Clinical differentiation of acute appendicitis and right colonic diverticulitis: A case-control study

BACKGROUND Acute right colonic diverticulitis (ARCD) is an important differential diagnosis of acute appendicitis (AA) in Asian countries because of the unusually high prevalence of right colonic diverticula. Due to qualitative improvement and the high penetration rate of computed tomography (CT) scanning in Japan, differentiation of ARCD and AA mainly depends on this modality. But cost, limited availability, and concern for radiation exposure make CT scanning problematic. Differential findings of ARCD from AA are based on several small studies that used univariate comparisons from Korea and Taiwan. Previous studies on clinical and laboratory differences between AA and ARCD are limited. AIM To determine clinical differences between AA and ARCD for differentiation of these two diagnoses by creating a logistic regression model. METHODS We performed an exploratory single-center retrospective case-control study evaluating 369 Japanese patients (age ≥ 16 years), 236 (64.0%) with AA and 133 (36.0%) with ARCD, who were hospitalized between 2012 and 2016. Diagnoses were confirmed by CT images. We compared age, sex, onset-to-visit interval, epigastric/periumbilical pain, right lower quadrant (RLQ) pain, nausea/vomiting, diarrhea, anorexia, medical history, body temperature, blood pressure, heart rate, RLQ tenderness, peritoneal signs, leukocyte count, and levels of serum creatinine, serum C-reactive protein (CRP), and serum alanine aminotrans-ferase. It is difficult for renewable energy resources to provide constant power with excellent quality for the grid system. This serial research proposes a power stabilization system with a pumped storage to guarantee power quality and capacity, while the outputs from the energy resources are at unstable and/or fluctuating conditions. The power stabilization system with a counter-rotating type pump-turbine unit was prepared and operated at the pumping and the turbine modes. The unit composed of the tandem impellers/runners connected to the inner and the outer armatures of the unique motor/generator. The experiments have verified that this type pump-turbine unit is reasonably effective to stabilize momentarily/instantaneously the fluctuating power from the renewable energy resources.

Technical Points Regarding New Enterostomy Formation for Incarcerated Stomal Prolapse in Loop Enterostomy

Aim: Incarcerated stomal prolapse is a rare complication of enterostomy. Numerous procedures have been described, such as additional laparotomy to fix the intraabdominal intestine in place, enterostomy revision, or correction of the prolapse following stoma creation. The authors report successful managements by stomal reconstruction and discuss several clinical points, including the techniques of surgical revision for incarcerated stomal prolapse in loop enterostomy. Patients: Case 1) A female infant weighing 2755 g was delivered at 34 weeks of gestation. On the first day after birth, a right supra-abdominal transverse incision of 10 cm in diameter was used for transverse loop colostomy in a cloacal malformation. Two centimeters of the stomal loop was approximated with sutures to prevent evisceration of the small intestine between the 2 limbs of the loop. Interrupted sutures of 5-0 absorbable monofilament secured the seromuscle of the colon to the peritoneum and fascia, and also to the skin. The distal limb of the colostomy prolapsed 11 months after birth. The physical findings revealed that 10 cm of the distal limb was intussuscepted. Case 2) A female infant weighing 2550 g was delivered at 39 weeks of gestation. A radiological examination by contrast enema showed no spastic rectum and colon, as in Hirschsprung’s disease. Under the laparotomy of a right supra-abdominal transverse incision of 5 cm in diameter, loop ileostomy was performed at 30cm on the proximal side of the cecum such as Case 1. Subsequently, the proximal limb of the ileostomy prolapsed 2 days after operation. The physical findings revealed that 10 cm of the proximal limb was intussuscepted. New enterostomy formation: Divided enterostomy was performed with 3-cm stitching of each limb. The stomal site was moved to the inside from the previous stomal site to oversew and fix by the rectal fascia. AIM To define clinical criteria to differentiate eosinophilic gastrointestinal disorder(Eo GD) in the esophagus. METHODS Our criteria were defined based on the analyses of the clinical presentation of eosinophilic esophagitis(Eo E), subepithelial eosinophilic esophagitis(s Eo E) and eosinophilic esophageal myositis(Eo EM), identified by endoscopy, manometry and serum immunoglobulin E levels(s-Ig E), in combination with histological and polymerase chain reaction analyses on esophageal tissue *** In five patients with Eo E, endoscopy revealed longitudinal furrows and white plaques in all, and fixed rings in two. In one patient with s Eo E and four with Eo EM, endoscopy showed luminal compression only. Using manometry, failed peristalsis was observed in patients with Eo E and s Eo E with some variation, while Eo EM was associated with hypercontractile or hypertensive peristalsis, with elevated s-Ig E. Histology revealed the following eosinophils per high-power field values. Eo E = 41.4 ± 7.9 in the epithelium and 2.3 ± 1.5 in the subepithelium; s Eo E = 3 in the epithelium and 35 in the subepithelium(conventional biopsy); Eo EM = none in the epithelium, 10.7 ± 11.7 in the subepithelium(conventional biopsy or endoscopic mucosal resection) and 46.8 ± 16.5 in the muscularis propria(peroral esophageal muscle biopsy). Presence of dilated epithelial intercellular space and downward papillae elongation were specific to Eo E. Eotaxin-3, IL-5 and IL-13 were overexpressed in Eo *** Based on clinical and histological data, we identified criteria, which differentiated between Eo E, s Eo E and Eo EM, and reflected a different pathogenesis between these esophageal Eo GDs.

