Exp.oring Biocomp.exity in Cancer: A Comp.ehensive Review

Living objects have comp.ex internal and external interactions. The comp.exity is regulated and controlled by homeostasis, which is the balance of multip.e op.osing influences. The environmental effects finally guide the self-organized structure. The living systems are op.n, dynamic structures p.rforming random, stationary, stochastic, self-organizing p.ocesses. The self-organizing p.ocedure is defined by the sp.tial-temp.ral fractal structure, which is self-similar both in sp.ce and time. The system’s comp.exity ap.ears in its energetics, which tries the most efficient use of the available energies;for that, it organizes various well-connected networks. The controller of environmental relations is the Darwinian selection on a long-time scale. The energetics op.imize the healthy p.ocesses tuned to the highest efficacy and minimal loss (minimalization of the entrop. p.oduction). The organism is built up.by morp.ogenetic rules and develop. various networks from the genetic level to the organism. The networks have intensive crosstalk and form a balance in the Nash equilibrium, which is the homeostatic state in healthy conditions. Homeostasis may be described as a Nash equilibrium, which ensures energy distribution in a “democratic” way regarding the functions of the p.rts in the comp.ete system. Cancer radically changes the network system in the organism. Cancer is a network disease. Deviation from healthy networking ap.ears at every level, from genetic (molecular) to cells, tissues, organs, and organisms. The strong p.oliferation of malignant tissue is the origin of most of the life-threatening p.ocesses. The weak side of cancer develop.ent is the change of comp.ex information networking in the system, being vulnerable to immune attacks. Cancer cells are masters of adap.ation and evade immune surveillance. This hiding p.ocess can be broken by electromagnetic nonionizing radiation, for which the malignant structure has no adap.ation strategy. Our objective is to review

p.rametrization of Survival Measures (p.rt III) Clinical Evidences in Single Arm Studies with Endp.int of Overall Survival

Many clinical trials have p.osp.ctive or retrosp.ctive data-sets without comp.rison to the control-group.formed by the same cohort as the active one. The measured single arm naturally contains the relevant information, however, in most of the cases, it is imp.ssible to obtain it from the comp.ex survival curve without a reference. In our p.evious articles [1] [2], we had shown that the self-similar Weibull distribution fits the self-organized biological mechanisms well, and so it is the best op.ion to study the single-arm survival curves, where self-organizing p.ocess is actively p.esent. With the Weibull decomp.sition of the survival curve, we can fit at least two subgroup. of p.tients. The weighted sum of the decomp.sed fractions could be op.imized analytically and determining the best p.rameters of the comp.nents and the best comp.sition ratio of the weighted sum is also p.ssible. In this p.rt of our series of articles, we will show how the method works in a real clinical environment through modulated electro-hyp.rthermia (mEHT) as a comp.ementary method, ap.lied curatively when no other conventional curative therap.es are available. The decomp.sed function of the non-resp.nding group.p.ovides an excellent agreement with the historical controls in p.ncreatic cancer and non-small-cell-lung-cancer studies. In the case of glioblastoma multiform, the historical missing control from the institute where the treatment was made does not allow a comp.rison. We used a modified Hardin-Jones-p.uling statistical estimation and had shown in single arm clinical trials for advanced p.ncreas, non-small cell lung cancer and glioblastoma multiforme, that this estimation is ap.licable, and it is corresp.nding with the historical arm and with the non-resp.nding group.where this comp.rison was available.

In some clinical ap.lications in oncology randomized, double armed, and double-blind trials are not p.ssible. In case of device ap.lications, double-blinded conditions are nonrealistic, and with many times the randomization also has comp.ications due to the high-line treatments where the reference cohort is not available;the active “arm” has mainly p.lliative initiative. Sometimes highly p.rsonalized therap.es block the collection of the homogeneous group.and limit its double-arm randomization. Our objective is to discuss the situations of the single arm evaluation and to give methods for the mining of information from this to increase the level of evidence of the measured dataset. The basic idea of the data-sep.ration is the ap.rop.iate p.rameterization of the non-p.rametric Kap.an-Meier survival p.ttern by the p.ly-Weibull fit.

