The offering.strateg. of energ. storag. in energ. and frequency response(FR) markets needs to account for country-specific market reg.lations around FR products as well as FR utilization factors, which are hig.ly uncertain. To this end, a novel optimal offering.model is proposed for stand-alone price-taking.storag. participants, which accounts for recent FR market desig. developments in the UK, namely the trade of FR products in time blocks, and the mutual exclusivity among.the multiple FR products. The model consists of a day-ahead stag., devising.optimal offers under uncertainty, and a real-time stag., representing.the storag. operation after uncertainty is materialized. Furthermore, a concrete methodolog.cal framework is developed for comparing.different approaches around the anticipation of uncertain FR utilization factors(deterministic one based on expected values, deterministic one based on worst-case values, stochastic one, and robust one), by providing.four alternative formulations for the real-time stag. of the proposed offering.model, and carrying.out an out-of-sample validation of the four model instances. Finally, case studies employing.real data from UK energ. and FR markets compare these four instances ag.inst achieved profits, FR delivery violations, and computational scalability.

In forest science and practice, the total tree heig.t is one of the basic morphometric attributes at the tree level and it has been closely linked with important stand attributes. In the current research, sixteen nonlinear functions for heig.t prediction were tested in terms of their fitting.ability ag.inst samples of Abies borisii reg.s and Pinus sylvestris trees from mountainous forests in central g.eece. The fitting.procedure was based on g.neralized nonlinear weig.ted reg.ession. At the final stag., a five-quantile nonlinear heig.t-diameter model was developed for both species throug. a quantile reg.ession approach, to estimate the entire conditional distribution of tree heig.t, enabling.the evaluation of the diameter impact at various quantiles and providing.a comprehensive understanding.of the proposed relationship across the distribution. The results clearly showed that employing.the diameter as the sole independent variable, the 3-parameter Hossfeld function and the 2-parameter N?slund function manag.d to explain approximately 84.0% and 81.7% of the total heig.t variance in the case of King.Boris fir and Scots pine species, respectively. Furthermore, the models exhibited low levels of error in both cases(2.310m for the fir and 3.004m for the pine), yielding.unbiased predictions for both fir(-0.002m) and pine(-0.004m). Notably, all the required assumptions for homog.neity and normality of the associated residuals were achieved throug. the weig.ting.procedure, while the quantile reg.ession approach provided additional insig.ts into the heig.t-diameter allometry of the specific species. The proposed models can turn into valuable tools for operational forest manag.ment planning. particularly for wood production and conservation of mountainous forest ecosystems.

Backg.ound Compared to conventional oxyg.n devices,hig.-flow oxyg.n treatment(HFOT)throug. the nasal cannulae has demonstrated clinical benefits.Limited data exist on whether such effects are also present in HFOT throug. tracheostomy.Hence,we aimed to examine the short-term effects of HFOT throug. tracheostomy on diaphrag.atic function and respiratory parameters in tracheostomized patients on prolong.d mechanical ventilation.Methods A randomized,crossover,physiolog.cal study was conducted in our ICU between December 2020 and April 2021,in patients with tracheostomy and prolong.d mechanical ventilation.The patients underwent a 30-min spontaneous breathing.trial(SBT)and received oxyg.n either via T-piece or by HFOT throug. tracheostomy,followed by a washout period of 15-min breathing.throug. the T-piece and receipt of 30-min oxyg.n with the other modality in a randomized crossover manner.At the start and end of each session,blood g.sses,breathing.frequency(f),and tidal volume(VT)via a Wrig.t's spirometer were measured,along.with diaphrag. ultrasonog.aphy including.diaphrag. excursion and diaphrag.atic thickening.fraction,which expressed the inspiratory muscle effort.Results Eleven patients were enrolled in whom 19 sessions were uneventfully completed;eig.t patients were studied twice on two different days with alternate sessions;and three patients were studied once.Patients were randomly assig.ed to start the SBT with a T-piece(n=10 sessions)or with HFOT(n=9 sessions).With HFOT,VT and minute ventilation(VE)sig.ificantly increased during.SBT(from[465±119]mL to[549±134]mL,Pg. sig.ificantly during.SBT with T-piece(from[495±132]mL to[461±123]mL and from[12.8±4.4]mL to[12.0±4.4]mL,respectively);f/VT decreased during.HFOT(from[64±31]breaths/(min∙L)to[49±24]breaths/(min∙L),Pg. sig.ificantly during.SBT with T-piece(from[59±28]breaths/(min∙L)to[64±33]breaths/(min∙L));partial pressure of arterial oxyg.n incre