Photochemical and thermal isomerization of various azobenzenes was systematically investigated to understand the correlation between the molecular structure and transcis isomerization characteristics of azobenzenes.A blue shift in π-π* absorption band of ortho-alkylated azobenzenes(1o and 2o) was observed together with a reduction in molar extinction coefficient(ε) in comparison with both meta-alkylated azobenzenes(4m and 5m) and 7p lacking the meta and ortho *** ortho-alkylated azobenzene,photochemical trans-to-cis isomerization and thermal back cis-to-trans isomerization in solution occurred slowly when compared with 4m,5m and *** half-life time of the cis form of 2o was found to be 380 h,which is about 8-50 times longer than those of comparable 4m,5m(43-13 h) and 7p(7 h).Furthermore,comparison of the molecular structure and isomerization characteristics of azobenzene thiol(2o and 5m) self-assembled monolayers on flat gold surfaces indicates that the trans-to-cis photoconversion in monolayer systems is influenced by steric hindrance and strong intermolecular interaction between azobenzene units.

Image quality and diagnostic performance of free-breathing diffusion-weighted imaging for hepatocellular carcinoma

To retrospectively evaluate the diagnostic performance of free-breathing diffusion-weighted imaging (FB-DWI) with modified imaging parameter settings for detecting hepatocellular carcinomas (HCCs). METHODSFifty-one patients at risk for HCC were scanned with both FB-DWI and respiratory-triggered DWI with the navigator echo respiratory-triggering technique (RT-DWI). Qualitatively, the sharpness of the liver contour, the image noise and the chemical shift artifacts on each DWI with b-values of 1000 s/mm2 were independently evaluated by three radiologists using 4-point scoring. We compared the image quality scores of each observer between the two DWI methods, using the Wilcoxon signed-rank test. Quantitatively, we compared the signal-to-noise ratios (SNRs) of the liver parenchyma and lesion-to-nonlesion contrast-to-noise ratios (CNRs) after measuring the signal intensity on each DWI with a b-factor of 1000 s/mm2. The average SNRs and CNRs between the two DWI methods were compared by the paired t-test. The detectability of HCC on each DWI was also analyzed by three radiologists. The detectability provided by the two DWI methods was compared using McNemar’s test. RESULTSFor all observers, the averaged image quality scores of FB-DWI were: Sharpness of the liver contour [observer (Obs)-1, 3.08 ± 0.81; Obs-2, 2.98 ± 0.73; Obs-3, 3.54 ± 0.75], those of the distortion (Obs-1, 2.94 ± 0.50; Obs-2, 2.71 ± 0.70; Obs-3, 3.27 ± 0.53), and the chemical shift artifacts (Obs-1, 3.38 ± 0.60; Obs-2, 3.15 ± 1.07; Obs-3, 3.21 ± 0.85). The averaged image quality scores of RT-DWI were: Sharpness of the liver contour (Obs-1, 2.33 ± 0.65; Obs-2, 2.37 ± 0.74; Obs-3, 2.75 ± 0.81), distortion (Obs-1, 2.81 ± 0.56; Obs-2, 2.25 ± 0.74; Obs-3, 2.96 ± 0.71), and the chemical shift artifacts (Obs-1, 2.92 ± 0.59; Obs-2, 2.21 ± 0.85; Obs-3, 2.77 ± 1.08). Computerized Scheme for Histological Classification of Masses with Architectural Distortions in Ultrasonographic Images

Architectural distortion is an important ultrasonographic indicator of breast cancer. However, it is difficult for clinicians to determine whether a given lesion is malignant because such distortions can be subtle in ultrasonographic images. In this paper, we report on a study to develop a computerized scheme for the histological classification of masses with architectural distortions as a differential diagnosis aid. Our database consisted of 72 ultrasonographic images obtained from 47 patients whose masses had architectural distortions. This included 51 malignant (35 invasive and 16 non-invasive carcinomas) and 21 benign masses. In the proposed method, the location of the masses and the area occupied by them were first determined by an experienced clinician. Fourteen objective features concerning masses with architectural distortions were then extracted automatically by taking into account subjective features commonly used by experienced clinicians to describe such masses. The k-nearest neighbors (k-NN) rule was finally used to distinguish three histological classifications. The proposed method yielded classification accuracy values of 91.4% (32/35) for invasive carcinoma, 75.0% (12/16) for noninvasive carcinoma, and 85.7% (18/21) for benign mass, respectively. The sensitivity and specificity values were 92.2% (47/51) and 85.7% (18/21), respectively. The positive predictive values (PPV) were 88.9% (32/36) for invasive carcinoma and 85.7% (12/14) for noninvasive carcinoma whereas the negative predictive values (NPV) were 81.8% (18/22) for benign mass. Thus, the proposed method can help the differential diagnosis of masses with architectural distortions in ultrasonographic images.
