AIM:To evaluate the effect of pantoprazole with a somatostatin adjunct in patients with acute non-variceal upper gastrointestinal bleeding(NVUGIB).METHODS:We performed a retrospective analysis of a prospective database in a tertiary care university *** October 2006 to October 2008,we enrolled 101 patients with NVUGIB that had a high-risk stigma on *** 24 h of hospital admission,all patients underwent endoscopic *** successful endoscopic hemostasis,all patients received an 80-mg bolus of pantoprazole followed by continuous intravenous infusion(8 mg/h for 72 h).The somatostatin adjunct group(n=49)also received a 250-μg bolus of somatostatin,followed by continuous infusion (250μg/h for 72 h).Early rebleeding rates,disappearance of endoscopic stigma and risk factors associated with early rebleeding were ***:Early rebleeding rates were not significantly different between treatment groups(12.2%vs 14.3%,P=0.766).Disappearance of endoscopic stigma on the second endoscopy was not significantly different between treatment groups(94.2%vs 95.9%,P=0.696).Multivariate analysis showed that the complete Rockall score was a significant risk factor for early rebleeding(P =0.044,OR:9.080,95%CI:1.062-77.595).CONCLUSION:The adjunctive use of somatostatin was not superior to pantoprazole monotherapy after successful endoscopic hemostasis in patients with NVUGIB.

Understanding microstructure and mechanical properties of(AlTa(0.76))xCoCrFeNi(2.1) eutectic high entropy alloys via thermo-physical parameters

(AlTa(0.76))xCoCrFeNi(2.1)(x values in molar ratio,x=0.1,0.3,0.5,0.7,1.0,and 1.5) alloys were designed to investigate the microstructure and mechanical properties of the eutectic high entropy alloys(EHEAs)consisting of FCC,B2,and Laves *** on the compositional variatio,clear microstructural variation was observed,as follows:(1) Group 1:FCC dendrite+Laves interdendrite(x=0.1),(2) Group 2:FCC dendrite+fine-eutectic structure consisting of FCC and Laves phases(x=0.3,0.5 and 0.7),(3) Group 3:B2 dendrite+bimodal eutectic structure [FCC/B2+Laves](x=1.0),(4) Group 4:Laves dendrite+eutectic structure consisting of B2 and Laves phases(x=1.5).As the fraction of Laves or B2 phases increases,yield stress increases from 293 to 2336 MPa,while the plastic strain decreases from 50 % to 2%.Thermo-physical parameters,such as mixing entropy(△S(mix)),mixing enthalpy(△H(mix)),valence electron concentration(VEC),and atomic size difference(δr),were calculated to understand the microstructural *** criteria(δr-VEC and δr-△H(mix)) were utilized to elucidate the formation of the eutectic structures in the present EHEAs,revealing the usefulness of the thermo-physical parameters in the development of EHEAs.

In this study,metallic glass is designed tunable for promising functional applications via a facile and stable,one-step *** approach about alloy design and process is based on the unique properties of the Ti-Cu-Ni-Sn metallic glass that are afforded by the varying oxygen affinities of the four constituent *** metallic glass exhibits homogeneous nucleation of oxides and uniform formation of the various metal *** alloy design and hydrothermal synthesis to customize the metallic glasses are the most critical factors that determine the characteristics of the metal oxides and the resulting nanostructures and photoelectrode *** enables the desired element to be selectively grown via a facile single-step *** results provide a promising road map for the application of metallic glasses to photoelectrochemical(PEC)solar water splitting,as well as various other new possibilities.

Ti45Cu40Ni7Zr5Sn2.5Si0.5alloys were prepared under various cooling rate conditions during *** alloys exhibited different volume fractions of B2 particles with 0～40 vol%in an amorphous *** bulk metallic glass of 1 mm diameter showed no macroscopic plasticity and exhibited the typical vein patterns in a maximum shear stress plane on the fracture ***,a bulk metallic glass composite containing the B2 particles revealed obvious plasticity(～5.6%)with a remarkable work-hardening behavior that resulted from a stress-induced martensitic transformation of the B2 ***,the composite displayed the complicated fracture morphologies containing of three types of fracture *** detailed investigations on the microstructural evolution,mechanical,deformation and fracture characteristics,the influence of B2 particle on overall behavior of the TiCu-based bulk metallic glass composites was elucidated.

Rigidity(or stiffness)of materials and extracellular matrix has proven to be one of the most significant extracellular physicochemical cues that can control diverse cell behaviors,such as contractility,motility,and spreading,and the resultant pathophysiological *** 2D materials engineered with tunable rigidity have enabled researchers to elucidate the roles of matrix biophysical cues in diverse cellular events,including migration,lineage specification,and mechanical ***,the recent findings accumulated under 3D environments with viscoelastic and remodeling properties pointed to the importance of dynamically changing rigidity in cell fate control,tissue repair,and disease ***,here we aim to highlight the works related with material/matrix-rigidity-mediated cell and tissue behaviors,with a brief outlook into the studies on the effects of material/matrix rigidity on cell behaviors in 2D systems,further discussion of the events and considerations in tissue-mimicking 3D conditions,and then examination of the in vivo findings that concern material/matrix *** current discussion will help understand the material/matrix-rigidity-mediated biological phenomena and further leverage the concepts to find therapeutic targets and to design implantable materials for the treatment of damaged and diseased tissues.

