Efficacy and safety of percutaneous transhepatic biliary radiofrequency ablation in patients with malignant obstructive jaundice

BACKGROUND Percutaneous transhepatic cholangiodrainage(PTCD)and endoscopic retrograde cholangiopancreatography/endoscopic nasobiliary drainage are the most common clinical procedures for jaundice control in patients with unresectable malignant obstructive jaundice,yet the safety and effect of endobiliary radiofrequency ablation(EB-RFA)combined PTCD is rarely reported,in this article,we report our experience of EB-RFA combined PTCD in such *** To retrospectively study the efficacy and safety of EB-RFA combined PTCD in patients with unresectable malignant obstructive *** Patients with unresectable malignant obstructive jaundice treated with EB-RFA under PTCD were selected,the bile ducts of the right posterior lobe was selected as the target bile ducts in all *** general conditions of all patients,preoperative tumour markers,total bilirubin(TBIL),direct bilirubin(DBIL),albumin(ALB),alkaline phosphatase(ALP),and glutamyl transferase(GGT)before and on the 7th day after the procedure,as well as perioperative complications,stent patency time and patient survival were *** All patients successfully completed the operation,TBIL and DBIL decreased significantly in all patients at the 7th postoperative day(P=0.009 and 0.006,respectively);the values of ALB,ALP and GGT also decreased compared with the preoperative period,but the difference was not statistically *** biliary bleeding occurred in 2 patients,which was improved after transfusion of blood and other conservative treatments,pancreatitis appeared in 1 patient after the operation,no serious complication and death happened after *** for 3 patients with loss of visits,the stent patency rate of the remaining 14 patients was 100%71%and 29%at the 1^(st),3^(rd),and 6^(th)postoperative months respectively,with a median survival of 4 *** EB-RFA under PTCD in patients with unresectable malignant obstructive jaundice has a satisfactory therapeu

Analysis of long-term outcome of modified gastric bypass for type 2 diabetes mellitus in Chinese patients

BACKGROUND Bariatric and metabolic surgery have been routinely performed following the rapid increase in obesity and metabolic diseases *** all evolving procedures,Roux-en-Y gastric bypass(RYGB)is considered the gold standard for surgical treatment of patients with type 2 diabetes mellitus(T2DM)and *** was introduced in China nearly 20 years ago,but the number of RYGB surgeries only accounts for 3.1%of the total number of weight loss and metabolic surgeries in China,it’s effect on Chinese people still needs further *** To investigate the effect and safety of a modified gastric bypass performed in Chinese patients with *** Patients with obesity and T2DM who underwent modified gastric bypass,with>5-year follow-up data,were *** All 37 patients underwent uneventful laparoscopic surgery,no patient was switched to laparotomy during the surgery,and no severe complications were *** weight and body mass index of the patients reduced from 84.6±17.3(60.0–140.0)kg and 30.9±5.0(24.7–46.2)kg/m2 to 67.1±12.2(24.7–46.2)kg and 24.6±3.9(17.7–36.5)kg/m2,respectively,and fasting plasma glucose and glycated hemoglobin decreased from 7.4±3.4 mmol/L and 8.2%±1.7%preoperatively to 6.5±1.3 mmol/L and 6.5%±0.9%5-years postoperatively,*** 29.7%(11/37)of the patients used hypoglycemic drugs 5-years postoperatively,and the complete remission rate of T2DM was 29.7%(11/37).Triglyceride level reduced significantly but high-density lipoprotein increased significantly(both P<0.05)compared with those during the preoperative *** and renal function improved significantly postoperatively,and binary logistic regression analysis revealed that the patients’preoperative history of T2DM and fasting C-peptide were significant prognostic factors influencing complete T2DM remission after RYGB(P=0.006 and 0.012,respectively).CONCLUSION The modified gastric bypass is a safe and feasible procedure for Chinese patients with obesity an

Rethinking attribute localization for zero-shot learning

Recent advancements in attribute localization have showcased its potential in discovering the intrinsic semantic knowledge for visual feature representations, thereby facilitating significant visual-semantic interactions essential for zero-shot learning(ZSL). However, the majority of existing attribute localization methods heavily rely on classification constraints, resulting in accurate localization of only a few attributes while neglecting the rest important attributes associated with other classes. This limitation hinders the discovery of the intrinsic semantic relationships between attributes and visual features across all *** address this problem, we propose a novel attribute localization refinement(ALR) module designed to enhance the model's ability to accurately localize all attributes. Essentially, we enhance weak discriminant attributes by grouping them and introduce weighted attribute regression to standardize the mapping values of semantic attributes. This module can be flexibly combined with existing attribute localization *** experiments show that when combined with the ALR module, the localization errors in existing methods are corrected, and state-of-the-art classification performance is achieved.

