Role of multi-parametric MRI of the prostate for screening and staging:Experience with over 1500 cases

Objective:Contemporary prostate cancer(PCa)screening modalities such as prostate specific antigen(PSA)and digital rectal examination(DRE)are limited in their ability to predict the detection of clinically significant ***-parametric magnetic resonance imaging(mpMRI)of the prostate has been explored as a staging modality for *** is known regarding its utility as a primary screening *** examined our experience with mpMRI as both a screening and staging ***:mpMRI studies performed between 2012 and 2014 in patients without PCa were cross-referenced with transrectal ultrasonography(TRUS)biopsy *** analyses were performed to determine association of mpMRI findings with overall cancer diagnoses and clinically significant(Gleason score≥7)*** analyses were then performed on patients with a history of prior negative biopsy and those without a history of TRUS *** studies were also cross-referenced with RP *** analyses determined predictive ability of extracapsular extension(ECE),seminal vesicle involvement(SVI),and pathologic evidence of clinically significant disease(Gleason score7).Results:Four hundred biopsy naive or prior negative biopsy patients had positive mpMRI *** sensitivity,specificity,positive and negative predictive values were 94%,37%,58%,and 87%,respectively and 95%,31%,42%,and 93%,respectively for overall cancer detection and Gleason score≥7 *** patients with no prior biopsy history,mpMRI sensitivity,specificity,positive and negative predictive values were 94%,36%,65%,and 82%,for all cancers,and 95%,30%,50%,and 89%for Gleason score7 lesions,*** those with prior negative biopsy sensitivity,specificity,positive and negative predictive values were 94%,37%,52%,and 90% for all cancers,and 96%,32%,36%,and 96% for Gleason score7 lesions,***-four patients underwent radical prostatectomy(RP)after *** size on mpMRI corr

While emerging data suggest nucleotide oligomerization domain receptor 1(NOD1),a cytoplasmic pattern recognition receptor,may play an important and complementary role in the immune response to bacterial infection,its role in cancer metastasis is entirely ***,we sought to determine the effects of NOD1 on ***1 expression in paired human primary colon cancer,human and murine colon cancer cells were determined using immunohistochemistry and immunoblotting(WB).Clinical significance of NOD1 was assessed using TCGA survival data.A series of in vitro and in vivo functional assays,including adhesion,migration,and metastasis,was conducted to assess the effect of NOD1.C12-iE-DAP,a highly selective NOD1 ligand derived from gram-negative bacteria,was used to activate ***130,a specific NOD1 inhibitor,was used to block C12-iE-DAP *** knockdown(KD)of NOD1 in human colon cancer cells(HT29)was constructed with shRNA lentiviral transduction and the functional assays were thus ***,the predominant signaling pathway of NOD1-activation was identified using WB and functional assays in the presence of specific kinase *** data demonstrate that NOD1 is highly expressed in human colorectal cancer(CRC)and human and murine CRC cell ***,we demonstrate that this increased NOD1 expression negatively impacts survival in patients with ***,we identify NOD1 activation by C12-iE-DAP augments CRC cell adhesion,migration and *** effects are predominantly mediated via the p38 mitogen activated protein kinase(MAPK)*** is the first study implicating NOD1 in cancer metastasis,and thus identifying this receptor as a putative therapeutic target.

Designing electrocatalysts for HER in alkaline conditions to overcome the sluggish kinetics associated with the additional water dissociation step is a recognized challenge in promoting the hydrogen *** this end,delicately tuning the atomic-scale structure and surface composition of nanoparticles is a common strategy and,specifically,making use of hybrid structures,can produce synergistic effects that lead to highly active ***,we present a core-shell catalyst of Ag@MoS_(2)that shows promising results towards the hydrogen evolution reaction(HER)in both 0.5 M H_(2)SO_(4)and 0.5 M *** this hybrid structure,the MoS_(2)shell is strained and defective,and charge transfer occurs between the conductive core and the shell,contributing to the electrocatalytic *** shelling process results in a large fraction of Ag_(2)S in the cores,and adjusting the relative fractions of Ag,Ag_(2)S,and MoS_(2)leads to improved catalytic activity and fast charge-transfer *** suggest that the enhancement of alkaline HER is associated with a cooperative effect of the interfaces,where the Ag(Ⅰ)sites in Ag_(2)S drive the water dissociation step,and the formed hydrogen subsequently recombines on the defective MoS_(2)*** study demonstrates the benefits of hybrid structures as functional nanomaterials and provides a scheme to activate MoS_(2)for HER in alkaline conditions.

Biermer Disease in an Unusual Neurological Presentation without Anemia: A Case Report

Background: Biermer disease is a megaloblastic disease caused by vitamin B12 deficiency. It is a rare clinical entity especially in subsahara Africa. Case presentation: We report the case of a 45 years old female patient who consulted for a one month history of generalised muscle cramps, weakness and numbness of all four limbs. Physical examination was relevant for a poor gait, poor coordination of both upper and lower limbs, a positive Romberg sign, normal muscle tone in all four limbs, reduced pallesthesia and deep tendon reflexes, abolished plantar reflexes. Paraclinical investigations revealed macrocytosis without anemia, a low cyanocobalamin (vitamin B12) level with a normal folic acid level, an atrophic corporeofundic mucosa which upon pathological analysis revealed a chronic atrophic gastritis with no Helicobacter pylori infection. Anti-intrinsic factor antibodies were positive while anti parietal cells antibodies were negative. The diagnosis of Biermer disease was considered and the patient did well on vitamin B12 supplementation. Conclusion: Though a rare disease, Biermer disease should be considered in a patient who consults for polyneuropathy even in the absence of anemia.