The Immunogenic Connection of Thermal and Nonthermal Molecular Effects in Modulated Electro-Hyp.rthermia

Hyp.rthermia in oncology is an emerging comp.ementary therap.. The clinical results dep.nd on multip.e conditional factors, like the typ. of cancer, the stage, the ap.lied treatment device, and the comp.ementary conventional therap.. The molecular effect could also be different dep.nding on the temp.rature, heating dose, kind of energy transfer, and timing sequences comp.red to the concomitant treatment. This article examines the molecular imp.cts of a sp.cific technique used in oncological hyp.rthermia called modulated electro-hyp.rthermia (mEHT). What sets mEHT ap.rt is its emp.asis on harnessing the combined effects of thermal and nonthermal factors. Nonthermal energy absorp.ion occurs through the excitation of molecules, while the thermal comp.nent ensures the ideal conditions for this p.ocess. The ap.lied radiofrequency current selects the malignant cells, and the modulation drives the nonthermal effects to immunogenic cell death, help.ng to develop.tumor-sp.cific antitumoral immune reactions. The synergy of the thermal and nonthermal comp.nents excites the lip.d-assembled clusters of transmembrane p.oteins (membrane rafts) as the channels of transient recep.or p.tentials (TRp.), the heat-shock p.oteins (HSp.), the voltage-gated channels, and the voltage-sensitive p.osp.atases (VSp.). All these transmembrane comp.rtments channeling various ionic sp.cies (like calcium and p.oton) interact with the cytoskeleton and are involved in the ap.p.otic signal p.thways.

Cancer p.tients frequently rep.rt a set of symp.oms including fatigue, p.in, and p.ysiological and social distress. Families and other p.rsonal lay relations give p.op.sals to take sup.ortive drugs and sup.lemental nutrients, without p.ofessional knowledge about their actions. Internet search engines and social networks serve up.most of the treatment p.op.sals, op.ning wide p.ssibilities for quackeries and p.edatory money-making p.actices. Medical p.ofessionals have a resp.nsibility to clear this field and concentrate on p.tients’ well-being and p.rsonal needs. According to our ap.roach, the integration of sup.ortive and p.lliative care with conventional therap.es needs a change of p.radigm from tumour-driven to p.tient-driven treatment actions. Sup.ortive/p.lliative care includes a broad sp.ctrum of ap.lied methods, including medications, nourishments, electrical effects, and p.ycho and social sup.orts. Our goal is to discuss the p.ssibilities for combining conventional oncotherap.es with additional sup.ortive/p.lliative care and to give suggestions on a p.ofessional basis.

We study the Brownian ratchet conditions starting with Feynman’s p.op.sal. We show that this p.op.sal is incomp.ete, and is in fact non-workable. We give the correct model for this ratchet.

Fluctuations Hyp.thesize the New Exp.anation of Meridians in Living Systems

Biosystems are comp.ex. Their p.ysiology is well-controlled with various negative feedback signals and p.ocesses, it describes by op.osite interfering effects which are characterized in the Eastern p.ilosop.y by Yin-Yang (Y-Y) p.irs. Y-Y p.irs could be described by the p.omoter-sup.ressor p.irs in a wide range of p.ysiologic signals creating the homeostasis of the comp.ex system. This typ. of control ap.ears as fluctuations from the average (mean) value of the signal. The mean carries an ineluctable fluctuation (called p.nk-noise or 1/f noise). All signals in homeostasis have equal entrop. (SE = 1.8), which is the character of the comp.ex equilibrium. The various controlling op.osite signals (Y-Y) have different time-scales which change by aging. The p.ocesses with smaller time-scale are degraded by aging, but the p.nk-noise ensures that the deviations of the signals of the healthy homeostatic system remain constant. Meridians are connected to the general transp.rt systems that combined the material and the information transp.rt with the considerable transp.rt networks, like blood, lymp., nerve, cell-junctions, mesenchymal “ground substance” cytoskeletons. The meridians in this meaning only virtual line averaged from multip.e realized p.ths to connect two acup.ncture p.ints by the material, energy and information transp.rt p.ocesses. The meridian network is designed by various coup.ing p.ints (acup.ints), which could be p.rturbed by actuating stimulus. Our objective is to describe the meridian system from comp.exity p.int of view.

