Perceptions of fishers to sea turtle bycatch,illegal capture and consumption in the San Ignacio-Navachiste-Macapule lagoon complex,Gulf of California,Mexico

In this study, 10% of all registered fishermen in the coastal towns of Navachiste in Sinaloa, in northwestern Mexico, answered a survey designed to collect data on their perceptions of the following topics: the impact of turtle meat consumption;human health;bycatch;illegal turtle fishing;the illegal sea turtle market;the lo-cal economy;pollution;environmental education;the success of protective legislation;and sea turtle-based ec-otourism. Perceptions were analyzed using the fuzzy logic method through classification into 5 fuzzy member-ship sets: VL, very low;L, low;M, moderate;h, high;Vh, very high. The 9 topics generated decision areas upon applying fuzzy inference that revealed the membership level of the answers in each fuzzy set. The eco-nomic potential of sea turtle-based ecotourism and the economic profitability of the illegal turtle meat market were perceived as VL. Conservation legislation was perceived as h, although inefficiently applied due to cor-ruption. Ecotourism and impacts on sea turtles were perceived as VL, because they were deemed unprofitable activities at the individual and community levels. Environmental education was perceived as L, because it cen-ters on nesting, hatching and releasing turtles and is directed at elementary and middle-school students. While fishers perceive a serious negative impact of fishing activities on sea turtles in the San Ignacio-Navachiste- Macapule area, they do not see themselves individually as part of the problem. Achieving sea turtle conserva-tion in this region requires: suitable ecotourism infrastructure, government investments in promotion, and stud-ies to estimate the minimum number of tourists needed to assure profitability.

The impact of lag effects produced by disturbances on primary production has been a major concern among ecologists during the last *** and extreme climatic events are imposing drastic reductions in radial growth of trees as evidenced in tree-rings series Dendrochronological samples are obtained at tree level but analyzed at an aggregated scale(i.e.,mean chronologies),although aggregating tree-ring chronology on a regional scale may reduce the possibility of studying the variability of individual tree response to drought,by amplifying the average population ***,we conducted experimental research in which 370 trees of 5 species were analyzed to assess the potential statistical and scaling issues that may occur when using regressionbased methods to analyze ecosystem responses to *** legacy effects were quantified using individual and aggregated ***,lag effects were validated using confidence and prediction intervals to identify values falling outside the certainty of the climate-growth model Individual scale legacy effects contrasted with confidence intervals were commonly distributed across species but were scarce when compared with prediction *** analysis of aggregated scale legacies detected significant growth reductions when validated using prediction intervals;however,individual scale legacy lag effects were not *** finding directly contrasts the results obtained when using an aggregated *** results provide empirical evidence on how aggregating ecological data to infer processes that emerge from an individual scale can lead to distorted *** therefore encourage the use of individual based statistical and ecological procedures to analyze tree rings as a means of further understanding the ecosystem responses to disturbances.

hepatogenous diabetes: Is it a neglected condition in chronic liver disease?

Diabetes mellitus(DM) that occurs because of chronic liver disease(CLD) is known as hepatogenous diabetes(hD). Although the association of diabetes and liver cirrhosis was described forty years ago, it was scarcely studied for long time. Patients suffering from this condition have low frequency of risk factors of type 2 DM. Its incidence is higher in CLD of viral, alcoholic and cryptogenic etiology. Its pathophysiology relates to liver damage, pancreatic dysfunction, interactions between hepatitis C virus(hCV) and glucose metabolism mechanisms and genetic susceptibility. It associates with increased rate of liver complications and hepatocellular carcinoma, and decreased 5-year survival rate. It reduces sustained virological response in hCV infected patients. In spite of these evidences, the American Diabetes Association does not recognize hD. In addition, the impact of glucose control on clinical outcomes of patients has not been evaluated. Treatment of diabetes may be difficult due to liver insufficiency and hepatotoxicity of antidiabetic drugs. Notwithstanding, no therapeutic guidelines have been implemented up to date. In this editorial, authors discuss the reasons why they think that hD may be a neglected pathological condition and call attention to the necessity for more clinical research on different fields of this disease.

Neutral protamine hagedorn/regular insulin in the treatment of inpatient hyperglycemia: Comparison of 3 basal-bolus regimens

AIM To compare the safety and efficacy or 3 basal-bolus regimens of neutral protamine hagedorn(NPh)/regular insulin in the management of inpatient *** We randomized 105 patients with blood glucose levelsbetween 140 and 400 mg/dL to a basal-bolus regimen of NPh insulin given once(n = 30), twice(n = 40) or three times(n = 35) daily, in addition to pre-meal regular insulin. Major outcomes included were differences in glycemic control, frequency of hypoglycemia and total insulin *** NPh insulin given in a once-daily regimen was associated with better glycemic control(58.3%) compared to twice daily(42.4%) and three times daily(48.9) regimens(P = 0.031). The frequency of hypoglycemia was similar between the three groups(2.0%, 0.7% and 1.2%, P = 0.21). The mean insulin dose at discharge was 0.48 ± 0.14 U/kg in the once-daily group compared to 0.69 ± 0.28 in the twice-daily, and 0.65 ± 0.20 in the three times daily regimens(P h insulin administered in a once-daily regimen resulted in improvement in glycemic control with similar rates of hypoglycemia compared to a twice-daily and a three times-daily regimen. Further studies are needed to evaluate whether this regimen could be implemented in all hospitalized patients with hyperglycemia.

