Side effects of cisplatin,especially dose-dependent nephrotoxicity,are major factors limiting its use in ***((S)-2,9-dihydroxy-1,10-dimethoxy-aporphine)is a natural alkaloid known for its strong antioxidant activity present in leaves/bark of boldo tree(Peumus boldus Molina),a native tree in ***,we aimed to investigate the nephroprotective effect of boldine and its underlying mechanisms on cisplatin-induced rat renal *** Wistar albino rats divided into 5 groups(Control,Cis,Bold.40,Cis+Bold.20,Cis+Bold.40 groups)were *** received boldine(20 or 40 mg/kg/day),or vehicle(saline)intraperitoneal for 14 days and a single dose cisplatin(7 mg/kg,ip)was applied on the 10th day to induce *** and kidney tissue were weighed to determine kidney *** urea nitrojen(BUN)and creatinine levels,the amount of thiobarbituric acid reactive substances(TBARS,an index of lipid peroxidation),superoxide dismutase(SOD),glutathione peroxidase(GPx)enzyme activities and tumor necrosis factor alpha(TNF-α)levels were measured and histopathologic examination was *** nitric oxide synthase(iNOS)and caspase-3 expressions were detected *** induced by cisplatin was apparent by elevated levels of BUN,creatinine,kidney index,TBARS and TNF-α,and decreased body weight,SOD and GPx enzyme *** with boldine protected the renal function at both boldine doses by fixing the renal damage markers,oxidative stress,caspase-3 and iNOS *** findings supported biochemical *** together these findings indicate that boldine has promising protective effect against cisplatin nephrotoxicity by improving oxidative stress,inflammation,histopathological alterations and by alleviating caspase 3 expression.

Impact of admission blood glucose levels on prognosis of elderly patients with ST elevation myocardial infarction treated by primary percutaneous coronary intervention

OBJECTIVE:Admission hyperglycemia in acute myocardial infarction (MI) is related with increased in-hospital and long term mortality and major cardiac adverse events。 We aimed to investigate how admission hyperglycemia affects the short and long term outcomes in elderly patients >65 years) after primary percutaneous coronary intervention for ST elevation myocardial infarction。 METHODS:We retrospectively analyzed 677 consecutive elderly patients (mean age 72.2 ±5.4)。 Patients were divided into two groups according to admission blood glucose levels。 Group : low glucose group (LLG), glucose 168 mg/dL。 RESULTS:In-hospital, long term mortality and in-hospital major adverse cardiac events were higher in the high admission blood glucose group (P 1, post-thrombolysis in MI <3 and admission blood glucose levels were independent predictors of in-hospital adverse cardiac events (P <0.001)。 CONCLUSIONS:Admission hyperglycemia in elderly patients presented with ST elevation myocardial infarction is an independent predictor of in-hospital major adverse cardiac events and is associated with in-hospital and long term mortality。

Background： Metabolic syndrome （MetS） is one of the high cardiovascular （CV） situations. Endothelial dysfunction, which is a common finding in patients with MetS, is related with increased CV risk. In patients with MetS, the effect of the major CV risk factors, not included in the MetS definition, on endothelial dysfunction is not well known. The aim of this study was to determine the effect of major CV risk factors such as gender, smoking, family history, and biochemical parameters on endothelial dysfunction in patients with MetS. Methods： The study was performed between December 2010 and August 2014. A total of 55 patients （ 15 females and 40 males） with MetS and 81 healthy controls （37 females and 44 males） with a body mass index 〈25 kg/m2 were enrolled in the study. Endothelial dysfunction was measured by flow-mediated dilatation （FMD）, oxidative stress parameters; high-sensitivity C-reactive protein （hs-CRP）, oxidized low-density lipoprotein （ox-LDL）, endothelial nitric oxide synthase （e-NOS）, nitric oxide, and cell adhesion markers; yon Willebrand factor, and e-selectin. Platelet aggregation （endothelial adenosine diphosphate）, total platelet count, and mean platelet volume were additionally analyzed and demographic parameters were explored. Student＇s t-test, Mann-Whitney U-test, and Chi-square test were used to analyze the results. Results： The fasting blood glucose （z = 3.52, P = 0.001 ）, hs-CRP （z - 3.23, P = 0.004）, ox-LDL （z = 2.62, P= 0,013）, and e-NOS （z = 2.22, P = 0.026） levels and cardiac risk score （z - 5.23, P 〈 0.001） were significantly higher in patients with MetS compared with the control group. Smoking was correlated with decreased FMD （χ2 = 9.26, P = 0.002） in MetS patients but not in the control group. Conclusions： Increased ox-LDL, hs-CRP, and e-NOS are likely to be a result of oxidative stress, a condition in which an imbalance occurs between the production and inactivation of reactive nitrogen and oxygen species. Efficacy of ankaferd blood stopper application on nonvariceal upper gastrointestinal bleeding

AIM:To prospectively assess the hemostatic efficacy of the endoscopic topical use of ankaferd blood stopper(ABS) in active non-variceal upper gastrointestinal system(GIS) ***:Endoscopy was performed on 220 patients under suspiciency of GIS *** with active non-variceal upper gastrointestinal bleeding(NVUGIB) with a spurting or oozing type were ***,8-10 cc of isotonic saline was sprayed to bleeding ***,8 cc of ABS was applied on lesions in which bleeding continued after isotonic saline *** other endoscopic therapeutic methods were applied on the lesions in which the bleeding did not stop after ***:Twenty-seven patients had an active NVUGIB with a spurting or oozing type and 193 patients were excluded from the study since they did not have non-variceal active bleeding.8 cc of ABS was sprayed on to the lesions of 26 patients whose bleeding continued after isotonic saline and in 19 of them,bleeding stopped after *** endoscopic treatment methods were applied to the remaining patients and the bleeding was stopped with these interventions in 6 of 7 ***:ABS is an effective method on NVUGIB,particularly on young patients with no *** may be considered as part of a combination treatment with other endoscopic methods.
