BACKGROUND Heart transplantation is recommended for the treatment of patients with refractory heart *** pain after heart transplantation is usually considered noncardiac owing to the denervated ***,data from case reports on tacrolimus-induced achalasia after heart transplantation are *** aimed to present a case of tacrolimus-induced achalasia that developed after heart tran-splantation,which was successfully relieved by laparoscopic Heller *** SUMMARY A 67-year-old man with a history of Type 2 diabetes mellitus,hyperlipidemia,and dilated cardiomyopathy had congestive heart failure following orthotopic heart transplantation with tacrolimus treatment 12 years *** the 10-year follow-up after the heart transplantation,the patient presented with persistent cough,dysphagia,heartburn,and retrosternal chest pain lasting for 2 *** endoscopy revealed no specific *** years later,the patient experienced the same symptoms,including chest pain lasting for 4 *** and manometry confirmed the presence of *** reports showed that discontinuing calcineurin inhibitor(CNI)treatment and endoscopic botulinum toxin injection could treat CNI-induced *** to the risk of rejection of the transplanted heart and considering the temporary benefits of botulinum toxin injection in achalasia,the patient underwent laparoscopic Heller *** was relieved without *** months later,he had no signs of recurrence of the *** In transplant patients with chest pain and gastrointestinal symptoms, CNIinducedachalasia may be one of the differential diagnoses. Esophagogram/manometry is useful for diagnosis.

Isolated gastric variceal bleeding caused by splenic lymphoma-associated splenic vein occlusion

Isolated gastric varices(IGV) can occur in patients with left-sided portal hypertension resulting from splenic vein occlusion caused by thrombosis or stenosis. In left-sided portal hypertension,blood flows retrogradely through the short and posterior gastric veins and the gastroepiploic veins,leading to the formation of an IGV. The most common causes of splenic vein occlusion are pancreatic diseases,such as pancreatic cancer,pancreatitis,or a pseudocyst. However,various other cancers,such as colon,gastric,or renal cancers,have also been known to cause splenic vein occlusion. Our patient presented with a rare case of IGV bleeding induced by splenic lymphoma-associated splenic vein occlusion. Splenectomy,splenic artery embolization,and stenting of the splenic vein are the current treatment choices. Chemotherapy,however,is an alternative effective treatment for splenic vein occlusion caused by chemotherapy-sensitive tumors. Our patient responded well to chemotherapy with a cyclophosphamide,hydroxydaunorubicin,oncovin,and prednisolone regimen,and the splenic vein occlusion resolved after the lymphoma regressed.

The safety of patients and the quality of medical care provided to them are vital for their *** study establishes a set of RFID(Radio Fre-quency Identiﬁcation)-based systems of patient care based on physiological sig-nals in the pursuit of a remote medical care *** RFID-based positioning system allows medical staff to continuously observe the patient's health and *** staff can thus respond to medical emergencies in time and appropriately care for the *** the COVID-19 pandemic broke out,the proposed system was used to provide timely information on the location and body temperature of patients who had been screened for the *** results of experiments and comparative analyses show that the proposed system is superior to competing systems in *** use of remote monitoring technology makes user interface easier to provide high-quality medical services to remote areas with sparse populations,and enables better care of the elderly and patients with mobility *** can be found from the experiments of this research that the accuracy of the position sensor and the ability of package delivery are the best among the other related *** presentation of the graphical interface is also the most cordial among human-computer interaction and the operation is simple and clear.

Software-as-a-service(SaaS)has received significant attention recently as one of three principal components of cloud computing,and it often deals with applications that run on top of a platform-as-a-service(PaaS)that in turn runs on top of infrastructure-as-a-service(IaaS).This paper provides an overview of SaaS including its architecture and major technical issues such as customization,multi-tenancy architecture,redundancy and recovery mechanisms,and ***,a SaaS system can have architecture relating to a database-oriented approach,middleware-oriented approach,service-oriented approach,or PaaS-oriented *** SaaS customization strategies can be used from light customization with manual coding to heavy customization where the SaaS system and its underlying PaaS systems are customized ***-tenancy architecture is an important feature of a SaaS and various trade-offs including security isolation,performance,and engineering effort need to be *** is important for a SaaS system to have multi-level redundancy and recovery mechanisms,and the SaaS system needs to coordinate these with the underlying PaaS ***,SaaS scalability mechanisms include a multi-level architecture with load balancers,automated data migration,and software design strategies.

Utilizing Blockchain Technology to Improve WSN Security for Sensor Data Transmission

This paper proposes a method for improving the data security of wireless sensor networks based on blockchain *** technology is applied to data transfer to build a highly secure wireless sensor *** this network,the relay stations use microcontrollers and embedded devices,and the microcontrollers,such as Raspberry Pi and Arduino Yun,represents mobile *** proposed system uses microcontrollers to facilitate the connection of various sensor *** adopting blockchain encryption,the security of sensing data can be effectively improved.A blockchain is a concatenated transaction record that is protected by *** section contains the encrypted hash of the previous section,the corresponding timestamp,and transaction *** transaction data denote the sensing data of the wireless sensing *** proposed system uses a hash value representation calculated by the Merkel-tree algorithm,which makes the transfer data of the system difficult to be tamped ***,the proposed system can serve as a private cloud data *** this study,the system visualizes the data uploaded by sensors and create relevant charts based on big data *** the webpage server of the proposed system is built on an embedded operating system,it is easy to model and visualize the corresponding graphics using Python or JavaScript programming ***,this study creates an embedded system mobile database and web server,which can utilize JavaScript program language and *** runtime environment to apply blockchain technology to mobile *** proposed method is verified by the experiment using about 1600 data *** results show that the possibility of data being changed is very small,and the probability of data being changed is almost zero.

With technological and social development in recent decades,people have begun pursuing more comfortable lives that frequently feature household pets that are treated like members of the *** average,one out of every three households has a *** has also led to the creation and growth of many businesses in the pet industry.A few companies have developed a system that allows busy office workers to remotely care for pets at home based on the Internet of Things and an intelligent adjustment *** owners of two dogs,the authors of this study observed their pets’living habits and recorded environmental conditions that appear suitable for *** data were then used to develop an automatic control system to care for *** observational data on the pets’habits and environment were written in a program in Arduino by using the ESP8266 Wi-Fi *** module and booster module control is the switch and setting of various household *** to the loop setting of the program,the system does not need to manually switch or adjust the electrical settings of the ***,the pet's living environment is assessed by using various *** use of Arduino programs helps develop a system that can automatically adjust the environment to one that is most suitable for the pet's comfort.

Enhancing Design of Visual-Servo Delayed System

A robust adaptive predictor is proposed to solve the time-varying and delay control problem of an overhead crane system with a stereo-vision servo. The predictor is based on the use of a recurrent neural network(RNN) with tapped delays, and is used to supply the real-time signal of the swing angle. There are two types of discrete-time controllers under investigation, i.e., the proportional-integral-derivative(PID) controller and the sliding controller. Firstly, a design principle of the neural predictor is developed to guarantee the convergence of its swing angle estimation. Then, an improved version of the particle swarm optimization algorithm, the parallel particle swarm optimization(PPSO) method is used to optimize the control parameters of these two types of controllers. Finally, a homemade overhead crane system equipped with the Kinect sensor for the visual servo is used to verify the proposed scheme. Experimental results demonstrate the effectiveness of the approach, which also show the parameter convergence in the predictor.
