BACKGROUND We invented Endoscopic Ruler,a new endoscopic device to measure the size of varices in patients with cirrhosis and portal *** To assess the feasibility and safety of Endoscopic Ruler,and evaluate the agreement on identifying large oesophageal varices(OV)between Endoscopic Ruler and the endoscopists,as well as the interobserver agreement on diagnosing large OV using Endoscopic *** We prospectively and consecutively enrolled patients with cirrhosis from 11 hospitals,all of whom got esophagogastroduodenoscopy(EGD)with Endoscopic *** primary study outcome was a successful measurement of the size of varices using Endoscopic *** secondary outcomes included adverse events,operation time,the agreement of identifying large OV between the objective measurement of Endoscopic Ruler and the empirical reading of endoscopists,together with the interobserver agreement on diagnosing large OV by Endoscopic *** From November 2020 to April 2022,a total of 120 eligible patients with cirrhosis were recruited and all of them underwent EGD examinations with Endoscopic Ruler successfully without any adverse *** median operation time of Endoscopic Ruler was 3.00 min[interquartile range(IQR):3.00 min].The kappa value between Endoscopic Ruler and the endoscopists while detecting large OV was 0.52,demonstrating a moderate *** kappa value for diagnosing large OV using Endoscopic Ruler among the six independent observers was 0.77,demonstrating a substantial *** The data demonstrates that Endoscopic Ruler is feasible and safe for measuring the size of varices in patients with cirrhosis and portal *** Ruler is potential to promote the clinical practice of the two-grade classification system of OV.

目的通过人类白细胞抗原（HLA）-B基因检测分析探讨广西地区范围哮喘儿童HLA-B等位基因遗传分布情况。方法将居住广西南宁地区无血缘关系的84例哮喘儿童和168例无哮喘和特应，性痰病的健康个体为对照组纳入研究。采用Pharmacia UniCAP系统检测哮喘儿童的血清总IgE水平，同时完成10种吸入性过敏原皮肤实验和肺功能检查。应用基因芯片法检测HLA-B的27个等位基因位点。计算疾病组和对照组的基因频率=基因出现频次数／（每组个体总数×2）×100％，进行χ^2检验和危险性分析。结果支气管哮喘组HLA-B56等位基因和HLA-B58等位基因频率（分别为5．36％和14．28％）明显高于健康对照组（分别为0．89％和8．04％），（χ^2值为7．973，P〈0．01和5．421，P〈0．05），比值比（OR）分别为6．6（95％可信限1．737～25．076）和2．09（95％可信限1．115～3．912）；而HLA-1346等位基因和HLA-B61等位基因频率在哮喘组（分别为6．55％和1．19％）低于对照组（分别为12．80％和4．76％），（χ^2值为5．197和4．308，P〈0．05），OR分别为0．438（95％可信限0．213～0．902）和0．232（95％可信限0．055～1．100）。结论HLA-B56等位基因和HLA-B58等位基因与广西南宁地区哮喘儿童易感性相关；HLA-B46等位基因对机体则可能有保护性。

目的探讨广西地区哮喘儿童人类白细胞抗原（HLA）-DR位点等位基因的遗传分布情况。方法将居住广西南宁地区无血缘关系的84例哮喘儿童（哮喘组）及168名无哮喘和特应性疾病的健康人（健康对照组）纳入研究。采用Pharmacia Un iCAP系统检测哮喘儿童血清总IgE水平,同时完成10种吸入性过敏原皮肤点刺实验和肺功能检查。应用基因芯片法检测HLA-DR的21个基因位点。结果哮喘组HLA-DR B1＊070X基因和HLA-DR B1＊11XX基因频率（2.98%、13.69%）高于对照组（0.3%、5.95%）（χ^2=6.915,9.478 P〈0.05,0.01）,优势比（OR）分别为10.57（95%可信限1.215-91.986）和2.79（95%可信限1.429-5.449）;HLA-DR B3（52）＊010X基因频率哮喘组（7.14%）显著低于对照组（13.99%）（χ^2=5.854 P〈0.05）,OR为0.429（95%可信限0.214-0.862）。结论HLA-DR B1＊070X基因和HLA-DR B1＊11XX基因与广西南宁地区哮喘儿童易感性相关;而HLA-DR B3（52）＊010X基因对机体可能有保护作用。

目的探索具有抗血管生成诱导血管正常化、抗炎、免疫调节作用的沙利度胺是否能够减轻VEGFR-TKI阿帕替尼的不良反应。方法本研究回顾性分析54例骨与软组织肉瘤患者,分为两组,接受阿帕替尼单药500 mg每天1次和沙利度胺100 mg每晚1次联合阿帕替尼500 mg每天1次治疗各27例,根据CTCAE 5.0评价药物不良反应,对比两组患者不良反应。结果总不良反应率:对照组为32.51%,实验组为15.23%,差异有统计学意义(P<0.001)。Ⅲ级及以上不良事件总发生率:对照组为2.26%,实验组为0%,差异有统计学意义(P<0.001)。白细胞减少:对照组48.15%,实验组18.52%,差异有统计学意义(P=0.020);对照组Ⅲ级及以上不良反应7.41%。血小板减少:对照组25.93%,实验组3.70%,差异有统计学意义(P=0.023);对照组Ⅲ级及以上不良反应率3.70%。腹泻:对照组40.74%,实验组14.81%,差异有统计学意义(P=0.036)。蛋白尿:对照组74.07%,实验组29.63%;差异有统计学意义(P=0.001);对照组Ⅲ级及以上不良反应率7.41%。手足综合征:对照组62.96%,实验组33.33%;差异有统计学意义(P=0.015);对照组Ⅲ级及以上的不良反应率为14.81%。结论沙利度胺能有效缓解阿帕替尼引起的腹泻、肾功能不良、蛋白尿等不良反应,提高患者对药物的耐受性。

