Heterotopic pregnancy is no more a medical breakthrough. It combines intrauterine pregnancy and extra-uterine pregnancy regardless of location. We report a case of intra-uterine pregnancy associated with a ruptured abdominal ectopic pregnancy located on the pelvic colon in a 29-year-old patient, third gestation, primigravida, having a live baby and a prior history of two abortions. She has blood group O negative of Rhesus. It has been diagnosed at the stage of the rupture of ectopic pregnancy. An emergency laparotomy performed under blood transfusion has revealed heavy hemoperitoneum (1100 ml), a ruptured abdominal extra-uterine localized on the pelvic under blood transfusion and a bulging uterus. We have proceeded with the aspiration of hemoperitoneum, the ablation of the extra-uterine pregnancy and the hemostasis of the section part. The post-operative follow-up has been easy. The intra-uterine pregnancy has developed normally and has given birth to a full-term live, and healthy baby.

Liver Abscesses in General Surgery at CsRef CI in Bamako Mali

Liver abscesses correspond to a newly formed cavity created by necrosis of the liver parenchyma induced by the pathogen. The aim of the present work was to study liver abscesses;determine the frequency;describe the clinical and paraclinical aspects, therapeutic and evolutionary modalities;determine the follow-up of treatment in order to assess the cost of treatment in the Reference Health Center of Commune I of Bamako in Mali. This prospective study, involving 30 cases of liver abscess, took place over a period of 24 months from January 2015 to December 2016 in the general surgery department of the Cs Ref of commune I. The liver abscess is very often the consequence of amoebiasis which is rampant in the underprivileged population and it remains topical in surgical practice in Mali. Our hospital frequency was 0.081% with an average age of 34.40 years and extremes of 16 and 61 years;a sex ratio of 2.3 in favor of men. The main clinical signs were fever (56.7%), hepatalgia (73.3%) and hepatomegaly (26.7%). Hepatic collections objectified on abdominal ultrasound were located in the right lobe in 70% of cases and unique in 62%. Amebic serology carried out in 100% was negative in 20%;10% of cases had undergone surgical treatment. The consequences were simple for all our patients. The average cost of care, approximately 100,000 FCFA, was significantly higher than the minimum wage (28,460 FCFA) in Mali.

Genetic Diversity Analysis of Badnaviruses Infecting Banana in Burkina Faso

Badnaviruses are serious plant pararetroviruses affecting banana and causes serious economic losses to banana production worldwide. This study aims to examine the variability of BSV and SCBV nature infecting banana in Burkina Faso. Polymerase Chain Reaction (PCR) used the Badna FP/RP specific primers for the RT/RNase H regions present in badnaviruses. The PCR yielded about 579 bp amplicons from banana infected by BSV and SCBV. The 38 BSV isolates recorded low nucleotide identity ranging from 58.9% - 98.1%. Based on percentage nucleotide sequence identity and phylogenetic analyse, BSV isolates were categorized into four groups: 1, 2, 3 and 4. Group 4 shared 76.9% - 100% identity with BSOL species. However, Groups 1 and 3 recorded a low identity ranging, from 76.8% - 79.2%, 68.8% - 79.7% with BSCV, and 72.8% - 79.0% between Group 2 and BSOLV. Groups 1, 2 and 3 were assigned to a potentially new BSV species. The two SCBV isolates recorded a low nucleotide identity of 68% among themselves indicating high diversity. In addition, SCBV_Cd and SCBV_CE showed high nucleotide identity 91.3% and 58.7% with SCBV_C and SCBV, when they were compared to all published SCBV genotypes. In addition, phylogenetic analysis revealed the segregation of SCBV isolates into two genotypes, SCBV_Cd in C and SCBV_CE segregated in a new genotype namely Z. Recombination analyses showed weak signatures of recombination among some of the BSV and SCBV sequences.

