Metastasis to Paranasal sinuses as the First Presenting sign of Renal Cell Carcinoma

The clear cell carcinoma is a relatively rare tumor, represented approximately 85% by renal cell carcinoma (RCC), but has a characteristic of early metastatic, which sometimes spread to unusual places. Metastatic tumors in the paranasal sinuses are very rare. This case report a 65 years-old patient who presented sinusitis and epitaxis as a first sign of renal cell carcinoma with metastasis to the right maxillary sinus, which was opereted at Lagoa Hospital—Rj and strengthen the inclusion of this disease in the differential diagnosis of sinus disease. The renal cell carcinoma is a relatively rare tumor, representing approximately 3% of malignant tumors. About a third of patients with renal cell carcinoma have metastasis at diagnosis. The sinuses are a rare location of metastasis. The maxillary sinuses and the paranasal sinuses are most commonly affected by metastatic tumors to the sinuses region. The most frequent primary tumor to metastasis to paranasal sinuses is the renal cell carcinoma. This report highlights the rarity of the case and the importance in the differential diagnosis of sinus disease.

应用具有决策支持系统(Dss)功能的sEDAM模型模拟黄河下游簸箕李引黄灌区的灌溉需求与渠系输配水状况,利用节水多准则分析方法评价农田与输配水系统的改善方案。以田间实测的各类典型数据资料为基础,采用随机方法生成与田间和各级渠道相关的数据,产生用于多准则分析的属性数据和性能指标。根据不同水平的与农田及输配水系统相关的决策变量,设计了8种用于改善农田与输配水系统的模拟方案,通过模拟的需水量与现状供水量之间的对比以及模拟的各类效用值随时间变化的趋势,对模拟结果进行分析。结果表明,采用改善的农田与输配水系统运行方案可有效地减少渠系渗漏量,较好地控制农田渗漏损失,利用田间径流排水作为补灌水源,达到节水增产、改善农田生态环境的目的。

The degree of vessel lumen narrowing is an independent predictor of ischemic stroke. New developments in carotid plaque morphology imaging (MR, CT), may bring new insights to the relationship between carotid atherosclerotic disease and stroke risk. Our aim is to review the stroke risk in a symptomatic patient with moderate carotid stenosis by CT imaging and histopathology. A 72-year-old patient with low ABCD2 scores TIA and moderate left internal carotid stenosis (50% by carotid ultrasound), was discharged with an optimized medical therapy. Four months later, he presented an ischemic stroke in the left frontal area. Carotid angiography showed a 60% stenosis in the left-internal carotid artery with a regular surface. CT plaque imaging detected a thin fibrous cap with calcification and an intraplaque hemorrhage (high-risk plaque). These findings were confirmed in the histolopathological study of the atherosclerotic plaque performed after the endarterectomy. After 1 year of follow-up, the patient returned independently to his daily activities. We propose, in this study, the inclusion of noninvasive plaque imaging in the evaluation of acute TIA with moderate carotid stenosis to better select patients with higher risk of stroke recurrence.

This study sets out a scheme for a controlled release delivery system using sBA-16 as a carrier matrix and Rutin as a drug (Rutin-sBA-16). Physicochemical characterizations were performed to confirm the structure of the sBA-16 for post-synthesis by scanning electron microscopy (sEM), Transmission Electron Microscopy (TEM) and X-ray Diffraction (XRD). The presence of Rutin-sBA-16 was confirmed by Fourier-transform infrared spectroscopy (FTIR) and Nitrogen adsorption-desorption isotherms at 77 K. The dissolution kinetics was evaluated by the Zero Order, First Order and Higuchi models, and Rutin quantification was carried out by High Performance Liquid Chromatography (HPLC). The best impregnation time, which was 8 hours, adsorbing 284 μg Rutin per mg of silica, and the maximum degree of dissolution occurred in a period of 20 - 25 h. The release kinetics of the Rutin was called Higuchi, and showed high linearity, with a correlation coefficient (R2) of 0.999 compared with 0.905 and 0.980 of the zero order and first order models respectively. The study shows the benefits of Rutin-sBA-16 as a drug delivery system.

