"Five steps four quadrants" modularized en bloc dissection technique for accessing hepatic hilum lymph nodes in laparoscopic pancreaticoduodenectomy

BACKGROUND Although en bloc dissection of hepatic hilum lymph nodes has many advantages in radical tumor treatment,the feasibility and safety of this approach for laparo-scopic pancreaticoduodenectomy(LPD)require further clinical evaluation and *** To explore the application value of the"five steps four quadrants"modularized en bloc dissection technique for accessing hepatic hilum lymph nodes in LPD *** A total of 52 patients who underwent LPD via the"five steps four quadrants"modularized en bloc dissection technique for hepatic hilum lymph nodes from April 2021 to july 2023 in our department were analyzed *** patients'body mass index(BMI),preoperative laboratory indices,intraoperative variables and postoperative complications were *** relationships between preoperative data and intraoperative lymph node dissection time and blood loss were also *** Among the 52 patients,36 were males and 16 were females,and the average age was 62.2±11.0 *** were 26 patients with pancreatic head cancer,16 patients with periampullary cancer,and 10 patients with distal bile duct *** BMI was 22.3±3.3 kg/m²,and the median total bilirubin(TBIL)concentration was 57.7(16.0-155.7)µmol/*** patients successfully underwent the"five steps four quadrants"modularized en bloc dissection technique without lymph node clearance-related complications such as postoperative bleeding or lymphatic *** analysis revealed significant associations between preoperative BMI(r=0.3581,P=0.0091),TBIL level(r=0.2988,P=0.0341),prothrombin time(r=0.3018,P=0.0297)and lymph node dissection ***,dissection time was significantly correlated with intraoperative blood loss(r=0.7744,P<0.0001).Further stratified analysis demonstrated that patients with a preoperative BMI≥21.9 kg/m²and a TIBL concentration≥57.7μmol/L had significantly longer lymph node dissection times(both P<0.05).CONCLUSION The"five steps four quadrants

Aging increases the risks of various diseases and the vulnerability to *** senescence is a hallmark of aging that contributes greatly to aging and aging-related *** study demonstrates that extracellular vesicles from human urine-derived stem cells(USC-EVs)efficiently inhibit cellular senescence in vitro and in *** intravenous injection of USC-EVs improves cognitive function,increases physical fitness and bone quality,and alleviates aging-related structural changes in different organs of senescence-accelerated mice and natural aging *** anti-aging effects of USC-EVs are not obviously affected by the USC donors’ages,genders,or health *** analysis reveals that USC-EVs are enriched with plasminogen activator urokinase(PLAU)and tissue inhibitor of metalloproteinases 1(TIMP1).These two proteins contribute importantly to the anti-senescent effects of USC-EVs associated with the inhibition of matrix metalloproteinases,cyclin-dependent kinase inhibitor 2A(P16INK4a),and cyclin-dependent kinase inhibitor 1A(P21cip1).These findings suggest a great potential of autologous USC-EVs as a promising anti-aging agent by transferring PLAU and TIMP1 proteins.

Effects of the urban landscape on heatwave-mortality associations in hong Kong: comparison of different heatwave definitions

Despite increased attention given to potential modifiers of temperature-mortality associations, evidence for variations between different urban landscape characteristics remains limited. It is in this context that in this paper effect modifications of multiple urban landscape characteristics are explored under different heatwave definitions for different age groups and gender in hong Kong, China. Daily meteorological data and heatwave-related mortality counts from 2008 to 2017 were collected from the hong Kong Census and Statistics Department, China. A case-only design was adopted, combined with logistic regression models to examine the modification effects of five urban landscape characteristics under six heatwave definitions. Stratified analyses were conducted to investigate age- and gender-specific effect modifications. It is found that individuals living in greener areas experienced lower levels of mortality during or immediately after heatwaves. In contrast, a higher building density and nighttime land surface temperature (LST) were associated with a higher heatwave-related mortality risk. Pronounced effect modifications of these urban landscape characteristics were observed under hotter and longer heatwaves, and in older adults (age ≥ 65 years) and males. The findings provide a scientific basis for policymakers and practitioners when considering measures for coping with hotter, longer, and more frequent heatwaves in the context of global climate change.

Temperature is an important environmental factor affecting heading date of *** its importance,genes responsible for temperature-sensitive heading in rice have remained *** previous study identified a quantitative trait locus qHd1 which advances heading date under high temperatures.A 9.5-kb insertion was found in the first intron of OsMADS51 in indica variety Zhenshan 97(ZS97).However,the function of this natural variant in controlling temperature sensitivity has not been *** this study,we used CRISPR/Cas9 to knock out the 9.5-kb insertion in *** conducted under cotrolled conditions in phytotrons confirmed that deletion increased temperature sensitivity and advanced heading by downregulating the expression level of ***-hybrid assays in yeast,ChIP-quantitative polymerase chain reaction,electrophoretic mobility shift,and luciferase-based transient transactivation assays collectively confirmed that OsMADS51 affects heading date by regulation of heading date gene *** further determined that the long non-coding RNA HEATINR is generated from the first intron of OsMADS51,offering an explanation for how the 9.5-kb insertion affects temperature *** also found that OsMADS51 was strongly selected in early/late-season rice varieties in South China,possibly accounting for their strong temperature *** insights not only advance our understanding of the molecular mechanisms underlying the temperature-responsive regulation of heading date in rice but also provide a valuable genetic target for molecular breeding.