Low Density Linear Polyethylene Reinforced with Alkali and MAPE Treated Fibers from Coffee Pulp

In this work fibers derived from coffee, hulls have been incorporated into Linear Low Density Polyethylene (LLDPE). The influence of the filler content on the thermal and physicomechanical properties of the composites obtained was assessed. The results showed that the incorporation of fibers was able to improve the thermostability of LLDPE/Coffee hulls fibers;comparing the treated fiber composite with untreated fiber composites, the chemical treatment reduces by 58.3% the water absorption, while increasing the elongation and tensile strength by about 48% and 17% respectively. Moreover, due to better interfacial interaction induced by MAPE, the corresponding composite exhibited better properties compared to the untreated fiber composite. Results are indicative of the fact that both mercerization and MAPE (coupling agent) have significant positive effects on the fiber-matrix interaction in terms of adhesion, wetting and dispersion, this treatment produced a better fiber distribution and consequently a more uniform composite morphology without voids and gaps between the fibers and the matrix, allowing the possibility to use higher fiber contents (up to 30% wt.) with acceptable mechanical properties.

Thermal and Physicochemical Characterization of Fibers from Coffee Hulls as Filler for Linear Low Density Polyethylene (LLDPE)

This work presents the thermal, physical and chemical characterization of Coffee canephora, from littoral region of Cameroon, for their use as reinforcement for polymeric materials. The infrared of coffee hulls shows the presence of a large peak intensity at 3299 cm-1 that can be attributed to O-H stretching group of alcohol (cellulose content in coffee pulp). The intensity 2926 cm-1 can be attributed to C-H stretching group of alkanes or the vibration of methoxy group of lignin. Thermo gravimetric analysis shows that around 440°C, the biomass has been completely consumed;the temperature profiles show a peak at 86°C that could correspond to the loss of water as evaporation at a percentage of 8%;the peak at 321°C is accompanied by a water loss of 64.50%; this temperature is assimilated to the degradation of hemicelluloses;the temperature range from 321°C to 401°C is accompanied by a loss of mass of 22.80%, which would be due to the degradation of cellulose. SEM images of the surface of raw coffee hulls, coffee hulls treated with caustic soda respectively clearly reveal gaps between the fibers. The results showed that the incorporation of coffee hulls fiber in LLDPE matrix might result in composites with suitable property application for various industrial fields

Investigation into Recent Temperature and Rainfall Trends in Mali Using Mann-Kendall Trend Test: Case Study of Bamako

Rainfall and temperature variability analysis is important for researchers and policy formulators in making critical decisions on water availability and use in communities. The Western Sahel, which comprises Mali is considered as one of the vulnerable regions to climate change, and also encountered the challenges of climatic shocks such as flood and drought. This research therefore sought to investigate climate change effects on hydrological events and trends in Sahelian rainfall intensity using Bamako (Mali) as a case study from 1991 to 2020, as limited data availability did not allow an extended period of study. Monthly observed data provided by MALI-METEO was used to validate daily rainfalls data from African Rainfall Climatology Version 2 (ARC2) satellite-based rainfall product on monthly basis. The validated model performance used Nash-Sutcliffe Efficiency (NSE) and Percent Bias (PBAIS) and gave results of 0.904 and 1.0506 respectively. Trends in annual maximum temperatures and rainfalls were analyzed using Mann-Kendall trend test. The result indicated that the trend in annual maximum rainfalls was decreasing, while annual total rainfall was increasing but not significant at 5% significance level. The rate of increase in annual total rainfalls was 0.475 mm/year according to the observed annual rainfall series and decreased to 0.68 mm/year in annual maximum. The analysis further found that annual maximum temperatures were increasing at the rate of 0.03°C/year at 5% significance level. To provide more accurate climate predictions, it is recommended that further studies on rainfall and temperature with data sets spanning 60 - 90 years be carried out.

Background: The rapid diagnostic tests play a pivotal role in the screening of viral markers in blood qualification for transfusion in limited resource setting. Therefore, it is important to assess their analytical performances to ensure their proper functioning. Material and Methods: We performed a cross- sectional study by successive recruitment to assess the diagnostic value of rapid diagnostic tests algorithms using ELISA as a reference test. A total of 661 blood from donors were enrolled for this study. Rapid Diagnostic Tests (RDTs) and ELISA tests were performed for each sample by a couple of double-blinded biotechnologists. Data were collected on case report form and captured in Microsoft Excel then the file was imported and analyzed using R software version 4.0.3. Results: The diagnostic accuracy for the algorithms are summarized in Table 1. Ultra-Pro Hernia System for Repair of Primary Complex Inguinal Hernia: Should It Be the Technique of Choice?

