Exenatide efficacy in unselected patients: Comparison with clinical trials

Exenatide is a Glucagon-Like Peptide-1 (GLP-1) analog improving fasting and post-prandial glycaemia, approved in patients treated with a combination of sulfonylureas and metformin and not achieving good metabolic control. This retrospective cohort study compared the efficacy and safety of exenatide in unselected patients with outcomes of FDA trials and other uncontrolled published studies. Exenatide was prescribed in 108 patients between April 2008 and December 2009, 80% of the patients were treated with 10 ug of exenatide twice daily. Mean follow up was 4.5 months for the 1st follow up. Changes in HbA1c (-1.1%), BMI (-1.1) and fasting glucose (-0.61 g/l) from baseline were significant, and coherent with controlled trials. 32% of patients had at least one side effect, often gastro intestinal. 23% stopped the treatment, a quarter (1/4) of which for inefficacity. Patients with a higher BMI (>40) (n = 29) at baseline did not show a better response in HbA1c or weight. Results at the second follow-up show a slight tendency of rising on HbA1c, BMI and fasting blood glucose. Finally, a third follow-up at a mean of 17 months is available for 11 patients, which shows stability in HA1c, BMI and fasting glucose. In our experience, Exenatide was safe and effective in reducing HbA1c and weight in patients who were able to tolerate it. Our results are close to results published elsewhere. There are still gaps in the literature on exenatide, especially on the long term effect, which need to be assessed by more publications.

BACKGROUND The association between the intestinal microbiota and psychiatric disorders is becoming increasingly *** gut microbiota contributes to colorectal carcinogenesis(CRC),as demonstrated with colibactin-producing Escherichia coli(CoPEC).AIM To evaluate the association between CoPEC prevalence and anxiety-and depressive-like behaviors with both preclinical and clinical *** Patients followed after a CRC surgery and for whom the prevalence of CoPEC has been investigated underwent a psychiatric *** were compared according to the CoPEC *** parallel C57BL6/J wild type mice and mice with a CRC susceptibility were chronically infected with a CoPEC *** behavior was assessed using the Elevated Plus Maze test,the Forced Swimming Test and the Behavior recognition system PhenoTyper®.RESULTS In a limited cohort,all patients with CoPEC colonization presented with psychiatric disorders several years before cancer diagnosis,whereas only one patient(17%)without CoPEC *** result was confirmed in C57BL6/J wildtype mice and in a CRC susceptibility mouse model(adenomatous polyposis colimultiple intestinal neoplasia/+).Mice exhibited a significant increase in anxiety-and depressive-like behaviors after chronic infection with a CoPEC *** This finding provides the first evidence that CoPEC infection can induce microbiota-gut-brain axis disturbances in addition to its procarcinogenic properties.

＜正＞ Thanks to their full time availability and in compliance with French and international recommendations, emergency departments are in the frontline when it comes to starting the initial treatment of people who have been exposed to HIV. This project aims at assessing the tolerance as well as the observance of triple therapy over a period of 1 month （lamivudine 150 mg + zidovudine 300mg - Com-bivir（R）- and nelfinavir 250 mg - Viracept（R） - ）. Methods: Prospective and descriptive study with clinical and biological follow - up （3 biological checkups）. Findings: 50 patients were included. The main side effects were digestive disorders （66% at the start of treatment and 42% by the end of treatment）, neurological disorders （36 % ）. asthenia （36 % ）. respiratory disorders （16 % ） and rheumatological disorders （8% ）. One patient suffered a mild biological pancreatitis 344 UI. L - 1 lipasemia). requiring a change in triple therapy. 12 % of patients had developed cytolysis （this disorder getting back to standard levels one month after start of treatment）. The tolerance of Combivir（ R） - Viracept（ R） therapy is satisfactory （only one change in treatment, no serious side effect）.
