We report here how a heterotopic penetrating peptic ulcer progressed to cause small bowel obstruction in a pa- tient with multiple previous negative investigations. The clinical presentation, radiographic features and patho- logical findings of this case are described, along with the salient lessons learnt. The added value of wireless cap- sule endoscopy （WCE） in such circumstances is debated.

The idea of using ultrashort X-ray pulses to obtain images of single proteins frozen in time has fascinated and inspired *** was one of the arguments for building X-ray free-electron *** to theory,the extremely intense pulses provide sufficient signal to dispense with using crystals as an amplifier,and the ultrashort pulse duration permits capturing the diffraction data before the sample inevitably *** was first demonstrated on biological samples a decade ago on the giant *** then,a large collaboration has been pushing the limit of the smallest sample that can be *** ability to capture snapshots on the timescale of atomic vibrations,while keeping the sample at room temperature,may allow probing the entire conformational phase space of *** we show the first observation of an X-ray diffraction pattern from a single protein,that of Escherichia coli GroEL which at 14 nm in diameter is the smallest biological sample ever imaged by X-rays,and demonstrate that the concept of diffraction before destruction extends to single *** the pattern,it is possible to determine the approximate orientation of the *** experiment demonstrates the feasibility of ultrafast imaging of single proteins,opening the way to single-molecule time-resolved studies on the femtosecond timescale.

最新研究表明贴身穿细美利奴羊毛的衣服对敏感型皮肤的皮疹有一定的治疗作用。为了确保这类过敏性皮肤穿着的舒适性,文章利用澳大利亚羊业合作研究中心的穿着试验数据和成衣,借助新近研发的羊毛舒适仪,对过敏性皮肤贴身穿轻薄毛织物的医用规格进行了研究。通过相关性分析发现:纯羊毛织物的刺痒性能很大程度上决定了穿着者的皮肤舒适度。进而通过使用多元变量主成分分析,辨别出人们对羊毛织物刺痒性能在不同的穿着环境和不同的活动强度下的感应模式,从而对织物的刺痒性能进行分类。最后通过对试穿服装的纤维细度和舒适度指数进行测试分析,确定了该类贴身穿毛织物的主要医用规格。包括舒适度指数不高于200,纤维的平均直径不大于17.5μm,直径大于25μm的纤维含量不高于2.0%。

An Analysis of Knowledge, Attitudes and Practices of Communities in Lake Victoria, Kenya on Microcystin Toxicity

Exposure to microcystin poses a potential health hazard to humans and other living organisms. This results from eutrophication and warrants an investigation into the problem of microcystin toxicity in Lake Victoria. This study was conducted in Homa Bay, Kisumu, Siaya and Busia counties to understand the effects of microcystin toxicity among fisherfolk and lake riparian communities. Data collection involved 90 semi-structured questionnaires, 11 key informant interviews and seven focus group discussions. Water samples were also collected and analysed for algal toxins. Data were analysed using Stata version 13 (Stata Corp, College Station, Texas, USA) and SPSS version 18.0. Majority (73.3%) of the fisherfolk were aware of microcystin toxicity in the lake with no significant difference in the awareness of microcystin toxicity between men and women (χ2 = 1.1, df = 1, p = 0.293). Most of the respondents relied on lake (48.9%) and tap water (47.8%) with paltry sourcing water from borehole (16.8%) and rain (7.8%). There was no association between level of education and water source (lake;χ2 = 1.61, df = 3, p = 0.656) and (tap;χ2 = 2.23, df = 3, p = 0.527). Fisherfolk was cognizant of the need to curb microcystin toxicity. There was a significant difference (p < 0.05) in the occurrence of microcystin during the wet season compared to the dry season. Cyanophytes were the most significant (p < 0.001) group of phytoplankton. When ingested, microcystin has a long-term effect and therefore pollution control is crucial.