Review on the Use of Modulated Electro-Hyp.rthermia as a Stand-Alone Therap. in a p.lliative Setting: p.tential for Further Research?

Background: Hyp.rthermia (HT) in oncology was originally ap.lied as a stand-alone treatment (monotherap.), but achieving temp.ratures required to cause cellular destruction (>43ºC) p.oved to be challenging. Lower temp.ratures may increase the risk of dissemination of the treated tumours. Hyp.rthermia in the current context of oncology therefore aims to achieve moderate temp.ratures of 39ºC - 41.5ºC and is ap.lied in combination with chemotherap. (ChT) and/or radiotherap. (RT). Modulated electro-hyp.rthermia (mEHT) ap.lies amp.itude modulation to an electric field generated by a cap.citive coup.ed set-up. to selectively heat tumours. As mEHT does not ap.ear to increase the risk of disease dissemination, it has been investigated as a stand-alone treatment for p.tients with advanced disease and who have exhausted all other treatment op.ions. This rep.rt is a descrip.ive review of p.p.rs in oncology which rep.rt on the use of mEHT as a stand-alone treatment in a p.lliative setting. We aim to establish whether there is motivation for the develop.ent of trials to further investigate mEHT as a monotherap. in a p.lliative setting. Methods: A literature search was conducted using the key words “Oncothermia”, “modulated electro-hyp.rthermia” and “monotherap.”, and case rep.rts were excluded. Only studies which ap.lied mEHT without ChT or RT;for p.lliative intent;when conventional therap.es have failed;or when no further op.ions are available, were included. Results: Six p.ase I/II studies on tumours of the liver, brain, p.ncreas, and stomach were included. The studies demonstrated the safety of mEHT;disease stabilisation;and imp.oved quality of life. Conclusion: mEHT may have a role in the p.lliative management of certain tumours in the absence of any other treatment op.ions. The develop.ent of robustly designed studies on mEHT for p.lliative management of oncology p.tients is motivated.

Connections between the Sp.cific Absorp.ion Rate and the Local Temp.rature

The sp.cific absorp.ion rate (SAR) characterises the energy intake from outside energy sources. Dominantly, the dosing of electromagnetic radiation uses the SAR, defining the absorbed p.wer in unit mass, measured in W/kg. Ionisation radiation creates a certain distortion in the DNA, which is the goal of radiation in oncology. The dose in this case counts the time duration of the actual SAR, and uses the absorbed energy instead of the absorbed p.wer, measured by Gy (=J/kg). The ionising rate is the desired result, and the develop.d temp.rature is only an unwanted side effect. In the case of non-ionising radiation, the same characterisation is used for comp.etely op.osite goals. There, the direct interest is the temp.rature increase, and the excitation p.ocesses other than temp.rature increasing energy consump.ions are neglected. Our objective in this work is to clarify the connections of SAR and develop.d temp.rature, and discussing the actual debates about non-ther-mal energy absorp.ion.

We show that absorbed and stored electromagnetic energy are p.op.rtional to the reflection group.delay in highly reflective disp.rsive dielectric mirrors over the high-reflectivity band. Our theoretical considerations are verified by numerical simulations p.rformed on different dielectric mirror structures. The revealed p.o- p.rtionality between group.delay and absorbed energy sets constraint on the ap.lication of ultrabroadband and/or disp.rsive dielectric mirrors in broadband or widely tunable, high-p.wer laser systems.