Natural gas hydrate(NGH)can cause pipeline blockages during the transportation of oil and gas under high pressures and low *** hydrate adhesion on pipelines is viewed as an efficient way to prevent NGH *** studies suggested the water film can greatly increase hydrate adhesion in gas-dominant ***,by performing the molecular dynamics simulations,we find in water-dominant system,the water film plays different roles in hydrate deposition on Fe and its corrosion ***,due to the strong affinity of water on Fe surface,the deposited hydrate cannot convert the adsorbed water into hydrate,thus,a water film *** water affinities decrease(Fe>Fe_(2)O_(3)>FeO>Fe_(3)O_(4)),adsorbed water would convert to amorphous hydrate on Fe_(2)O_(3)and form the ordered hydrate on FeO and Fe_(3)O_(4)after hydrate *** absorbed water film converts to amorphous or to hydrate,the adhesion strength of hydrate continuously increases(Feliquid layer,the process of which becomes harder as liquid layer *** a result,contrary to gas-dominant system,the water film plays the weakening roles on hydrate adhesion in water-dominant ***,our results can help to better understand the hydrate deposition mechanisms on Fe and its corrosion surfaces and suggest hydrate deposition can be adjusted by changing water affinities on pipeline surfaces.

Neuroprotective effects of chaperone-mediated autophagy in neurodegenerative diseases

Chaperone-mediated autophagy is one of three types of autophagy and is characterized by the selective degradation of proteins. Chaperone-mediated autophagy contributes to energy balance and helps maintain cellular homeostasis, while providing nutrients and support for cell survival. Chaperone-mediated autophagy activity can be detected in almost all cells, including neurons. Owing to the extreme sensitivity of neurons to their environmental changes, maintaining neuronal homeostasis is critical for neuronal growth and survival. Chaperone-mediated autophagy dysfunction is closely related to central nervous system diseases. It has been shown that neuronal damage and cell death are accompanied by chaperone-mediated autophagy dysfunction. Under certain conditions, regulation of chaperone-mediated autophagy activity attenuates neurotoxicity. In this paper, we review the changes in chaperone-mediated autophagy in neurodegenerative diseases, brain injury, glioma, and autoimmune diseases. We also summarize the most recent research progress on chaperone-mediated autophagy regulation and discuss the potential of chaperone-mediated autophagy as a therapeutic target for central nervous system diseases.

BACKGROUND The recurrence rate of liver cancer after surgery is *** ablation(RFA)combined with transcatheter arterial chemoembolization(TACE)is an effective treatment for liver cancer;however,its efficacy in recurrent liver cancer remains *** To investigate the clinical effect of TACE combined with RFA in the treatment of recurrent liver *** Ninety patients with recurrent liver cancer were divided into 2 groups according to treatment plan:Control(RFA alone);and experimental[TACE combined with RFA(TACE+RFA)].The incidence of increased alanine aminotransferase levels,complications,and other indices were compared between the two groups before and after the *** One month after the procedures,the short-term efficacy rate and Karnofsky Performance Status scores of the experimental group were significantly higher than those of the control group(Plirubin levels were lower than those in the control group(Plications between the two groups(P>0.05).CONCLUSION TACE+RFA was effective for the treatment of recurrent liver cancer and significantly reduced AFP levels and improved various indices of liver function.

In situ plasmonic & electrochemical fiber-optic sensor for multi-metal-ions detection

High precision and high throughput detection of heavy metal ions is essential for water quality monitoring and ***,we propose a plasmonic & electrochemical dual-mode fiber sensing probe for label-free and real-time detection of multiple ions(Pb2+and Cu2+as examples).This sensor comprises a multimode fiber-single mode fiber reflection probe,the outer surface of which is coated with a gold nanofilm to excite the surface plasmon resonance(SPR) optically and simultaneously serves as an electrochemical working *** traditional electrochemical detection,the enrichment of ions cannot be detected in ***,by utilizing the plasmonic & electrochemical dual-mode detection method,various kinds of metal ions can be deposited onto the gold nanofilm and selectively oxidized during forward potential scanning,and the entire electrochemical process can be monitored by SPR *** experimentally demonstrate that the sensor can simultaneously detect Pb2+and Cu2+in a mixed solution in real-time,providing a linear response over the ion concentration range from 10-12to 10-7M and offering an excellent detection limit(1.69×10-14–5.49×10-13M).The proposed dual-mode fiber sensor has the benefits of remote sensing,compact footprint,and cost-effectiveness and shows excellent potential for water quality risk management in difficult-to-reach environments.