Purpose: The introduction of light weight three-dimension meshes into the field of inguinal hernia repair showed excellent outcomes. Ultrapro Hernia System (UHS) is one of these three-dimension meshes that allow reinforcing the pre-peritoneal space with minimal fixation. It improves the patient quality of life with rapid resumption of the daily activities. The aim of this study is to evaluate the short-term outcomes of open mesh repair of primary complex inguinal hernia using UHS mesh. Methods: Between November 2013 and November 2015, seventy five male patients with complex primary inguinal hernia were submitted to open inguinal hernia mesh repair using the UHS. Results: The mean age was 46.1 years. Thirty three patients had inguinoscrotal hernias, 14 with Nyhus type IIIA, 22 with Nyhus type IIIB, & 6 patients had strangulated hernias. The mean operative time was 48 minutes. No operative complications were recorded. Post-operatively, the mean VAS scale on 1 day, 1 week, & 1 month was 3.22, 1.2, & 0.3 respectively. The mean duration of oral analgesics use was 2.1 days. None of the patients reported any chronic pain at 12 months post-operatively. Two cases of superficial wound infection were recorded that were treated by oral antibiotics. No recurrence was reported during a mean follow-up period of 26 months. Conclusion: Inguinal hernia repair using UHS is an effective technique that combines the advantages of the anterior and the pre-peritoneal approaches. It improves the patient’s quality of life with minimal recurrence rates. A larger number of patients with longer follow up periods are needed to increase the validity of our results.

Artificial induction of labor (AIL) is the set of techniques intended to induce childbirth in order to reduce maternal-fetal mortality. The aim of this paper was to study the maternal and fetal prognosis after the artificial induction of labor with misoprostol. It was a descriptive prospective study conducted from September 1, 2018 to February 28, 2019 at the maternity unit of the Referral Health Center (RHC) in commune I of Bamako. It was about any pregnant woman at term who had received misoprostol as part of the artificial induction of labor. 102 cases of artificial induction with misoprostol 200 μg were collected out of 3641 deliveries, or a frequency of 2.8%. The age group 28 - 32 years accounted for 56.86% with an average age of 28.8 years. Multiparous represented 54.90% of the number. The history of diabetes and hypertension involved 37.25% of the pregnant women. The main indication was premature rupture of membranes (PRM) (40.2%). Induction was performed between the 37th and 41st week of amenorrhea (69.6%). The trigger index according to BISHOP was greater than 7 (50.98%). Induction was done only with 3/4 of misoprostol tablet 200 μg (43.14%). The average labor time was 7 hours (89.22%). The vaginal birth concerned 92.16% of pregnant women. An APGAR10 was noted in 97.85% of newborns after 05 minutes. AIL with misoprostol was practiced at the Center. The main indication was the premature rupture of the membranes. It improves the maternal and fetal prognosis.

Role of Laparoscopic Surgery in the Management of Female Infertility at the Department of Gynecology of Hôpital du Mali

Infertility is a socio-cultural drama in Africa, especially in Mali and remains difficult for couples to overcome. Laparoscopy, also called minimally invasive surgery or keyhole surgery, is an operative technique that permits to explore the pelvis and perform an appropriate therapeutic procedure. The objective of our work was to assess the role of laparoscopic surgery in the treatment of female infertility at Hopital du Mali. It was a retrospective descriptive study that was conducted over a period of 5 years (January 2013 to December 2018). Any patients followed for infertility and who underwent laparoscopic surgery in the gynecology department over a period of two (2) years were included. The outcome of laparoscopic surgery was evaluated in terms of conception of pregnancy. We had collected 103 infertile patients out of 2984, with a frequency of 3.45%. The mean age of our patients was 30.2 years. Housewives and out-of-school women accounted for 68.93% and 54.37% respectively. Nulligravida represented 41.75%. Regarding infertility, it was primary in 41.75% and secondary in 58.25%, with an average duration of 6.2 years. Systematic chlamydial serology was positive in 62.14% of our patients. Hysterosalpingography (HSG) revealed bilateral tubal obstruction in 53.33% of cases. Laparoscopy found tubal adhesions in 97.80% of cases. Adhesiolysis was the main procedure performed with 84.47% bilateral tubal patency achieved in the methylene blue test. Among our patients who had contracted a pregnancy (22.99%), 85% had carried their pregnancies to term. Tubal damage was the main cause of infertility. Adhesiolysis remains the main laparoscopic procedure for treatment.