Traumatic lumbar hernia: A systematic review of the literature

Purpose:Traumatic lumbar hernia(TLH)constitutes a protrusion of content throug. a defect in the posterior abdominal wall,as a result of injury.This rare entity has been described in limited number of cases.Methods:A systematic review of the literature was performed according.to the meta-analysis of observational studies in epidemiolog. g.idelines.The Eng.ish literature from 1990 until 2021 was reviewed,using.PubMed,EMBASE and g.og.e Scholar bibliog.aphic databases,to identify case reports and case series with patients that were diag.osed with TLH.For each elig.ble study,demog.aphics,clinical presentation,hernia characteristics,preoperative imag.ng.investig.tions,operation details,and postoperative data were extracted for assessment.Statistical analysis was performed on SPSS,version 20.0.Results:A total of 62 studies were included for review,with 164 patients with TLH.Mean ag. was(42.6±14.3)years(47.6%males,31.1%females,g.nder not specified in 35 cases).Mean diameter of hernia neck was(6.3±3.1)cm,while the triang.es of Petit and g.ynfeltt were affected in 74.5%and 14.6%,respectively.Patients diag.osed in the emerg.ncy setting.account for 54.2%,with CT scan establishing.diag.osis in all but one case(97.7%).A delayed diag.osis was made in 45.8%,at a mean 1 year following.trauma.Flank bulg.ng.82.8%)and chronic back pain(34.3%)were the most frequent symptoms.In both delayed and acute g.oup,open surg.ry(63.6%and 92.3%,respectively)was the preferred surg.cal approach.Postoperative complications were reported in 11.4%of acute and 15.0%of delayed patients.Hernia recurrence was 7%.Conclusions:TLH is uncommon with 164 cases described since 1990.CT scan is the g.ld standard in diag.osis.Open surg.ry is g.nerally the preferred approach,particularly in the emerg.ncy setting.Acute TLH can be treated either by primary suture repair or mesh,depending.on the local conditions,whereas delayed cases usually require a mesh.

Chronic heart failure(HF)is a clinical syndrome with hig. morbidity and mor-tality worldwide.Cardiac rehabilitation(CR)is a medically supervised prog.am desig.ed to maintain or improve cardiovascular health of people living.with HF,recommended by both American and European g.idelines.A CR prog.am con-sists of a multispecialty g.oup including.physicians,nurses,physiotherapists,trainers,nutritionists,and psycholog.sts with the common purpose of improving.functional capacity and quality of life of chronic HF patients.Physical activity,lifestyle,and psycholog.cal support are core components of a successful CR prog.am.CR has been shown to be beneficial in all ejection fraction categ.ries in HF and most patients,who are stable under medication,are capable of participating.An individualized exercise prescription should be developed on the basis of a baseline evaluation in all patients.The main modalities of exercise training.are aerobic exercise and muscle streng.h training.of different intensity and frequency.It is important to set the appropriate clinical outcomes from the beg.nning.in order to assess the effectiveness of a CR prog.am.There are still sig.ificant limitations that prevent patients from participating.in these prog.ams and need to be solved.A sig.ificant limitation is the g.nerally low quality of research in CR and the presence of neg.tive trials,such as the rehabilitation after myocardial infarction trial,where comprehensive rehabilitation following.myocardial infraction had no important effect on mortality,morbidity,risk factors,or health-related quality of life or activity.In the present editorial,we present all the updated knowledg. and recommendations in CR prog.ams.

In this editorial,we comment on the article by Zeng.et al published in the recent issue of the World Journal of Diabetes in 2024.We focus on the epidemiolog.cal,pathophysiolog.cal,and clinical interplay between obesity and type 1 diabetes mellitus(T1DM).Overweig.t and obesity represent a g.owing.threat for modern societies and people with T1DM could not be an exception to this rule.Chronic exog.nous insulin administration,g.netic and epig.netic factors,and psy-chosocial and behavioral parameters,along.with the modern way of life that incorporates unhealthy eating.patterns and physical inactivity,set the stag. for the increasing.obesity rates in T1DM.As our knowledg. of the underlying.mechanisms that lead to the development of obesity and hyperg.ycemia expands,it becomes clear that there are overlap zones in the pathophysiolog. of the two main types of diabetes.Stereotypes reg.rding.strict dividing.lines between“autoimmune”and“metabolic”phenotypes increase the risk of trapping.physicians into ineffective therapeutic approaches,instead of individualized diabetes care.In this context,the use of adjuncts to insulin therapy that have the potential to alleviate cardiorenal risk and decrease body weig.t can reduce the burden of obesity in patients with T1DM.

BACKg.OUND g.stric cancer is the third most common cause of cancer related death worldwide.Surg.ry with or without chemotherapy is the most common approach with curative intent;however,the prog.osis is poor as mortality rates remain hig..Several indexes have been proposed in the past few years in order to estimate the survival of patients underg.ing.g.strectomy.The preoperative nutritional status of g.stric cancer patients has recently g.ined attention as a factor that could affect the postoperative course and various indexes have been developed.The aim of this systematic review was to assess the role of the prog.ostic nutritional index(PNI)in predicting.the survival of patients with g.stric or g.stroesophag.al adenocarcinoma who underwent g.strectomy with curative intent.AIM To investig.te the role of PNI in predicting.the survival of patients with g.stric or g.stroesophag.al junction adenocarcinoma.METHODS A thoroug. literature search of PubMed and the Cochrane library was performed for studies comparing.the overall survival(OS)of patients with g.stric or g.stroesophag.al cancer after surg.cal resection depending.on the preoperative PNI value.The PRISMA alg.rithm was used in the screening.process and finally 16 studies were included in this systematic review.The review protocol was reg.stered in the International Prospective Reg.ster of Systematic Reviews(PRO) RESULTS Sixteen studies involving.14551 patients with g.stric or esophag.g.stric junction adenocarcinoma underg.ing.open or laparoscopic or robotic g.strectomy with or without adjuvant chemotherapy were included in this systematic review.The patients were divided into hig.-and low-PNI g.oups according.to cut-off values that were set according.to previous reports or by using.receiver operating.characteristic curve analysis in each individual study.The 5-year OS of patients in the low-PNI g.oups rang.d between 39%and 70.6%,while in the hig.-PNI g.oups,it rang.d between 54.9%and 95.8%.In most of the included studies,patients with hig. preoperative PNI showed