Background Many organizations have issued recommendations to limit elective surgery during the coronavirus disease 2019(COVID-19)*** surveyed providers of children’s surgical care working in low-income and middle-income countries(LMICs)to understand their perspectives on surgical management in the wake of the COVID-19 pandemic and how they were subsequently modifying their surgical care *** A survey of children’s surgery providers in LMICs was *** reported how their perioperative practice had changed in response to *** were also presented with 26 specific procedures and asked which of these procedures they were allowed to perform and which they felt they should be allowed to *** in surgical practice reported by respondents were analyzed *** A total of 132 responses were obtained from 120 unique institutions across 30 LMICs.117/120 institutions(97.5%)had issued formal guidance on delaying or limiting elective children’s surgical *** in LICs were less likely to have issued guidance on elective surgery compared with middle-income facilities(82%in LICs vs 99%in lower middle-income countries and 100%in upper middle-income countries,p=0.036).Although 122(97%)providers believed cases should be limited during a global pandemic,there was no procedure where more than 61%of providers agreed cases should be delayed or *** There is little consensus on which procedures should be limited or delayed among LMIC *** of testing capacity and local,context-specific guidelines may be a better strategy than international consensus,given the disparities in availability of preoperative testing and the lack of consensus towards which procedures should be delayed.

Background While 5%of 247 million global malaria cases are reported in Uganda,it is also a top refugee hosting country in Africa,with over 1.36 million *** malaria being an emerging challenge for humanitarian response in refugee settlements,little is known about its risk *** study aimed to investigate the risk factors for malaria infections among children under 5 years of age in refugee settlements in *** We utilized data from Uganda’s Malaria Indicator Survey which was conducted between December 2018 and February 2019 at the peak of malaria *** this national survey,household level information was obtained using standardized questionnaires and a total of 7787 children under 5 years of age were tested for malaria using mainly the rapid diagnostic *** focused on 675 malaria tested children under five in refugee settlements located in Yumbe,Arua,Adjumani,Moyo,Lamwo,Kiryadongo,Kyegegwa,Kamwenge and Isingiro *** extracted variables included prevalence of malaria,demographic,social-economic and environmental *** logistic regression was used to identify and define the malaria associated risk *** Overall,malaria prevalence in all refugee settlements across the nine hosting districts was 36.6%.Malaria infections were higher in refugee settlements located in Isingiro(98.7%),Kyegegwa(58.6%)and Arua(57.4%)*** risk factors were significantly associated with acquisition of malaria including fetching water from open water sources[adjusted odds ratio(aOR)=1.22,95%CI:0.08–0.59,P=0.002],boreholes(aOR=2.11,95%CI:0.91–4.89,P=0.018)and water tanks(aOR=4.47,95%CI:1.67–11.9,P=0.002).Other factors included pit-latrines(aOR=1.48,95%CI:1.03–2.13,P=0.033),open defecation(aOR=3.29,95%CI:1.54–7.05,P=0.002),lack of insecticide treated bed nets(aOR=1.15,95%CI:0.43–3.13,P=0.003)and knowledge on the causes of malaria(aOR=1.09,95%CI:0.79–1.51,P=0.005).Conclusions The persistence of the malaria infect

Effects of Grounding (Earthing) on Massage Therapists: An Exploratory Study

Background:In essential hypertension,renal noradrenaline(NA)spillover is commonly increased and in experimental hypertension renal sympathectomy abolishes the blood pressure *** renal sympathetic nerve ablation with catheter-based radiofrequency energy(Symplicity,Ardian Inc.)was tested in patients with resistant hypertension(SBP above 160 mmHg on 3 or more anti-hypertensive drugs,including a diuretic).Methods: Office blood pressure(BP)and safety data were acquired prior to,and at 1,3,6 and 12 months post-procedure. Whole body and renal noradrenaline spillover were measured with isotope dilution in a subset of 10 patients at baseline and 30 days ***:45 pts with a baseline mean office blood pressure of 177/ 101 mmHg(SD 20/15),(mean 4.7 antihypertensive medications)were studied;office blood pressures after procedure were reduced by 14/10,21/10,22/11,24/11,and 27/17 mmHg at 1,3,6,9,and 12 months,respectively. It is well known that massage therapists often develop a number of health problems relatively early on in their career. A preliminary study showed that grounding massage therapists during their work may alleviate some of the health problems they encounter. A doubled-blind randomized controlled trial was designed to examine the effects of working and sleeping grounded for 4 weeks on massage therapists’ blood viscosity, stress (through HRV), inflammation (IFN-γ, IL-6, TNF-α, and hsCRP) and oxidative stress (MPO and MDA) biomarkers. The results show stress reduction as measured by heart rate, respir-atory rate and hear rate variability (HRV) and a lowering effect on blood viscosity that lasted for at least one week after ungrounding, with systolic blood viscosity becoming significantly lower at the end of the study. Inflammation markers (IFN-γ, TNF-α, and hsCRP) increased rapidly, within one week, after ungrounding. The findings suggest that grounding is beneficial for massage therapists in multiple domains relevant to health and wellbeing.