Diagnostic and therapeutic challenges of gastrointestinal angiodysplasias: A critical review and view points

Gastrointestinal angiodysplasias (GIADs), also called angioectasias, are the most frequent vascular lesions. Its precise prevalence is unknown since most of them are asymptomatic. however, the incidence may be increasing since GIADs affect individuals aged more than 60 years and population life expectancy is globally increasing worldwide. They are responsible of about 5% to 10% of all gastrointestinal bleeding (GIB) cases. Most GIADs are placed in small bowel, where are the cause of 50 to 60% of obscure GIB diagnosed with video capsule endoscopy. They may be the cause of fatal severe bleeding episodes;nevertheless, recurrent overt or occult bleeding episodes requiring repeated expensive treatments and disturbing patient’s quality-of-life are more frequently observed. Diagnosis and treatment of GIADs (particularly those placed in small bowel) are a great challenge due to insidious disease behavior, inaccessibility to affected sites and limitations of available diagnostic procedures. hemorrhagic causality out of the actively bleeding lesions detected by diagnostic procedures may be difficult to establish. No treatment guidelines are currently available, so there is a high variability in the management of these patients. In this review, the epidemiology and pathophysiology of GIADs and the status in the diagnosis and treatment, with special emphasis on small bowel angiodysplasias based on multiple publications, are critically discussed. In addition, a classification of GIADs based on their endoscopic characteristics is proposed. Finally, some aspects that need to be clarified in future research studies are highlighted.

A review of helicobacter pylori diagnosis, treatment, and methods to detect eradication

helicobacter pylori (h. pylori) affects nearly half of the world’s population and, thus, is one of the most frequent and persistent bacterial infections worldwide. h. pylori is associated with peptic ulcer disease, gastric ulcers, mucosa-associated lymphoid tissue lymphoma, and gastric cancer. Various diagnostic methods exist to detect infection, and the choice of one method or another depends on several factors, such as accessibility, advantages and disadvantages of each method, cost, and the age of patients. Once h. pylori infection is diagnosed, the clinician decides whether treatment is necessity, according to the patient’s clinical condition. Typically, eradication of h. pylori is recommended for treatment and prevention of the infection. Cure rates with the standard triple therapy are acceptable, and effective quadruple therapies, sequential therapies, and concomitant therapies have been introduced as key alternatives to treat h. pylori infection. In this work, we review the main diagnostic methods used to identify h. pylori infection and to confirm eradication of infection. In addition, key factors related to treatment are reviewed.

Meckel's diverticulum diagnosis by video capsule endoscopy: A case report and review of literature

BACKGROUND Meckel’s diverticulum （MD） occurs predominantly in chil-dren and adolescents. It is rarely diagnosed in adults. Preoperative diagnosis is diffcult due to low sensitivity of the radiological imaging studies. The role of video capsule endoscopy （VCE） in the diagnosis of MD is unknown, and the endoscopic patterns are not defned. We will describe four of our cases of MD evaluated with VCE and make a review of the literature focusing on the endoscopic *** SUMMARYWe present four cases of MD confirmed by surgery. They were all adult males with ages going from 18 to 50 years, referred to our service from 2004 to 2018, due to obscure gastrointestinal bleeding （OGIB）. TheyGarcía-Compeán D et al . Merckel’s diverticulum diagnosis by VCEhad a history of 1 mo to 10 years of overt and occult bleeding episodes. Laboratory blood test showed an iron-deficiency anemia from 4 to 9 g/dL of hemoglo-bin that required multiple hospitalizations and blood transfusions in all cases. Repeated upper digestive endoscopies and colonoscopies were negative. Small bowel was examined with VCE, which revealed double lumen images in all cases, one with polyps and three with circumferential ulcers in the diverticulum. however, based on VCE findings, preoperative diagnosis of MD was suggested only in two patients. Capsule was retain-ed in one patient, which was recovered with surgery. The anatomopathological report revealed ulcerated ectopic gastric mucosa in all *** is useful for the diagnosis of MD. however, endo-scopic characteristics must be recognized in order to establish preoperative diagnosis.

Isotropic compression behavior of granular assembly with non-spherical particles by X-ray micro-computed tomography and discrete element modeling

The particle morphological properties,such as sphericity,concavity and convexity,of a granular assembly significantly affect its macroscopic and microscopic compressive behaviors under isotropic loading ***,limited studies on investigating the microscopic behavior of the granular assembly with real particle shapes under isotropic compression were *** this study,X-ray computed tomography(mCT)and discrete element modeling(DEM)were utilized to investigate isotropic compression behavior of the granular assembly with regard to the particle morphological properties,such as particle sphericity,concavity and interparticle *** mCT was first used to extract the particle morphological parameters and then the DEM was utilized to numerically investigate the influences of the particle morphological properties on the isotropic compression *** image reconstruction from mCT images indicated that the presented particle quantification algorithm was robust,and the presented microscopic analysis via the DEM simulation demonstrated that the particle surface concavity significantly affected the isotropic compression *** observations of the particle connectivity and local void ratio distribution also provided insights into the granular assembly under isotropic *** found that the particle concavity and interparticle friction influenced the most of the isotropic compression behavior of the granular assemblies.
