What is already known about this topic?Addressing health disparities is a worldwide priority,with a well-established acknowledgment of the influence of childhood circumstances on these discrepancies.In China,particularly among the elderly,health inequalities are a notable concern.What is added by this report?The inequality in healthy aging has increased from 2011 to 2020,both in general and concerning childhood factors.Nevertheless,the impact of early-life healthcare access and parental health behaviors on healthy aging gaps has reduced among older adults in better health within the top segment of healthy aging.What are the implications for public health practice?Efforts towards reducing regional health disparities and improving healthcare access for children,along with promoting the health and well-being of parents,especially in economically disadvantaged households,are crucial policy considerations.

BACKGrOUND Liver transplantation(LT)is the only curative treatment for end-stage liver disease.however,LT recipients are susceptible to infection,which is the leading cause of early mortality after LT.Klebsiella pneumoniae infections(KPIs)in the bloodstream are common in LT recipients.We hypothesized that KPIs and carbapenemresistant Klebsiella pneumoniae(CrKP)infections may affect the outcomes of LT recipients.AIM To assess KPI incidence,timing,distribution,drug resistance,and risk factors following LT and its association with outcomes.METhODS This retrospective study included 406 patients undergoing LT at The Third Xiangya hospital of Central South University,a tertiary hospital,from January 2015 to January 2023.We investigated the risk factors for KPIs and assessed the impact of KPIs and CrKP infections on the prognosis of LT recipients using logistic regression analysis.rESULTS KPI incidence was 7.9%(n=32),with lung/thoracic cavity the most frequent site of infection;the median time from LT to KPI onset was 7.5 d.Of 44 Klebsiella pneumoniae isolates,43(97.7%)and 34(77.3%)were susceptible to polymyxin B or ceftazidime/avibactam and tigecycline,respectively;>70%were resistant to piperacillin/tazobactam,ceftazidime,cefepime,aztreonam,meropenem,and levofloxacin.Female sex[odds ratio(Or)=2.827,95%confidence interval(CI):1.256-6.364;P=0.012],pre-LT diabetes(Or=2.794,95%CI:1.070-7.294;P=0.036),day 1 post-LT alanine aminotransferase(ALT)levels≥1500 U/L(Or=3.645,95%CI:1.671-7.950;P=0.001),and post-LT urethral catheter duration over 4 d(Or=2.266,95%CI:1.016-5.054;P=0.046)were risk factors for KPI.CrKP infections,but not KPIs,were risk factors for 6-month all-cause mortality post-LT.CONCLUSION KPIs occur frequently and rapidly after LT.risk factors include female sex,pre-LT diabetes,increased post-LT ALT levels,and urethral catheter duration.CrKP infections,and not KPIs,affect mortality.

In this study,the gas-solid flow process in the blast furnace raceway is numerically simulated using coupled computational fluid dynamics and the discrete element method(CFD-DEM).The coke reaction kinetics data are imported into the DEM model to reproduce the consumption process of each coke particle.The effects of inlet gas velocity and angle on the morphology of the raceway,coke consumption rate,coke bed temperature,and particle size distribution in the blast process are systematically investigated and analyzed.The results show that the consumption of coke particles promotes the formation of raceways during the blast process.At the same time,a coke mixture layer is produced at the edge of the raceway.The higher the inlet gas velocity,the thicker the coke mixture layer in the middle and upper parts of the raceway region,and the larger the proportion of small particles in the coke mixture layer.The effect of the inlet gas angle on the raceway region is less than the inlet gas velocity.however,with the increase in the inlet gas angle,the high-temperature region of the coke bed extends downward gradually,which is conducive to activating the hearth.

Bigeye tuna Thunnus obesus is an important migratory species that forages deeply,and El Niño events highly influence its distribution in the eastern Pacific Ocean.While sea surface temperature is widely recognized as the main factor affecting bigeye tuna(BET)distribution during El Niño events,the roles of different types of El Niño and subsurface oceanic signals,such as ocean heat content and mixed layer depth,remain unclear.We conducted A spatial-temporal analysis to investigate the relationship among BET distribution,El Niño events,and the underlying oceanic signals to address this knowledge gap.We used monthly purse seine fisheries data of BET in the eastern tropical Pacific Ocean(ETPO)from 1994 to 2012 and extracted the central-Pacific El Niño(CPEN)indices based on Niño 3 and Niño 4indexes.Furthermore,we employed Explainable Artificial Intelligence(XAI)models to identify the main patterns and feature importance of the six environmental variables and used information flow analysis to determine the causality between the selected factors and BET distribution.Finally,we analyzed Argo datasets to calculate the vertical,horizontal,and zonal mean temperature differences during CPEN and normal years to clarify the oceanic thermodynamic structure differences between the two types of years.Our findings reveal that BET distribution during the CPEN years is mainly driven by advection feedback of subsurface warmer thermal signals and vertically warmer habitats in the CPEN domain area,especially in high-yield fishing areas.The high frequency of CPEN events will likely lead to the westward shift of fisheries centers.

Microglia are resident immune cells in the central nervous system(CNS)that play vital roles in CNS development,homeostasis and disease pathogenesis.Genetic defects in microglia lead to microglial dysfunction,which in turn leads to neurological disorders.The correction of the specific genetic defects in microglia in these disorders can lead to therapeutic effects.Traditional genetic defect correction approaches are dependent on viral vectorbased genetic defect corrections.however,the viruses used in these approaches,including adeno-associated viruses,lentiviruses and retroviruses,do not primarily target microglia;therefore,viral vector-based genetic defect corrections are ineffective in microglia.Microglia replacement is a novel approach to correct microglial genetic defects via replacing microglia of genetic defects with allogenic healthy microglia.In this paper,we systematically review the history,rationale and therapeutic perspectives of microglia replacement,which would be a novel strategy for treating CNS disorders.

The unstable fluid flow and severe free surface fluctuations in the wheel and belt caster can affect the quality of the cast bar.The lower level height tends to entrap inclusions in the molten metal.On the other hand,the higher level height makes the production process more dangerous due to the overflow of high temperature fluid from the mold.A computational model of the molten metal pouring process was established.The transient fluid flow and free surface fluctuations behavior were calculated using the three-dimensional large eddy simulation model and the volume of fluid model.The results show that the flow velocity of the main jet gradually decreases under the influence of the low kinetic energy fluid in the mold.There is an obvious oscillation in the tail of the jet,while the flow field is asymmetric in space.The jet is closer to the inside radius side due to the Coanda effect,and there is a recirculation zone on the inside radius and the outside radius respectively,according to the 10 s time-averaged results.Compared with the industrial observation and simulation results,the shape of the free surface is a wave that varies with time.In addition,the free surface height is lowest and the flow velocity is highest in the region near the jet.

This paper investigates the effective capacity of a point-to-point ultra-reliable low latency communication(UrLLC)transmission over multiple parallel sub-channels at finite blocklength(FBL)with imperfect channel state information(CSI).Based on reasonable assumptions and approximations,we derive the effective capacity as a function of the pilot length,decoding error probability,transmit power and the sub-channel number.Then we reveal significant impact of the above parameters on the effective capacity.A closed-form lower bound of the effective capacity is derived and an alternating optimization based algorithm is proposed to find the optimal pilot length and decoding error probability.Simulation results validate our theoretical analysis and show that the closedform lower bound is very tight.In addition,through the simulations of the optimized effective capacity,insights for pilot length and decoding error probability optimization are provided to evaluate the optimal parameters in realistic systems.
