急性冠脉综合征瘀毒互结证诊断标准研究

目的基于冠心病“瘀毒”病因病机的理论认识,建立急性冠脉综合征瘀毒互结证诊断标准。方法采用文献系统分析与横断面临床研究构建条目池,采用德尔菲法专家咨询对条目进行筛选,采用层次分析法确定各条目权重。在此基础上,结合条目权重及临床实用性原则建立诊断标准。结果文献系统分析筛选条目43个,横断面临床研究纳入患者400例,筛选条目39个,归纳分类后形成包含67个条目的条目池;经过两轮德尔菲专家咨询,筛选保留条目28个;通过层次分析法,确定高权重类条目(0.0430~0.1976)9个、中权重类条目(0.0153~0.0289)9个与低权重类条目(0.0060~0.0147)10个。结合临床实用性原则,优化形成包括必要指标(心绞痛程度进行性加重、近1个月反复发作性心绞痛等)、理化指标(心脏肌钙蛋白或肌酸激酶同工酶升高等)及舌象脉象(舌青紫或紫暗等、脉弦细或脉涩)3个维度的急性冠脉综合征瘀毒互结证诊断标准。两轮专家问卷咨询肯德尔和谐系数分别为0.368、0.395,格朗巴赫系数为分别为0.967、0.893,提示专家意见一致性好,结果可信度高。结论该诊断标准涵盖了反映瘀毒互结致病特点的宏观表征和理化指标,可指导急性冠脉综合征瘀毒互结证的诊断。

文章研究了饲料中添加不同水平的白术提取物(Extraction of Atractylodes macrocephala Koidz,EAMK)对日本沼虾(Macrobrachium nipponense)生产性能、器官指数、血液生化指标、抗氧化能力、消化系统健康和抗病力的影响。实验以初始体重为(0.21±0.05)g的日本沼虾为研究对象,设置6个饲料处理,白术提取物添加量为0、100、200、400、600和1200 mg/kg(Control、A100、A200、A400、A600和A1200)。每个饲料处理设6个重复,每个养殖缸放入60尾虾,实验周期为75d,每天记录采食量。养殖实验期间监测水体温度为(20±3)℃,pH为7.8—8.2,亚硝酸盐为0.16—0.2 mg/L,氨氮为0.02—0.1 mg/L,溶氧>6.0 mg/L。在实验结束后,分析日本沼虾生长性能、血清生化指标、肠道消化酶和免疫指标及肝胰腺抗氧化能力,观察日本沼虾的组织结构,用嗜水气单胞菌(Aeromonas hydrophila)进行攻毒并记录存活率。结果表明:饲料中添加200 mg/kg的白术提取物可以显著提高日本沼虾的增重率(WGR)和特定生长率(SGR),并显著降低饲料系数(FC;P0.05);A100组的血清葡萄糖(Glu)含量显著高于对照组(P<0.05),并能显著降低谷丙转氨酶(ALT)活性(P<0.05);各白术提取物处理组的血清甘油三酯(TG)和总胆固醇含量(TC)均显著低于对照组(P<0.05);A400组的肝胰腺GSH-Px活性显著高于A100组和对照组(P<0.05),A600组的肝胰腺CAT活性显著高于其他各组(P<0.05),A200和400组的肠道iNOS的活性显著高于对照组(P<0.05);A400组的日本沼虾肠绒毛显著高于对照组(P<0.05),并且减轻了肝细胞的空泡化,促进了鳃的完整性;A100处理组的嗜水气单胞菌攻毒后存活率显著高于对照组(P<0.05)。综上所述,白术提取物可以提高日本沼虾的生长性能和抗氧化能力,提高抗病力,在日本沼虾饲料中适宜添加量为100—400 mg/kg。

Porous organic polymers(POPs) are porous materials composed of light elements such as C, H, N, and O. The benign characters,including large surface area, good physical and chemical stability, well-defined chemical composition, wide ranges of monomer selection, and strong designability, have made POPs one of the frontiers in materials research. In this review, we discussed the design and synthesis of various POP materials that mainly led by Chinese scientists, including conjugated microporous polymers(CMPs), porous aromatic frameworks(PAFs), and hypercrosslinked porous polymers(HCPs), as well as crystalline POPs comprised of covalent organic frameworks(COFs) and a special class of COFs with triazine rings, covalent triazine frameworks(CTFs), and supramolecular organic frameworks(SOFs), and sorted out their main applications in adsorption, separation,catalysis, and electrochemistry fields.

BACKGROUND The phosphoinositide 3-kinase/protein kinase-B/mechanistic target of rapamycin(PI3K/Akt/mTOR) signalling pathway is crucial for cell survival, differentiation, apoptosis and metabolism. Xihuang pills(XHP) are a traditional Chinese preparation with antitumour properties. They inhibit the growth of breast cancer, glioma, and other tumours by regulating the PI3K/Akt/mTOR signalling pathway. However, the effects and mechanisms of action of XHP in hepatocellular carcinoma(HCC) remain unclear. Regulation of the PI3K/Akt/mTOR signalling pathway effectively inhibits the progression of HCC. However, no study has focused on the XHPassociated PI3K/Akt/mTOR signalling pathway. Therefore, we hypothesized that XHP might play a role in inhibiting HCC through the PI3K/Akt/mTOR signalling *** To confirm the effect of XHP on HCC and the possible mechanisms *** The chemical constituents and active components of XHP were analysed using ultra-performance liquid chromatography-quadrupole time of flight mass spectrometry(UPLC-Q-TOF-MS). Cellbased experiments and in vivo xenograft tumour experiments were utilized to evaluate the effect of XHP on HCC tumorigenesis. First, SMMC-7721 cells were incubated with different concentrations of XHP(0, 0.3125, 0.625, 1.25, and 2.5 mg/mL) for 12 h, 24 h and 48 h. Cell viability was assessed using the CCK-8 assay, followed by an assessment of cell migration using a wound healing ***, the effect of XHP on the apoptosis of SMMC-7721 cells was evaluated. SMMC-7721 cells were stained with fluorescein isothiocyanate and annexin V/propidium iodide. The number of apoptotic cells and cell cycle distribution were measured using flow cytometry. Association of Overlapped and Un-overlapped Comorbidities with COVID-19 Severity and Treatment Outcomes: A Retrospective Cohort Study from Nine Provinces in China

Objective Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains *** A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of *** Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two *** Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. Women with comorbidities were more likely to have severe disease, while men with comorbidities were more likely to have unfavorable treatment outcomes.