A Practical Method to Evaluate and Verify Dose Calculation Algorithms in the Treatment Planning System of Radiation Therapy

Purpose: To introduce a practical method of using an Electron Density Phantom (EDP) to evaluate different dose calculation algorithms for photon beams in a treatment planning system (TPS) and to commission the Anisotropic Analytical Algorithm (AAA) with inhomogeneity correction in Varian Eclipse TPS. Methods and Materials: The same EDP with various tissue-equivalent plugs (water, lung exhale, lung inhale, liver, breast, muscle, adipose, dense bone, trabecular bone) used to calibrate the computed tomography (CT) simulator was adopted to evaluate different dose calculation algorithms in a TPS by measuring the actual dose delivered to the EDP. The treatment plans with a 6-Megavolt (MV) single field of 20 × 20, 10 × 10, and 4 × 4 cm2 field sizes were created based on the CT images of the EDP. A dose of 200 cGy was prescribed to the exhale-lung insert. Dose calculations were performed with AAA with inhomogeneity correction, Pencil Beam Convolution (PBC), and AAA without inhomogeneity correction. The plans were delivered and the actual doses were measured using radiation dosimetry devices MapCheck, EDR2-film, and ionization chamber respectively. Measured doses were compared with the calculated doses from the treatment plans. Results: The calculated dose using the AAA with inhomogeneity correction was most consistent with the measured dose. The dose discrepancy for all types of tissues covered by beam fields is at the level of 2%. The effect of AAA inhomogeneity correction for lung tissues is over 14%. Conclusions: The use of EDP and Map Check to evaluate and commission the dose calculation algorithms in a TPS is practical. In Varian Eclipse TPS, the AAA with inhomogeneity correction should be used for treatment planning especially when lung tissues are involved in a small radiation field.

As biomarkers are important in the early diagnosis ofAlzheimer’s disease （AD）, the frst collab-orative work of recruiting early-onset familial AD （EO-FAD） families in Canada and China was initiated in 2012. The registration networks have collected hundreds of pedigrees, for which genetic screening, neuropsycholog-ical tests and amyloid and tau imaging was used to study diagnostic biomarkers for preclinical and mild cognitive impairment （MCI） stages of AD. Besides identifying ped-igrees with novel mutations in presenilins （PSENs）/amy-loid precursor protein （APP）, the program has benefted training of Chinese research fellows, AD clinical trials forprevention,the ethical concernsfor clinical fndings, and other collaborative projects with Chinese investiga-tors. Further research of the collaborative program may facilitate the testing and clinical use of novel treatments for EOFAD and late onset AD and contribute to dementia prevention strategies in Canada and China.

Recurrent missense mutation of GDF5 (p.R438L) causes proximal symphalangism in a British family

Proximal symphalangism(SYM1B)(OMIM 615298) is an autosomal dominant developmental disorder affecting joint fusion. It is characterized by variable fusions of the proximal interphalangeal joints of the hands,typically of the ring and little finger,with the thumb typically being spared. SYM1 is frequently associated with coalition of tarsal bones and conductive hearing loss. Molecular studies have identified two possible genetic aetiologies for this syndrome,NOG and GDF5. We herein present a British caucasian family with SYM1 B caused by a mutation of the GDF5 gene. A mother and her three children presented to the orthopaedic outpatient department predominantly for feet related problems. All patients had multiple tarsal coalitions and hand involvement in the form of either brachydactyly or symphalangism of the proximal and middle phalanx of the little fingers. Genetic testing in the eldest child and his mother identified a heterozygous missense mutation in GDF5 c.1313G>T(p.R438L),thereby establishing SYM1 B as the cause of the orthopaedic problems in this family. There were no mutations identified in the NOG gene. This report highlights the importance of thorough history taking,including a three generation family history,and detailed clinical examination of children with fixed planovalgus feet and other family members to detect rare skeletal dysplasia conditions causing pain and deformity,and provides details of the spectrum of problems associated with SYM1 B.

Are local plague endemicity and ecological characteristics of vectors and reservoirs related? A case study in north-east Tanzania

The pattern of plague endemicity in Tanzania is characterized by continuous re-appearance of the disease in some locations, while in other neighbouring villages the disease has never or seldom been observed. To understand the reasons for this pattern, we studied small mammal and flea species composition, diversity and relative abundances in two plague-endemic and two plague-free locations. We asked （a） whether fleas more abundant in plague-endemic locations differ in their characteristic abundance and the degree of their host specificity from fleas more abundant in plague-free locations and （b） whether hosts most abundant in p/ague-endemic locations differ in the diversity of their flea assemblages from hosts most abundant in p/ague-free locations. We characterized （a） each host species by species richness and degree of taxonomic relatedness of its flea assemblages and （b） each flea species by its mean abundance and size, and degree of taxonomic relatedness of its host spectrum and compared their relative abundances between locations. No significant difference between plague-endemic and plague-free locations in either host density or any variable related to flea abundance or diversity was found. However, there was marginally significant effect of taxonomic distinctness of a flea assemblage barboured by a host on its probability to be more abundant in either plague-endemic or plague-free locations. Furthermore, hosts more abundant in plague-endemic locations tended to harbour closely-related fleas. Finally, while opportunistic and specialist fleas were equally distributed in both sets of locations, fleas exploiting distantly-related hosts were found mainly in plague-free locations during the rainy season. We suggest that the observed patterns might arise due to seasonal and spatial differences in local microclimatic conditions and landscape connectivity [ Current Zoology 55 （3）： 200 - 211, 2009].