The intense and extreme rains in southeastern Brazil are spatially and temporally dynamic, corroborating their substantial complexity regarding understanding and associations. Therefore, the present study aimed at determining threshold values for events of intense and extreme rainfall in the region of são Carlos/sP using a climatic index, as well as temporal and spatial observations. The RClimdex script, Rnn index, and the detection of outliers were employed in order to mark and establish intense and extreme rainfall thresholds for the region. Values of 10 mm and 20 mm of rain were considered typical and of greater recurrence, and their incidence over a period of 24 hours did not necessarily denote intense events. In turn, values of 35, 46, and 60 mm indicate pluviometric rates that impact on significant disasters, as verified in the IPMET/UNEsP natural disasters database. It is important to emphasize that values below 60 mm of daily rainfall may also indicate disaster contexts. However, they do not exclude the necessity to verify the intensity, duration, and frequency of intense rain events, and can delineate thresholds for territorial management organizations in their planning.

BACKGROUND Acute fibroid complications are ***,failure to recognize and treat acute complications expeditiously when they occur can lead to catastrophic,even deadly,*** is a rare but potentially fatal condition resulting from infarction and infection of a fibroid through bacterial seeding and direct,hematogenous,or lymphatic *** though the diagnosis is established through clinical and laboratory findings,imaging is an important complementary method to support the suspected *** sUMMARY Herein,we report a case of a pyomyoma in a nulliparous woman previously diagnosed with uterine leiomyomatosis according to ultrasound *** patient had previously attended the emergency room due to hypogastric pain unresponsive to *** a week of persistent pain,she developed sepsis without any identifiable *** resonance imaging revealed findings compatible with uterine myomatosis with red degeneration,and a possible diagnosis of a pyomyoma was made according to the imaging findings along with the patient’s clinical *** decided to perform myomectomy(which is an infrequently performed surgical treatment due to the procedure’s intrinsic implic-ations)due to the patient’s desire to preserve *** results revealed a uterine leiomyoma with coagulative and liquefactive necrosis,while the tissue culture showed gram-negative cocci bacteria,which were successfully treated using antibiotic *** patient’s health status improved after several *** The main diagnostic tools to evaluate pyomyomas are the clinical and laboratory findings as well as tissue ***,magnetic resonance imaging can help to corroborate these findings as well as to better characterize myomas with its different complications.

We summarize our results about the quantization of a charged particle motion without spin inside a flat box under a static electromagnetic field with Landau’s gauge for the magnetic field, where Fourier’s transformation was used to analyze the problem, to point out that there exists a wave function which is different to that one given by Landau with the same Landau’s levels. The quantization of the magnetic flux is deduced differently to previous one, and a new solution is presented for the case of symmetric gauge of the magnetic field, and having the same Landau’s levels.

The increasing demand for new packages with increased shelf life properties has stimulated the increase of research in the active packaging sector. The use of antimicrobial agents requires an in-depth study of their properties to avoid loss of efficiency of the polymer processing. In this context, the objective of this work was to evaluate the preparation of an 18% ethylene vinyl acetate copolymer (EVA) nanocomposite and zinc oxide (ZnO) as microbicidal nanoparticle, prepared in a monosulfon extruder. The nanoparticle was modified with octadecylamine and EVA 18 nanocomposite films were prepared and compared to the systems containing modified nanoparticle. These new materials were characterized by thermogravimetric analysis (TGA), Differential scanning Calorimetry (DsC), X-Ray Diffraction (XRD), Dynamic Mechanical Analysis (DMA), Time Domain Nuclear Magnetic Resonance (NMR) to investigate the effect of zinc oxide nanoparticles on thermal properties, EVA crystallinity and antimicrobial effect. The TGA showed a tendency of increase of the thermal stability in different proportions of ZnO. DsC results did not show significant changes in thermal parameters. The XRD analysis showed an increase in the degree of crystallinity of the nanocomposites in relation to the EVA matrix and change in the crystallinity with the increase of ZnO percentages. DMA analysis indicates change in structural organization through the variation of storage modulus, loss, and tan delta. Time domain NMR data corroborate with XRD data through the change in molecular mobility.
