Clinical indices and mortality of hospitalized avian influenza A (H7N9) patients in Guangdong, China

Background:six epidemic waves of human infection with avian influenza A (H7N9) virus have emerged in China with high mortality. However, study on quantitative relationship between clinical indices in ill persons and H7N9 outcome (fatal and nonfatal) is still unclear. A retrospective cohort study was conducted to collect laboratory-confirmed cases with H7N9 viral infection from 2013 to 2015 in 23 hospitals across 13 cities in Guangdong Province, ***:Multivariable logistic regression model and classification tree model analyses were used to detect the threshold of selected clinical indices and risk factors for H7N9 death. The receiver operating characteristic curve (ROC) and analyses were used to compare survival and death distributions and differences between indices. A total of 143 cases with 90 survivors and 53 deaths were ***:Average age (Odds Ratio (OR)=1.036, 95% Confidence Interval (CI)=1.016-1.057), interval days between dates of onset and confirmation (OR=1.078, 95% CI=1.004-1.157), interval days between onset and oseltamivir treatment (OR=5.923, 95% CI=1.877-18.687), body temperature (BT) (OR=3.612, 95% CI=1.914-6.815), white blood cell count (WBC) (OR=1.212, 95% CI=1.092-1.346) were significantly associated with H7N9 death after adjusting for confounders. The chance of death from H7N9 infection was 80.0% if BT was over 38.1 °C, and chance of death is 67.4% if WBC count was higher than 9.5 (109/L). Only 27.1% of patients who began oseltamivir treatment less than 9.5 days after disease onset died, compared to 68.8% of those who started treatment more than 15.5 days after ***:The intervals between date of onset and confirmation of diagnosis, between date of onset to oseltamivir treatment, age, BT and WBC are found to be the best predictors of H7N9 mortality.

Recent studies investigating possible causes of male subfertility have largely focused on how lifestyle or environmental factors impact on the process of spermatogenesis, Markedly, fewer studies have investigated those risk factors that result in reduced sperm quality, such as poor sperm motility. The speed at which sperm swim is a major predictor of fertility and is extremely variable in human populations. It has been hypothesized that offspring sex may be adaptively manipulated to maximize the offspring＇s reproductive fitness （e.g., parents with genes for good male fertility traits, such as high sperm speed, would produce primarily sons and fewer daughters because the offspring will inherit advantageous male fertility genes）. Conversely, parents with poor male fertility genes would produce primarily daughters, We tested whether there was an association between how fast a man＇s sperm swam and the sex bias of his siblings in a sample of men attending clinic for fertility investigations with their partner and with a wide range of semen characteristics, including sperm speed. We found that the sex bias of a man＇s siblings is associated with his sperm speed; men with female-biased siblings had significantly slower sperm （judged using computer-assisted sperm analysis （CAsA）） than men from male-biased sibships. This observation suggests family composition is an important factor that needs to be considered in future eDidemiological and clinical studies of human fertility,

The horizontal single belt casting(HsBC)incorporating a single-impingement feeding system was simulated with an improved numerical *** experiments were carried out on the pilot-scale caster for *** results show that the meniscus turbulence neither comes from the tundish region,nor from the impingement between the melt and the moving *** is the moving belt that gives rise to this high turbulence region,and this region can stir the melt near the *** feeding system studied and the moving belt give rise to a buffer region,which can optimize casting parameter variations,especially melt depth changes in the *** temperature change rate of the bottom surface of the strip is around 4 times faster than that of the upper surface.

Highly Branched Poly(α-Methylene-γ-Butyrolactone) from Ring-Opening Homopolymerization

New heteroleptic lanthanide complex [L1ILaN{si(CH3)2}] (1) containing tridentate [ONO] ancillary ligand was synthesized from an acid-base ligand exchange reaction with ligands H2L1 and corresponding homoleptic lanthanide compound La[N{si(CH3)3}2]3. Meanwhile, dimeric complexes [L1LaCl] (2) and [L1ILaCl] (3) were prepared from salt metathesis reaction between one equivalent of ligands H2LI,II, three equivalent of NaN{si(CH3)3}2, and one equivalent of LaCl3. These compounds were characterized by nuclear magnetic resonance (300 MHz) and elemental analysis. These complexes were used as catalysts in the ring-opening homopolymerization of α-methylene-γ-butyrolactone. While compound 1 did not show any significant reactivity, compounds 2 and 3 gave significant amount of highly branched poly(α-methylene-γ-butyrolactone) as confirmed by 1H NMR spectroscopy and Malvern’s triple detector GPCMax analysis in DMsO with molecular weights of over 500,000 Dalton. The glass-transition temperatures of the branched polymer samples were determined using a Dynamic Mechanical Analyzer, DMA Q800.

This study investigated functional enhancement strategies for whole egg(WE),egg yolk(EY)and whey protein concentrate(WPC)with a view to reducing their required concentrations in formulations and the resulting environmental *** and EY underwent a combined phospholipase PLA2 and controlled heat treatment(WE2 and EY2)while WPC was partially-denatured via controlled heating(pdWPC).WE2 and EY2 samples were mixed with pdWPC and were also control-heated(h)with ***2-WPC(h)foams proved more stable than their untreated ***2,EY2 and EY2-pdWPC showed higher emulsifying properties than their untreated ***,EY2 and EY2-WPC(h)hydrogels proved more viscoelastic than their untreated ***2 and EY2-WPC(h)displayed an additional 45 kDa protein band,which could correspond to surface-active apoproteins released from *** work highlighted the potential to enhance WE,EY and WPC functionality via combined partial denaturation treatments and via synergy between WPC and WE or EY when co-processed.

Background/Purpose: There is a considerable variation in the use of vascular imaging techniques in the preoperative assessment of children scheduled for liver transplantation. Duplex Doppler ultrasound scan (Us), magnetic resonance angiography (MRA), and conventional angiography are used to varying extents. The authors compared the results of preoperative vascular imaging studies with operative findings to determine their accuracy and usefulness. Methods: Results of preoperative vascular imaging in 37 consecutive children undergoing cadaveric liver transplantation were compared with operative findings. Those undergoing relatively elective transplantations were investigated by Us and MRA (group 1), whereas those requiring urgent transplants were assessed only by Us (group 2). Results: The median age of the cohort (15 boys; 22 girls) was 4 years (19 days to 16 years) and the median weight was 17 kg (2.9 to 82 kg). In group 1 (n = 26), 20 children had a normalcaliber, patent portal vein at transplant and 6 had a narrow but patent portal vein requiring venous reconstruction in 4. The sensitivity and specificity of MRA in the detection of an abnormally narrow portal vein were 100% (6/6) and 95% (19/20), respectively. If reversed or absent flow in the portal vein on Us was taken as an indication of a potentially abnormal vein, the sensitivity and specificity of Doppler Us were 83% (5/6) and 95% (19/20), respectively. Magnetic resonance angiography revealed arterial anomalies in 4 children but failed to detect small accessory hepatic arteries in 5. The single patient with an aberrant vena cava was identified by MRA. In group 2 (n = 11), venous findings at operation and on Us were concordant in 10 (91% ) cases; one infant with reversed flow in the portal vein on Us had a thrombosed vein at surgery.