Background:The scored Patient-Generated Subjective Global Assessment(PG-SGA)has been widely used to assess the nutritional status of cancer *** purpose of this study is to compare the differences in PG-SGA scores and the 7 domain scores of the PG-SGA in male and female cancer ***:This study was conducted at 72 hospitals from July 2013 to December 2018,a part of the Investigation on Nutritional Status and its Clinical Outcomes of Common *** PG-SGA was recorded to evaluate the nutritional status of patients.A total of 19,528 patients with 13 common malignancies were included in this *** t test and the χ^(2) test were applied to analyze the sex diferences in the 7 domain *** Cancer Genome Atlas(TCGA)database was used to analyze the expression levels of symptom-related ***:There were significant sex dfferences in the PG-SGA(P=0.032),notably in patients with gastric cancer(male vs female:9.09±4.86 vs 9.58±5.07,P=0.005)and esophageal cancer(9.64±4.90 vs 10.46±4.96,P=0.011)and the average total PG-SGA of female patients was slightly higher than that of male patients(7.64±4.98 vs 7.77±5.14).The differences were mainly related to the weight,eating,symptom,as well as activity and physical function scores in the stratified *** causes of the sex differences were the rates of nausea,vomiting,dry mouth,and other symptoms,in both gastric and esophageal cancer *** of the TCGA database suggested that most of the related genes were sex neutral,except for genes related to dysphagia in gastric cancer(VEGFC was higher in female patients,VEGFA and VEGFB higher in male patients).Conclusions:There are sex differences in the PG-SGA scores in patients with various tumor types(female patients generally had higher scores than male patients),with differences mainly in the weight,eating,symptom,as well as activity and physical function *** sex differences in PG-SGA scores might be due to the differences i

通过制备15批厚朴温中汤物质基准冻干粉样品,测定其特征图谱、指标成分含量及出膏率,明确特征峰的归属及其与对照图谱的相似度范围,厚朴酚、橙皮苷、甘草酸、乔松素的含量范围及转移率范围,出膏率范围及变化幅度范围等。结果表明物质基准特征图谱与15批物质基准生成的对照图谱R的相似度大于0.90;明确除茯苓外其余药味在全方特征图谱中特征峰的归属,共归属19个特征峰,经对照品指认12个特征峰信息,其中,4个来自姜厚朴、5个来自橘红、2个来自木香、3个来自炙甘草、4个来自草豆蔻、1个来自干姜、生姜;15批物质基准指标成分含量范围及转移率范围分别为橙皮苷0.80%~1.14%,20.25%~39.61%,甘草酸0.49%~0.79%,23.09%~33.87%,乔松素0.03%~0.07%,3.55%~10.09%,厚朴酚0.15%~0.38%,8.08%~24.35%;出膏率范围及出膏率变化幅度分别为22.60%~25.57%,12.67%~23.68%。该研究引入特征图谱、指标成分含量、出膏率等概念,探索厚朴温中汤物质基准量值传递关系,初步拟定厚朴温中汤物质基准的质量标准,为类似方剂的开发研究提供思路。

Unintended pregnancy is a global issue, with approximately 85,000,000 women around the world having unintended pregnancy annually. The contents of clinical application of women’s contraceptive methods are very wide, involving multiple areas. This consensus deeply discusses the specific contraceptive needs at different statuses, combined with gynecological diseases, postabortion contraception, and postpartum family planning, ensuring the correct use of contraceptive methods under the corresponding status. The top priority of the consensus is the specific contraception consensus section for women combined with gynecological diseases because medical treatment effect as well as side effects should be weighed carefully. The consensus is to make high-efficiency and individual contraceptive strategy for different groups based on multidisciplinary(gynecology, obstetrics, and family planning) and multidimensional aspects, which can provide uniform guidance for medical and health organizations under the condition as relevant global guidance or consensus is still lacking.

The prevalence of malnutrition is high among oncology patients in China. Although the Patient- Generated Subjective Global Assessment (PG-SGA) is widely recognized as an important nutritional assessment tool, it has not been validated for Chinese cancer patients. The purpose of this study was to conduct an analysis of nutrition screening and assessment data to validate the Chinese version of the PG-SGA in Chinese patients with lung cancer. This was an observational, cross-sectional study of 2,000 consecutive adult patients with lung cancer treated at several tertiary hospitals in China. Anthropometric and patient descriptive data were collected. The PG-SGA generated a score for the nutritional risk and a global rating for the nutritional status. The internal consistency, external consistency, and construct validity were evaluated, and a known-groups comparison and principal component analyses were conducted to evaluate the reliability and validity of the PG-SGA. The known-groups comparison demonstrated the ability of the PG-SGA to differentiate lung cancer patients of different nutritional and body mass index (BMI) groups. A confirmatory factor analysis supported the original four-factor structure of the PG-SGA. In terms of the internal and external consistency, the PG-SGA demonstrated intra-class correlation coefficients (ICC) up to 0.987. Alternative form validity was also demonstrated between Box 4 of the PG-SGA and the Karnofsky Performance Status score, with excellent external consistency (r = 0.972, P < 0.001). The construct validity was supported, and selected questionnaire dimensions were evident in the principal component analysis. Significant Spearman correlations (P < 0.001) were demonstrated. The Chinese version of the PG-SGA demonstrated significant reliability and sufficient exploratory properties to support its validity.
