and oxidative stress. In so many words, Oxidative stress and genotoxicity result in psychological disorders in radiology staff. So, radiology staff should have more attention, as they suffered from some psychological disorders.

Road traffic deaths in Kashan region, Iran： An eight-year study （2006-2013）

Purpose： The aim of this study was to analyze the trend of road traffic fatalities in Kashan Region, lran, in a period of eight years. Methods： Through a cross-sectional study, all road traffic deaths classified under the VO1-V99 codes according to ICD-10 in Kashan region, central Iran, from March 2006 to March 2013 and population data were collected from the registration system of Kashan University of Medical Sciences. Years of lost life （YLL） and mortality rates were calculated regarding age, gender and year of the accident. Generalized linear model （GLM） with Poisson log-linear link was used to evaluate the effects of the mentioned variables on mortality rate. Results： During the period of the study （8 years）, 928 people （767 men） died due to road traffic injuries （RTIs）. The total YLL was 20,818. The mortality rate due to RTIs has been declined constantly from 43.1 in March 2006 to 21.1 per 100,000 in March 2013. The highest mortality rate was found in the age group of over 60 years old and the lowest in the age group of 0-14 years old. Both mortality rate and YLL was greater in men than in women. Poisson regression showed that age, gender and year of the accidents had a significant effect on mortality rate （p 〈 0.001 ）. Conclusion： Although there has been a constant decline of mortality rate in Kashan area within the study period, the value remains higher than the mean level of Eastern Mediterranean region and the global average, which is a notable fact for policymakers and authorities.

目的:研究大豆提取物抗戊四唑所致卵巢切除大鼠痉挛的作用。方法:雌性Wistar大鼠随机分为4组(n=15),分别为假手术组、卵巢切除组、低剂量大豆提取物组和高剂量大豆提取物组。假手术组大鼠行假手术,其余3组大鼠行卵巢切除术。每组大鼠再随机分为2个亚组,其中一个亚组(n=7)每日予低剂量戊四唑(40mg/kg)腹腔内注射,连续14d;另一亚组(n=8)予一次高剂量戊四唑(90mg/kg)腹腔内注射。在戊四唑注射前30min,低剂量大豆提取物(20mg/kg)及高剂量大豆提取物(60mg/kg)组分别腹腔内注射大豆提取物,假手术组及卵巢切除组注射生理盐水。上述步骤完成后,将大鼠置于树脂玻璃箱中观察60min。结果:重复低剂量注射戊四唑14d,卵巢切除组大鼠的痉挛评分在第3、5、8、10、11、12及13天低于假手术组(P<0.05或P<0.01),而低剂量大豆提取物组和高剂量大豆提取物组大鼠的痉挛评分高于卵巢切除组(P<0.05或P<0.01)。一次性高剂量注射戊四唑后,与假手术组相比,卵巢切除组大鼠阵发性强直性抽搐潜伏期延长,而最小阵发性痉挛潜伏期无明显变化;与卵巢切除组相比,低剂量及高剂量的大豆提取物显著缩短了大鼠阵发性强直性抽搐及最小阵发性痉挛的潜伏期(P<0.01)。结论:雌激素能够影响戊四唑所致大鼠痉挛的严重程度,而大豆中所含的植物雌激素也具有类似作用,这种作用还有待更多的研究予以证实。

The prevalence of coronary artery disease (CAD) is increasing in Iran. Patients with depression who have a myocardial infarction are more likely to die and patients who have depressive symptoms during hospitalization may have increased cardiovascular events. This study aimed to determine the relationship between anxiety, depression and coronary artery disease among patients undergoing angiography in Ghaem Hospital, Mashhad. This was a case-control study conducted between September 2011 and August 2012 among patients undergoing coronary angiography in Ghaem Hospital, Mashhad, Iran. There were 486 cases that were found to have one or more coronary stenoses, with a stenosis of equal or more than 50% of the diameter of at least one major coronary artery. The other patient group consisted of the patients in whom the coronary artery stenosis was less than 50% in diameter which was classified as angiography negative, and a control group that consisted of 440 healthy adults aged 18 years old and above who were selected among people who attended for routine medical checkup and medical examination of employment. The dependent variables were Beck Anxiety and Depression Inventory scores and the independent factors were coronary artery disease, and socioeconomic profiles. Validated and reliability-tested questionnaires were used for data collection. The mean age of patients was 55.75 ± 10.64 years and in the healthy group was 55.83 ± 8.55 years;there was no significant difference in age between subject groups (p = 0.897) nor a significant difference in the gender frequency distribution of subjects (p = 0.610). Purpose:Injuries are one of the leading causes of death and lead to a high social and financial *** patterns can vary significantly among different age groups and body *** study aimed to evaluate the relationship between mechanism of injury,patient comorbidities and severity of ***:The study included trauma patients from July 2016 to June 2018,who were admitted to Sina Hospital,Tehran,*** inclusion criteria were all injured patients who had at least one of the following:hospital length of stay more than 24 h,death in hospital,and transfer from the intensive care unit of another *** collection was performed using the National Trauma Registry of Iran minimum ***:The most common injury mechanism was road traffic injuries(49.0%),followed by falls(25.5%).The mean age of those who fell was significantly higher in comparison with other mechanisms(p<0.001).Severe extremity injuries occurred more often in the fall group than in the vehicle collision group(69.0%vs.43.5%,p<0.001).Moreover,cases of severe multiple trauma were higher amongst vehicle collisions than injuries caused by falls(27.8%vs.12.9%,p=0.003).Conclusion:Comparing falls with motor vehicle collisions,patients who fell were older and sustained more extremity *** injured by motor vehicle collision were more likely to have sustained multiple trauma than those presenting with *** of the relationship between mechanisms and consequences of injuries may lead to more effective interventions.
