Outcome of pancreaticoduodenectomy with extended retroperito- neal lymphadenectomy for adenocarcinoma of the head of the pancreas

Background Nowadays, there is a remarkable rise in resectability rate of periampullary adenocarcinoma and the mortality and morbidity of the pancreaticoduodenectomy procedure have been reduced remarkably, while the 5 year survival rates of patients with carcinoma of the head of the pancreas are still below 25%. We conducted this retrospective study to evaluate the clinical outcome of radical pancreaticoduodenectomy with extended retroperitoneal lymphadenectomy as a surgical therapy for adenocarcinoma of the head of the pancreas. Methods Twenty cases with adenocarcinoma of the head of the pancreas were treated by standard pancreaticoduodenectomy （removing only the peripancreatic lymph nodes en bloc with the tumour） from 1994 to 1997, and 46 cases with the same disease underwent extended retroperitoneal lymphadenectomy associated with standard pancreaticoduodenectomy from 1998 to 2002. The patients for whom there were insufficient follow-up data, or who had received postoperative adjuvant therapy, were excluded from the analysis. Clinical and pathological parameters of both groups were reviewed. The postoperative morbidity, mortality and survival data were compared statistically. Results Demographic and histopathological characteristics were similar in the two groups of patients. Performance of the extended lymphadenectomy lengthened the procedure. The mean total number of lymph nodes resected was significantly higher in the radical group （ P〈0. 05 ）. Of the 46 cases in the radical group, 26% （12/46） had metastatic adenocarcinoma in the resected retroperitoneal lymph nodes. There was one perioperative death in the standard group, and two in the radical group. Postoperative diarrhoea and lymphatic leakage were only observed in the radical group. Transfusion requirements and postoperative morbidity rates did not differ between the two groups. First-line chemoimmunotherapy(with or without bevacizumab)has improved outcomes in advanced non-small cell lung cancer(NSCLC).Here,this open-label,multi-cohort phase II study(NCT05329025)was done to investigate the safety and efficacy of QL1706(a single bifunctional MabPair product against PD-1 and CTLA-4)and chemotherapy with or without bevacizumab in this *** were enrolled into five different cohorts based on genotype(cohorts 1-4,epidermal growth factor receptor[EGFR]wild-type;cohort 5,EGFR-mutant and progressed on EGFR-tyrosine kinase inhibitors[TKIs]).Between June 11,2021 and December 29,2021,91 patients were *** frequent treatment-related adverse events(TRAEs)included decreased appetite(60[65.9%]),anemia(60[65.9%]),infusion-related reactions(48[52.7%]),and pruritus(44[48.4%]).Grade≥3 TRAEs occurred in 30(33.0%)***-seven(45%)patients with wild-type EGFR achieved partial response(PR)(objective response rate[ORR]=45%)and had a median progression-free survival(mPFS)of 6.8 months(95%CI:5.2-9.7).For 31 patients harboring mutated EGFR,17(54.8%)achieved PR(ORR=54.8%),with an mPFS of 8.5 months(95%CI:5.72-not evaluable).Overall,QL1706 plus chemotherapy,regardless of having bevacizumab,was generally tolerable and had promising antitumor activity for EGFR wild-type advanced NSCLC in first-line ***,QL1706 plus chemotherapy and bevacizumab showed favorable antitumor activity for patients who had EGFR mutated NSCLC but failed in TKI therapy,demonstrating a potential for treating this population.

Fabrication of surface passivated two-dimensional MFI zeolite for alkylation between toluene with methanol

Stability of water yield in watersheds

The hydrological system of a watershed is intricately influenced by both underlying characteristics and climate *** the variability in water yield is essential for effective water resources management and water security in the context of changing *** this study,we adopted the Budyko framework and leveraged simulations from the CMIP6 model to investigate the compensation effects of climate and underlying characteristics on watershed water *** on Taylor expansion and Budyko framework,we estimated the sensitivity of watershed water yield to climate and underlying characteristics(the first-and second-order partial derivatives).By combining external watershed characteristics(e.g.,water yield ratios and underlying characteristics)with internal sensitivity coefficients,this study further used vine copula and principal component analysis to quantify the stability of watershed water *** findings show:(1)Water-yield changes related to underlying characteristics could be offset by climate-related water-yield changes across all climate zones,maintaining the water yield ratio steady(i.e.,the compensation effects).(2)However,global watersheds will turn more sensitive to underlying characteristics and less sensitive to climate variation in the *** climate-and underlying-related sensitivities increase in watersheds with arid climates.(3)The stability of watershed water yield will gradually diminish in the *** 1901–1950to 2051–2100,the global stability of 280 watersheds drops from 0.054 to 0.021(i.e.,stability index identified by the joint probability).Particularly,the largest change in stability of water yield reaches-0.347±0.18 in arid *** semi-arid,semihumid,and humid regions,the changes are-0.039±0.010,-0.028±0.005,and-0.005±0.002,*** findings provide a reference for the future sustainable water resources development under climate change,highlighting the vulnerability of the water resources in ari

Underdetermined direction of arrival estimation with nonuniform linear motion sampling based on a small unmanned aerial vehicle platform

Uniform linear array(ULA)radars are widely used in the collision-avoidance radar systems of small unmanned aerial vehicles(UAVs).In practice,a ULA's multi-target direction of arrival(DOA)estimation performance suffers from significant performance degradation owing to the limited number of physical *** improve the underdetermined DOA estimation performance of a ULA radar mounted on a small UAV platform,we propose a nonuniform linear motion sampling underdetermined DOA estimation *** the motion of the UAV platform,the echo signal is sampled at different ***,according to the concept of difference co-array,a virtual ULA with multiple array elements and a large aperture is synthesized to increase the degrees of freedom(DOFs).Through position analysis of the original and motion arrays,we propose a nonuniform linear motion sampling method based on ULA for determining the optimal *** the condition of no increase in the aperture of the physical array,the proposed method obtains a high DOF with fewer sampling runs and greatly improves the underdetermined DOA estimation performance of *** results of numerical simulations conducted herein verify the superior performance of the proposed method.

Tuning the solubility of polysulfides for constructing practical lithium-sulfur battery

Experimental and numerical analyses of the effect of fibre content on the close-in blast performance of a UHPFRC beam

Limited research has been conducted on the influences of fiber content on close-in blasting characteristics for ultrahigh-performance fiber-reinforced concrete(UHPFRC)*** paper aims to address this knowledge gap through experimental and mesoscale numerical *** were conducted on ten UHPFRC beams built with varying steel fiber volumetric fractions subjected to close-in explosive ***,this study considered other parameters,such as the longitudinal reinforcement type and *** the case of UHPFRC beams featuring normal-strength longitudinal reinforcement of diametersΦ12,Φ16,andΦ20,a reduction in maximum displacement by magnitudes of19.6%,19.5%,and 17.4%was observed,respectively,as the volumetric fractions of fiber increased from1.0%to 2.5%.In addition,increasing the longitudinal reinforcement ratio and using high-strength steel longitudinal reinforcement both significantly reduced the deformation characteristics and increase the blasting resistances of UHPFRC ***,the effects on the local crushing and spalling damage were not significant.A mesoscale finite element model,which considers the impacts of fiber parameters on UHPFRC beam behaviors,was also established and well correlated with the test ***,parametric analyses were further conducted to examine the impacts of the steel fiber content and length and the hybrid effects of various types of microfibers and steel fibers on the blasting performance of UHPFRC beams.