The programmed cell death ligand 1(PD-L1)and its receptor programmed cell death 1(PD-1)deliver inhibitory signals to regulate immunological tolerance during immune-mediated ***,the role of PD-1 signaling and its blockade effect on human dental pulp stem cells(h DPSCs)differentiation into the osteo-/odontogenic lineage remain *** show here that PD-L1 expression,but not PD-1,is downregulated during osteo-/odontogenic differentiation of h ***,PD-L1/PD-1 signaling has been shown to negatively regulate the osteo-/odontogenic differentiation of h ***,depletion of either PD-L1 or PD-1 expression increased ERK and AKT phosphorylation levels through the upregulation of Ras enzyme activity,which plays a pivotal role during h DPSCs osteo-/odontogenic *** with nivolumab(a human anti-PD-1 monoclonal antibody),which targets PD-1 to prevent PD-L1 binding,successfully enhanced osteo-/odontogenic differentiation of h DPSCs through enhanced Ras activity-mediated phosphorylation of ERK and *** findings underscore that downregulation of PD-L1 expression accompanies during osteo-/odontogenic differentiation,and h DPSCs-intrinsic PD-1 signaling inhibits osteo-/odontogenic *** findings provide a significant basis that PD-1 blockade could be effective immunotherapeutic strategies in h DPSCs-mediated dental pulp regeneration.

BACKGROUND Vasculitis,a systemic disorder with inflammation of blood vessel walls,can develop broad spectrum of signs and symptoms according to involvement of various organs,and therefore,early diagnosis of vasculitis is *** herein describe a patient who developed a special case of systemic vasculitis with mononeuropathy multiplex,rectal perforation and antiphospholipid syndrome(APS)presented with pulmonary *** SUMMARY A 61-year-old woman visited hospital with complaints of myalgia and occasional *** was initially diagnosed as proctitis and treated with antibiotics,however,there was no *** addition,she also complained right foot drop with hypesthesia,and left 2^(nd) and 3^(rd) finger tingling *** underwent nerve conduction study for evaluation,and it revealed sensorimotor polyneuropathy in the left arm and bilateral *** sural nerve biopsy strongly suggested vasculitic *** on nerve biopsy and clinical manifestation,she was diagnosed with vasculitis and treated with immunosuppressive *** treatment,sudden rectal perforation and pulmonary thromboembolism occurred,and further laboratory study suggested probable concomitant *** Hartmann operation was performed for rectal perforation,and anti-coagulation therapy was started for *** few cycles of immunosuppressive therapy,tingling sensation and weakness in her hand and foot had been partially recovered and vasculitis was considered to be *** Vasculitis can be presented with a variety of signs and symptoms,therefore,clinicians should always consider the possibility of diagnosis.

Universality class of wave chaos emerges in many areas of science,such as molecular dynamics,optics,and network *** this work,we generalize the wave chaos theory to cavity lattice systems by discovering the intrinsic coupling of the crystal momentum to the internal cavity *** cavity-momentum locking substitutes the role of the deformed boundary shape in the ordinary single microcavity problem,providing a new platform for the in situ study of microcavity light *** transmutation of wave chaos in periodic lattices leads to a phase space reconfiguration that induces a dynamical localization *** degenerate scar-mode spinors hybridize and non-trivially localize around regular islands in phase *** addition,we find that the momentum coupling becomes maximal at the Brillouin zone boundary,so the intercavity chaotic modes coupling and wave confinement are significantly *** work pioneers the study of intertwining wave chaos in periodic systems and provide useful applications in light dynamics control.

The moiré superlattice of misaligned atomic bilayers paves the way for designing a new class of materials with wide *** this work,we propose a photonic analog of the moiré superlattice based on dielectric resonator *** sharp contrast to van der Waals materials with weak interlayer coupling,we realize the strong coupling regime in a moiré superlattice,characterized by cascades of robust flat bands at large ***,we find that these flat bands are characterized by a non-trivial band topology,the origin of which is the moiré pattern of the resonator *** physical manifestation of the flat band topology is a robust one-dimensional conducting channel on edge,protected by the reflection symmetry of the moiré *** explicitly breaking the underlying reflection symmetry on the boundary terminations,we show that the first-order topological edge modes naturally deform into higher-order topological corner *** work pioneers the physics of topological phases in the designable platform of photonic moiré superlattices beyond the weakly coupled regime.
